      • Role of Neoadjuvant Chemotherapy in the Management of Advanced Ovarian Cancer

        Objective: To analyze efficacy of neoadjuvant chemotherapy for advanced ovarian cancer. Materials and Methods: A total of 107 patients with advanced ovarian cancer undergoing cytoreductive surgery were divided into a neoadjuvant chemotherapy group (n=61) and a primary debulking group (n=46) and retrospectively analyzed. Platinum-based adjuvant chemotherapy was applied to both groups after cytoreductive surgery ande overall and progression-free survival times were calculated. Results: No significant difference was observed in duration of hospitalization ($20.8{\pm}6.1$ vs. $20.2{\pm}5.4$ days, p>0.05). The operation time of neoadjuvant chemotherapy group was shorter than the initial surgery group ($3.1{\pm}0.7$ vs. $3.4{\pm}0.8$ h, p<0.05). There were no significant differences in median overall survival time between neoadjuvant chemotherapy group and surgery group (42 vs. 55 months, p>0.05). Similarly, there was no difference in median progression-free survival between neoadjuvant chemotherapy group and surgery group (16 vs. 17 months, p>0.05). The surgical residual tumor size demonstrated no significant difference between initial surgery and neoadjuvant chemotherapy groups (p>0.05). Multivariate analysis showed that more than 3 cycles of regimen with neoadjuvant chemotherapy was associated with more resistance to chemotherapy compared with patients without receiving neoadjuvant chemotherapy (OR: 5.962, 95%CI: 1.184-30.030, p<0.05). Conclusions:Neoadjuvant chemotherapy can shorten the operation time. However, it does not improve survival rates of advanced ovarian cancer patients.

      • Perioperative Comprehensive Supportive Care Interventions for Chinese Patients with Esophageal Carcinoma: a Prospective Study

        Objective: To assess the effects of perioperative comprehensive supportive care interventions on outcome of Chinese esophageal cancer patients in a prospective study. Methods: 60 patients with primary esophageal carcinoma were randomized into an intervention group (IG, n=31) and a control group (CG, n=29). The Chinese version of symptom checklist-90 (SCL-90) was adopted to assess their psychological status. The interventions, including health education, psychological support, stress management, coping strategies and behavior training, were carried out in 3 phases (preoperative, postoperative I and postoperative II), and psychological effects were thereafter evaluated accordingly before surgery, and 1 week, 4 weeks and 24 weeks post-surgery. Medical costs were estimated at discharge. Survival of patients was estimated each year post-surgery. General health status and satisfaction-with-hospital were surveyed by a follow-up questionnaire 4 years post-surgery. Results: All the subjects demonstrated higher scores in the preoperative phase than the normal range of Chinese population concerning 7 psychological domains including somatization, obsessive-compulsive, depression, anxiety, hostility, phobic anxiety and paranoid ideation. Although no significant difference was observed between the two groups at admission, the scores of IG, which tended to decrease at a faster rate, were generally lower than those of CG at weeks 1, 4 and 24 post-surgery. The length of hospital stay and medical costs of IG were significantly less than those of CG and satisfaction-with-hospital was better. However, there was no significant difference in 4-year survival or health status between two groups. Conclusions: Appropriate perioperative comprehensive supportive care interventions help to improve the psychological state of Chinese patients with esophageal carcinoma, to reduce health care costs and to promote satisfaction of patients and their families with hospital.

        Establishment of a novel myocarditis mouse model based on cyclosporine A

        Background: Myocarditis is a myocardial injury that can easily cause adolescent death. Traditional research models of animal invasion with viral components, lipopolysaccharide (LPS) or porcine myocardial myosin, among others, have the shortcomings of potential biological safety hazards and high animal mortality. Objective: To explore the construction of a novel myocarditis model with cyclosporine A and the potential genes and pathways associated with it. Methods: BALB/c mice were used in this study, and cyclosporin A and LPS were injected into the peritoneal cavity of mice. The successful establishment of the model was assessed by detecting serum myocardial injury markers and inflammatory factors levels, HE, IHC staining, and RT-qPCR methods. Key genes were obtained using the GSE35182 dataset from the GEO database and validated with the RT-qPCR method. Results: We found that a large number of inflammatory cells infiltrated the myocardium of mice in each group of Cyclosporin A constructed model, while the expression of inflammatory factor indicators was increased, and this model has the characteristics of high degree of local inflammation in myocardial tissue, low mortality, and safe and non-toxic treatment. Using GSE35182 data, we selected 18 Hub genes and validated Hub genes in myocardial tissue with RT-qPCR and found that multiple signaling pathways such as Toll-likereceptor signaling pathway(TLRs), Rap1 signal pathway(Rap1), and Chemokine signaling pathway may be involved in the development of myocarditis. Conclusion: Cyclosporin A can construct a new myocarditis model, and TLRs, Chemokines and Rap1 signaling pathways may be the core pathways of myocarditis.

        Phase separation phenomena and thermodynamics of octylphenol polyoxyethylene ether in micellar solutions: Effect of high concentration of inorganic salts and linear correlation

        The phase separationphenomena or cloud points (CP) behaviorof nonionic octylphenol polyoxyethylene etherwith oxyethylene glycol ether unit of 15 (OP-15) were investigated in aqueous micellar solutions containinghigh concentrations of inorganic salts including NaCl, KCl, MgCl2, CaCl2, BaCl2, AlCl3, FeCl3, Na2SO4and Na2SiO3. The change in CP induced by salts and its mechanism are discussed theoretically. In aqueous solutionscontaining high concentration of salt, for thefirst time the linear relationship between CP and saltconcentrationiswellestablishedtocharacterizethedepressionofCP.Basedonpseudophaseseparationmodel,thermodynamic parameters are estimated and adopted to further describe the phase separation process andthe interactions between OP-15 molecules or the OP-15/water interactions. Thermodynamic parametersincluding standard Gibbs energy change (DG0CP), enthalpy change (DH0CP) and entropy change (DS0CP)indicate that the additions of different salts result in the change in entropy-driven nonspontaneousprocess of phase separation. These data, which are from the compensation temperatures and the changeof molar heat capacity, also prove that different ions induce the change in the interactions betweensurfactant molecules or the surfactant–water interaction and thus affect the depression of CP. Thesefindings help with understanding the effect of inorganic salts, especially the high concentration of salt, onCP of nonionic surfactant and then broadening the application of nonionic surfactant based on CP such asthe extraction of biomaterials in biological samples derived from animals or plants, etc.

        Seismic performance of the concrete-encased CFST column to RC beam joints: Analytical study

        A finite element analysis (FEA) model is established to investigate the concrete-encased concrete-filled steel tubular (CFST) column to reinforced concrete (RC) beam joints under cyclic loading. The feasibility of the FEA model is verified by a set of test results, consisting of the failure modes, the exposed view of connections, the crack distributions and development, and the hysteretic relationships. The full-range analysis is conducted to investigate the stress and strain development process in the composite joint by using this FEA model. The internal force distributions of different components, as well as the deformation distributions, are analyzed under different failure modes. The proposed connections are investigated under dimensional and material parameters, and the proper constructional details of the connections are recommended. Parameters of the beam-column joints, including material strength, confinement factor, reinforcement ratio, diameter of steel tube to sectional width ratio, beam to column linear bending stiffness ratio and beam shear span ratio are evaluated. Furthermore, the key parameters affecting the failure modes and the corresponding parameters ranges are proposed in this paper.

        Surface Analysis and Clinical Study of Connective Tissue Around Calcium Phosphate Coated Titanium Dental Implants in a Canine Model

        <P>A part of the titanium dental implant surface, which will meet connective tissue after being inserted in mandibular bone, was coated with calcium phosphate by ion beam assisted deposition (IBAD). Twenty-four implants of coated and uncoated group were randomly placed in mandibles after 3 months of premolars extraction in beagle dogs. All the implants were firmly anchored in the bone and had no clinical signs of peri-implant inflammation after 3-month insertion. The probing depth in calcium phosphate coated group is 1.9±0.2 mm, less than in control group (2.1±0.2 mm) (p<0.05). Most of the probing sites tended to bleed upon probing in two groups. It might indicate that the calcium phosphate coating has the potential to promote soft tissue integration.</P>

        Enantioselective Esterification of Ibuprofen by a Novel Thermophilic Biocatalyst: APE1547

        The enantioselective esterification of ibuprofen catalyzed by a novel thermophilic esterase (APE1547)from the archaeon Aeropyrum pernix K1 was successfully conducted in organic solvents. The effects of acyl acceptor,substrate ratio, organic solvent, temperature, and water activity were investigated. Under optimum conditions, the highest enantioselectivity (E = 38.1) was obtained with a higher enzyme activity (216.5 μmol/g/h). Celites were added into the reaction mixture to remove the water produced in the esterification. The reaction achieved its equilibrium in approximately 96 h with a conversion of 57 and 99%(ee) of the un-reacted (S)-ibuprofen obtained.

        A comparative study on chemical composition of total saponins extracted from fermented and white ginseng under the effect of macrophage phagocytotic function

        In this study, white ginseng was used as the raw material, which was fermented with Paecilomyces hepiali through solid culture medium, to produce ginsenosides with modified chemical composition. The characteristic chemical markers of the products thus produced were investigated using rapid resolution liquid chromatography coupled with quadrupole time-of-flight mass spectrometry (RRLCeQTOFeMS). Chemical profiling data were obtained, which were then subjected to multivariate statistical analysis for the systematic comparison of active ingredients in white ginseng and fermented ginseng to understand the beneficial properties of ginsenoside metabolites. In addition, the effects of these components on biological activity were investigated to understand the improvements in the phagocytic function of macrophages in zebrafish. According to the established RRLCeQTOFeMS chemical profiling, the contents in ginsenosides of high molecular weight, especially malonylated protopanaxadiol ginsenosides, were slightly reduced due to the fermentation, which were hydrolyzed into rare and minor ginsenosides. Moreover, the facilitation of macrophage phagocytic function in zebrafish following treatment with different ginseng extracts confirmed that the fermented ginseng is superior to white ginseng. Our results prove that there is a profound change in chemical constituents of ginsenosides during the fermentation process, which has a significant effect on the biological activity of these compounds.
