Radiofrequency ablation (RFA) is a minimally invasive management strategy that has been widely applied for benign and recurrent malignant thyroid lesions as an alternative to surgery in Taiwan. Members of academic societies for specialists in interventional radiology, endocrinology, and endocrine surgery collaborated to develop the first consensus regarding thyroid RFA in Taiwan. The modified Delphi method was used to reach a consensus. Based on a comprehensive review of recent and valuable literature and expert opinions, the recommendations included indications, pre-procedural evaluations, procedural techniques, post-procedural monitoring, efficacy, and safety, providing a comprehensive review of the application of RFA. The consensus effectively consolidates advice regarding thyroid RFA in clinical practice for local experts.

Background and ObjectivesAge is a traditional risk factor for open-heart surgery. The efficacy and safety of transcatheter edge-to-edge mitral valve repair, using MitraClip (Abbott Vascular), has been demonstrated in patients with severe mitral regurgitation (MR). Since octogenarians or older patients are usually deferred to receive open-heart surgery, the main interest of this study is to elucidate the procedural safety and long-term clinical impact of MitraClip in elderly patients. MethodsPatients with symptomatic severe MR were evaluated by the heart team. For those with high or prohibitive surgical risks, transcatheter mitral valve repair was performed in hybrid operation room. Transthoracic echocardiography (TTE), blood tests, and six-minute walk test (6MWT) were performed before, 1-month, 6-months, and 1 year after index procedure. ResultsA total of 46 consecutive patients receiving MitraClip procedure were enrolled. Nineteen patients (84.2±4.0 years) were over 80-year-old and 27 (73.4±11.1 years) were younger than 80. Compare to baseline, the significant reduction in MR severity was achieved after the procedure and sustained. All the patients benefited from significant improvement in New York Heart Association functional class. The 6-minute walk test (6MWT) increased from 259±114 to 319±92 meters (p=0.03) at 1 year. The overall 1-year survival rate was 80% in the elderly and 88% in those <80 years, p=0.590. Baseline 6MWT was a predictor for all-cause mortality (odds ratio, 0.99; 95% confidence interval, 0.982–0.999; p=0.026) after the MitraClip procedure. ConclusionsTrans-catheter edge-to-edge mitral valve repairs are safe and have positive clinical impact in subjects with severe MR, even in advanced age.

Background: The initiation of sodium-glucose cotransporter-2 inhibitors (SGLT2i) typically leads to a reversible initial dip in estimated glomerular filtration rate (eGFR). The implications of this phenomenon on clinical outcomes are not well-defined.Methods: We searched MEDLINE, Embase, and Cochrane Library from inception to March 23, 2023 to identify randomized controlled trials and cohort studies comparing kidney and cardiovascular outcomes in patients with and without initial eGFR dip after initiating SGLT2i. Pooled estimates were calculated using random-effect meta-analysis.Results: We included seven studies in our analysis, which revealed that an initial eGFR dip following the initiation of SGLT2i was associated with less annual eGFR decline (mean difference, 0.64; 95% confidence interval [CI], 0.437 to 0.843) regardless of baseline eGFR. The risk of major adverse kidney events was similar between the non-dipping and dipping groups but reduced in patients with a ≤10% eGFR dip (hazard ratio [HR], 0.915; 95% CI, 0.865 to 0.967). No significant differences were observed in the composite of hospitalized heart failure and cardiovascular death (HR, 0.824; 95% CI, 0.633 to 1.074), hospitalized heart failure (HR, 1.059; 95% CI, 0.574 to 1.952), or all-cause mortality (HR, 0.83; 95% CI, 0.589 to 1.170). The risk of serious adverse events (AEs), discontinuation of SGLT2i due to AEs, kidney-related AEs, and volume depletion were similar between the two groups. Patients with >10% eGFR dip had increased risk of hyperkalemia compared to the non-dipping group.Conclusion: Initial eGFR dip after initiating SGLT2i might be associated with less annual eGFR decline. There were no significant disparities in the risks of adverse cardiovascular outcomes between the dipping and non-dipping groups.
Interleukin-20 targets podocytes and is upregulated in experimental murine diabetic nephropathy

Interleukin (IL)-20, a proinflammatory cytokine of the IL-10 family, is involved in acute and chronic renal failure. The aim of this study was to elucidate the role of IL-20 during diabetic nephropathy development. We found that IL-20 and its receptor IL-20R1 were upregulated in the kidneys of mice and rats with STZ-induced diabetes. In vitro, IL-20 induced MMP-9, MCP-1, TGF-β1 and VEGF expression in podocytes. IL-20 was upregulated by hydrogen peroxide, high-dose glucose and TGF-β1. In addition, IL-20 induced apoptosis in podocytes by activating caspase-8. In STZ-induced early diabetic nephropathy, IL-20R1-deficient mice had lower blood glucose and serum BUN levels and a smaller glomerular area than did wild-type controls. Anti-IL-20 monoclonal antibody (7E) treatment reduced blood glucose and the glomerular area and improved renal functions in mice in the early stage of STZ-induced diabetic nephropathy. ELISA showed that the serum IL-20 level was higher in patients with diabetes mellitus than in healthy controls. The findings of this study suggest that IL-20 induces cell apoptosis of podocytes and plays a role in the pathogenesis of early diabetic nephropathy.
Preparation and characterization of Y-TZP powders coated with alumina

In this study, alumina was coated onto yttria tetragonally stabilized zirconia (Y-TZP)core powder using aluminum nitrate with urea in an aqueous solution. Boehmite coating onto the surface of Y-TZP powders was confirmed using transmission electron microscopy (TEM), energy dispersive spectroscopy (EDS), Fourier transform infrared (FTIR) spectroscopy and zeta potential measurements. The sintering behaviors of Y-TZP powders with and without an alumina coating were investigated using a dilatometer and scanning electron microscopy (SEM). Boehmite coating onto the surface of Y-TZP powders significantly improved the densification of the resulting ceramic bodies. The relative density of the Y-TZP with a boehmite coating sintered at 1350oC can reach 97.3%. Purpose Undertaking a standard-setting exercise is a common method for setting pass/fail cut scores for high-stakes examinations. The recently introduced equal Z standard-setting method (EZ method) has been found to be a valid and effective alternative for the commonly used Angoff and Hofstee methods and their variants. The current study aims to estimate the minimum number of panelists required for obtaining acceptable and reliable cut scores using the EZ method. Methods The primary data were extracted from 31 panelists who used the EZ method for setting cut scores for a 12-station of medical school’s final objective structured clinical examination (OSCE) in Taiwan. For this study, a new data set composed of 1,000 random samples of different panel sizes, ranging from 5 to 25 panelists, was established and analyzed. Analysis of variance was performed to measure the differences in the cut scores set by the sampled groups, across all sizes within each station. Results On average, a panel of 10 experts or more yielded cut scores with confidence more than or equal to 90% and 15 experts yielded cut scores with confidence more than or equal to 95%. No significant differences in cut scores associated with panel size were identified for panels of 5 or more experts. Conclusion The EZ method was found to be valid and feasible. Less than an hour was required for 12 panelists to assess 12 OSCE stations. Calculating the cut scores required only basic statistical skills.
Provable Secure Brand-new Multi-auction Mechanism with Dynamic Identity

Different from traditional auctions, electronic auctions provide a platform to allow bidders and auctioneers merchandise to each other over network anytime and anywhere. Auctioneers can publish information of goods, and bidders can choose the interested targets through this bidding platform. To ensure the fairness and security of electronic auctions, Li et al. have proposed a practical electronic auction scheme which can confirm the requirement of strong anonymity, bidding privacy, and secret bidding price. However, we have found out that Li et al.'s scheme may lurk the risk of the denial-of-service attack during the bidding phase in a sealed-bid auction. Thus, we propose a brand-new sealed-bid auction mechanism, in which the essentials of e-auction can be firmly preserved. In particular, each bidder only needs to register at the center once and then can join to multiple plays launched by different auctioneers. Moreover, the correctness of mutual authentication is confirmed according to the BAN logic model.

In this study, the predatory efficacy of Orius strigicollis (Poppius), a predacious flower bug, against the twospotted spider mite (TSSM), Tetranychus urticae Koch, on strawberry (Fragaria ananassa) was investigated. Our results showed that release of 2, 4, 6, and 12 neonates of O. strigicollis per strawberry plant could suppress 58.9, 72.6, 88.6, and 92.8% of T. urticae, respectively, at 14 days after treatment. A single release of 12 neonate bugs per plant achieved and maintained, over a period of 3 months, 82% to 98% control of T. urticae population. A biweekly release of 6 neonate bugs per plant; likewise, suppressed ca. 90% of T. urticae population throughout the same period. However, predatory efficacy was reduced by an over-release of predators which may cause cannibalism as well as lower possibility of finding prey. Since the released neonates survived well to adults with offspring that retained their predatory ability, this study has demonstrated that O. strigicollis is a predatorwith good potential for sustainable suppression of T. urticae on strawberry, and one application of O. strigicollis neonates at a predator:prey ratio of 1:200 egg plus motile TSSM or 1:60 motiles per strawberry plant promises a season-long control and substantial management of spider mite infestation.
