      • Increased expression of connexin43 on the aortic valve in the hypercholesterolemic rabbit model.

        <P>BACKGROUND: Aortic valve sclerosis is associated with increased risk of cardiovascular death and myocardial infarction. However, the relevance of connexin43 in aortic valve sclerosis remains unclear. We hypothesized that the mechanism regulating aortic valve sclerosis is associated with the alteration of cell-to-cell communication. METHODS: Twenty male New Zealand rabbits were divided into two groups. Group 1 (n = 10) were fed a normal chow diet, while those in group 2 (n = 10) received a diet containing 1% cholesterol for 12 weeks. After utanizing the animals, the aortic valves were excised for analysis. RESULTS: Myofibroblasts and macrophages were more highly expressed in the cholesterol diet group. Osteopontin and connexin43 were found to concentrate within the endothelial layer on the aortic side of the valve leaflets in the cholesterol diet group. A real-time polymerase chain reaction revealed increased connexin43 and osteopontin mRNA levels in the hypercholesterolemic aortic valves. CONCLUSIONS: The present study demonstrates that hypercholesterolemia increases the expression of connexin43 in the rabbit aortic valve. The results suggest that alterations in gap junctional intercellular communication via connexin43 gap junctions may play a role in the development of aortic valve sclerosis.</P>

      • An Experimental Study on the Effect of Hypothermic Oxygenated Cardioplegic Solution on Myocardial Protection during Prolonged Aortic Cross-clamping

        This study was experimental undertaken to evaluate the effect of hypothermia oxygenated cardioplegic solution on myocardial protection during prolonged aortic cross-clamping under cardiopulmonary bypass. Dogs were divided into two groups-control group (received hypothermic unoxygenated cardioplegic solution) and experimental group (received hypothermic oxygenated cardioplegic solution). Coronary sinus effluent was obtained at once and 30, 60, 90 minutes after cross clamping for the determination of pH, PCO_2, PO_2, and lactate level during the infusion of cardioplegic solution and myocardial biopsies were obtained after cessation of 90 minutes of aortic cross-clamping. The results obtained were as follows: 1. There was no significant differences in the pH and PCO_2; between the oxygenated and unoxygenated cardioplegic solution but the PO_2 of the oxygenated solution was 4 times greater than unoxygenated solution, and also the oxygenated solution had a significantly greater oxygen content (2.02±0.5 ml O_2/min) and had much more oxygen delivery than unoxygenated solution. 2. The myocardial oxygen consumption and the myocardial oxygen extraction in oxygenated group were 1.63 ml O_2/100 ml and 67.32% respectively, which was greater than those in unoxygenated group. 3. Regarding to pH and PCO_2 of coronary sinus effluent, the pH shifted to acidosis from 60 minutes, PCO_2 increased from 90 minutes of aortic cross-clamping, and PO_2 markedly decreased from 90 minutes of aortic cross-clamping in unoxygenated group. 4. The lactate concentration of coronary sinus effluent revealed relatively normal in both groups, but showed slight increase up to 27.5±4.56 mg/100 ml at 90 minutes of aortic cross-clamping in unoxygenated group. 5. On electron microscopic study, the ultrastructural integrity of myocardial cells in oxygenated group was well preserved within 90 minutes. Slight swelling and deformity of mitochondria, interfibrillar widening, and disarrangement of myofibrilles were observed at 90 minutes after aortic cross-clamping in unoxygenated group. From these results, the use of hypothermic oxygenated cardioplegic solution seemed to be effective and better method for preservation of ischemic myocardium during the prolonged aortic cross-clamping.

      • Valve Replacement with St. Jude Medical Valve

        Consequently, overall mortality was 10.3%(37/357) during follow-up period. Actuarial survival rates after 6 yrs were 94.6% for MVR. 96.8% for AVR. 95.8% for DVR and 100% for TVR. In conclusion, the st. Jude Medical prosthesis can be considered to be an effective valve with a low rate of valve-related complication when there is correct anticoagulant therapy. Our group continues to use the St. Jude Medical Valve in all patients requiring Valve replacement except in those who can not take warfarin anticoagulation.

      • Somatic mutations of the <i>ERBB4</i> kinase domain in human cancers

        <P><B>Abstract</B></P><P>The EGFR family consists of 4 receptor tyrosine kinases, EGFR (ERBB1), ERBB2 (HER2), ERBB3 (HER3) and ERBB4 (HER4). Recent reports revealed that the kinase domains of both <I>EGFR</I> (<I>ERBB1</I>) and <I>ERBB2</I> gene were somatically mutated in human cancers, raising the possibility that the other ERBB members possess somatic mutations in human cancers. Here, we performed mutational analysis of the <I>ERBB4</I> kinase domain by polymerase chain reaction–single‐strand conformation polymorphism assay in 595 cancer tissues from stomach, lung, colon and breast. We detected the <I>ERBB4</I> somatic mutations in 3 of 180 gastric carcinomas (1.7%), 3 of 104 colorectal carcinomas (2.9%), 5 of 217 nonsmall cell lung cancers (2.3%) and 1 of 94 breast carcinomas (1.1%). The 12 <I>ERBB4</I> mutations consisted of 1 in‐frame duplication mutation and 8 missense mutations in the exons, and 3 mutations in the introns. We simultaneously analyzed the somatic mutations of <I>EGFR</I>, <I>ERBB2, K‐RAS</I>, <I>PIK3CA</I> and <I>BRAF</I> genes in the 12 samples with the <I>ERBB4</I> mutations and found that 1 gastric carcinoma with <I>ERBB4</I> mutation also harbored <I>K‐RAS</I> gene mutation. Our study demonstrated that in addition to <I>EGFR</I> and <I>ERBB2</I>, somatic mutation of the kinase domain of <I>ERBB4</I> occurs in the common human cancers, and suggested that alterations of ERBB4‐mediated signaling pathway by <I>ERBB4</I> mutations may contribute to the development of human cancers. © 2005 Wiley‐Liss, Inc.</P>

        Identification of the Elements Regulating the Expression of the Phospholipase C Delta1

        Recently we cloned the phospholipase C delta1 (PLC-d1) promoter region and found that PLC-d1 is selec-tively expressed in several tissues. In order to estab-lish the common and cell-type specific transcriptional elements, 1.8 kilobase of the 5¢-flanking region of PLC-d1 was characterized in several cell lines. A tran-sient transfection assay of the -1787-Luc construct in several cell lines demonstrated the potential transcrip-tional enhancement of reporter activities in the neuroblastoma cell [SK-N-BE(2)C] and kidney cell lines (Cos-7), but not in liver cell lines (Chang liver cell). Transient transfection assays of a series of 5¢ 3¢ deletion constructs of the PLC-d1 promoter and electronic mobility shift assays suggested that the E-box and HFH3 binding sites are cell-type specific elements, and that Sp-1 is a major transcriptional activator of a majority of cell lines. Our findings, therefore, indicate that the combination of several elements within the 5¢-flanking region of the PLC-d1 gene dictates its restricted expression in several cell lines.

        Molecular characterization of avian rotavirus isolated in Korea

        An avian rotavirus (AvRV-2) was isolated from feces of broilers suffering from acute gastroenteritis in 2011. It was the first avian rotavirus isolated in Korea. To investigate the molecular characteristics of AvRV-2, the VP4, VP6, VP7 and NSP4 gene nucleotide sequences were determined and compared with those of rotavirus strains available in the GenBank database. The phylogenetic tree of VP7 gene showed that AvRV-2 had a high degree of nucleotide sequence homology (93.4% to 94.7%) with those of rotaviruses belonging to genotype G19 cluster. The phylogenetic tree of the VP4 gene revealed a high degree of nucleotide sequence homology (95.8% to 95.9%) with genotype P[30] rotaviruses isolated from chickens. The VP6 and NSP4 gene nucleotide sequences showed the highest identities with those of avian strains with 95.3% to 96.4% and 90.3% to 92.2%, respectively. Genetic characterization of the VP4, VP6, VP7 and NSP4 showed that AvRV-2 strain was most closely related to chicken rotavirus strains from Germany and Japan. Comparative nucleotide sequences and phylogenetic analysis indicated that avian rotavirus isolated from broilers belonged to genotype G19P[30] and it was the first report on avian rotavirus infection in Korea.

      • Eradication of <i>Plasmodium falciparum</i> from Erythrocytes by Controlled Reactive Oxygen Species via Photodynamic Inactivation Coupled with Photofunctional Nanoparticles

        <P>We investigated the antimalarial effect of photodynamic inactivation (PDI) coupled with magnetic nanoparticles (MNPs) as a potential strategy to combat the emergence of drug-resistant malaria and resurgence of malaria after treatment. Because the malarial parasite proliferates within erythrocytes, PDI agents need to be taken up by erythrocytes to eradicate the parasite. We used photofunctional MNPs as the PDI agent because nanosized particles were selectively taken up by Plasmodium-infected erythrocytes and remained within the intracellular space due to the enhanced permeability and retention effect. Also, the magnetism of Fe3O4 nanoparticles can easily be utilized for the collection of photofunctional nanoparticles (PFNs), and the uptaken PFNs infected the erythrocytes after photodynamic treatment with external magnetics. Photofunctionality was provided by a photosensitizer, namely, pheophorbide A, which generates reactive oxygen species (ROS) under irradiation. PAs were covalently bonded to the surface of the MNPs. The morphology and structural characteristics of the MNPs were investigated by scanning electron microscopy and X-ray diffraction (XRD), whereas the photophysical properties of the PFNs were studied with Fourier transform infrared, absorption, and emission spectroscopies. Generation of singlet oxygen, a major ROS, was directly confirmed with time -resolved phosphorescence spectroscopy. To evaluate the ability of PFNs to kill malarial parasites, the PDI effect of PFNs was evaluated within the infected erythrocytes. Furthermore, malarial parasites were completely eradicated from the erythrocytes after PDI treatment using PFNs on the basis of an 8 day erythrocyte culture test.</P>

        Functional outcomes of Gartland III supracondylar humerus fractures with early neurovascular complications in children : A retrospective observational study

        <P><B>Abstract</B></P><P>This was a retrospective observational study. The aim of this study was to evaluate functional outcomes in children treated for Gartland III supracondylar humerus (SCH) fracture with neurovascular (NV) injuries using validated outcome measures. A secondary goal was to determine whether clinical parameters such as age at injury, sex, weight, fracture site, and/or direction of displacement could predict NV injury at the time of fracture or long-term functional outcomes in these patients.</P><P>One hundred fifty-four patients of Gartland III SCH fractures between March 2004 and May 2013 were studied retrospectively. The patients were divided into 2 groups according to the presence of NV injury. Medical records and radiographs were reviewed to assess several parameters, including age, sex, weight, treatment intervention, the extremity involved, direction of fracture displacement, and NV injury. Functional outcome was assessed on final follow-up using the Pediatric Outcomes Data Collection Instrument (PODCI) and Quick Disabilities of the Arm, Shoulder, and Hand (Quick DASH) outcome measures. Statistical analysis was used to determine the relationship between NV injury and functional outcomes.</P><P>There were 33 cases with Gartland III SCH fracture associated with NV injuries (10 cases of vascular compromise, 14 cases of neural injury, and 9 cases involving both vascular compromise and neural injury). There were significant differences between the 2 groups in age (<I>P</I>  =  .048), weight (<I>P</I>  =  .009), and direction of displacement (<I>P</I>  =  .004). Vascular compromise and median nerve injury were most common in fractures with posterolateral displacement, and radial nerve injuries were common in fractures with posteromedial displacement. The mean global function score in the PODCI was 91.4 points, and the mean Quick DASH score was 11.7 points, with excellent functional outcomes. No differences in outcomes were identified based upon age, fracture site, sex, weight, direction of displacement, or operative technique in NV injury patients (<I>P</I> > .05).</P><P>The majority of patients with Gartland III SCH fractures associated with NV injuries returned to a high functioning level after treatment of their injuries. NV injury does not appear to influence functional outcomes. Good functional results can be expected regardless of age, fracture site, sex, weight, direction of displacement, and operative technique.</P>
