      • Low-loss silicon-on-quartz (SoQ)-based aperture-coupled antenna on RF MEMS packaging platform

        <P>This letter presents a micromachined aperture-coupled antenna which improves antenna performances as well as simplifies a fabrication process. The SoQ (silicon-on-quartz) bonding and BCB (benzocyclobutene) adhesive bonding are employed for a radio frequency microelectro-mechanical system (RF-MEMS) packaging platform. The SoQ/BCB packaging platform makes the air cavity and the quartz substrate where the aperture-coupled antenna is stacked. Therefore, radiation performances can be enhanced from low-loss air/quartz substrates while a silicon substrate has almost no effects on radiation. Furthermore, the proposed method enables simultaneous integration of the antenna with the RF MEMS devices through a single packaging process. In this letter, the antenna is manufactured based on the proposed method and the fabrication process is carefully described.         Comparison between SLAP Repair and Biceps Tenodesis with Concomitant Rotator Cuff Repair in Patients Older Than 45 Years: Minimum 2-Year Clinical and Imaging Outcomes

        Background: There is controversy over how to surgically treat symptomatic superior labrum anterior to posterior (SLAP) tears in middle-aged patients with concomitant rotator cuff tears. The aim of the study was to compare the clinical and imaging outcomes of SLAP repair versus biceps tenodesis (BT) each combined with arthroscopic rotator cuff repair (ARCR). Methods: We retrospectively reviewed 35 patients older than 45 years who underwent arthroscopic surgery to manage concomitant SLAP tears and rotator cuff tears. In addition to ARCR, 17 patients underwent SLAP repair, whereas 18 patients underwent BT. Shoulder range of motion (ROM), visual analog scale (VAS) for pain, American Shoulder and Elbow Surgeons (ASES) score, Constant score, and University of California at Los Angeles (UCLA) score were used for clinical assessment. The integrity of rotator cuff repair and change of superior labrum-biceps complex were evaluated by postoperative magnetic resonance imaging (MRI). Results: There was significant improvement in the pain VAS and all functional scores in both groups (p < 0.001) at a mean followup of 29.4 ± 11.4 months (range, 24–84 months) postoperatively. Shoulder ROM showed significant improvement postoperatively (p < 0.05). No significant difference in outcomes could be found between the 2 groups after surgery. The retear rate of rotator cuff repair on MRI was 11.8% in the SLAP repair group and 11.1% in the BT group. Conclusions: In middle-aged patients with combined SLAP lesions and rotator cuff tears, both SLAP repair and BT can be safe adjuncts to ARCR.

      • Rotator cuff repair with autologous fascia lata augmentation for biologically unfavorable rotator cuff tear: a minimum two-year follow-up

        Introduction and Background Large to massive cuff tear with advanced fatty degeneration and failed repair are unfavorable factors that affect the outcomes after rotator cuff repair. Various additional procedures have been employed to promote tissue healing in these group of patients. The aim of the study was to report the minimum two-year follow-up outcome of rotator cuff repair with autologous fascia lata augmentation in the biologically unfavorable treatment group. Material and Method This was a retrospective study of 13 patients (aged 63.4 ± 6.3 years) who underwent open rotator cuff repair with autologous fascia lata augmentation. The indications included large to massive rotator cuff tear with high-grade fatty degeneration defined as Goutallier grade 3 or 4 (11 patients) and retear after previous cuff surgery (2 patients). Clinical outcomes were assessed according to the pain VAS, ASES shoulder score at yearly follow-up. Postoperative imaging study (MRI or ultrasonography) was evaluated for integrity of rotator cuff repair when available. Results At the 1-year follow-up, there was significant improvement of pain VAS (from 4.9 ± 1.6 to 1.4 ± 1.0, p < 0.001) and ASES score (from 51.2 ± 19.7 to 84.6 ± 7.9, p < 0.001). At the minimum 2-year follow-up (mean 34 months), these improved outcomes were maintained (pain VAS 1.0 ± 0.9 and ASES score 87.2 ± 11.3). No patient underwent secondary surgery at the final follow-up. Nine patients were available for imaging study (8 for MRI study, 1 for ultrasonography) at 14.3 ± 5.7 months postoperatively. One patient showed retear (Sugaya type 5) and two patients showed thinning (Sugaya type 3) of repaired tendon. Conclusions In rotator cuff repair for patients with biologically unfavorable factors, autologous fascia lata augmentation can be a promising adjunct to promote healing and improve the clinical outcome.

        Debridement, antibiotics, and implant retention in infected shoulder arthroplasty caused by Serratia marcescens: a case report

        Periprosthetic joint infection (PJI) is one of the most devastating complications that can occur after shoulder arthroplasty. Although staged revision arthroplasty is the standard treatment in many cases, surgical intervention with debridement, antibiotics, and implant retention (DAIR) can be an effective option for acute PJI. We report a complex case of infected reverse shoulder arthroplasty (RSA) in a 73-year-old male. The patient had been previously treated for infected nonunion of a proximal humerus fracture caused by methicillin-resistant Staphylococcus epidermidis. He presented with a sinus tract 16 days after the implantation of RSA and was diagnosed with PJI caused by Serratia marcescens. The patient was successfully treated with DAIR and was free of infection at the last follow-up visit at 4 years postoperatively.

      • Low-Profile Pattern-Reconfigurable Antenna with Vertical and Horizontal Shorting Lines in Grounded CPW Technology

        <P>A novel low-profile reconfigurable antenna is proposed for changing radiation patterns. The antenna is constructed on a circular patch fed by a grounded coplanar waveguide (GCPW). The circular patch has three vertical shorting lines and one horizontal shorting line. A p-i-n diode switch on the horizontal shorting line is used to configure the radiation pattern of the antenna. In the off state, the maximum radiation intensity occurs at <TEX>$\pm 45^\circ$</TEX> to the circular patch. In the on state, the maximum radiation intensity occurs perpendicular to the circular patch. Simulation and measurement results at 2.3 GHz validate the reconfigurability of the proposed design.</P>

      • Substrate-Integrated-Waveguide (SIW) Single-Pole-Double-Throw (SPDT) Switch for X-Band Applications

        <P>A substrate-integrated-waveguide (SIW) single-pole double-throw (SPDT) switch is presented for X-band applications. Inductive posts with rectangular slots are embedded in the SIW to control the traveling EM wave. By adding PIN diodes on the top slots of the inductive posts, we achieved the ability to switch in the SIW technology; then, an SIW SPDT switch was designed. The proposed SPDT switch has a measured isolation (S31) of greater than 10 dB, isolation (S32) of greater than 15 dB, and an insertion loss (IL) of less than 2.55 dB, from 8.24-10.36 GHz.</P>

        Usability evaluation of physical products involves characterizing complex physical interactions between humans and products. Human models known as manikins have been widely utilized as usability evaluation tools for automobile interior package design. When combined with computer-aided design software programs, such manikins can be used to simulate driving postures and evaluate driver-interior fits early in the design process, and therefore, may greatly facilitate achieving high-quality design in a cost-efficient manner. The purpose of this study was to define a set of manikins for designing automobile interior packages for the South Korean male population. These manikins were conceptualized as "boundary" manikins, which represent individuals lacking in certain physical capacities or having usability-related issues (e.g., an individual with the 5th percentile forward reach capability, an individual with the 95th percentile shoulder width). Such boundary manikins can serve as an efficient tool for determining if an automobile interior design accommodates the majority of the population. The boundary manikins were selected from the large sample of Korean males whose anthropometric dimensions were described in the recent Size Korea anthropometric database. For each male in the database, his comfortable driving posture was represented using a kinematic body linkage model and various physical capacity measured and usability-related characteristics relevant to driver accommodation were evaluated. For each such measure, a boundary manikin was selected among the Korean males. The manikins defined in this study are expected to serve as tools for ergonomic design of automobile interior packages. The manikin selection method developed in this study was implemented as a generic software program useful for various product design applications.

      • Monopole-Like and Boresight Pattern Reconfigurable Antenna

        <P>A novel pattern reconfigurable antenna is proposed to achieve two different radiation patterns by controlling the bias voltage of switches. Initially, the proposed antenna operates as a top-loaded monopole antenna fed by a co-planar waveguide (CPW). When the slot from the CPW is disturbed by switching on PIN diodes, it operates as a slot antenna. Therefore, when the switch turns on, the maximum radiation intensity occurs at the normal to the aperture surface defined as the boresight direction, which is similar to a slot antenna. When the switch turns off, the maximum radiation intensity occurs at right angles to the boresight direction, which is similar to a monopole antenna. The pattern reconfigurable capability is successfully demonstrated at 2.3 GHz by full-wave simulation and measurement.</P>

        60-㎓ 렉테나를 이용한 효율적 DC 전력 관리 체계

        In this paper, a DC power management scheme is proposed for efficient battery operation of 60-㎓ receivers. It consists of a rectenna and a zero-cross detector. The harmonic rejection capability of a circular sector antenna is used to simplify the rectenna, The zero-cross detector is introduced to increase sensitivity. When the received power density is larger than 0.013 ㎽/㎠, it can generate enough DC power to activate an RF receiver. Experimental data is used to verify the proposed idea.
