Background/Aims: There are few available data regarding the association between the single nucleotide polymorphisms (SNPs) of the gene encoding interleukin 28B (IL28B) and a sustained virologic response (SVR) to peginterferon (PEG-IFN) plus ribavirin (RBV) therapy in Korean chronic hepatitis C patients. Methods: This was a retrospective cohort study of 156 patients with chronic hepatitis C virus (HCV) infection who received combination treatment of PEG-IFN plus RBV. Blood samples from these patients were analyzed to identify the IL28B SNPs at rs12979860, rs12980275, rs8099917, and rs8103142. Association analyses were performed to evaluate the relationships between each IL28B SNP and SVRs. Results: Seventy six patients with HCV genotype 1 and 80 with genotype non-1 were enrolled. The frequencies of rs12979860 CC and CT genotypes were 90.4% and 9.6%, respectively; those of rs12980275 AA and AG genotypes were 87.2% and 12.8%, respectively; those of rs8099917 TT and TG genotypes were 92.3% and 7.7%, respectively; and those of rs8103142 TT and CT genotypes were 90.4% and 9.6%, respectively. Among the patients with HCV genotype 1, the SVR rates were 69.7% and 80.0% for rs12979860 CC and CT, respectively (P=0.71). Among the HCV genotype non-1 patients, SVR rates were 88.0% and 100% for rs12979860 CC and CT (P=1.00), respectively. Conclusions: Genotypes of the IL28B SNP that are known to be favorable were present in most of the Korean patients with chronic hepatitis C in this study. Moreover, the IL28B SNP did not influence the SVR rate in either the HCV genotype 1 or non-1 patients. Therefore, IL28B SNP analysis might be not useful for the initial assessment, prediction of treatment outcomes, or treatment decision-making of Korean chronic hepatitis C patients. Background: The outcome of hepatitis B virus (HBV) infection after liver transplantation (LT) was improved by hepatitis B immunoglobulin (HBIG) and nucles(t)ide analogue (NUA). However, HBV recurrence after LT is critical because the recurrence is occasionally accompanied by a progressive destruction of graft and poor survival. The aims of this study were to investigate the significance HBV recurrence and identity factors associated with HBV recurrence. Methods: From October 1999 to February 2011, a total of 2684 consecutive LT recipients who underwent HBV-associated LT were retrospectively enrolled from 7 transplantation centers in Korea. Results: Prophylaxis regimens were HBIG monotherapy (67.7%) or a combination of HBIG with NUA (22.3%). The recurrence rate of HBV was 6.1% (164 recipients) during mean follow-up duration of 10.9 years. The median time from transplantation to recurrence was 2.1 years (0.1-7.9 years). Of the 1,071 patients with hepatocellular carcinoma (HCC) prior to LT, 155 patients (14.5%) had HCC recurrence after transplantation, and 48 patients (31.0%) had HBV recurrence. Of the 48 patients with recurrence of both HBV and HCC, 25 patients (52.1%) experienced HBV recurrence after HCC recurrence. In the multivariate analysis, pretransplant HCC, pretransplant HBV DNA above 5.5 log copies/mL was independent clinical factors influencing HBV recurrence after LT. The mortality rate among the recipients with HBV recurrence was 34.1% (56 recipients). Mean overall survival was 6.4 years in the HBV-recurrence group and 9.9 years in the HBV-nonrecurrence group (p<0.001). HBV recurrence was not an independent prognostic factor for overall survival. HCC recurrence was the most important factor for overall survival. Conclusions: The overall outcome of LT in HBV-related liver disease was excellent with the current prophylaxis regimen Choon Hyuck David Kwon,8 Suk-Koo Lee8 Pretransplant HBV DNA and HCC were important factors for HBV recurrence. HBV recurrence after LT did not significantly influence on the overall survival without combining of HCC recurrence.

The eco-friendly preservatives such as ACQ or CUAZ have been used in landscape architectural facilities these days. In this study, the decay efficacies of ACQ treatments were evaluated according to domestic veneer species, concentration of ACQ, weathering test, adhesive type, and fungus type. In case of veneer species, hinoki cypress and yellow poplar showed the highest and lowest decay resistance, respectively. And the decay resistance appeared to be greater in plywoods bonded by resorcinol resin-bonded plywood and non-weathering treated plywood than polyurethane resin-bonded plywood and weathering treated plywood.

<P>It was found that the Ag electrode layer in a transmission electron microscopy (TEM) specimen of an inverted polymer solar cell structure of Ag/PEDOT:PSS/P3HT:PCBM/TiO<SUB>2</SUB>/ITO/glass (where PEDOT is poly(3,4-ethylenedioxythiophene), PSS is polystyrene sulfonate, and ITO is indium tin oxide) was broken down into particles as time passed. In order to investigate the cause of Ag particle formation and the effect of the degradation on the performance of solar cells, the temporal change of the cross-sectional TEM micrographs was examined together with energy-dispersive X-ray spectroscopy (EDS) analysis and electron tomography. Temporal degradation of Ag/Si and Ag/1 nm-Ti/PEDOT:PSS/ITO/glass structures was also studied. Absorption of water by the PEDOT:PSS layer followed by corrosion of the grain boundaries of the Ag layer by the corrosive water was thought to be the reason of Ag particle formation and fast performance lowering of the device.</P><P><B>Graphic Abstract</B> <IMG SRC='http://pubs.acs.org/appl/literatum/publisher/achs/journals/content/aamick/2012/aamick.2012.4.issue-10/am301818z/production/images/medium/am-2012-01818z_0016.gif'></P>

BACKGROUND:: Therapeutic drug monitoring (TDM) of posaconazole is usually performed 1 week after starting the drug because of its long half-life. However, previous studies showed that measuring the posaconazole plasma concentration (PPC) on day 3 is effective for predicting steady-state levels. The purpose of this study was to evaluate the relevance of early TDM (day 3) of posaconazole for achieving an optimal PPC. METHODS:: This prospective study was conducted from September 2014 to August 2016. A total of 148 patients with acute myeloid leukemia or myelodysplastic syndromes received a 200 mg posaconazole oral suspension 3 times daily for fungal prophylaxis. During the period from September 2014 to December 2015 (control group), no dose adjustment was performed on day 3. During the period from January 2016 to Aug 2016 (early TDM group), the frequency of posaconazole 200-mg administration was increased to 4 times daily in patients whose PPC on day 3 was <400 ng/mL. The cutoff value for optimal PPC on day 8 was defined as 500 ng/mL. RESULTS:: In 21 of 107 patients (20%) in the control group, PPC was <400 ng/mL on day 3. In 15 (71%) of these 21 patients, the PPC was suboptimal on day 8. In the early TDM group, the PPC was <400 ng/mL on day 3 in 4 of 41 patients (10%). After increasing the posaconazole administration frequency in these 4 patients, PPC was suboptimal on day 8 in 1 patient (25%). In both groups, 104 patients had a PPC of ≥500 ng/mL on day 3, but 7% (7/104) of these had a suboptimal level on day 8. CONCLUSIONS:: Early TDM on day 3 for posaconazole suspension may help more patients achieve optimal drug levels on day 8, although TDM on day 8 is needed even in patients with optimal levels on day 3.

<P>Reducing sugar, 2-deoxy-<SMALL>D</SMALL>-ribose (dRib), produces reactive oxygen species through autoxidation and protein glycosylation and causes osteoblast dysfunction. Kaempferol, a natural flavonoid, was investigated to determine whether it could influence dRib-induced cellular dysfunction and oxidative cell damage in the MC3T3-E1 mouse osteoblastic cell line. Osteoblastic cells were treated with 30 m<SMALL>M</SMALL> dRib in the presence or absence of kaempferol (10<SUP>−9</SUP>—10<SUP>−5</SUP> <SMALL>M</SMALL>) and markers of osteoblast function and lipid peroxidation were subsequently examined. Kaempferol (10<SUP>−9</SUP>—10<SUP>−5</SUP> <SMALL>M</SMALL>) significantly inhibited the dRib-induced decrease in growth of MC3T3-E1 osteoblastic cells. In addition, treatment with kaempferol resulted in a significant elevation of alkaline phosphatase (ALP) activity, collagen content, and mineralization in the cells. Treatment with kaempferol increased osteoprotegerin (OPG) secretion and decreased malondialdehyde (MDA) contents of MC3T3-E1 osteoblastic cells in the presence of 30 m<SMALL>M</SMALL> dRib. Taken together, these results suggest that kaempferol inhibits dRib-induced osteoblastic cell damage and may be useful for the treatment of diabetes-related bone disease.</P>
The pregnancy outcome after intrauterine insemination according to uterine position

목적: To determine whether the uterine position of women affects the outcome of intrauterine insemination 방법: Data were obtained from the medical records of 148 patients who underwent intrauterine insemination for infertility treatment between January 2011 and May 2016. All patients were examined by transabdominal ultrasonography before insemination procedure. The uterine position was recorded as anteverted or retroverted. All intrauterine inseminations were performed by experienced gynecologic endocrinologists. 결과: 102 of 148 patients(68.9%) had anteverted uterus and 46 of 148 patients(31.1%) had retroverted uterus. Clinical pregnancy rate of total patients was 18.2%. There were no significant differences in clinical pregnancy rate between women with anteverted uterus and retroverted uterus(17.6% and 19.6%, p=0.780). 결론: After intrauterine insemination procedure for infertility treatment, women with retroverted uterus had similar pregnancy outcome compared to whom with anteverted uterus. The uterine position did not affect the outcome of intrauterine insemination.
Development of MK $III^{TM}$ Type Large Arctic LNG Carrier

It is a very challenging work to design large Arctic LNG carrier, since LNG carrier requires high reliability for the structural safety and the environment of Arctic region is known to be very severe. Therefore, special attention should be paid for the verifying the structural safety of LNG career particularly with regard to LNG leakage. In this paper, the safety of the hull structure and cargo containment system of 208K MK $III^{TM}$ type LNG carriers with Arc4 is investigated based on the direct calculation of ice loads as well as wave loads. From the whole investigation, it is clear that the developed vessel - 208K MK $III^{TM}$ type LNG carrier with RMRS Ice class Arc4 - has enough strength and is safe to be operated in Arctic region.

Sebaceous epithelioma and sebaceoma have been used to describe lesions consisting of primarily germinative seboblasts. Others have used the term sebaceoma to describe benign sebaceous proliferations deep in the dermis as opposed to the more superficial sebaceous adenoma. It appears as a yellowish papule, nodule, or plaque on the scalp and face, on which there are abundant sebaceous glands. But, sebaceous neoplsms of the ear and external ear canal are very rare. Only one case of sebaceoma of the earlobe have been reported in Korean literature. We report a case of sebaceoma occurring in 65-year-old woman. The patient presented with a solitary asymptomatic 4.0×2.0cm sized verrucous yellowish colored plaque with ulcer on the left preauricular area for 20years ago. Skin biopsy specimen showed well-circumscribed lesion composed of undifferentiated basaloid cells and mature sebaceous cells in relatively preserved lobulated architectures. On immunohistochemical study, the tumor cells were positive for CK7, Ki-67, D2-40. Because of these histologic findings, sebaceoma was diagnosed. She treated with surgical excision.
