Mean Platelet Volume Could be a Possible Biomarker for Papillary Thyroid Carcinomas

Thyroid cancer is the most prevalent endocrine cancer and is evident in nearly 5% of thyroid nodules. The correlation between mean platelet volume (MPV) and many other cancer types has been investigated previously. However, the correlation between papillary thyroid carcinoma (PTC) and MPV has not yet been studied in detail. The aim of this study was to examine whether MPV would be a useful inflammatory marker to differentiate PTC patients from cases of benign goiter and healthy controls. Preoperative MPV levels in patients with PTC were found to be significantly higher when compared with benign goiter patients and healthy controls ((respectively, 8.05 femtoliter (fl), 7.57 fl, 7.36 fl, p=0.001). After surgical treatment of PTC patients, a significant decrease in MPV levels was seen (8.05 fl versus 7.60 fl, p=0.005). ROC analysis suggested 7.81 as the cut-off value for MPV (AUC=0.729, sensitivity 60%, specificity 80%). In conclusion, maybe changes in MPV levels can be used as an easily available biomarker for monitoring the risk of PTC in patients with thyroid nodules, enabling early diagnosis of PTC.
An Iatrogenically Unmasked Life Threatening Disease: Brugada Syndrome

Brugada syndrome is a life threatening disease that is usually overlooked during emergency service admissions. It is characterized by typical electrocardiography resembling right bundle branch block, static or dynamic ST-segment elevation in leads V 1-3. There is familial tendency in some cases. A majority of patients have a structurally normal heart and are likely to remain asymptomatic, however they may present to emergency departments with syncope and various serious arrhythmias. Therefore it is crucially important for emergency medicine physicians not to omit this potential diagnosis. Herein we report a case with Brugada syndrome which was iatrogenically unmasked after propafenone administration for atrial fibrillation.

Prosthetic valves are being widely used in the treatment of heart valve disease. Prosthetic valve endocarditis (PVE) is one of the most catastrophic complications seen in these patients. In particular, prosthetic valve dehiscence can lead to acute decompensation, pulmonary edema, and cardiogenic shock. Here, we discuss the medical management of late PVE in a patient with a prior history of late and redo early PVE and recurrent dehiscence. According to the present case, we can summarize the learning points as follows. A prior history of infective endocarditis increases the risk of relapse or recurrence, and these patients should be evaluated very cautiously to prevent late complications. Adequate debridement of infected material is of paramount importance to prevent relapse. A history of dehiscence is associated with increased risk of relapse and recurrent dehiscence.

Background: Atherosclerosis and endothelial dysfunction are main pathological event for developing ischemic heart disease. It is damaged that the endothelial produced vasoactive and antithrombotic agents in the process of atherosclerosis and endothelial dysfunction occurs. Microalbuminuria is an indicator for the endothelial dysfunction. In this study, it was investigated the relationship between microalbuminuria and acute coronary syndrome. Methods: This study was consisted up 35 acute coronary syndrome patients without diabetes mellitus. Metabolic parameters and microalbuminuria in 24 hours urinary collection of the patients were analyzed. Above 30 mg/dl daily urinary albumin excretion was described as microalbuminuria (30-300 mg/dl). Microalbuminuric patients were followed up for one year. Patients parameters` mean and standart deviation were calculated. It was statiscically meaningful, p<0,05. Results: 27 male and 8 female, total 35 patients were added in the study group. It was determinated that the mortality and reinfarctus were higher in microalbuminuric patient. 3 reinfarctus and 3 mortality were determinated in normoalbuminuric patients. Fasting triglycerid and homocystein levels were lower in microalbuminuric patients. Conclusions: It was demonstrated that the prognostic signifi cance of microalbuminuria in the acute coronary syndrome patients for one year follow up.

We present a case of oral myiasis in a 15-year-old boy with tuberculosis meningitis. The diagnosis was based on the visual presence of wriggling larvae about 1 cm in size and on the microscopic features of the maggots, especially those relating to stigmatic structures. The larvae were identified as thirdstage larvae of Sarcophaga sp.

Background/Aims: Double-balloon enteroscopy (DBE) allows for the diagnoses and treatment of small bowel tumors (SBTs). Thisstudy aimed to evaluate the utility of DBE for the diagnosis and treatment of SBTs. Methods: Patients diagnosed with SBTs who underwent DBE were included in this study. According to their endoscopic appearances,they were categorized as polyps or masses, and according to their histological characteristics, they were categorized as benign or malignantSBTs. Results: A total of 704 patients were retrospectively analyzed, and 90 (12.8%) were diagnosed with SBTs. According to their endoscopicappearance, 48 (53.3%) had polyps and 42 (46.7%) had masses. Additionally, 53 (58.9%) and 37 (41.1%) patients had malignant andbenign SBTs, respectively, depending on their histological characteristics. Patients diagnosed with polyps were younger than those diagnosedwith masses (p<0.001). Patients diagnosed with benign SBTs were younger than those diagnosed with malignant SBT(p<0.001). Overall, histological diagnosis was determined using DBE in 73 (81.1%) patients. Conclusions: DBE is a useful method for diagnosing SBTs. Additionally, the histological type of the lesion can be determined usingDBE.

Purpose: Since laparoscopic appendectomy was first described, various modifications, such as single port incisionless-intracorporeal conventional equipmentendoscopic surgery (SPICES), have been described for reducing pain and improving cosmetic results. In the retrocecal and retrocolic positions, attachments to the lateral peritoneum and cecum may lead to difficulties during SPICES, which is performed with only one port. Here, we present the effects of variations in the position of the vermiform appendix in treating acute appendicitis with SPICES. Methods: We retrospectively reviewed 52 children who underwent SPICES for acute appendicitis between March 2010 and November 2011 in our institution. One group (group A) consisted of 30 patients (mean age, 10.5 ± 2.5 years) with retrocecal appendix, while the other group (group B) included 22 patients (mean age, 10.9 ± 2.3 years) with the appendix lying free in the peritoneal cavity. Results: There were no significant differences between groups in terms of patient age, gender, success rate of SPICES, mean operating time, mean follow-up period, overall complication rates or mean postoperative hospitalization period. Conclusion: These results suggest that SPICES is a safe and feasible approach even in patients with retrocecal acute appendicitis.
Effects of flexible plate attached to the rear of the cylinder on flow structure

In this experimental study, the flow structure in the wake flow region was investigated with the Particle image velocimetry technique (PIV) by attaching elastic plates at different lengths behind the cylinder. The flow structure occurred at the wake flow region altered depending on the length of the flexible matter. In this experiment, the strips with the lengths of 75, 90, 120, 135 and 180 mm were used to control instabilities. Diameter of the cylinder (D) is 60 mm and the water height (h w ) is 600 mm. Reynolds number was kept constant as 5000 based on cylinder diameter. The images were captured at mid-height of the cylinder (h m ) which is 250 mm. As a result of experimental studies, attached flexible strip suppressed vortex shedding occurred in the behind of the cylinder and it is observed that effect of the length flexible of the strip is pretty much. Maximum level of flow characteristics such as Reynolds stress, fluctuation velocities and turbulent kinetic energy were decreased with flexible splitter plate and shifted through the downstream region.
