Mean Platelet Volume Could be a Possible Biomarker for Papillary Thyroid Carcinomas

Thyroid cancer is the most prevalent endocrine cancer and is evident in nearly 5% of thyroid nodules. The correlation between mean platelet volume (MPV) and many other cancer types has been investigated previously. However, the correlation between papillary thyroid carcinoma (PTC) and MPV has not yet been studied in detail. The aim of this study was to examine whether MPV would be a useful inflammatory marker to differentiate PTC patients from cases of benign goiter and healthy controls. Preoperative MPV levels in patients with PTC were found to be significantly higher when compared with benign goiter patients and healthy controls ((respectively, 8.05 femtoliter (fl), 7.57 fl, 7.36 fl, p=0.001). After surgical treatment of PTC patients, a significant decrease in MPV levels was seen (8.05 fl versus 7.60 fl, p=0.005). ROC analysis suggested 7.81 as the cut-off value for MPV (AUC=0.729, sensitivity 60%, specificity 80%). In conclusion, maybe changes in MPV levels can be used as an easily available biomarker for monitoring the risk of PTC in patients with thyroid nodules, enabling early diagnosis of PTC.

Background/Aims: Double-balloon enteroscopy (DBE) allows for the diagnoses and treatment of small bowel tumors (SBTs). Thisstudy aimed to evaluate the utility of DBE for the diagnosis and treatment of SBTs. Methods: Patients diagnosed with SBTs who underwent DBE were included in this study. According to their endoscopic appearances,they were categorized as polyps or masses, and according to their histological characteristics, they were categorized as benign or malignantSBTs. Results: A total of 704 patients were retrospectively analyzed, and 90 (12.8%) were diagnosed with SBTs. According to their endoscopicappearance, 48 (53.3%) had polyps and 42 (46.7%) had masses. Additionally, 53 (58.9%) and 37 (41.1%) patients had malignant andbenign SBTs, respectively, depending on their histological characteristics. Patients diagnosed with polyps were younger than those diagnosedwith masses (p<0.001). Patients diagnosed with benign SBTs were younger than those diagnosed with malignant SBT(p<0.001). Overall, histological diagnosis was determined using DBE in 73 (81.1%) patients. Conclusions: DBE is a useful method for diagnosing SBTs. Additionally, the histological type of the lesion can be determined usingDBE.

Background: The central obesity and arterial hypertension (AH) are associated with the basic pathogenetic mechanism of metabolic syndrome (MS) - insulin resistance (IR). Leptin - the adipose tissue hormone/cytokine plays an important role in AH pathogenesis in MS. Objectives: to find out the influence of antihypertensive therapy in MS patients with AH on the leptin level. Methods: 65 persons included in the study were divided into two groups: the main group - 40 patients (15 men and 25 women) with AH and MS, mean age 49,0±6,2 years, and the control group - 25 healthy people (11 men and 14 women), mean age - 43,7±7,2 years. The main group was randomized into two subgroups: 20 patients of the first subgroup were treated by lisinopril (5-10 mg per day) and 20 patients of the second subgroup - by bisoprolol (5-10 mg per day) during 12 weeks. The anthropometrical measurements, biochemical blood analyses, fasting serum insulin and leptin levels estimations (by ELISA) were carried out in both groups before and after the therapy. IR index was evaluated by Homeostasis Model Assessment 2 (HOMA2) calculator. Results: The mean baseline leptin levels were reliably higher in both subgroups compared to control group (56,1±35,3 ng/ml and 58,2±25,5 ng/ml, p<0,001). The decrease of its levels was revealed in both groups, but just in lisinopril subgroup these changes were statistically reliable (Δ= -6,67±6,8 ng/ml, p<0,001 and Δ= 1,47±5,94 ng/ml, p>0,05). IR-HOMA2 that was significantly increased in both subgroups at baseline also reliably decreased only in the first subgroup (Δ IR-HOMA2 = -0,73±1,25, p=0,02). Conclusions: The antihypertensive therapy with lisinopril leads to IR-HOMA2 and serum leptin levels decrease that can demonstrate the pathogenetic effects of lisinopril in MS, while bisoprolol doesn`t influence significantly those parameters.
Synthesis, Characterization and Supercapacitive Performances of Yttrium Doped Cobalt Oxide Films

Cobalt oxide and yttrium doped cobalt oxide films are produced with spraying method. The films are coated on glass and indium tin oxide substrates at 450 C. The obtained films have been examined by X-ray diffractometer and scanning electron microscopy devices. In this examination, yttrium doping is seen to cause deterioration in crystal scructure. Moreover, films’ electrochemical features have been studied. The electrochemical measurements of samples are performed with cyclic voltammetry, electrodes supercapasitor curves and impedance spectrometers have been measured. Electrochemical super capasitor performance test has been measured in 6 M KOH for electrolyte and has been studied with the speed of 0.1 V/s. It has been calculated that yttrium doping increases capacity and the maximum capacitance value is measured as 296 F/g

Background: The adipocytokine/hormone leptin produced by adipose tissue plays the key role in pathogenesis of metabolic syndrome (MS) and its main components: insulin resistance (IR), arterial hypertension and dyslipidemia. Objectives: to reveal the correlations between the leptin level and the most important MS components parameters. Methods: 115 persons included in the study were divided into two groups: the main group - 90 patients (36 men and 54 women) with MS, mean age 50,6±7,3 years, and the control group - 25 healthy people (11 men and 14 women), mean age - 37,1±7,2 years. The anthropometrical measurements, biochemical blood analyses, heart and liver ultrasonography and fasting serum insulin and leptin levels estimation (by ELISA) were carried out in both groups. IR index was evaluated by Homeostasis Model Assessment 2 (HOMA2) calculator (version 2.2.). Results: The mean leptin level in patient with MS was 56,9±27,5 that was signifi cantly higher than in control group (p<0,001). Reliable positive correlations between the BMI, the waist circumference (WC) and leptin level, and negative correlation of a hormone rate with waist-to-hip ratio (WHR) were revealed in the study (R (Spirmen correlation coeffi cient) = 0,36, p<0,001; R=0,29, p=0,006 and R = - 0,6, p<0,001 for BMI, WC and WHR, respectively). Reliable positive correlations between leptin level and systolic and diastolic BP were also revealed (R=0,5, p<0,001 and R=0,34, p=0,001 for SBP and DBP, respectively). More over, leptin level showed strong reliable positive correlation with fasting serum insulin level, IR-NOMA2 and cholesterol level (R=0,59, p<0,001; R=0,65, p<0,001; R=0,23, p=0,04 for insulin, IR-NOMA2 and cholesterol, respectively,). Conclusions: There was revealed the raised serum leptin level in patients with MS compared to healthy people. Leptin level signifi cantly correlated with the parameters that characterize the most important components of MS.
Coexistence of Darier's Disease and Acrokeratosis Verruciformis of Hopf

Darier's disease and acrokeratosis verruciformis are inherited with the same pattern and they share similar clinical properties. We present a patient with both Darier's disease and acrokeratosis verruciformis. The patient had had brownish and skin-colored papules, initially on her face and neck, since the age of 25. Similar lesions had gradually spread to various parts of her body and lastly the dorsal aspect of her feet. On dermatological examination, brownish and skin-colored, 2 to 5mm, keratotic papules were observed on her face, neck, both axillae, inframammary regions, and dorsa of the hands and feet. The nail examination revealed “V” shaped notches and longitudinal ridges on the fingers of both hands. The biopsy taken from the neck showed typical findings for Darier's disease. The biopsy from the dorsa of the left foot revealed the distinctive structures of acrokeratosis verruciformis. In this article the clinical and histopathological findings of these diseases are reviewed based on our patient.

Background: Atherosclerosis and endothelial dysfunction are main pathological event for developing ischemic heart disease. It is damaged that the endothelial produced vasoactive and antithrombotic agents in the process of atherosclerosis and endothelial dysfunction occurs. Microalbuminuria is an indicator for the endothelial dysfunction. In this study, it was investigated the relationship between microalbuminuria and acute coronary syndrome. Methods: This study was consisted up 35 acute coronary syndrome patients without diabetes mellitus. Metabolic parameters and microalbuminuria in 24 hours urinary collection of the patients were analyzed. Above 30 mg/dl daily urinary albumin excretion was described as microalbuminuria (30-300 mg/dl). Microalbuminuric patients were followed up for one year. Patients parameters` mean and standart deviation were calculated. It was statiscically meaningful, p<0,05. Results: 27 male and 8 female, total 35 patients were added in the study group. It was determinated that the mortality and reinfarctus were higher in microalbuminuric patient. 3 reinfarctus and 3 mortality were determinated in normoalbuminuric patients. Fasting triglycerid and homocystein levels were lower in microalbuminuric patients. Conclusions: It was demonstrated that the prognostic signifi cance of microalbuminuria in the acute coronary syndrome patients for one year follow up.

We present a case of oral myiasis in a 15-year-old boy with tuberculosis meningitis. The diagnosis was based on the visual presence of wriggling larvae about 1 cm in size and on the microscopic features of the maggots, especially those relating to stigmatic structures. The larvae were identified as thirdstage larvae of Sarcophaga sp.
An Iatrogenically Unmasked Life Threatening Disease: Brugada Syndrome

Brugada syndrome is a life threatening disease that is usually overlooked during emergency service admissions. It is characterized by typical electrocardiography resembling right bundle branch block, static or dynamic ST-segment elevation in leads V 1-3. There is familial tendency in some cases. A majority of patients have a structurally normal heart and are likely to remain asymptomatic, however they may present to emergency departments with syncope and various serious arrhythmias. Therefore it is crucially important for emergency medicine physicians not to omit this potential diagnosis. Herein we report a case with Brugada syndrome which was iatrogenically unmasked after propafenone administration for atrial fibrillation.
