        A Modified, Direct Neck Lift Technique: The Cervical Wave-Plasty

        Background Major problems with cervicoplasty by direct skin excision include the subjective nature of skin markings preoperatively and the confusing array of procedures offered. This technique incorporates curved incisions, resulting in a wave-like scar, which is why the procedure is called a "wave-plasty". Methods This prospective study includes 37 patients who underwent wave-plasty procedures from 2004 to 2015. Skin pinching technique was used to mark the anterior neck preoperatively in a reproducible fashion. Intra-operatively, redundant skin was excised, along with excess fat when necessary, and closed to form a wave-shaped scar. Patients were asked to follow up at 1 week, 6 weeks, and 6 months after surgery. Results The mean operation time was 70.8 minutes. The majority (81.3%) was satisfied with their progress. On a scale of 1 to 10 (1 being the worst, and 10 being the best), the scars were objectively graded on average 5.5 when viewed from the front and 7.3 when seen from the side 6 months after surgery. Complications consisted of one partial wound dehiscence (2.3%), one incidence of hypertrophic scarring (2.3%), and two cases of under-resection requiring revision (5.4%). Conclusions In select patients, surgical rejuvenation of the neck may be obtained through wave-like incisions to remove redundant cervical skin when other options are not available. The technique is reproducible, easily teachable and carries low morbidity and high patient satisfaction in carefully chosen patients.

        Structural damage detection based on MAC flexibility and frequency using moth-flame algorithm

        Vibration-based structural damage detection through optimization algorithms and minimization of objective function has recently become an interesting research topic. Application of various objective functions as well as optimization algorithms may affect damage diagnosis quality. This paper proposes a new damage identification method using Moth-Flame Optimization (MFO). MFO is a nature-inspired algorithm based on moth’s ability to navigate in dark. Objective function consists of a term with modal assurance criterion flexibility and natural frequency. To show the performance of the said method, two numerical examples including truss and shear frame have been studied. Furthermore, Los Alamos National Laboratory test structure was used for validation purposes. Finite element model for both experimental and numerical examples was created by MATLAB software to extract modal properties of the structure. Mode shapes and natural frequencies were contaminated with noise in above mentioned numerical examples. In the meantime, one of the classical optimization algorithms called particle swarm optimization was compared with MFO. In short, results obtained from numerical and experimental examples showed that the presented method is efficient in damage identification.

      • Effects of Breast Self-Examination Consultation Based on the Health Belief Model on Knowledge and Performance of Iranian Women Aged Over 40 Years

        Background: Breast cancer is the most prevalent malignancy in women worldwide; lack of awareness of symptoms and delay on diagnosis of breast cancer are the main causes of mortality among women. This study was conducted with the purpose of assessing the effect of educational consulting for breast self-examination (BSE) based on the health belief model (HBM) on the knowledge and performance of women over 40 years attending health care centers in Hamadan, Iran. Materials and Methods: In this quasi-experimental study, eligible women admitted to health centers in Hamadan city in 2015 were randomly assigned to intervention and control groups (n=75 in each group). The intervention group received 4 weekly sessions of breast cancer screening consulting based on the HBM. Control group received only routine care. Knowledge, HBM constructs, and BSE practice were compared between the groups before, immediately after and three months after the consultation. Results: Before the intervention, no significant differences were observed in knowledge, health belief and practice between two groups. However, after the intervention a significant difference was observed between two groups in mean scores of perceived benefits, perceived barriers, self-efficacy and the health motivations (p <0.05). Significant differences were also observed in terms of knowledge and BSE practice (p <0.01). Conclusions: The results indicate the importance of consultation on knowledge and beliefs to improve BSE performance and prevention of breast cancer in Iranian women.

        Analyzing consolidation data to predict smear zone characteristics induced by vertical drain installation for soft soil improvement

        In this paper, the effects of variability of smear zone characteristics induced by installation of prefabricated vertical drains on the preloading design are investigated employing analytical and numerical approaches. Conventional radial consolidation theory has been adopted to conduct analytical parametric studies considering variations of smear zone permeability and extent. FLAC 2D finite difference software has been employed to conduct the numerical simulations. The finite difference analyses have been verified using three case studies including two embankments and a large-scale laboratory consolidometer with a central geosynthetic vertical drain. A comprehensive numerical parametric study is conducted to investigate the influence of smear zone permeability and extent on the model predictions. Furthermore, the construction of the trial embankment is recommended as a reliable solution to estimate accurate smear zone properties and minimise the post construction settlement. A back-calculation procedure is employed to determine the minimum required waiting time after construction of the trial embankment to predict the smear zone characteristics precisely. Results of this study indicate that the accurate smear zone permeability and extent can be back-calculated when 30% degree of consolidation is obtained after construction of the trial embankment.

        Institutional Quality and Economic Growth: A Dynamic Panel Data Analysis of MICs and HICs for 2000–2020

        We investigate the dynamic impact of institutions on economic growth using a panel dataset of 77 countries, divided into MICs and HICs for the period 2000-2020. We critically examine the available institutional indices and construct three weighted indices from 20 indicators closely related to the meaning of the term ‘institutions’ as the ‘rules of the game’ defined by Douglas North. Next, we use the Generalized Method of Moments (GMM) to show that institutions significantly influence economic growth through investment and trade more than the total factor productivity channel. While the quality of the legal system and property rights and regulatory quality all positively and significantly influence output per capita, output gains from each unit of improvement in the quality of legal systems and protection of private property rights are comparatively higher than gains from a unit of improvement in the regulatory environment. An average MIC gains relatively more from improving its quality of legal system and property rights, whereas an average HIC benefits relatively more from each unit of improvement in its regulatory environment. The results from the Granger non-causality test demonstrate and unidirectional causality from institutions to economic growth in MICs but no significant causal relationship between institutions and economic growth in HICs

        Characteristic Behavior of Aluminum-Stainless Laminated Sheet in Redrawing

        Using a rigid-plastic finite element program deep drawing, direct and reverse redrawing of two layer aluminum-austenitic stainless steel (AL-ASS) laminated sheets have been simulated. The results of simulations as the variation of drawing ratios with thickness ratio and setting condition are presented. They show that to access the highest drawing ratios in direct and reverse redrawing, thickness ratio should be about 1/3 (one layer aluminum and three layer stainless steel) and the setting conditions are opposite to each other. In another word, while in direct redrawing contact of austenitic stainless steel with punch leads to the maximum drawing ratio, in reverse redrawing, aluminum should contact the punch in order to access highest drawing ratio. An explanation for this finding is offered through the thickness strain distribution.

        Digital implant placement accuracy: a clinical study on a fully-guided flapless single-unit immediate-loading protocol

        Aims The primary aim of the present study was to measure the discrepancy between the virtual and the actual position of the single-unit implants placed via a digitally-designed fully-guided surgical template using a flapless surgical technique. Prefabricated provisional restorations and periodontal factors were evaluated after the immediate loading of implants and 3 months after the surgery, respectively. Materials and methods Fourteen implants in nine patients were virtually planned after importing intraoral scans and cone-beam computed tomography (CBCT) records into 3D planning software. Accordingly, fully-guided surgical templates, customized abutments, and provisional restorations were designed and fabricated. The implant position after the surgery was compared with its virtual counterpart in terms of angular and apical linear deviations. Implants were immediately loaded after the surgery, and the occlusal level of the delivered provisional restorations was compared with their designed positions. Early implant failure, bleeding on probing, and peri-implant pockets were documented on the 3-month follow-up. Results A mean angular deviation of 5.07 ± 2.06° and a mean apical linear deviation of 1.74 ± 0.63 mm resulted. Two out of 14 implants failed within the first 3 months of the surgery, and the occlusal level difference was calculated for nine prefabricated provisional restorations. Conclusions DIONAVI protocol has been evaluated regarding its accuracy, and an estimation of the expected deviation is presented to the clinicians using this protocol. However, before widespread use, immediate-loading protocols and provisional restorations must be studied further. Trial registration IRCT, IRCT20211208053334N1. Registered 6 August 2022.
