Factor Structure and Validation of the Revised Suicide Crisis Inventory in a Korean Population

Objective Because of the exceptionally high suicide rates in South Korea, new assessment methods are needed to improve suicide prevention. The current study aims to validate the revised Suicide Crisis Inventory-2 (SCI-2), a self-report measure that assesses a cognitiveaffective pre-suicidal state in a Korean sample.Methods With data from 1,061 community adults in South Korea, confirmatory factor analyses were first conducted to test the proposed one-factor and five-factor structures of the SCI-2. Also, an exploratory factor analysis (EFA) was performed to examine possible alternative factor structure of the inventory.Results The one-factor model of the SCI-2 resulted in good model fit and similarly, the five-factor model also exhibited strong fit. Comparing the two models, the five-factor was evaluated as the superior model fit. An alternative 4-factor model derived from EFA exhibited a comparable model fit. The Korean version of the SCI-2 had high internal consistency and strong concurrent validity in relation to symptoms of suicidal ideation, depression, and anxiety.Conclusion The SCI-2 is an appropriate and a valid tool for measuring one’s proximity to imminent suicide risk. However, the exact factor structure of the SCI-2 may be culture-sensitive and warrants further study.

도덕교육과 사고력교육과의 관련성은 무엇인가? 흔히들 사고력교육은 도덕교육과 관련없는 것으로서 도덕교육외적 영역으로 치부하는 경향이 있다. 도덕적 판단력교육은 도덕적 사고력 교육에 의존하며 도덕적 사고력교육은 사고력교육에 의존한다. 특히 배려적 사고기술, 창의적 사고기술등 사고력이 기술의 수준에 다다를 때 습관화 즉 덕의 교육 인격교육과 밀접하게 연결될 수 있다. 도덕적 사고력교육 또는 사고력교육이 도덕판단 교육과 어떻게 관련되고 있는지에 대해서는 최근 괄목할만한 연구들이 있어왔다. 도덕적 상상력의 교육이 대표적인 최근의 연구라면 논리적 상호성과 공정성의 개념과의 관계에 대한 피아제의 연구는 고전적인 것에 속한다. 그럼에도 불구하고 개별적인 연구들이 도덕판단의 다측면적 성격을 그려내지 못한다는 비판을 받아왔다. 저자들은 비판적 사고기술, 창의적 사고기술, 배려적 사고기술이 도덕판단에 다같이 작용하며 이러한 새로운 개념이 종래의 도덕원리에 의한 판단이라는 도덕판단 개념에 에 대체되어야한다고 본다. 본 연구는 이와 같은 입장의 연장선상에서 구체적으로 어떻게 사고력교육이 이루어질 수 있는가를 모색한 것이다. 특히 어린이 철학 프로그램의 하나인 노마의 발견(Harry Stottlemeyer`s Discovery)을 중심으로 살펴보았다이를 통해 어떻게 탐구가 일어나며 탐구에 필요한 사고기술과 사고양식은 무엇인가를 규명하고자 하였다. 이 논문은 도덕판단 교육에 있어 종래의 도덕추론의 한계를 지적한다. 도덕문제 해결에 있어서 논리의 형식적 적용을 강조하는 도덕추론과 구체적 맥락에서의 적합한 판단을 위해서 다국면적 기준들을 고려하는 윤리탐구의 방식은 엄연한 차이가 있다. 새로운 도덕교육을 위해서는 도덕추론을 포함하는 윤리탐구적 접근이 적용되어야 할 것이다.

The common γ-chain (γc) plays a central role in signaling by IL-2 and other γc-dependent cytokines. Here we report that activated T cells produce an alternatively spliced form of γc mRNA that results in protein expression and secretion of the γc extracellular domain. The soluble form of γc (sγc) is present in serum and directly binds to IL-2Rβ and IL-7Rα proteins on T cells to inhibit cytokine signaling and promote inflammation. sγc suppressed IL-7 signaling to impair naive T cell survival during homeostasis and exacerbated Th17-cell-mediated inflammation by inhibiting IL-2 signaling upon T cell activation. Reciprocally, the severity of Th17-cell-mediated inflammatory diseases was markedly diminished in mice lacking sγc. Thus, sγc expression is a naturally occurring immunomodulator that regulates γc cytokine signaling and controls T cell activation and differentiation.

Full-thickness burns almost always require skin grafting because the severity prevents the natural regenerative process. Here, we illustrate the potential to promote natural skin regeneration utilizing growth factors and regenerative proteins in purified amniotic fluid (PAF). A 24-year-old female suffered from full-thickness burns to her legs resulting from a rogue firework. This initiated a treatment plan involving extensive rehabilitation and skin grafting. Instead, the patient was treated with PAF (4 mL) administered topically over 2 days. Forty-five minutes following application, her pain decreased from 10/10 to 2/10. After two treatments her overall recovery time decreased by 75%–leaving her physicians surprised at the obviated need for skin grafting. Topical PAF relieved pain within minutes, making it easier to clean the affected area, enhancing sleep, and allowing more range of motion and autonomy. The efficiency of PAF to reduce pain allowed the patient to discontinue the use of prescribed hydrocodone, an opioid that is fueling an epidemic of abuse and addiction. Accelerated recovery time allowed the patient to return to work sooner, having broader economic implications on medical costs and personal time off. Finally, PAF enhanced the body’s own capacity to heal itself, avoiding painful and costly grafting and decreasing additional donor site scarring.

Lymph nodes as barriers to T‐cell rejuvenation in aging mice and nonhuman primates

<P><B>Abstract</B></P><P>In youth, thymic involution curtails production of new naïve T cells, placing the onus of T‐cell maintenance upon secondary lymphoid organs (SLO). This peripheral maintenance preserves the size of the T‐cell pool for much of the lifespan, but wanes in the last third of life, leading to a dearth of naïve T cells in blood and SLO, and contributing to suboptimal immune defense. Both keratinocyte growth factor (KGF) and sex steroid ablation (SSA) have been shown to transiently increase the size and cellularity of the old thymus. It is less clear whether this increase can improve protection of old animals from infectious challenge. Here, we directly measured the extent to which thymic rejuvenation benefits the peripheral T‐cell compartment of old mice and nonhuman primates. Following treatment of old animals with either KGF or SSA, we observed robust rejuvenation of thymic size and cellularity, and, in a reporter mouse model, an increase in recent thymic emigrants (RTE) in the blood. However, few RTE were found in the spleen and even fewer in the lymph nodes, and SSA‐treated mice showed no improvement in immune defense against West Nile virus. In parallel, we found increased disorganization and fibrosis in old LN of both mice and nonhuman primates. These results suggest that SLO defects with aging can negate the effects of successful thymic rejuvenation in immune defense.</P>

<P>The zinc-finger protein Ikaros is a key player in T-cell development and a potent tumor suppressor in thymocytes. To understand the molecular basis of its function, we disabled Ikaros activity in vivo using a dominant negative Ikaros transgene (DN-IkTg). In DN-IkTg mice, T-cell development was severely suppressed, and positively selected thymocytes clonally expanded, resulting in a small thymus with a heavily skewed T-cell receptor (TCR) repertoire. Notably, DN-IkTg induced vigorous proliferation concomitant to downregulation of antiapoptotic factor expression such as Bcl2. Ikaros activity was required during positive selection, and specifically at the CD4<SUP>+</SUP>CD8<SUP>lo</SUP> intermediate stage of thymocyte differentiation, where it prevented persistent TCR signals from inducing aberrant proliferation and expansion. In particular, DN-IkTg induced the accumulation of CD4 single-positive (SP) thymocytes with a developmentally transitional phenotype, and it imposed a developmental arrest accompanied by massive apoptosis. Thus, we identified an in vivo requirement for Ikaros function, which is to suppress the proliferative potential of persistent TCR signals and to promote the survival and differentiation of positively selected thymocytes.</P>
