        Comparison of Reading Speed after Bilateral Bifocal and Trifocal Intraocular Lens Implantation

        Purpose: : To evaluate and compare visual acuity and reading speed for Korean language between a diffractive bifocal and trifocal intraocular lens (IOL) of the same material and haptic design. Methods: We reviewed the medical records of the patients who had undergone bilateral cataract surgery with bifocal IOLs (AT LISA 801) on the both eyes (bifocal group) and trifocal IOLs (AT LISA tri 839 MP, trifocal group). The main outcome measures were the uncorrected distance, intermediate, and near visual acuity (uncorrected distance visual acuity [UCDVA], uncorrected intermediate visual acuity [UCIVA], and uncorrected near visual acuity [UCNVA]) and corrected distance, near, and distance-corrected intermediate visual acuity (corrected distance visual acuity [CDVA], corrected near visual acuity [CNVA], and distance-corrected intermediate visual acuity [DCIVA]) at last postoperative follow-up month. Reading speeds for Korean language were measured to check near visual function. Results: Fourteen eyes (7 patients) were included in the bifocal group and 32 eyes of 16 patients in the trifocal group. There were no statistical differences between the two groups with respect to UCDVA, UCNVA, CDVA, and CNVA. However, UCIVA (0.35 vs. 0.22 logarithm of the minimum angle of resolution [logMAR], p < 0.01) and DCIVA (0.34 vs. 0.20 logMAR, p < 0.01) were significantly better in the trifocal group than in the bifocal group. The mean reading speed for logMAR 0.5 optotype (point 10) was 86.50 words per minute (wpm) in the bifocal group and 81.48 wpm in the trifocal group without a significant difference (p = 0.70). Conclusions: Trifocal IOLs provided the same level of distance and near visual acuity and reading speed as that of bifocal IOLs with better intermediate visual acuity

        Korea hypertension fact sheet 2022: analysis of nationwide population-based data with a special focus on hypertension in the elderly

        Background The Korean Society of Hypertension has published the Korea Hypertension Fact Sheet 2022 to provide an overview of the magnitude and management status of hypertension and their recent trends. Methods The Fact Sheets were based on the analyses of Korean adults aged 20 years or older of the 1998–2020 Korea National Health and Nutrition Examination Survey and the 2002–2020 National Health Insurance Big Data. Results As of 2020, 29.4% of the adult population aged 20 or older in Korea, about 12.6 million people, have high blood pressure, of which 5.0 million (40%) are 65 years of age or older and 1.2 million (10%) are 80 years of age or older. Among those with hypertension, the awareness rate is 69%, the treatment rate is 65%, and the control rate is 47%. The number of people diagnosed with hypertension increased from 3.0 million in 2002 to 10.5 million in 2020. During the same period, the number of people using antihypertensive medication increased from 2.5 million to 9.9 million, and the number of people adherent to treatment increased from 0.6 million to 7.4 million. Among those treated for hypertension in 2020, 74% used angiotensin blockers, 61% used calcium channel blockers, 24% used diuretics, and 15% used beta blockers. Combination therapy with at least two classes of antihypertensive medication consisted of 60% of all antihypertensive prescriptions. The number of people with hypertension aged 65 or older is increasing very rapidly compared to those aged 20–64. Awareness and treatment rates of hypertension improved rapidly, especially in those aged 65 or older, but the rate of improvement slowed since 2012. Conclusions In Korea, the level of hypertension management is improving, but the absolute number of people with hypertension, especially elderly hypertension, is increasing due to the rapid aging of the population. It is necessary to develop more efficient and target-specific policies to control blood pressure and prevent cardiovascular disease.

        Transforming Growth Factor-β : Biology and Clinical Relevance

        Transforming growth factor-β is a pleiotropic growth factor that has enthralled many investigators for approximately two decades. In addition to many reports that have clarified the basic mechanism of transforming growth factor-β signal transduction, numerous laboratories have published on the clinical implication/application of transforming growth factor-β To name a few, dysregulation of transforming growth factor-β signaling plays a role in carcinogenesis, autoimmunity, angiogenesis, and wound healing. In this report, we will review these clinical implications of transforming growth factor-β.

      • 무선 Ad-hoc 네트워크에서 RTS/CTS의 사용여부에 관한 연구

        무선 Ad-hoc 네트워크상에서 hidden node problem을 해결하기 위해 도입된 RTS/CTS의 사용여부는 네트워크의 처리량(throughput)에 영향을 미친다. 이는 RTS/CTS가 hidden node problem을 해결해주지만 거기에서 발생하는 overhead 때문에 처리량이 떨어질 수도 있기 때문이다. 기존 관련 연구에서는 RTS/CTS의 사용여부를 패킷의 크기에 따라 달리 해야 한다고 말하고 있다. 그리고 이를 구현하기 위해서 RTSThreshold라는 값을 사용하고 있다. 만약 이 값보다 패킷의 크기가 작으면 RTS/CTS는 사용 되지 않고, 만약 이 값보다 패킷의 크기가 크면 RTS/CTS가 사용된다. 이렇게 RTS/CTS의 사용여부를 결정해주는 RTSThreshold값을 결정하기 위해선 네트워크의 처리량에 영향을 줄 수 있는 모든 인자들을 고려해 보아야 할 것이다. 조사한 바에 의하면 이미 관련 연구에서 physical preamble의 크기, 노드의 수, 그리고 data rate와 RTSThreshold와의 관계를 연구 했었지만, 그밖에 네트워크의 처리량에 영향을 줄 수 있는 carrier sense range나 네트워크의 크기와의 관계는 연구가 되지 않았었다. 따라서 본 논문에서는 이 두가지 인자와 RTSThreshold와의 관계를 살펴보고자 모의실험을 수행하였고 그 결과를 분석하였다. 두 인자가 어느 정도 RTSThreshold를 결정하는데 영향을 줄 것이라 예상했었지만, 실험 결과를 분석한 결과 carrier sense range나 네트워크의 크기는 RTSThreshold를 결정하는데 크게 영향을 주지 않는 것으로 나타났다.

        기존 ITS의 한계를 극복하고 최근 C-ITS 사업이 확장됨에 따라 사고를 방지하는 안전 운전과 사고를 빠르게 대응하여 2차 사고를 막을 수 있는 교통체계가 설립되고 있다. 단방향으로 정보를 수집하는 ITS와 달리 C-ITS는 차량과 차량, 차량과 인프라 간 양방향으로 정보를 공유할 수 있어 교통안전 관리에 더욱 효과적이다. 성공적인 C-ITS의 상용화를 위하여 국내 실정에 맞는 V2X 기술의 통신 규격의 결정이 필요하다. 본 논문에서 국내 V2X 통신 방식인 WAVE와 C-V2X의 차이점을 기술하고 국내 C-ITS 통신 규격 전망에 대해 소개한다.

      • Diagnostic challenge for the rare lysosomal storage disease: Late infantile GM1 gangliosidosis

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>GM1 gangliosidosis is a rare lysosomal storage disorder caused by GLB1 mutations. Because of its extreme rarity and symptoms that overlap with other neurodegenerative diseases, its diagnosis is sometimes challenging, especially in the late infantile form with less severe phenotype. We aim to expand the clinical and genetic spectrum of late infantile GM1 gangliosidosis.</P> <P><B>Methods</B></P> <P>We confirmed a diagnosis of GM1 gangliosidosis based on <I>GLB1</I> mutations and/or the deficiency of β-galactosidase activity. We identified the first two cases by whole-exome sequencing, and then the other six cases by direct sequencing of <I>GLB1</I> with enzyme analysis.</P> <P><B>Results</B></P> <P>All eight patients presented with developmental delay or regression during late infancy and later developed epilepsy, mostly intractable generalized tonic seizures. No clinical signs of storage disorders were noted except for skeletal abnormalities. Interestingly, we found aspartate transaminase (AST) elevations alone with normal alanine transaminase (ALT) levels in all patients. The recurrent mutation, p.D448V in GLB1, accounted for 50.0% of total alleles in our cohort.</P> <P><B>Conclusions</B></P> <P>With a high index of clinical suspicion, skeletal survey and AST level would be important for early diagnosis of GM1 gangliosidosis. In addition, we would highlight the clinical usefulness of whole-exome sequencing in the diagnosis of non-classical presentation of ultra-rare neurodegenerative disease in children.</P>

      • Discretization Approach for the Very Light Aircraft Configurations Validation Using Computational Fluid Dynamics

        In this paper the development of unstructured grid discretization approach is presented. The objective of this study is to better establish the effects of mesh generation techniques and grid convergence on velocity fields and particle deposition patterns to determine the optimal aerodynamic characteristics. In order to achieve this objective, cell-centred and vertex-centred approaches are employed and compared with experimental data. The cell-centred approach result indicates that the aerodynamic characteristic was captured faster in the stall condition, although with a different trend after the stall condition compared with the experimental data. The vertex-centred approach result indicates that the aerodynamic characteristic trend captured is comparable to the experimental data. From the model an advance study can also be achieved for this vertex-centred discretization approach with discrete vertex variables in several important areas.

        Grid Discretization Study for the Efficient Aerodynamic Analysis of the Very Light Aircraft (VLA) Configuration

        In this research the development of unstructured grid discretization solution techniques is presented. The purpose is to describe such a conservative discretization scheme applied for experimental validation work. The objective of this paper is to better establish the effects of mesh generation techniques on velocity fields and particle deposition patterns to determine the optimal aerodynamic characteristics. In order to achieve the objective, the mesh surface discretization approaches used the VLA prototype manufacturing tolerance zone of the outer surface. There were 3 schemes for this discretization study implementation. They are solver validation, grid convergence study and surface tolerance study. A solver validation work was implemented for the simple 2D and 3D model to get the optimum solver for the VLA model. A grid convergence study was also conducted with a different growth factor and cell spacing, the amount of mesh can be controlled. With several amount of mesh we can get the converged amount of mesh compared to experimental data. The density around surface model can be calculated by controlling the number of element in every important and sensitive surface area of the model. The solver validation work result provided the optimum solver to employ in the VLA model analysis calculation. The convergence study approach result indicated that the aerodynamic trend characteristic was captured smooth enough compared with the experimental data. During the surface tolerance scheme, it could catch the aerodynamics data of the experiment data. The discretization studies made the validation work more efficient way to achieve the purpose of this paper.
