        Haemorrhagic Transformation after Ischaemic Stroke in Patients Taking Non-vitamin K Antagonist Oral Anticoagulants

        Background and Purpose To evaluate the frequency and outcome of haemorrhagic transformation (HT) after ischaemic stroke in patients treated with non-vitamin K antagonist oral anticoagulants (NOACs). Methods Patients with stroke on treatment with a NOAC were prospectively enrolled in this multicentre observational study between February 2012 and 2015. Brain imaging at admission and follow-up imaging until day 7 were reviewed for HT. Functional outcome was assessed by the modified Rankin scale (mRS) before the index event, at discharge, and at 3-months. Results 231 patients without recanalisation therapy (no-RT), and 32 patients with RT were eligible for analysis. Any HT was present at admission in 9/231 no-RT patients (3.9%, 95% CI 2.0 to 7.3) and in none of the patients with RT. In patients with follow-up imaging (no-RT, n=129, and RT, n=32), HT was present in 14.0% (no-RT; 95% CI, 8.9 to 21.1), and 40.6% (RT, 95% CI, 25.5 to 57.8), respectively. After adjustment for stroke severity, this difference between the no-RT and RT groups became non-significant. Symptomatic ICH was observed in 1 patient per group. HT was not associated with unfavourable outcome (mRS 3-6) at 3-months in multivariable analysis. Resumption of OAC after stroke was delayed in patients with HT compared to those without (15 d [IQR, 5–26] vs. 1 d [0–4], P<0.001). Conclusions The frequency and severity of HT after stroke on NOAC appears similar to previous reports for vitamin K antagonists and no anticoagulation. Whether asymptomatic HT should delay resumption of preventive anticoagulation requires further investigation.

        A Taiwan Strait Conflict Simulation Model

        산업 디자인 교육 커리큘럼의 Upcycling 디자인 통합의 장점과 한계에 대한 연구

        이 연구는 한국의 산업 디자인 학부생들을 대상으로, 폐기물의 창의적 재사용과 관련된 디자인 연습이 폐기물의 복합적인 문제와 지속 가능한 디자인에 대해 소개하는데 실제적으로 효과적인지 평가하는 것을 목표로 주어진, 다운스트림 업사이클 과제의 결과를 분석한다. 이 분석은 폐기물을 기능성 제품으로 변형시키는 능력 뿐만이 아니라 생성된 제품이 실제로 사용 가능하고 계속적으로 기능적인지의 여부 및 생물학적, 기술적 영양소의 재순환에 효과적으로 참여하는지 여부를 함께 평가한다. 이 연구 결과에 따르면, 비록 대다수 학생들이 작업 결과에 어느 정도의 만족을 표했더라도, 결과물의 디자인 분석 결과 제품의 성능이 좋지 않거나, 경우에 따라서는 환경에 부정적인 영향을 미치거나, 재료의 재순환도 개선되지 않았다. 결과적으로 이러한 디자인 연습이 지속가능성에 대한 학생들의 행동을 뚜렷이 개선시키지 못하고, 오히려 낭비적인 행동을 용서받을 변명의 정도에 그치거나, 현재의 택하고-만들고-버리는 쓰레기 처리 패러다임에 효과적으로 도전하지 않는다는 것을 보여준다. 이러한 활동의 다소 실망스러운 결과는 그러한 디자인 연습과제들이, 그들 스스로 효과적이지는 않더라도, 오늘날 폐기물 문제가 가진 복합성과 어려움을 이해하는 것에 오히려 사용될 여지가 있으며, 요람에서 요람으로 또는 순환 경제와 같은 폐쇄 루프형 시스템을 효과적으로 도입해 지속 불가능한 현재의 소비, 생산 방식을 변경해야 한다는 시급한 필요성을 강조한다. This study analyses the result of a downstream upcycling exercise by Korean undergraduate industrial design students. It aims at assessing whether design exercises involving the creative reuse of waste, are effective at introducing students to the issues of waste and Sustainable design. The analysis focuses not only on the ability to transform waste into functional products but also assess whether the products created are viable and functional and whether they effectively participate in the recirculation of biological and technical nutrients. The result of the study indicates that although the majority of the students expressed satisfaction with the result of their work, the analysis revealed the products to have poor performances, in some case negative environmental impact and didn't improve recirculation of the materials. The results also indicate that the exercise did not improve student’s behavior towards sustainability, may excuse wasteful behaviors and does not effectively challenge the current take-make-waste paradigm. The disappointing results of the activity indicate that such exercise, while not effective on their own, should rather be used to highlight the complexity of the problem of waste, the urgent need to change our unsustainable current modes of consumption and effectively introduce holistic closed-loop systems such as the Cradle to Cradle or the Circular Economy.

      • Photometric redshifts for the Kilo-Degree Survey : Machine-learning analysis with artificial neural networks

        <P>We present a machine-learning photometric redshift (ML photo-<I>z</I>) analysis of the Kilo-Degree Survey Data Release 3 (KiDS DR3), using two neural-network based techniques: ANNz2 and MLPQNA. Despite limited coverage of spectroscopic training sets, these ML codes provide photo-<I>z</I>s of quality comparable to, if not better than, those from the Bayesian Photometric Redshift (BPZ) code, at least up to <I>z</I>phot ≲ 0.9 and <I>r</I> ≲ 23.5. At the bright end of <I>r</I> ≲ 20, where very complete spectroscopic data overlapping with KiDS are available, the performance of the ML photo-<I>z</I>s clearly surpasses that of BPZ, currently the primary photo-<I>z</I> method for KiDS. Using the Galaxy And Mass Assembly (GAMA) spectroscopic survey as calibration, we furthermore study how photo-<I>z</I>s improve for bright sources when photometric parameters additional to magnitudes are included in the photo-<I>z</I> derivation, as well as when VIKING and WISE infrared (IR) bands are added. While the fiducial four-band <I>ugri</I> setup gives a photo-<I>z</I> bias 〈<I>δz</I>/(1 + <I>z</I>)〉 = −2 × 10<SUP>−4</SUP> and scatter <I>σδz/(1+z)</I> < 0.022 at mean 〈<I>z</I>〉 = 0.23, combining magnitudes, colours, and galaxy sizes reduces the scatter by ~7% and the bias by an order of magnitude. Once the <I>ugri</I> and IR magnitudes are joined into 12-band photometry spanning up to 12 <I>μ</I>m, the scatter decreases by more than 10% over the fiducial case. Finally, using the 12 bands together with optical colours and linear sizes gives 〈<I>δz</I>/(1 + <I>z</I>)〉 < 4 × 10<SUP>−5</SUP> and <I>σ</I><I>δz</I>/(1+<I>z</I>) < 0.019. This paper also serves as a reference for two public photo-<I>z</I> catalogues accompanying KiDS DR3, both obtained using the ANNz2 code. The first one, of general purpose, includes all the 39 million KiDS sources with four-band <I>ugri</I> measurements in DR3. The second dataset, optimised for low-redshift studies such as galaxy-galaxy lensing, is limited to <I>r</I> ≲ 20, and provides photo-<I>z</I>s of much better quality than in the full-depth case thanks to incorporating optical magnitudes, colours, and sizes in the GAMA-calibrated photo-<I>z</I> derivation.</P>

        Quantification of Aging-Related Decreases in Sensory Innervation of the Bladder Trigone in Rats

        Purpose: The prevalence of lower urinary tract symptoms (LUTS), characterized by problems regarding storage and/or voiding of urine, is known to significantly increase with age. Effective communication between the lower urinary tract and the central nervous system (CNS) is essential for the optimal function of this system, and heavily relies on the efficient interaction between the bladder urothelium and the afferent nerve fibers situated in close proximity to the urothelium within the lamina propria. Methods: We aimed to quantify aging-related differences in the expression of calcitonin gene-related peptide (CGRP, an established marker for sensory nerve fibers) in the trigonal mucosal layers of young (3–4 months) and aged (25–30 months) rats. We evaluated trigonal tissue from 3 animals per age group. Tissue was serially sectioned at 10 μm and stained for CGRP. Images were taken along the full length of the tissue. For each image we computed the total CGRP-positive area (μm2 ) and the median value for each animal was used for further analysis. Results: Upon statistical analysis the aged rats show a significantly lower CGRP-positive area compared to young rats (P=0.0049). These results indicate that aging has a negative effect on the area of CGRP-positive signal in the trigone. Conclusions: The structural and functional integrity of the sensory web in the trigonum of rats is negatively affected by the aging process, potentially leading to impaired communication between the bladder urothelium the CNS. Consequently, these perturbations in the sensory system may contribute to the pathogenesis or exacerbation LUTS.

        Eruption of Metastatic Paraganglioma After Successful Therapy with 177Lu/90Y-DOTATOC and 177Lu-DOTATATE

        Metastatic paraganglioma treatment options are limited. Peptide receptor radionuclide therapy (PRRT) has been introduced as a novel management option for metastatic neuroendocrine tumors demonstrating safety, efficacy, and increased quality of life.We present two cases of marked progression of metastatic paraganglioma following initial partial response to PRRT. Given their positivity on 68Ga-DOTATATE PET/CT and 111In-octreotide SPECT, they underwent PRRT. Imaging following treatment revealed significant improvement in size and intensity, with some foci nearly completely resolved in one patient, and disease regression with a decrease in the number and size of bone and liver lesions in the second patient.Within months, repeat imaging in both patients revealed extensive metastatic disease with new lesions, which eventually lead to their deaths. The mechanism for rapid disease progression after partial response is not well understood, although it could be related to initially high Ki-67 levels or 18F-FDG PET/CT SUVmax values. However, naturally rapid disease progression despite PRRT response cannot be excluded. This finding warrants the importance of proper patient counseling along with early and accurate pre-PRRT assessment, taking into consideration the above potential risk factors for therapy response in order to personalize treatment regimens and achieve maximum patient benefit.
