      • HBV : PO-04 ; Antiviral efficacy of entecavir and adefovir combination therapy in chronic hepatitis B patients with previous nucleos(t)ide analogues failures

        Background: Entecavir (ETV) and adefovir (ADV) combination can be used as a rescue therapy for patients who had failed nucleos(t)ide analogues where tenofovir has not been introduced. We evaluated the efficacy of ETV and ADV combination therapy in patients who had failed to previous nucleos(t)ide analogue (NA). Methods: This study included consecutive 66 compensated chronic hepatitis B (CHB) patients who have failed to previous NAs. All patients received ETV and ADV combination therapy for at least 6 months. While all of the patients had experienced lamivudine (LAM), 48 (72.7%) had experienced ADV and 53 (80.3%) had been treated with ETV. Genetic resistance to LAM, ADV, and ETV at baseline were detected in 65 (98.5%), 12 (18.2%), and 36 (54.5%), respectively. Results: Median HBV DNA level was 5.63 (1.86∼8.23) log IU/mL and fifty nine patients had HBeAg positivity. During a median of 15 (6-38) months of ETV and ADV combination therapy, virological response (VR, < 60 IU/mL by PCR assay) was achieved in 18 of 66 (27.3%) patients. Cumulative VR rates were 21.5%, 28.4%, 36.6% and 36.6% at 6, 12, 24 and 36 months, respectively. When we investigated predictors for VR, baseline ADV resistance along with low baseline ALT level (p=0.027) were significantly associated with decreased VR from multivariate analysis. Conclusions: ETV and ADV combination rescue therapy showed limited efficacy for CHB patients who had failed to previous NAs, however, it can be considered as a rescue therapy in patients who do not have resistance to ADV and high baseline ALT level.

      • Overexpression of Reactive Oxygen Species Scavenger Enzymes Is Associated with a Good Prognosis in Triple-Negative Breast Cancer

        <P>Abstract</P><P><B><I>Objective:</I></B> The aim of this study was to investigate the expression of redox proteins in triple-negative breast cancer (TNBC), a breast cancer type which harbors an aggressive phenotype, and to demonstrate the relationships between the expression of reactive oxygen species (ROS) markers and clinical outcome. <B><I>Methods:</I></B> From 2000 to 2005, we analyzed tissue samples from 135 cases of TNBC obtained from curative surgeries at the Severance Hospital, Seoul, South Korea. Immunochemical staining for redox proteins was performed. <B><I>Results:</I></B> The median patient age was 48 (range 27-76) years. TNM staging was as follows: T1 (n = 52, 38.5%), T2 (n = 81, 60%), T3 (n = 2, 1.5%), N0 (n = 88, 65.2%), N1 (n = 35, 25.9%), N2 (n = 8, 5.9%) and N3 (n = 4, 3.0%). Median follow-up was 59 (range 12-99) months. The high-expression catalase group tended to have a lower N stage (p = 0.016), lower tumor recurrence (p = 0.02) as well as longer overall survival rates (p = 0.05). After adjusting for patient age as well as tumor and nodal stage, we found that primary breast cancers with high catalase expression levels did not have an improved clinical outcome. A high MTC4 expression in the stroma was strongly associated with increased manganese superoxide dismutase expression (p = 0.015) and a significantly longer overall survival (p = 0.032). <B><I>Conclusion:</I></B> Considering the lack of targeted molecules as well as molecular heterogeneity of TNBC, ROS markers may provide clues to clinical outcome in TNBC.</P><P>© 2014 S.         Changes in Cardiac Structure and Function After Kidney Transplantation: A New Perspective Based on Strain Imaging

        BACKGROUND: We aimed to investigate left ventricular (LV) global longitudinal strain (GLS) in end-stage renal disease patients and its change after kidney transplantation (KT). METHODS: We retrospectively reviewed patients who underwent KT between 2007 and 2018 at two tertiary centers. We analyzed 488 patients (median age, 53 years; 58% male) who had obtained echocardiography both before and within 3 years after KT. Conventional echocardiography and LV GLS assessed by two-dimensional speckle-tracking echocardiography were comprehensively analyzed. Patients were classified into three groups according to the absolute value of pre-KT LV GLS (|LV GLS|). We compared longitudinal changes of cardiac structure and function according to pre-KT |LV GLS|. RESULTS: Correlation between pre-KT LV EF and |LV GLS| were statistically significant, but the constant was not high (r = 0.292, p < 0.001). |LV GLS| was widely distributed at corresponding LV EF, especially when the LV EF was > 50%. Patients with severely impaired pre-KT |LV GLS| had significantly larger LV dimension, LV mass index, left atrial volume index, and E/e’ and lower LV EF, compared to mildly and moderately reduced pre-KT |LV GLS|. After KT, the LV EF, LV mass index, and |LV GLS| were significantly improved in three groups. Patients with severely impaired pre-KT |LV GLS| showed the most prominent improvement of LV EF and |LV GLS| after KT, compared to other groups. CONCLUSIONS: Improvements in LV structure and function after KT were observed in patients throughout the full spectrum of pre-KT |LV GLS|.

        Clinical Implications and Determinants of Left Atrial Mechanical Dysfunction in Patients With Stroke

        <P>Background and Purpose-The evaluation of sources of cardioembolism with transesophageal echocardiography (TEE) in patients with stroke is crucial but semi-invasive. We hypothesized that the size and mechanical function of the left atrium (LA) assessed by transthoracic echocardiography (TTE) could provide useful information on high risk of cardioembolism on TEE in patients with stroke. Furthermore, we sought to define the determinants of LA mechanical dysfunction in these patients. Methods-A total of 248 patients with acute ischemic stroke (147 men; 64 +/- 13 years) who underwent 2-dimensional and speckle tracking TTE followed by TEE were analyzed. Results-LA appendage emptying velocity, prevalence of LA or LA appendage thrombus, prevalence of aortic plaques, and incidence of embolic stroke showed significant differences among the 4 groups classified according to the median values of the LA volume index and global LA longitudinal strain (LALS). Patients at high risk of cardioembolism evidenced by TEE revealed significantly larger LA volume index and lower global LALS than those without. Global LALS (cutoff, 11.5%; area under the curve, 0.947; sensitivity, 100%; specificity, 91%; P<0.001) revealed a significantly better diagnostic power (P=0.04) for LA or LA appendage thrombus than LA volume index (cutoff, 36.2 mL/m(2); area under the curve, 0.823; sensitivity, 88%; specificity, 75%; P=0.002). Age, left ventricular systolic function, LA volume index, and pulse wave velocity were independent determinants for global LALS. Conclusions-LA mechanical dysfunction is closely associated with high risks of cardioembolism. Global LALS assessed by speckle tracking TTE well discriminates the presence of LA or LA appendage thrombus on TEE in patients with acute ischemic stroke.</P>

      • L-type amino acid transporter SLC43A2 in ovarian cancer regulate accumulation and suppressive function of MDSC in TME

        Objective: Ovarian cancer is the most lethal malignant cancer in women. Myeloid-derived suppressor cells (MDSCs) accumulation in tumor microenvironment (TME) contribute poor prognosis of ovarian cancer patients. Tumor cells consume high amount of nutrition to meet their bioenergetics/biosynthetic demands. High nutrition demand of tumor cells leads to deprivation of several nutrition in TME, making that tumor-infiltrating immune cells compete with cancer cells for nutrition to maintain their own function. SLC43A2 is a type of L-type amino acid transporter found in various cancers, resulting in methionine deprivation in TME. Methods: The TCGA data sets were used for analyzing the correlation of SLC43A2 mRNA, MDSC markers, and overall survival of ovarian cancer patients. Whole blood was collected from fifteen patients who underwent debulking surgery in Kangwon National University Hospital. PBMC and serum was isolated from the whole blood. Immune cells in PBMC were analyzed with flowcytometry. Methionine was measured with LC/MS method. Results: We found that SLC43A2 mRNA negatively correlates with overall survival and mRNA of MDSC markers including CD33, CD14, CD15 and PD-L1 from ovarian cancer patients as assessed with TCGA data sets. Additionally, serum methionine concentration showed negative correlation with percent of CD11b+ CD33+ cells (represented as MDSC phenotype) in PBMCs donated from ovarian cancer patient cohort. We also found that tumor growth was delayed and tumor-infiltrating granulocytic MDSCs was reduced in SLC43A2 KO melanoma-bearing mice as compared to WT-bearing mice. Gr-1+ MDSCs isolated from SLC43A2 KO melanoma-bearing mice revealed reduced T cell suppression activity than MDSCs isolated from WT melanoma-bearing mice. Conclusion: These findings suggested that alteration of TME metabolism due to high level of SLC43A2 expression in cancer cells resulting in methionine deprivation can affect MDSC physiology and anti-cancer immunity.

        The Korean Organ Transplant Registry (KOTRY): Second Official Adult Heart Transplant Report

        Background and Objectives: This second adult heart transplantation (HTx) report is based on Korean Organ Transplant Registry data submitted on 400 HTxs in recipients of all ages. Methods: From March 2014 to December 2017, a total of 400 HTxs were performed at 4 major centers in Korea. We analyzed demographics and characteristics according to transplant years. Patterns of immunosuppression, allograft rejection, and survival after HTx were analyzed. Donor and recipient age were highlighted. Results: Some distinct differences in HTx statistics were noted. Mean donor age increased significantly in the most recent years compared to 2014–2015, while mean recipient age did not change. The proportion of patients on pre-transplant extracorporeal membrane oxygenation (ECMO) increased over time. One-year and intermediate-term survival was significantly worse in patients on pre-transplant ECMO compared to those without mechanical support. Over the years, tacrolimus has increased to become the most frequently used calcineurin inhibitor over cyclosporine, while the number of patients using steroids both at discharge and 1-year follow-up has declined. Age did not affect 1-year survival, but significantly affected intermediate-term survival. Conclusions: From 2014 to 2017, centers were willing to accept older donors to address increasing organ shortages and more patients received transplant under ECMO care. Increasing age was a strong independent factor for intermediate-term survival, however, post-transplant comorbidities did not differ among age groups. Further studies with longer follow-up duration are needed to better understand age-related post-transplant prognosis.

        AN ADAPTIVE FINITE DIFFERENCE METHOD USING FAR-FIELD BOUNDARY CONDITIONS FOR THE BLACK-SCHOLES EQUATION

        We present an accurate and efficient numerical method for solving the Black-Scholes equation. The method uses an adaptive grid technique which is based on a far-field boundary position and the Peclet condition. We present the algorithm for the automatic adaptive grid generation: First, we determine a priori suitable far-field boundary location using the mathematical model parameters. Second, generate the uniform fine grid around the non-smooth point of the payoff and a non-uniform grid in the remaining regions. Numerical tests are presented to demonstrate the accuracy and efficiency of the proposed method. The results show that the computational time is reduced substantially with the accuracy being maintained.

        By structure-borne effect of small reverberation room, Based on various experimental data, it is showed that structure-borne effect plays an important role of the small chamber performance under the frequency range of 400 Hz. When the structure-borne effect is removed, the accuracy to follow the mass law behavior has been expanded down to 200 Hz frequency range. We also examined the correction factor using the SEA simulation in addition to mass law. It turns out that SEA simulation is also appropriate for estimating the transmission loss of the test specimen. We may conclude that if the structure-borne effect is removed, it would be able to measure the transmission loss characteristics using a small chamber with validation down to 200 Hz frequency range.
