        Proposed Use of Thickener According to Fluid Intake on Videofluoroscopic Swallowing Studies: Preliminary Study in Normal Healthy Persons

        Objective To examine the characteristics and changes in the pharyngeal phase of swallowing according to fluid viscosity in normal healthy persons, to help determine fluid intake methods in more detail than the use of standardized fluid.Methods This was a prospective observational study involving 10 normal healthy adults. While the participants sequentially took in fluids with 10 different viscosities changes in the pharyngeal phase of the swallowing process were monitored using videofluoroscopic swallowing studies (VFSS). Twenty parameters of the pharyngeal phase, including epiglottis contact, laryngeal elevation, pharyngeal constriction, and upper esophageal sphincter opening, were determined and compared.Results No significant viscosity-based changes in epiglottis contact, laryngeal elevation, or upper esophageal sphincter-opening duration of the pharyngeal phase were observed. However, pharyngeal transit time and time from the start of the pharyngeal phase to peak pharyngeal constriction were significantly delayed upon intake of fluid with viscosities of 150.0 centipoise (cP) and 200.0 cP.Conclusion VFSS analysis of fluid intake may require the use of fluids of various concen trations to determine a suitable viscosity of thickener mixture for each subject.

      • Purification and Characterization of Protein Methylase II from Porcine Testis

        Protein methylase II (S-adenosyl-L-methionine : protein O-methyl-transferase, EC 2.1.1.24; PM II) was purified approximately 1250-fold from porcine testis by fractional precipitation and DEAE-cellulose chromatography, followed by gel filtration on a Sephadex G-75 column and HPLC on a Protein Pak 125 column. The molecular weight of the enzyme was estimated to be 33,000 daltons by SDS-PAGE, which agreed with the value determined by gel filtration. Isoelectric focusing of purified PM II showed a single protein species with an isoelectric point of 6.2. The optimum pH for the reaction was 6.0. The $K_m$ value of the enzyme was $1{\times}10^{-5}M$ with a $V_{max}$ value of 769 pmol/min/mg of enzyme. S-adenosyl-L-homocysteine is a competitive inhibitor of PM II with a $K_i$ value of $1.38{\times}10^{-6}M$.

        고관절 관절경의 초기 경험과 합병증

        Purpose: To report our early experience of hip arthroscopy. Materials and Methods: Forty one patients (45 hips) who underwent hip arthroscopy from November 2007 to February 2009 were enrolled in the study. The population was divided into 3 groups in sequence. The operation time, fluoroscopic time, traction time, and complications were analyzed. Results: There was no significant difference in operation time and traction time between the three groups. The fluoroscopic time decreased from 41 minutes in group I to 11 minutes in group III (p<0.01). There were four cases of nerve palsy; 2 pudendal nerve palsy in group I, 1 pudendal nerve palsy in group II, and 1 peroneal nerve palsy in Group III. All cases recovered completely within few days after surgery. Iatrogenic labral injury occurred in 4, 3 and 0 hips in group I, II and III, respectively. Iatrogenic cartilage damage to the femoral head occurred in 2, 3 and 1 hip in group I, II and III, respectively, which did not affect the outcomes. Conclusion: Definite improvement in the hip arthroscopy technique was observed in the fluoroscopic time. No serious complications, such as death or permanent nerve palsy, were encountered.

        컴퓨터단층검사에서 AAPM Phantom을 이용한 변조전달함수 평가

        In clinical computed tomography (CT), regular quality assurance (QA) has been required. This study is to evaluate the MTF for analyzing the spatial resolution using AAPM phantom in CT exam. The dual source somatom definition flash (siemens healthcare, forchheim, Germany), the brilliance 64 (philips medical system Netherlands) and aquilion 64 (toshiba medical system, Japan) were used in this study. The quantitative evaluation was performed using the image J (wayne rasband national institutes of health, USA) and chart method which is measurement of modulation transfer function (MTF). In MTF evaluation, the spatial frequencies corresponding to the 50% MTF for the CT systems were 0.58, 0.28, and 0.59 mm-1, respectively and the 10% MTF for the CT systems were 1.63, 0.89, and 1.21 mm-1, respectively. This study could evaluate the characteristic of spatial resolution of MTF using chart method, suggesting the quantitative evaluation method using the data. 임상적으로 computed tomography (CT)에서 정기적인 quality assurance (QA)는 요구되는 사항이다. 본 논문에서는 CT 검사에서 공간분해능에 대해서 AAPM Phantom을 이용하여 정확한 성능 분석을 위해 변조전달함수를 평가하고자 한다. CT장비는 dual source somatom definition flash (siemens healthcare, forchheim, Germany), brilliance 64 (phili ps m edi cal system N etherlands), a qui li on 64 ( toshiba m edi cal system, Japan)를 사용하였으며, l mage J(wayne rasband national institutes of health, USA)프로그램을 실행하여 변조전달함수의 측정 방법인 chart method를 이용하여 영상에 대해서 정량적인 평가방법을 시행하였다. 변조전달함수를 이용하여 평가한 결과, 공간주파수를 이용한 데이터 값이 높아짐에 따라 변조전달함수는 감소됨을 알 수 있었다. 그리고 CT 장비에서 변조전달함수는 공간주파수 (50%)에 있어서는 0.58, 0.28 0.59 mm-1으로 나타났으며, 변조전달함수 공간주파수 (10%)에 있어서는 1.63, 0.89 1.21 mm-1으로 나타났다. 본 연구에서는 chart method를 이용하여 변조전달함수의 공간해상력에 대한 특성을 확인하고, 정량적인 평가방안을 제시하였다는 점에서 학술적 의의를 둘 수 있다.

        Uterine torsion, which most often occurs during pregnancy, is rare. Torsion of a nongravid uterus is extremely rare and is associated with pelvic adhesions, uterine leiomyomas, or uterine anomalies. We experienced a case of uterine torsion, with a huge subserosal leiomyoma and multiple intramural leiomyomas, in a postmenopausal woman, one that showed the most extreme degree of rotation among the known cases of uterine torsion. Necrosis of the uterus and both adnexae was observed. A total abdominal hysterectomy with a bilateral salpingo-oophorectomy was performed. We present this case with a brief review of the relevant literature.

        간접평판형 검출기에서 국제전자기술위원회 기준을 통한 잡음전력스펙트럼 비교 연구

        The purpose of this study was to compare image quality of indirect digital radiography (IDR) system using the International Electro-technical Commission standard (IEC 62220-1), and to suggest the analysis of noise power spectrum (NPS) which were applied to IEC 62220-1 in medical imaging. In this study, Pixium 4600 (Trixell, France) which is in-direct flat panel detector (FPD) was used. The size of image receptor (IR) is 7 × 17 inch (matrix 3001 × 3001) which performed 14bit processing and pixel pitch is 143 μm. In IEC standard, NPS evaluation were applied to RQA3, RQA5, RQA7 and RQA9. Because of different radiation quality, each region of interesting (ROI) were compared. The results of NPS indicated up to 3.5 mm -¹ including low Nyquist frequency. RQA5 indicated the lowest NPS and the others indicated higher NPS results relatively. NPS result of ROI a38 was higher than ROI a92 and this result indicated that there are more noise in left (cathode) than right (anode). This study were to evaluate NPS by using different radiation quality and setting the each ROI, and to suggest the quantitative methods of measuring NPS

        방사선치료 시 전자포털 영상장치를 이용한 잡음전력스펙트럼 수평 및 수직 측정비교

        영상화 품질향상은 방사선 치료영역과 진단영역의 영상화 검사와 같이 quality assurance (QA) 장비들의 특성평가에 매우 중요한 요소이다. 본 연구의 목적은 치료장비의 megavoltage X-ray 에너지를 사용하여 측정방법에 있어서 NPS에 대한 수평, 수직적인 방법에 대한 측정결과를 비교 평가하는 것이다. 잡음전력스펙트럼 평가 방법들은 국제 전기 규격 international electro-technical commission (IEC 62220-1) 기준을 사용하여 치료 영역에 적용되었다. 사용한 전자포털 영상장치(electronic portal imaging device, EPID)는 Siemens BEAMVIEWPLUS, Elekta iViewGT 그리고 Var ian ClinacR iX aS1000이었다. 데이터들로부터 각각의 주파수 값에 대응하는 노이즈 값들의 평균에 대해서, 각각의 주파수 별로 대응해서 표현하였으며, 수평 수직되어있는 주파수별 평균값의 노이즈값을 합산해서 정량적으로 장비마다 비교 평가하였다. 결과값은 Elekta iView에서 4가지 방법론에 의한 가로방향에서의 horizontal측정된 NPS값과 vertical 세로방향에서의 측정치는 비슷한 경향성에 따라서 horizontal과 vertical값은 3~5%정도의 차이가 있었으며, Siemens BEAMVIEWPLUS, Varian ClinacR iX aS1000에서 horizontal측정된 NPS값과 vertical 측정된 NPS값은 편차의 차이가 전체값의 평균으로 했을 시 horizontal과 vertical값은 15%정도의 차이가 나타남을 보였다. 본 연구에서는 수평 수직되어있는 주파수별 평균값의 노이즈값을 합산해서 정량적으로 장비마다 비교 평가하였으며, 정량적인 평가방안을 제시하였다는 점을 둘 수 있다. The quality assurance (QA) is very important for diagnostic field and radiation therapy field to evaluate the characteristic of devices. The purpose of this study was to compare different NPS methodologies results which are measuring NPS with regard to horizontal and vertical directions by using megavoltage X-ray energies. The NPS evaluation methods were applied to the International Electro-technical Commission standard (IEC 62220-1). The electronic portal imaging devices (EPID) devices such as Siemens BEAMVIEWPLUS, Elekta iViewGT and Varian ClinacR iX aS1000 were used. NPS data were expressed by corresponding each frequency about average of noise value corresponding the each frequency, and NPS were evaluated quantitatively by totaling up the noise values of average frequency which are on horizontal and vertical directions. In NPS results for Elekta iViewGT, NPS of horizontal and vertical by using 4 methods were indicated the difference of 3~5% between horizontal and vertical direction. In the results of Siemens BEAMVIEWPLUS and Varian ClinacR iX aS1000, the NPS of horizontal and vertical direction were indicated the difference of 15% when averaging the whole values. This study were evaluated the NPS of each devices by totaling up the noise values of average frequency which are on horizontal and vertical directions suggesting the quantitative evaluation method using the data.

        Dilemma in Severity Classification of Incongruent Hallux Valgus Deformity Using Radiological Measurements

        목적: 비상합성 무지 외반증의 수술 계획 수립을 위해 기존에 사용되던 각도 계측에 의한 분류법의 유용성을 재 고찰하고자 한다. 대상 및 방법: 비상합성 무지 외반증 257예를 연구 대상으로 하여 이들의 무지 외반각과 제1-2 중족골간 각을 측정하였다. Mann에 의한 방사선적 계측에 따른 분류 방법으로 무지 외반각과 제1-2 중족골간 각 사이의 중증도를 비교하였고, 이들의 중증도가 서로 일치하는지를 확인하였다. 비상합성 변형의 분포 정도를 관찰하기 위해 원위 중족 관절각 또한 계측하였다. 결과: 무지 외반각은 평균 36.1° (범위, 16-60°)이었으며 제1-2 중족골간 각은 평균 15.8° (범위, 5-30°)이었다. 무지 외반각과 제1-2 중족골간 각의 중증도가 서로 일치하는 예는 126예(49.1%)이었고 131예(50.9%)에서는 중증도가 일치하지 않았다. 원위 중족 관절각은 평균 15.6°(범위, 0-40°)이었으며 133예(51.8%)에서 정상치 이상(> 15°)을 보였다. 결론: 무지 외반각과 제1-2 중족골간 각의 중증도가 서로 일치하지 않는 한계가 있으므로, 수술 방법 결정을 위한 지침으로써 방사선학적 분류를 이용하기 위해서는 기존의 분류 체계가 보완되어야 할 것이다. 무지 외반각은 제1 중족족지 관절 주변의 골성 변형이나 외측방 아탈구 등에 의해 영향을 받으므로, 무지 외반각에 영향을 마치는 이러한 요소들을 세분하고 이를 교정해야 할 대상으로 고려하여야 할 것으로 사료된다. Purpose: To study the availability of the radiological classification for incongruent hallux valgus deformities, which has been used as the key to the algorithm for selecting surgical options. Materials and Methods: To determine radiological severity, 257 cases of incongruent hallux valgus deformities were studied. The hallux valgus angle (HVA) and the 1/2 intermetatarsal angle (IMA) were measured in each case. Following Mann's radiological classification system, the HVA and IMA were compared to determine any similarity in severity. The distal metatarsal articular angle (DMAA) was also measured to observe the extent of its effect on the incongruent deformity. Results: The HVA was 36.1° on average (range, 16-60°) and the 1/2 IMA was 15.8° on average (range, 5-30°). In 126 cases (49.1%), the severity between the 1/2 IMA and HVA coincided with the index classification, whereas. 131 cases (50.9%) did not correspond. Overall, the severity of approximately half the cases, could not be graded. The DMAA was 15.6° on average (range, 0-40°) and 133 cases (51.8%) demonstrated a value above normal value (>15°). Conclusion: The radiological classification system used as a guide for surgical decision-making needs to be revised, due to the overt limits of correspondence between the severity of the 1/2 IMA and HVA. Because the HVA would be influenced by both the lateral subluxation and bony deformity around the first metatarsophalangeal joint, it is reasonable to consider these factors on the HVA as elements to be corrected individually, instead of the HVA alone.

        Laparoscopic total extraperitoneal hernia repair of fallopian tube indirect inguinal hernia in reproductive aged woman: a case report

        An indirect inguinal hernia containing the fallopian tube alone is extremely rare in reproductive-aged women without any genital tract anomalies. Despite this rarity, early diagnosis and adequate management is important to prevent strangulation and recurrence. We present a case of an indirect inguinal hernia containing only the fallopian tube in the hernia sac, which was successfully reduced by using a laparoscopic total extraperitoneal approach and repaired with a polypropylene mesh.
