<P><B>AIM</B></P><P>To clarify the role of neoadjuvant concurrent chemoradiotherapy (NACCRT) followed by surgical resection for localized or locally advanced perihilar cholangiocarcinoma (CCA).</P><P><B>METHODS</B></P><P>We retrospectively reviewed 57 patients who underwent surgical resection with or without NACCRT for perihilar CCA; 12 patients received NACCRT and 45 patients did not received NACCRT. Patients with locally advanced perihilar CCA requiring NACCRT were defined as follows: (1) a mass involving unilateral branches of the portal vein or hepatic artery with insufficient volume of the anticipated remnant lobe; or (2) an infiltrating mass in the main portal vein that was too long for reconstruction, identified at preoperative staging.</P><P><B>RESULTS</B></P><P>The median disease-free survival (DFS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 26.0 and 15.1 mo, respectively (<I>P</I> = 0.91). The median overall survival (OS) durations of the neoadjuvant and non-neoadjuvant CCRT groups were 32.9 and 27.1 mo, respectively (<I>P</I> = 0.26). The NACCRT group showed a downstaging tendency compared to the non-NACCRT group as compared with the tumor stage confirmed by histological examination after surgery and the tumor stage confirmed by imaging test at the time of diagnosis (<I>P</I> = 0.01).</P><P><B>CONCLUSION</B></P><P>NACCRT does not prolong DFS and OS in localized or locally advanced perihilar CCA. However, NACCRT may allow tumor downstaging and improve tumor resectability.</P>

As science and technology infiltrates every aspects of modern society in terms of economic and social growth and development, funding for research and development (R&D) is growing rapidly. Republic of Korea is not an exception in this trend and the R&D funding in Korea has been grown about 10% every year, recently. However, as the scope and size of funding grows exponentially, need for monitoring and managing these R&D projects becoming more and more imminent. Though different types of project management systems were developed by a variety of agencies and departments and used in monitoring and managing, these systems were developed as standalone silo type systems. These systems are not connected to each other while the same researchers may involved in different projects across agencies and department. Also, these management systems are not linked to the banking systems in which real transactions of funding occurs, such as cost reimbursement and financial audit of each R&D accounts. Historically, a few fraud and malappropriation cases were found and indicted. However, as the number of these incidents grows along with the growth of R&D funding, a large scale integration/linking of project management systems and banking ystems. Realizing the importance of systems integration among agencies as well as with the banking systems, situational requirements analyses were conducted concerning the current state of R&D management system. As a results, a Real-time Case Management System (RCMS) was proposed as a solution to current problems. In this paper, the collected systems requirements were documents with analyses of the situation, the architecture of the integrated systems with more user-friendly technological alternatives. This large scale linkage requires interface standardization as well as modularization of interfaces. Proposed systems architecture is introduced here with technical details of Jex Framework used,, followed by resulting technical and economic performance of the Realtime Cashflow Management System (RCMS).

Purpose: To evaluate the efficacy of anterior segment swept-source optical coherence tomography (SS-OCT) for examining horizontal extraocular muscle thickness, distance from the corneal limbus to the insertion of the horizontal extraocular muscle (limbus to insertion distance), and scleral thickness in Korean pediatric strabismus patients. Methods: This study included pediatric strabismus patients between 5 and 10 years of age. Children with any ocular disease other than strabismus or a history of ocular surgery were excluded. SS-OCT was used to measure horizontal extraocular muscle thickness, limbus to insertion distance, and scleral thickness. Eyes were classified into subgroups by sex, spherical equivalent of the refractive error (measured with cycloplegic refraction), and fixating/ deviating eye. Results: One patient initially included in this study was excluded due to poor cooperation. Of the remaining 35 eyes of 20 patients, 19 eyes (54.3%) were from male patients and 16 eyes (45.7%) were from female patients. The mean patient age was 7.86 ± 1.38 years. Lateral scleral thickness was greater in male eyes than in female eyes (p = 0.048). No other differences were noted between male and female children. Additionally, there were no statistically significant differences between fixating and deviating eyes or among spherical equivalent groups for any parameter examined. Conclusions: Anterior segment SS-OCT can successfully and comfortably measure horizontal extraocular muscle and scleral anatomy in children. These measurements may be helpful for treatment and follow-up of pediatric strabismus patients.

Coronary artery injury after thoracic injury is very rare, but can result in serious acute myocardial infarction (MI). It can be easily mistaken for chest wall pain or cardiac contusion if relying solely on a history and physical examination. We herein report a rare case of a 60-year-old female patient who presented with inferior wall ST-segment elevation MI due to right coronary artery dissection following blunt chest trauma after a traffic accident. Successful primary percutaneous coronary intervention was performed without complications. (Korean J Med 2013;84:847-850)

전극 전해질 복합체가 하나로 구성된 단위전지는 전기출력이 낮아 원하는 전기출력을 얻기 위하여 단위전지를 여러 장 직렬로 쌓아 스택을 구성하여야 한다. 본 연구에서는 400W급 직접메탄을 연료전지 스택을 제조하여, 운전 조건의 최적화를 위한 실험을 수행하였고, 한국형 휴머노이드 로봇인 Hubo에 적용하여 그 동특성을 평가하였다. 0.8M 메탄을 용액은 260 ml/min의 유량을, 공기는 42LPM을 스택에 주입하여 400W의 성능을 나타내었다. 연료전지 스택의 온도와 전압의 편차는 작았으며 정상 운전시스템의 온도는 약 70?C를 유지하였다. 로봇의 필요 전력이 높을 경우, 배터리와 연료전지가 동시에 부하 변동에 대응하였다 방전된 배터리는 로봇의 필요 전력이 낮아질 경우, 충전되어 이후의 방전 상황에 대비하였다. 연료전지 시스템 적용 결과 로봇의 성공적인 운전을 확인할 수 있었다.

본 논문에서는 제어밸브에 대한 유공압적, 기계적, 전기적 부분에 대하여 전반적인 기술 적인 사항 및 설계 기술들을 다루었다. 특히 제어밸브를 이용한 시험으로부터 도출하기 어려운 많은 요소의 동특성을 시스템 모사코드인 AMESim 을 이용하여 예측해보았고, 일반 적인 물리적 동특성도 또한 실제 시험결과와 비교 검토하였다. 또한 제어밸브의 단품 상태에서의 동특성 외에 시스템 연계 상황에서의 특성도 예측하고자 하였고, 이를 근간으로 하 여 발사체 엔진 추진제공급시스템의 제어 관련 분야에 응용하였다. This paper deals with technological and design skills for all pneumatical, hydraulical, mechanical, and electrical parts related to control valve. Especially, a variety of dynamic characteristics, which are not easily extracted from field tests using control valves, are studied by system-simulation code, AMESim. The simulated results are also compared and examined with actual testing results in terms of physical dynamic characteristics. In addition, this paper contains the simulated system characteristics including dynamic characteristics in component level of valve itself. Based on the results, it is applied to control system of propellant feeding system in liquid-propellant engine of satellite launcher.

차세대 하이테크 스마트 의류는 복합 차원에서의 감성적인 요소를 섬유·패션기술에 IT융합 기술을 이용하여 제공하고 있다. 생체신호를 이용한 감성은 모호하여 정량적이고 객관적인 측정이 어렵고, 그 표현도 제한된 감성 어휘에 의하여 나타나기 때문에 구체적으로 파악하는 것은 어려운 일이다. 이를 위하여 제품의 기능적 측면뿐만 아니라 정서적 감정과 선호도가 반영된 제품의 설계나 디자인 또한 요구되고 있다. 본 논문에서는 생체신호 측정을 위한 웨어러블 기반의 심전도 측정 의복을 제안하였다. 착용자가 평소 자주 입는 티셔츠를 응용하여 답답해하거나 불편하지 않게 제작하고 소매 형태로 신축성있는 소재를 사용한다. 인체의 형태에 따라 의복과 바이오센서의 전극이 안정적으로 밀착될 수 있도록 고탄력 밴드를 이용하여 일자형으로 제작하였다. 심전도 측정 의복을 착용에 의해 수집된 심전도 ECG 파형을 수집하고 심박변화율을 계산하는 시뮬레이션을 개발한다.
