The inhibitory effect of extracts from 15 edible plants on the protease of human immunodeficiency virus (HIV) type 1 was investigated. Protease activity was determined by incubating the extracts in a reaction mixture containing protease and substrate His-Lys-Ala-Arg-Val-Leu-(p-NO$_2$-Phe)-Glu-Ala-Nle-Ser-NH$_2$ to inhibit proteolytic cleavage. Of various plants tested, the leaves of Cedrela sinensis inhibited the HIV-1 protease by 42% at a concentration of 100$\mu\textrm{g}$/ml. A major flavonoid isolated from the leaves of C. sinensis, quercetin 3-O-$\alpha$-L-rhamnoside showed inhibitory activity of 19% at a concentration of 100$\mu$M.

Terfenadine is widely used because of nonsedating effect. But it could rarely provoke a potentially lethal ventricular tachyarrhythmia. Recently, we experienced two cases of torsades de pointes(TDP) occurred after combined use of terfenadine and ketoconazole in usual dose. In one case, 31-yr-old female presented palpitation and recurrent syncope of sudden onset after ingestion of terfenadine 60mg and ketoconzole 200mg 5 times. On attack, ECG showed a polymorphic ventricular tachycardia, and after attack, showed prolongation of QT interval and TU wave changes. Her laboratory findings were not contributory. TDP was controlled with MgSO₄ and isoproterenol infusion. Then, QT interval was normalized and no further episode occurred. In the other case, 32-yr-old female presented palpitation and recurrent syncope of sudden onset after ingestion of terfenadine 60mg and ketoconzole 200mg 5 times, ECG showed prolongation of QT interval and TU wave changes. Her laboratory findings were not contributory. TDP was controlled with MgSO₄ and isoproterenol infusion. Then, QT interval was normalized and no further episode occurred.

The Sit-and-Reach Test (SRT) is commonly used to assess flexibility of the spine and length of the hamstring muscle. The purpose of this study was to describe hamstring muscle length as reflected by use of the SRT, the Hip Joint Angle (HJA) and Back length(from C7 to S2) in children, to examine the correlation between Back length, SRT and HJA measurements and to examine gender differences. The 162 subjects were participated without known musculoskeletal and neurological impairments of their spine or lower extremities. In the Long-sitting position, the Back length, SRT and HJA measurements were obtained. A mean Back length value of 46.2㎝, a mean SRT value of 29.4㎝ and a mean HJA of 77.0 degrees were obtained including all subjects. There was a strong correlation between the SRT and HJA measurements (r=.66). There were a difference between boys and girls in Back(p=.0019) and HJA measurements (p=.015). The results of this study suggest that measurements for the SRT and HJA were correlated than Sack. The HJA measurements guide treatment more effectively than do Back length and SRT measurements.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Although there have been several reports that prasugrel can improve clinical outcomes, the efficacy and safety of prasugrel is unknown in Korean patients with acute myocardial infarction (AMI) undergoing successful revascularization.</P> <P><B>Methods</B></P> <P>A total of 4421 patients [637 patients were prescribed prasugrel (60/10 or 5mg, loading/maintenance dose) and 3784 patients clopidogrel (600 or 300/75mg)] with AMI undergoing successful revascularization were enrolled from the core clinical cohort of Korea Acute Myocardial Infarction Registry-National Institute of Health.</P> <P><B>Results</B></P> <P>After propensity score matching (637 pairs), there were no significant differences in baseline clinical and procedural characteristics and in-hospital medications between the two groups. The primary efficacy endpoint, defined as the composite of cardiac death, MI, stroke, or target vessel revascularization at 6 months showed no significant difference between prasugrel and clopidogrel (2.4% vs. 2.9%, <I>p</I> =0.593). Also, no difference was observed in the composite of cardiac death, MI, or stroke during hospitalization between two groups (0.8% vs. 0.9%, <I>p</I> =0.762). However, the incidence of in-hospital Thrombolysis in Myocardial Infarction (TIMI) major or minor bleeding was significantly higher in prasugrel compared with clopidogrel (5.3% vs. 2.7%, <I>p</I> =0.015). In multivariate linear regression analysis, trans-femoral intervention, use of glycoprotein IIb/IIIa inhibitors, use of calcium channel blocker, and use of prasugrel were independent predictors of in-hospital TIMI major or minor bleeding [odds ratio (OR)=6.918; 95% confidence interval (CI)=2.453–19.510, OR=2.577; 95% CI=1.406–4.724, OR=4.016; 95% CI=1.382–11.668, OR=2.022; 95% CI=1.101–3.714].</P> <P><B>Conclusions</B></P> <P>Our study shows that the recommended dose of prasugrel had significantly higher in-hospital bleeding complications without reducing ischemic events compared with clopidogrel. However, further large-scale, long-term, randomized clinical trials are required to accurately assess the efficacy and safety of prasgurel and to find out the optimal dose for Korean AMI patients.</P>

실내모형실험과 개별요소법을 이용한 지반 공동 및 이완영역 모사에 관한 연구

지반함몰의 발생은 하수관 파손으로 인한 토사유실이 주원인으로써 그 대책을 수립하기 위해서는 공동과 이완영역의 발생, 주변 지반의 거동을 이해하여야 할 필요성이 있다. 본 논문에서는 지중 하수관의 파손으로 인한 지반함몰 메커니즘 분석을 위해 실내모형실험과 개별요소 수치해석을 실시하였다. 실내모형실험에서는 알루미늄 봉과 트랩도어를 이용하여 모형지반의 거동을 모사하였고, 개별요소 수치해석은 모형실험과 동일한 경계조건으로 수행하여 그 결과를 모형실험 결과와 비교분석 하였다. 모형실험 및 개별요소 결과로부터 토사유실로 인한 공동과 이완영역의 형상 및 규모를 파악하였으며, 공동 주변 지반의 간극비 분포 특성 및 이완정도를 파악할 수 있었다. Ground subsidence mainly occurs due to the soil wash-away caused by cracked sewer pipes. It is necessary to understand the behavior surrounding soils with the formation of cavity and relaxation zone to set up counterplan. In this paper, a series of laboratory model tests and numerical analyses (Discrete Element Method) were performed to investigate the ground subsidence mechanism due to sewer pipe damage. For model tests, aluminum rod and trap door were used to simulate the behavior of model ground. Test results were compared with the numerical analyses conducted under the same boundary conditions with model tests. From this study, it was investigated the shape and size of cavity and relaxation zone due to the soil wash-away and a void ratio distribution of surrounding soils with relaxation properties.
마이크로 벤치-플로우 리액터를 이용한 LNT 촉매의 NO 흡장과 정화성능에 관한 연구

본 연구에서는 디젤엔진에서 배출되는 질소산화물의 저감을 위한 후처리장치인 LNT(Lean NOx Trap, 흡장형 De-NOx 촉매)의 특성을 파악하였다. 먼저 희박한 배출가스 상태에서의 질소산화물 중 산화질소에 대한 촉매의 기본적인 흡장성능을 알아본 후, 다양한 환원제를 분사하여 인위적으로 배출가스를 농후한 상태를 만들었다. 농후한 상태에서는 희박한 상태에서 촉매 내에 흡장되어 있던 산화질소가 촉매의 환원반응에 의해서 질소로 전환된 후 촉 매후단부에서 산화질소 배출농도를 측정하였다. 본 연구에서 사용된 LNT(Lean NOx Trap)시편은 실제 디젤 차량에 서 사용되는 LNT 촉매로부터 Reactor에 장착될 수 있도록 작은 사이즈로 절단 및 가공된 후, SUS304의 stainless 재질로 재가공 처리한 후에 Micro bench-flow reactor에 장착하였다. 분사된 피드가스성분들은 실제 배출가스의 분위기를 만들기 위해서 각각 3가지의 가열성분, 비가열 성분으로 나누어 분사된다. 이러한 조건들에서 다양한 반응온도와 공간속도를 반응변수로 하여 LNT(Lean NOx Trap)의 흡장성능과 환원제종류에 따른 산화질소의 배출특성을 파악하였다. We carry out an experimental investigation to analyze the basic performance of NO(nitric oxide) storage in a lean phase and also analyze the NO reduction achieved by the spraying of reducing agents in the rich phase of the exhaust gas in an LNT(Lean NOx Trap). This is an after-treatment system used to reduce the NOx emissions from a diesel engine. If the stored NO is reduced, we measure the outlet concentration downstream of the LNT. The test LNT material used in the experiments is commercial LNT. After being canned into stainless-steel (SUS304), it was built in a micro bench-flow reactor. Compositions of feed gases, three heated and three no heated gases were sprayed upstream of the LNT to analyze the characteristics. We use various temperatures and space velocities as response variables.

<P>The accumulation of extracellular matrix proteins is a common feature of fibrotic kidney diseases. Accumulating evidence suggests that TGF-beta and plasminogen activator inhibitor type 1 (PAI-1) promote the development of renal fibrosis by stimulating the generation and inhibiting the removal of matrix proteins. The small heterodimer partner (SHP) represses PAI-1 expression in the liver by inhibiting TGF-beta signaling, but whether SHP inhibits renal fibrosis is unknown. Here, unilateral ureteral obstruction (UUO) markedly increased the expression of PAI-1, type I collagen, and fibronectin but decreased SHP gene expression. Moreover, in kidneys of SHP-/- mice, the expression of PAI-1, type I collagen, fibronectin and alpha-smooth muscle actin (alpha-SMA) were higher compared with those in kidneys of wild-type mice. In addition, loss of SHP accelerated renal fibrosis after UUO. Adenovirus-mediated overexpression of SHP in cultured rat mesangial cells and renal tubular epithelial cells inhibited TGF-beta-stimulated expression of PAI-1, type I collagen, and fibronectin. SHP inhibited TGF-beta- and Smad3-stimulated PAI-1 promoter activities as well as TGF-beta-stimulated binding of Smad3 to its consensus response element on the PAI-1 promoter. Similarly, in vivo, adenovirus-mediated overexpression of SHP in the kidney inhibited the expression of UUO-induced PAI-1, type I collagen, fibronectin, and alpha-SMA. In summary, SHP attenuates renal fibrosis in obstructive nephropathy, making its pathway a possible therapeutic target for chronic kidney disease.</P>
