        콩고 민주 공화국의 ICT 보급 및 전자 정부 배치의 과제

        많은 개발 도상국이 전자 정부, 전자 상거래, 전자 건강 및 기타 계획을 포함한 중요한 정보 통신 기술(ICT) 프로젝트를 시작했다. 그러나 선진국과 달리 ICT의 보급은 개발도상국에서 빠르고 효과적이지 않았다. 보고된 바에 따르면, 구현된 대부분의 전자 이니셔티브는 의도한 목표를 달성하지 못하거나 중간에 중단되었다. 이 연구의 목적은 콩고 민주 공화국의 전자 정부 발전을 평가하고 글로벌 전자 정부 설문 조사의 성과를 포함하여 전자 서비스 제공 순위가 좋지 않은 원인을 검토하고 향후 순위를 개선하기 위한 솔루션을 제안한다. 이 분야의 여러 IT 전문가 및 정부 당국과 인터뷰를 진행했고, 이 주제에 대하여 유사한 문제를 겪고 있는 다른 개발도상국에도 도움이 될수 있도록 연구 결과의 범위를 일반화하고 확장하였다.

        eGIF4DRC: E-Government Interoperability Framework For DRC Based On Service Oriented Architecture(SOA)

        The evolution of the Internet and Web has led governments to reshape traditional public services and supply the faster, better and more responsive access to public services to citizens called e-Government. E-Government is not only the electronic interactions between citizens and government agencies but also the co-operations among independent government affiliated agencies where different systems exist. These cooperations raise research challenges of heterogeneous interoperating environment in e-Government. This paper will examine the e-Government aspects and then identify the requirements in order to propose a G2G and G2B interoperability architecture based on Service-Oriented Architecture (SOA) approach, the new trend of software architecture and distributed computing technology, along with technical standards, maturity model, and some key actions meant to make the initiative sustainable in the longer term. It is built on top of existing experiences in e-Government Interoperability Frameworks (eGIFs) of some countries across the globe and it addresses some specific peculiarities and needs of developing countries, like DRC.

      • F-35 Predictors of mortality in Patients with Rheumatoid Arthritis-Associated Interstitial Lung Disease

        Background: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) has variable clinical course and poor prognosis; however, risk factors of them are not well defined. Methods: Patients diagnosed with RA-ILD (n=158; surgical lung biopsy, n=34) from May 1995 to July 2017 at Asan Medical Center were included and clinical data and high resolution CT (HRCT) scan images retrospectively analyzed. HRCT pattern was categorized as usual interstitial pneumonia (UIP) or non-UIP pattern, and the extent of emphysema, GGO, reticulation, consolidation, traction bronchiectasis and honeycombing was semiquantitatively scored. Fibrosis index was defined as sum of reticulation and honeycombing scores. Results: Median follow-up period was 54 months. Mean age was 60.9 year, 42.4% were male, 43.0% were smokers, and 41.8% showed UIP pattern on HRCT, respectively. 42.4% of patients died during follow-up. Non-survivors were older (64.0 vs. 58.6 years) and had more male (52.2 vs. 35.2%), ever smoker (55.2 vs. 34.1%), lower FVC (71.6 vs. 78.2% predicted) and DLco (56.9 vs. 64.9% predicted), and more UIP pattern (59.7 vs. 28.6%) and higher fibrosis index (27.0 vs. 13.5%) compared to survivors. In univariate Cox analysis, age, gender, smoking history, rheumatoid factor titer, FVC, DLco, UIP pattern and fibrosis index were significant; however, in the multivariate analysis, age (HR, 1.074), UIP pattern (HR, 1.944) and fibrosis index (HR, 1.053) were only significant. Conclusions: Older age, UIP pattern and higher fibrosis index on HRCT could predict poor prognosis in RA-ILD.

        <P><B>Purpose</B></P><P>To describe the stroke-related Twitter network through analysis of the #Stroke hashtag.</P><P><B>Materials and methods</B></P><P>621 653 tweets containing the #Stroke hashtag were analyzed from 20 March 2012 to 31 January 2018. Twitter activity metrics, engagement, user characteristics, content analysis, and network analysis were obtained using the healthcare social media analytics platform, Symplur Signals.</P><P><B>Results</B></P><P>The number of users, the number of impressions, and the number of tweets containing the #Stroke hashtag increased by an annual average of 64.9%, 87.7%, and 89.2% over the past 6 years from 20 March 2012 to 31January 2018. 69 371 tweets (11.2%) contained novel content and 48 568 tweets (7.8%) related to patient care. 181 120 (29.1%) tweets contained at least one image and 436 132 tweets (70.2%) contained links to outside resources. Stroke prevention, diabetes, and atrial fibrillation were commonly discussed topics. With regard to engagement, there were 259 438 retweets (41.7%), 366 561 mentions (59.0%), and 8549 replies (1.4%). Physicians and patients authored 52 197 (8.4%) and 41 822 (6.7%) tweets, respectively. Advocate organizations, patients, and non-healthcare individuals most frequently used the #Stroke hashtag on Twitter.</P><P><B>Conclusion</B></P><P>The use of the #Stroke hashtag on Twitter has grown significantly over the 6-year study period. The majority of the discussions were held between stroke support groups and non-healthcare-related individuals, with discussion content centering around stroke prevention, stroke symptoms, associated medical conditions, and treatment options.</P>

      • Prognostic role of blood KL-6 in rheumatoid arthritis-associated interstitial lung disease

        Background: Rheumatoid arthritis-associated interstitial lung disease (RA-ILD) has a variable clinical course for which predicting prognosis is difficult. However, the role of blood biomarkers in RA-ILD is ill-defined. The aim of this study was to investigate the prognostic value of Krebs von den Lungen-6 (KL-6) levels in RA-ILD patients. Methods: The medical records of 84 patients with RA-ILD were retrospectively reviewed. Plasma KL-6 levels were measured by Nanopia KL-6 assay (SEKISUI MEDICAL, Tokyo), using latex-enhanced immunoturbidimetric assay. Results: The median follow-up period was 69 months. Mean age was 61.4 years, 45.2% were men, 44.0% were ever-smokers, and 35.7% showed a usual interstitial pneumonia (UIP) pattern on high-resolution computed tomography. The median KL-6 level at baseline was 741.2 U/mL (interquartile range, 439.7-1308.9 U/mL). On multivariate logistic regression analysis, a high KL-6 level (≥ 640 U/mL) was an independent predictor of a UIP pattern (odds ratio [OR], 5.906; P=0.004) with old age (OR, 1.129, P=0.002). On multivariate Cox analysis, a high KL-6 level (≥ 685 U/mL) was independently associated with mortality (hazard ratio [HR], 2.984; P=0.016) with a UIP pattern (HR, 4.034; P=0.002), male sex (HR, 3.610; P=0.001), old age (HR, 1.061; P=0.030), and low forced vital capacity (HR, 0.957; P=0.002). Conclusions: Our results suggest that KL-6 levels may be useful for predicting a UIP pattern and prognosis in patients with RA-ILD.

        Burkholderia thailandensis outer membrane vesicles exert antimicrobial activity against drug-resistant and competitor microbial species

        Gram-negative bacteria secrete outer membrane vesicles (OMVs) that play critical roles in intraspecies, interspecies, and bacteria-environment interactions. Some OMVs, such as those produced by Pseudomonas aeruginosa, have previously been shown to possess antimicrobial activity against competitor species. In the current study, we demonstrate that OMVs from Burkholderia thailandensis inhibit the growth of drug-sensitive and drug-resistant bacteria and fungi. We show that a number of antimicrobial compounds, including peptidoglycan hydrolases, 4-hydroxy-3-methyl-2-(2-nonenyl)- quinoline (HMNQ) and long-chain rhamnolipid are present in or tightly associate with B. thailandensis OMVs. Furthermore, we demonstrate that HMNQ and rhamnolipid possess antimicrobial and antibiofilm properties against methicillin- resistant Staphylococcus aureus (MRSA). These findings indicate that B. thailandensis secretes antimicrobial OMVs that may impart a survival advantage by eliminating competition. In addition, bacterial OMVs may represent an untapped resource of novel therapeutics effective against biofilm- forming and multidrug-resistant organisms.

        Performance of the Achutha Menon Centre Diabetes Risk Score in Identifying Prevalent Diabetes in Tamil Nadu, India

        Background: The Achutha Menon Centre Diabetes Risk Score (AMCDRS), which was developed in rural Kerala State, South India, had not previously been externally validated. We examined the performance of the AMCDRS in urban and rural areas in the district of Vellore in the South Indian state of Tamil Nadu, and compared it with other diabetes risk scores developed from India. Methods: We used the data from 4,896 participants (30 to 64 years) of a cross-sectional study conducted in Vellore (2010 to 2012), to calculate the AMCDRS scores using age, family history, and waist circumference. Sensitivity, specificity, positive predictive value (PPV), and negative predictive values (NPV), and the area under the receiver operating characteristic curve (AROC) were calculated for undiagnosed and total diabetes. Results: Of the 4,896 individuals surveyed, 274 (5.6%) had undiagnosed diabetes and 759 (15.5%) had total diabetes. The AMCDRS, with an optimum cut-point of ≥4, identified 45.0% for further testing with 59.5% sensitivity, 60.5% specificity, 9.1% PPV, 95.8% NPV, and an AROC of 0.639 (95% confidence interval [CI], 0.608 to 0.670) for undiagnosed diabetes. The corresponding figures for total diabetes were 75.1%, 60.5%, 25.9%, 93.0%, and 0.731 (95% CI, 0.713 to 0.750), respectively. The AROC for the AMCDRS was not significantly different from that of the Indian Diabetes Risk Score, the Ramachandran or the Chaturvedi risk scores for total diabetes, but was significantly lower than the AROC of the Chaturvedi score for undiagnosed diabetes. Conclusion: The AMCDRS is a simple diabetes risk score that can be used to screen for undiagnosed and total diabetes in lowresource primary care settings in India. However, it probably requires recalibration to improve its performance for undiagnosed diabetes.

        Aptamer-targeted Antigen Delivery

        <P>Effective therapeutic vaccines often require activation of T cell-mediated immunity. Robust T cell activation, including CD8 T cell responses, can be achieved using antibodies or antibody fragments to direct antigens of interest to professional antigen presenting cells. This approach represents an important advance in enhancing vaccine efficacy. Nucleic acid aptamers present a promising alternative to protein-based targeting approaches. We have selected aptamers that specifically bind the murine receptor, DEC205, a C-type lectin expressed predominantly on the surface of CD8α<SUP>+</SUP> dendritic cells (DCs) that has been shown to be efficient at facilitating antigen crosspresentation and subsequent CD8<SUP>+</SUP> T cell activation. Using a minimized aptamer conjugated to the model antigen ovalbumin (OVA), DEC205-targeted antigen crosspresentation was verified <I>in vitro</I> and <I>in vivo</I> by proliferation and cytokine production by primary murine CD8<SUP>+</SUP> T cells expressing a T cell receptor specific for the major histocompatibility complex (MHC) I-restricted OVA<SUB>257–264</SUB> peptide SIINFEKL. Compared with a nonspecific ribonucleic acid (RNA) of similar length, DEC205 aptamer-OVA-mediated antigen delivery stimulated strong proliferation and production of interferon (IFN)-γ and interleukin (IL)-2. The immune responses elicited by aptamer-OVA conjugates were sufficient to inhibit the growth of established OVA-expressing B16 tumor cells. Our results demonstrate a new application of aptamer technology for the development of effective T cell-mediated vaccines.</P>

        Midterm outcomes of suture anchor fixation for displaced olecranon fractures

        Background: Displaced olecranon fractures constitute a challenging problem for elbow surgeons. The purpose of this study is to evaluate the role of suture anchor fixation for treating patients with displaced olecranon fractures. Methods: A retrospective review was performed for all consecutive patients with displaced olecranon fractures treated with suture anchor fixation with at least 2 years of clinical follow-up. Surgical repair was performed acutely in all cases with nonmetallic suture anchors in a double-row configuration utilizing suture augmentation via the triceps tendon. Osseous union and perioperative complications were uniformly assessed. Results: Suture anchor fixation was performed on 17 patients with displaced olecranon fractures. Functional outcome scores were collected from 12 patients (70.6%). The mean age at the time of surgery was 65.6 years, and the mean follow-up was 5.6 years. Sixteen of 17 patients (94%) achieved osseous union in an acceptable position. No hardware-related complications or fixation failure occurred. Mean postoperative shortened disabilities of the arm, shoulder, and hand (QuickDASH) score was 3.8±6.9, and mean Oxford Elbow Score was 47.5±1.0, with nine patients (75%) achieving a perfect score. Conclusions: Suture anchor fixation of displaced olecranon fractures resulted in excellent midterm functional outcomes. Additionally, this technique resulted in high rates of osseous union without any hardware-related complications or fixation failures. Level of evidence: IV.
