        Association between Postoperative Neck Pain and Intraoperative Transcranial Motor-Evoked Potential Waveforms of the Trapezius Muscles in Patients with Cervical Myelopathy Who Underwent Cervical Laminoplasty

        Study Design: Retrospective study.Purpose: Cervical laminoplasty is safe and effective for treating cervical myelopathy but has a higher frequency of postoperative axial pain compared to other methods. Several studies have reported on the causes of postoperative axial pain, but none have fully elucidated them. This study aimed to investigate the association between postoperative neck pain and intraoperative transcranial motor-evoked potential (MEP) waveforms of the trapezius muscles using transcranial MEPs.Overview of Literature: Few studies have investigated the association between postoperative neck pain and intraoperative transcranial MEP waveforms of the trapezius muscles in patients with cervical laminoplasty.Methods: A total of 79 patients with cervical myelopathy who underwent cervical laminoplasty at our facility between June 2010 and March 2013 were included in this study. Intraoperative control and final waveform were evaluated based on the trapezius muscle MEPs by measuring the latency and amplitude. A neck pain group comprised patients with higher neck pain Visual Analog Scale scores from preoperative value to 1 year postoperatively. The cross-sectional areas of the trapezius muscles and the MEP latencies and amplitudes were compared between patients with and without neck pain.Results: The latency and amplitude of the control waveforms were not significantly different between groups. The neck pain group had a significantly shorter final waveform latency (neck pain: 23.6±2.5, no neck pain: 25.8±4.5; p =0.019) and significantly larger amplitude (neck pain: 2,125±1,077, no neck pain: 1,630±966; p =0.041) than the no neck pain group.Conclusions: Postoperative neck pain was associated with the final waveform latency and amplitude of the trapezius muscle MEPs during cervical laminoplasty. Intraoperative electrophysiological trapezius muscle abnormalities could cause postoperative neck pain.

      • Study of Mild-Stall Wings with a Fixed Slat

        Much attention has been paid to PAY (Personal Air Vehicle) as a new-generation small aircraft, It is expected that PAY will be widely used as a short-range transport aircraft in the future for countermeasure against traffic jam, One of the key technologies to make PAY much popular is to enhance the safety, which is crucial to small planes, In order to enhance the safety of PAY, mild-stall high-lift characteristics of the wing are especially needed. This study aims to propose a mild-stall wing with a fixed slat for PAY use, Two mild-stall airfoils are combined to form a new rectangular wing: NACA4415 and SLAT4415, NACA4415 is known as a conventional mild-stall airfoil, and SLAT4415 is a fixed slat airfoil based on NACA4415 for further improvement in stall characteristic, The higher lift characteristic ofSLAT4415 was obtained via the optimization by Genetic Algorithm. Designed wings composed ofNAC4415 and SLAT4415 airfoils are numerically analyzed by solving the Navier-Stokes equations on the unstructured mesh to examine the performance at high-angle of attack for low-speed region, The computational results indicate that designed wings are able to achieve mild-stall characteristics, Lift coefficients of the designed wings tend to drop at 22 degrees of A.o.A, however, increase again as A.o.A becomes higher, For further analysis, numerical simulations for the wing that has a taper or that has only SLAT4415 airfoil were also conducted, As a result, it was proved that the designed rectangular wing that has SLAT4415 airfoil of fixed slat only in outboard wing had very mild stall characteristic.

      • Photo-controlled gene expression by fluorescein-labeled antisense oligonucleotides in combination with visible light irradiation

        A new concept of "photo" -antisense method has been evaluated, where the inhibition of gene expression by the conventional antisense method is enhanced by photochemical binding between antisense oligonucleotides conjugated with photo-reactive compound and target mRNA or DNA. Fluorescein labeled oligodeoxyribonucleotides (F-DNA) was delivered to cell nuclei in the encapsulated form in multilamellar lecithin liposomes with neutral charge. F-DNA was previously shown to photo-bind to the complementary stranded DNA, and the delivery system using neutral liposome to be effective in normal human keratinocytes. In the present study, we used human kidney cancer G401.2/6TG.1 cell line to be advantageous in reproducible experiments. p53 was adopted as a target gene since antisense sequence information has been accumulated. The nuclear localization ofF-DNA was identified by comparing the fluorescence ofF-DNA with that of Hoechst 33258 under fluorescence microscope. After 7hr incubation to accumulate p53 protein induced by UV -B, p53 protein was quantified by Western blot. After 2hrs from F-DNA application, about 30% of cell population incorporated F-DNA in their nuclei with some morphological change possibly due to liposomal toxicity. Irradiation of visible light longer than 400nm from solar simulator at this time enhanced the inhibitory action of antisense F-DNA. The present results suggest that photo-antisense method is promising to control gene expression in time and space dependent manner. Further improvement of F-DNA delivery to cancer cells in the stability and toxicity is in progress. progress.

      • Evaluation of the Association between Neck Pain and the Trapezius Muscles in Patients with Cervical Myelopathy Using Motor Evoked Potential: A Retrospective Study

        Study Design: Retrospective study. Purpose: We aimed to use motor evoked potentials (MEPs) to examine the association of electrophysiological assessment of the trapezius muscle with neck pain. Overview of Literature: Previous reports on the association of neck pain with the trapezius muscle have focused on surface electromyograms and muscle oxygenation; however, to our knowledge, none of these studies included detailed data on MEPs. Methods: The study included 100 patients with cervical myelopathy who underwent surgery at the National Center for Geriatrics and Gerontology in Obu, Japan from June 2010 to March 2013. Before the surgery, neck pain was evaluated using a Visual Analog Scale (a score ≥50 indicated neck pain and a score <50 indicated no neck pain). The preoperative cross-sectional areas of the trapezius muscles were measured with cervical magnetic resonance imaging sagittal T2-weighted images. Cranial stimulation under general anesthesia was used to derive the MEPs, enabling the measurement of latency and amplitude, using preoperative MEPs of the trapezius muscles. Results: The MEP of the trapezius muscle in patients with neck pain had significantly shorter latencies than those in patients who did not have neck pain. However, there was no significant difference in the amplitude between patients with and without neck pain. However, this tended to be greater in patients with neck pain as compared to that in those without neck pain. The cross-sectional area of the trapezius muscle in patients with neck pain was significantly smaller than that in those who did not have neck pain. Conclusions: MEPs revealed electrophysiological abnormalities of the trapezius muscles in patients with neck pain, supporting a relationship of neck pain with the trapezius muscles.

      • Absorption Spectroscopy of Biological Specimens Near X-ray Absorption Edges of Constituent Elements

        Absorption spectra of biological specimens in the soft X-ray region have been presented with special reference to the XANES (X-ray absorption Near Edge Structure) of constituent elements. Absorption spectrum in this wavelength region is characterized by the absorption edges from which elemental content could be derived. In addition, XANES has a characteristic profile for chemical environment around the element such as chemical bond. Using the specific absorption peak we can assign not only the chemical bond but also molecules having such a chemical bond. In the present paper, absorption spectrum of DNA was measured in the wavelength range from 1.5nm to 5nm. Spectrum of Chinese Hamster Ovary (CHO) cells was compared with the DNA spectrum. XANES were distinct at the K absorption edges of major elements, C, N and O. In the spectrum of the cells prominent peaks at the L absorption edge of minor element Ca were also detectable. XANES profiles in small local areas in a cell could also be measured in combination with X-ray microscopy. These give information about local chemical environment in a cell. XANES at the phosphorus K absorption edge in a human HeLa cell was successfully obtained corresponding to a sharp and intensive XANES peak of DNA.

        Therapeutic Endoscopic Treatment Plus Maintenance Dimethyl Sulfoxide Therapy Prolongs Recurrence-Free Time in Patients With Hunner Type Interstitial Cystitis: A Pilot Study

        Purpose: To evaluate whether hydrodistention with fulguration of Hunner lesions (HD/FUL) plus maintenance dimethyl sulfoxide (DMSO) therapy prolongs the recurrence-free time in patients with Hunner type interstitial cystitis (IC). Methods: The study enrolled patients with Hunner type IC who required repeat HD/FUL due to recurrence of IC symptoms after the first HD/FUL at our institution. All patients received a second HD/FUL plus maintenance DMSO therapy. The maintenance DMSO therapy was performed every 2 weeks for a total of 8 instillations, and then once every 4 weeks thereafter. The recurrencefree time from HD/FUL to therapeutic failure was estimated using the Kaplan-Meier method. The recurrence-free time between the first HD/FUL and second HD/FUL plus maintenance DMSO therapy was statistically compared using the log-rank test. Results: A total of 21 patients (mean age, 66.3±10.8 years) with Hunner type IC were evaluated. The recurrence-free time for the second HD/FUL plus maintenance DMSO therapy was significantly longer than that for the first HD/FUL (P<0.0001). The median recurrence-free time for the first HD/FUL was 10.1 months, while that for the second HD/FUL plus maintenance DMSO therapy has yet to be reached. The recurrence-free rate for the first HD/FUL was 81.0% at 6 months, 38.1% at 1 year, 9.5% at 2 years, and 4.8% at 3 years. In contrast, the rate for the second HD/FUL plus maintenance DMSO therapy was 100% at 6 months, 94.7% at 1 year, 82.6% at 2 years, and 82.6% at 3 years. There were no significant differences in efficacy between the first and second HD/FUL. Conclusions: HD/FUL plus maintenance DMSO therapy clearly prolongs the recurrence-free time compared with HD/FUL alone in Hunner type IC.

      • Effect of alendronate on muscle mass: Investigation in patients with osteoporosis

        Objectives: Many osteoporosis drugs reliably increase bone mass in the elderly; if these drugs also had a positive effect on muscle, their benefit would be even greater. We examined the effect of alendronate monotherapy on muscle mass in patients with osteoporosis. Methods: In this retrospective cohort, case-control study, patients from an osteoporosis database were divided into 2 groups: alendronate-treated patients (group A; n ¼ 199) and a control group receiving no drug treatment (group C; n ¼ 233). Appendicular skeletal muscle mass (ASM) and skeletal muscle mass index (SMI) measured by dual-energy X-ray absorptiometry were assessed at approximately 1 year. The change in muscle mass was compared between the groups. Results: At baseline, group A included more women and had lower height, weight, bone mineral content, and muscle mass than group C. A comparison of changes after 1 yeardadjusted for age, sex, observation period, body mass index and initial valuesdrevealed that the muscle mass in group A showed increases by 0.137 kg/m2 in SMI, 514 g in ASM, and 319 g in lower limb muscle mass (LLM). Group C showed no changes in muscle mass. A significant difference in the amount of change in ASM and LLM was found between the groups after adjustment: 2.5 times and 4.4 times higher, respectively, in groups A and C. However, the difference in SMI disappeared after adjustment. Conclusions: This is the first study to show that alendronate may have a positive effect not only on bone, but on muscle as well.

      • Modeling of Quantum Computer by using Quantum Petri Net

        Petri Net that is an effective tool for concurrent systems, has been extended to Quantum Petri Net. The conventional modeling method of quantum computer using Quantum Petri Net, have several drawbacks. This paper defines new model and examines conditions to solve these drawbacks, and proposes new model from modeling quantum Turing machine, and shows that the model can simulate more general calculation processes than which the conventional model.
