Impact of Guidelines on Clinical Practice : Intravenous Heparin Use for Acute Ischemic Stroke

<P>Background and Purpose-Since its introduction, controversy has existed about the administration of intravenous heparin for the treatment of acute ischemic stroke. We studied trends in the intravenous heparin use during a 6-year time period and the potential influence of clinical guidelines in national language on intravenous heparin administration in Korea. Methods-On the basis of a prospective nationwide multicenter stroke registry, we collected data on patients with acute ischemic stroke who arrived within 7 days of symptom onset during the time period 2008 to 2013. We studied patient demographics, prestroke medical history, stroke characteristics, and stroke treatment. Data from a total of 23 425 patients from 12 university hospitals or regional stroke centers were analyzed. Results-The administration of intravenous heparin steadily decreased throughout the study period: 9.7% in 2008, 10.9% in 2009, 9.4% in 2010, 6.0% in 2011, 4.7% in 2012, and 4.3% in 2013 (P for trend < 0.001). The reduced intravenous heparin use was associated with moderate stroke severity, atrial fibrillation, and stroke of cardioembolic, other-, and undetermined etiology. In a multivariable logistic model, increase of 1 calendar year (odds ratio, 0.89; 95% confidence interval, 0.84-0.95; P < 0.001) and release of clinical practice guidelines in Korean (odd ratio, 0.74; 95% confidence interval, 0.59-0.91; P < 0.01) were independent factors associated with reduction in the frequency of intravenous heparin use. Conclusions-Use of intravenous heparin for acute ischemic stroke treatment has decreased in Korea, and this change may be attributable to the spread and successful implementation of regional clinical practice guidelines.</P>

The experiment was conducted to get basic information for cotton breeding program through seven-variety diallel cross population. Significant variance analysis was observed in days to flowering, days to maturity, plant height, length of stem internode, bolls per plant, boll weight, seeds per boll, lint weight per boll, 100-seed weight, staple length, length of seed, width of seed and lint percentage. Based on the Vr-Wr graphical analysis, the characters, days to flowering, bolls per plant, seeds per boll, lint weight per boll, 100-seed weight, staple length, length of seed and width of seed were found to inherit incomplete dominance, and the characters of days to maturity, plant height, length of stem internode, boll weight and lint percentage were assumed to inherit over dominance. The component of genetic variance analysis for days to flowering, lint weight per boll, 100-seed weight, staple length, length of seed, width of seed and lint percentage showed that additive effects were higher than dominant effects. The narrow-sense heritability(h²ns) for days to flowering, bolls per plant, boll weight, seeds per boll, lint weight per boll, 100-seed weight, staple length and length of seed showed high values as more than 0.41. The estimate of broad-sense heritability(h²bs) value was higher than that of h²ns because of the low importance of dominance effects.
Fluoride uptake effect by the iontophoresis device on tooth enamel

Objectives: The authors have experimented the micro volume of the fluoride concentration on the tooth surface with EPMA and EDS, in order to suggest the proper method of fluoride topical application in clinical. Methods: 38 samples from 19 molar teeth extracted, were selected such conditions as the simple application with 2.0% NaF, iontophoresis method, for 4 minutes for 16 times, and 0.2% NaF for 80 or 160 times, were given to each experimental groups, and compared the EPMA photos and the atomic percentages checked with EDS data with the control groups, as no-application of fluoride or simple application of 2.0% NaF or 0.2%. Results: There were no evidence of fluoride invasion on tooth enamel and DEJ by simple application of 2.0% NaF application for 16 times and it was the similar phenomena to the control group as no-fluoried treatment, by EPMA findings. Fluoride component was found at the DEJ area of the tooth enamel on experimental group 2.0% NaF application for 4 minutes for 16 times with iontophoresis method, by EPMA findings. Otherwise there were no remarkable evidence for fluoride invasion on 0.2% NaF application for 4 minutes for 80 or 160 times with iontophoresis method, by EPMA findings. Both of the cses to use the professional fluoride iontophoresis kit and the self unit, relative atomic percentage of fluoride were more than those of the cases on the control group as simple application without use of the iontophoresis kits. Conclusion: It was recommended that to use the iontophoresis kit were available and effective to increase the more fluoride invasion, both in the professional kit and the self unit.

<P>The effects of capsaicin (CAP), a transient receptor potential vanilloid subtype 1 (TRPV1) agonist, were determined on nigrostriatal dopamine (DA) neurons in the 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) mouse model of Parkinson's disease (PD). The results showed that TRPV1 activation by CAP rescued nigrostriatal DA neurons, enhanced striatal DA functions and improved behavioral recovery in MPTP-treated mice. CAP neuroprotection was associated with reduced expression of proinflammatory cytokines (tumor necrosis factor-α and interleukin-1β) and reactive oxygen species/reactive nitrogen species from activated microglia-derived NADPH oxidase, inducible nitric oxide synthase or reactive astrocyte-derived myeloidperoxidase. These beneficial effects of CAP were reversed by treatment with the TRPV1 antagonists capsazepine and iodo-resiniferatoxin, indicating TRPV1 involvement. This study demonstrates that TRPV1 activation by CAP protects nigrostriatal DA neurons via inhibition of glial activation-mediated oxidative stress and neuroinflammation in the MPTP mouse model of PD. These results suggest that CAP and its analogs may be beneficial therapeutic agents for the treatment of PD and other neurodegenerative disorders that are associated with neuroinflammation and glial activation-derived oxidative damage.</P>
Air Pollution Is Associated With Ischemic Stroke via Cardiogenic Embolism

<P>Conclusions-Our findings suggest that the short-term exposure to air pollutants is associated with cardioembolic stroke, and greater care should be taken for those susceptible to cerebral embolism during peak pollution periods. Public and environmental health policies to reduce air pollution could help slow down global increasing trends of cardioembolic stroke.</P>

Human embryonic stem cells (hESCs) have the potential to differentiate into various cell types, and the three germ layers in vivo and in vitro. They are therefore useful in transplantation and tissue engineering. Here, we describe the expression patterns of selected steroid receptor mRNAs - estrogen receptor-α (ER-α), ER-β, glucocorticoid receptor (GR), and progesterone receptor (PR) - in undifferentiated hESCs and embryoid bodies (EBs) cultured for 2, 4, and 6 d, as assessed by real-time PCR, in order to define the possible influence of steroid hormones on the differentiation of hESCs. These receptor mRNAs were expressed in undifferentiated hESCs and EBs. The expression of PR mRNA only decreased during the differentiation of EBs but not of hESCs. Immunohistochemical analysis gave strong staining of ER-α, ER-β, and GR proteins in the nuclei of hESCs and EBs, whereas PR was not detected. We also examined the potential of these steroid hormones to direct the differentiation of hESCs in vitro. The expression of 11 cell-specific markers representing 3 germ layers and 5 tissue types was used to assess the differentiation of hESCs. We found that certain endodermal marker genes were either only expressed in the estrogen-treated group or their expression was stimulated in that group, suggesting that steroid hormones can control the differentiation of hESCs into various cell types.

An endophytic bacterial strain YC5480 producing antifungal and phytotoxic compounds simultaneously was isolated from the surface sterilized root of Artemisia sp. collected at Jinju area, Korea. The bacterial strain was identified as a species of Pseudomonas brassicacearum based on its 16S rRNA gene sequence analysis and physiological and biochemical characteristics. The seed germination and growth of monocot and dicot plants were inhibited by culture filtrate (1/10-strength Tryptic Soy Broth) of the strain. The germination rate of radish seeds in the culture filtrate differed in various culture media. Only 20% of radish seeds germinated in the culture media of 1/2 TSB for 5 days incubation. Mycelial growth of fungal pathogens, Colletotrichum gloeosporioides, Fusarium oxysporum and Phytophthora capsici was also inhibited by the culture filtrate of the strain YC5480. An antifungal compound, KS-1 with slight inhibitory activity of radish seed germination at 1,000 ppm and a seed germination inhibitory compound, KS-2 without suppression of fungal growth were produced simultaneously in TSB. The compounds KS-1 and KS-2 were identified to be 2,4-diacetylphloroglucinol (DAPG) and 2,4,6-trihydroxyacetophenone (THA), respectively.

An infiltration was realized as an important part of ventilation to change fresh air with contaminated air in a house. In this study, it was measured the infiltration in new apartment buildings according to Seasons, height etc. We measured the infiltration by using tracer gas method and did for 24hours. To quantify the infiltration, Sulfur Hexafluoride(SF?) gas was used. The Constant Concentration was conducted during experimental process and the concentration of the gas was measured with a gas monitor. In the results of the measurement, the infiltration were appeared over 0.6 ACH in winter and about 0.2 ACH in summer and autumn. Also the infiltration were measured more highly at high and low stories than middle stories in winter.

표준치료법인 전 폐하세척술에서 효과를 보지 못했던 폐포단백증 환자에서 GM-CSF 흡입법을 이용하여 임상적으로 증상, 폐기능, 흉부단층촬영에서 호전을 보인 증례를 보고하고자 한다. Pulmonary alveolar proteinosis (PAP) is a rare condition that is treated using whole lung lavage. A recent study suggested that granulocyte-macrophage colony stimulating factor (GM-CSF) plays roles in both the pathogenesis and treatment of PAP. We present a 69-year-old man with PAP who deteriorated despite bilateral whole lung lavage; that said, his symptoms, chest X-ray findings, and pulmonary function test improved after GM-CSF inhalation therapy over 12 months. GM-CSF therapy is an effective treatment modality for PAP.
