        稀有 顔面裂에 對한 臨床的 考察

        In classifying the congenital clefts of the face and palate, various principles have been in use, i.e. embryologocal, geneticl, morphological, etc. from a practical point of view, a simple clssification satisfying the embryological, clinical, as well as genetical requirement: Group Ⅰ ; primary palate, Group Ⅱ; primary and secondary palate, Group Ⅲ; secondary palate alone and Group Ⅳ; Rare clefts. Group Ⅳ; the rare, atypical clefts, stand numerically for a small part of the complete material, even of large series of clefts. Fogh-Anderson reported in his monograph?? on 48 patients within this category out of a series of 3988 cases of facial clefts during 30 yeare. Burial repoted on 97 patient out of nearly 4000 cases of facial clefts during 40 year, i.e. well over 2%. Authors reported on 7 cases of rare, atypical cleft deformities out a hospital materials off 496 cases of facial clefts during 10 years. A survey will be given of this materials of rare facial clefts and their characteristics, as regards etiology and its treatment.

        Multiple Intelligences and Christian Education

        Accepted widely by psychologists and educators and applied in a variety of ways in educational fields, the theory of multiple intelligences proposes a different understanding of intelligence. Howard Gardner has defined intelligence as the capacity to solve problems or to fashion products that are valued in one or more cultural settings. Based on his definition of intelligence, Gardner has described various intelligences through which individuals learn and teach information. The theory of multiple intelligences has strong implications for Christian education. This paper has attempted to provide a brief review of multiple intelligences connected with Gardner’s theory and its implications for Christian teaching and the learning process.

        Hcparin이 백서 피부판에 미치는 효과에 대한 실험적 연구

        In recent years many attempts have been made clinically and experimentally to improve the blood supply in ischemic random pattern flaps. Maintaining the blood supply present in the flap is very important for he flap survival. thrombus formation is one of the major determinate factors responsible for failure of a constructed skin ffap survival. Many authors suggested the factors that evoke thrombosis in acute skin flap; Vasoconstriction by sympathetic nerve stimulation(Palmer, 1970), erythrocyte aggregating due to sluggish flow in the microvasculature (Wells, 1964), prostaglandin cause vasconstriction and platelet aggregation (Gordon, 1967), and increased platelet adhesiveness after operation (Ham, 1957). But, it is not clear which plays major role in thrombus formation, it may be all togather or with others. This experiments were designed to clarify that effectiveness of heparin administrated various time various time on survival of random pattern flaps in rats. They revealed the following results; 1. The survival of flaps of rats treated with systemic administration of heparin (100 U/kg) at both immediate and 6ths postoperation was significantly improved comparing with the control animals. 2. But, there was little or no improvement in the flap survival of the groups treated with heparin 12 hrs postoperation and there after. 3. No side effects of heparin administration (100 U/kg, intraperitoneal) were noticed in this study, although the blood clotting time was prolonged 6 minutes twice long as normal.

        한국인 심상완동맥 요골측부지에 관한 해부학적 연구

        In the field of reconstructive surgery, the skin flap has played a major role and is continuously developing. The free flap transfer has been used easily with the advent of microvascular surgery. The reconstruction of hand or foot require particular prerequisites such as thin and even flap containing nerve and bone. Since Song(1982) had reported the forearm and upper arm flap, a few clinical experience on upper arm flap has been reported. The upper arm flap was suggested as one of better flap donor sites which satisfies the above mentioned requirements. However, neither the general anatomy textbooks mention about the distribution of upper arm blood vessel in detail nor any domestic reports are available. We observed and analysed the origin and pathway of the deep brachial artery, radial collateral branch(R.C.B.) of deep brachial artery, vena comitans of R.C.B., their diameters at various points, skin flap territories upon dye injection in R.C.B., relationship between R.C.B. and radial nerve, pathway of posterior upper arm cutaneous nerve, by the dissection of 27 adult Korean cavaders(25 : bilateral, 2 : unilateral) and the distance between lateral intermuscular septum and lateral epicondyle was measured from 30 male Korean doctors of Severance Hospital. The results were summerized as follow: 1. Deep brachial artery arose alone from brachial artery in 27 cases(52%), arose from the brachial artery with the superior ulnar collateral branch in 16 cases(30%) and directly from axillrya a. in 4 cases(8%). The diameter at the origin of deep brachial artery was 2.2±0.37mm in average. (range 1.5∼2.7mm) 2. Deep brachial artery wound posteriorly in the spiral groove of the humerus deep to triceps muscle and entered the depths of the lateral intermuscular septum between the deltoid insertion anteriorly and the triceps muscle posteriorly. Radial collateral branch of deep brachial artery proceeded distally in the lateral intermuscular septum between triceps posteriorly and brachialis and brachioradialis muscle anteriorly. 3. The diameters at its pathway of R.C.B. were 1.5±0.27mm(proximal) 1.0±0.29mm(middle), 0.75±0.15mm(distal) in average. 4. The diameter of R.C.B. was bigger in male than in female and right side was also bigger than left. 5. The number of cutaneous branch of R.C.B. was 3.6±0.6 in average.(range3∼5) 6. Diameters of vena comitans of R.C.B. was 1.7±0.20mm(range: 1.2∼2.0mm) 7. Posterior cutaneous nerve of upper arm and foream runs along the R.C.B. but only 34 cases(65%) distributed upper arm exclusively. 8. The cutaneous territory after 2% methylene blue injection in R.C.B. was 8×12cm and long axis of territory was parallel to axis of limb. 9. The distance between ateral intermuscular septum and latleral epicondyle in living adult male was 13.2±0.42cm. (range: 12.5∼14.5cm) 10. Considering above results, the introduction of lateral upper arm flap based on R.C.B. may be an useful adjunct for reconstruction of hand or foot.

        부정유합된 안면골 골절의 임상례

        This series comprised 4 patients with malunited facial bone fractures treated in the department of Plastic and Reconstructive Surgery at the Yonsei University Medical Center. Generally, it might be stated that acute fractures of the facial bones are not a surgical emergency. But it is desirable to institute definitive as soon as possible after injury but not before the general condition of the patient warrants. The management of deformity due to malunited facial bone fracture are discussd. The malunion can be managed by osteotomy and realignment of the fragments. The postoperative results are parallel to severity of trauma and duration from injury to management.

        안면부 박피술에 대한 임상적 고찰

        During the past 3 decades, certain cosmetic defect and diseases of the skin have been treated by highly technical procedure of dermabrasion, also known as surgical planing or wire brush surgery. Dermabrasion is an office procedures in which various thickness of skin are planed to depth necessary for removal of defects by use of highspeed, rotary, abrasive instruments. Within seven to ten days, healing without scarring occurs beneath a crust in an "open-to-air" manner. Kurtin first described this technique in 1953 as an improvement of the sandpaper treatment, and the technique employed today is essentially the same as that originally described by Kurtin. The indications for dermabrasion are acne and pock scars, but recently the planing provides a non-scarring method for the treatment of existing keratoses, as well as a purpose of rhytidectomy. A total of 220 cases of facial dermabrasion performed during the last 10 years at Severance Hospital was reviewed and analysed upon age and sex distribution, scar depth and distribution, trial dermabrasion, post-operative changes(edema, discharge, crust and cracking), follow-up study and complications.

        皮膚移植에서 供與部創傷 治癒過程에 關한 臨床的 考察

        Many plastic surgeons are called upon from time to time to carry out skin grafting procedures. Often the sebsequent donor site does not receive the attention or expert management it deserve. The result is an inordinately uncomfortable patient or, worse, delayed healing or infection with conversion to full-thickness loss. The most important goal of donor site care is rapid re-epithelization without infection and minimum discomfort to the patient. So, many techniques have been used in managing the split-thickness skin graft donor site. Basically, these can be categolized into the following dressing classification: occlusive, open and semi-open method. During the past 6 months, 48 cases of the donor dressing at plastic surgery department of Severance Hospital. In the author's observation, the semi-open method is rapid re-epithelization with minimum discomfort compare ti open and occlusive method. Which result suspect the early wound open and dry up after bleeding control is the key of donor wound healing.

        Poland씨 흉근 異狀의 유방재건 치험례

        Poland's syndrome means a congenital unilateral pectoralis muscle defect, often accompained by the deformity of the ipsilateral upper extrimity. Besides the pectoralis major muscle defect, the defects of deltoid, supra-and infra-supinatus, latissimus dorsi, rectus abdominis, teres major & minor etc. may occur as muscle deformity. And also breast tissue, nipple and areolar, subcutaneous fat deficiency and abnormality of costal cartilage and anterior ends of ribs may be associated. In the ipsilateral upper extrimity, various abnormlity inclding sysdactly, brachydactyly may be associated. This paper describes an experience of Poland's pectoralis muscle anomaly, which was corrected with latissimus muscular island flap transposition and silastic prosthesis augmentation.

        몸의 영성과 기독교교육

        Traditionally, spirituality stands for a life of asceticism and contemplation. In many religious traditions, spirituality has been associated with the disciplined life of monastic communities which have consciously separated themselves from everyday life and its conflicting demands. This ascetic tradition of spirituality has emphasized 'ascetica' and 'mystica.' However, a new approach(body-oriented) to the spirituality contributes to the rediscovery and re-affirmation of another dimension of spirituality. Instead of following the dualistic tradition, it stresses holistic spirituality that seeks to integrate all aspects of what it means to be human and perceives the role of body(brain) in the religious experience positively. This article also challenges the traditional understanding of spirituality to move beyond its merely narcisstic and transcendental status by focusing on spirituality of the body(especially the brain) and proposes a new model of Christian Education for spirituality. To that end it will briefly explore the relationship between the body and religious experience in the perspective of neurothelogy. Through such reviewing, it will be shown that Christian Education has to be more concern about ⑴the body as medium for the development of spirituality, ⑵the importance of immanent transcendence, and ⑶the mind and body unity. In conclusion, this article will point out that Christian Education for spirituality is successfully performed through considering spirituality of the body.
