As the DT of micro businesses emerges as an important task, the government is also promoting support projects such as policy establishment and micro business education. This study aims to develop a new index (checklist) that can objectively measure the level of digital literacy required for DT in the tertiary industry, which accounts for the largest share of micro business owners. In this study, indicators were derived through review of existing studies and FGI, and the validity and reliability of Likert 5 were measured for decision makers in the tertiary industry. In the field of digital literacy for micro business owners, a total of 22 indicators were developed, largely composed of basic technology environment competency, information utilization competency, information dissemination and production capability, and mind recognition capability. This study has academic significance in that it can contribute to accurately understanding the digital capabilities of micro business owners by developing a digital literacy index for micro business owners, a specific group lacking in research.
셀 브로드캐스팅 보안 인증시스템 및 비즈니스 모델에 관한 연구

공유 서비스나 사물인터넷기술이 확산됨에 따라 보안인증이 적용되어야 하는 분야와 그 범위가 넓어지고 있다. 이동을 전제로 하는 서비스들에 있어서는 기존의 개인 소유물이나 특성에 관한 보안인증만으로는 충분치가 않아서 위치에 기반한 인증기술들도 개발이 되고 있다. 하지만 기존 위치기반 보안인증기술의 대부분은 주로 서버 근접도나 위성위치추적을 활용하는 기술로 개발이 되고 있어서 그 활용의 범위가 좁고 이동 시 핸드오버 문제가 발생한다. 본 연구에서는 이러한 기존의 위치기반 보안인증기술보다 그 활용의 범위가 넓고 따라서 편의성과 사업성이 높은 셀 브로드캐스팅(Cell Broadcasting)기술 활용 위치기반 보안인증기술에 대해서 소개하고 이와 관련된 응용 사례 및 비즈니스 모델들을 소개하였다. 기술 개발의 현황과 아울러 이를 활용하여 사업을 개발하고 있는 기업 사례를 심층 분석하여 비즈니스 모델의 변화가 어떻게 이루어지고 있는지를 자세히 분석하였다. 여기서 제안된 셀브로드캐스팅 보안인증시스템은 다각화되고 있는 미래형 비즈니스모델의 근간이 되는 기술이며 기존의 위치인증방법 및 시스템을 보완한 모델로 그 의미가 있다. With the rapidly changing era of the fourth industrial revolution, the utilization of IT technology is increasing. In addition, the demand for security authentication is increasing as shared services or IoT technologies are being developed as new business models. Security authentication is becoming increasingly important for all intelligent devices such as self-driving cars. However, most location-based security authentication technologies are being developed mainly with technologies that utilize server proximity or satellite location tracking, which limits the scope of their physical use. Location-based security authentication technology has recently been developed as a complementary replacement technology. In this study, we introduce location-based security authentication technology using cell broadcasting technology, which has a wider range of applications and is more convenient and business-friendly than existing location-based security authentication technologies. We also introduced application cases and business models related to this. In addition to the current status of technology development, we analyzed current changes in business models being employed. Based on our analysis results, this study draws the implication that technology diversification is necessary to improve the performance of innovative technologies. It is meaningful that it has found and studied advanced technologies other than existing location authentication methods and systems.

저자들은 33세 남자 환자에서 비장 경색을 동반한 췌장 미부 종양으로 오인된 부비장 및 비장 결핵을 경험하였기에 문헌고찰과 함께 보고하고자 한다. Abdominal tuberculosis usually affects the gastrointestinal tract, peritoneum, lymph nodes, liver or spleen. Tuberculosis of the spleen is uncommon, except when associated with miliary dissemination. We report a case of a 33-year-old man with tuberculosis of the accessory spleen, which was originally suspected to be a distal pancreatic tumor. He was admitted with a history of left upper quadrant abdominal pain for 3 months. Computed tomography imaging of the abdomen revealed a 4.5 cm sized poorly defined hypodense mass in the distal pancreas and showed multiple focal hypodense lesions in the enlarged spleen. We performed distal pancreatectomy and splenectomy under the preoperative expectation of a distal pancreatic tumor. Microscopic examination of the specimens revealed accessory splenic tuberculosis associated with splenic tuberculosis. Following this, he was treated with appropriate antituberculosis drugs and was discharged without any complications. (Korean J Med 79:53-56, 2010)
크론병 환자에서 아자치오프린 용량 적정은 어떻게 할 것인가? 백혈구 정상 하한치와 내약성에 근거한 최대 용량 적정 방법

목적: 일반적으로 크론병 치료에 있어 체중에 근거한 azathioprine (AZA) 사용(2.5 mg/kg/day)이 권고되고 있지만 많은 환자들에서 골수억제 부작용이 나타나고 있다. 이 연구에서는 크론병 환자에서 관해 유지를 위한 AZA의 최대용량을 체중에 근거하지 않고 백혈구의 정상 하한치에 근거해 적정하는 방법에 대해 알아보고자 하였다. 대상 및 방법: 고신대학교 복음병원에서 크론병으로 진단받은후 2010년에서 2011년까지 고신대학교 복음병원에서 추적 관찰하던 78명의 크론병 환자들 중 내약성과 4,000/mm3 이상의 백혈구 수치를 동시에 만족시키면서 스테로이드 없이 관해를 유지하는 AZA 최대용량을 사용하는 환자들을 연구에 포함시켰다. 적정된 최대용량의 AZA 용량과 체중과의 관계에 대해 살펴보았다. 결과: 총 42명의 환자가 연구에 포함되었으며, 그 중 32명이 남자였고 평균 나이는 31세였다. 내약성과 4,000/mm3 이상의 백혈구 수치를 동시에 만족시키면서 스테로이드 없이 관해를 유지하는 AZA 최대용량은 49.1 mg/day였다. 체중당 용량은 0.87 mg/kg/day였고, 체중당 AZA 용량은 체중(γ=?0.51, p=0.01), 체질량지수(γ=?0.33, p=0.034)와 음의 상관관계가 있었다. 40세 이하 군에서의 체중당 AZA의 용량은 40세 이상 군에서보다 의미있게 많았다(p=0.039). 결론: AZA의 용량을 체중에만 근거하여 결정하는 것은 저용량 또는 고용량의 AZA 용량을 초래하여, 결과적으로 부가적인 치료를 필요로 하거나 심각한 부작용을 일으킬 수도 있다. 따라서, 백혈구의 정상 하한치와 내약성에 근거한 최대용량 적정 방법은 AZA 용량 결정에 있어 새롭고 가치있는 방법이다. Background/Aims: Although general guidelines have suggested weight-based dosing of azathioprine (AZA, 2.5 mg/kg/day) for Crohn`s disease (CD), a substantial number of patients develop bone marrow suppression. The aim of this study was to evaluate the maximum dose of AZA not based on weight but titrated according to the lower limit of leukocyte count for maintaining remission in patients with CD. Methods: Among a total of seventy-eight patients with CD, who had been followed-up at Kosin University Gospel Hospital (Busan, Korea) from 2010 to 2011, those treated with the maximum dose of AZA meeting both drug-tolerability and leukocytes count of more than 4,000/mm3 for steroid-free maintaining remission were enrolled. The titrated maximum AZA dose and its relationship with weight were evaluated. Results: A total of 42 patients (male, 32 patients; mean age, 31 years) were enrolled. The maximum dose of AZA was 49.1 mg/day. The dose per weight was 0.87 mg/kg/day and negatively correlated with body weight (γ=?0.51, p=0.01) and BMI (γ=?0.33, p=0.034). AZA dose per weight in the below 40 years old group was significantly higher than that in the above 40 years old group (p=0.039). Conclusions: Dose decision of AZA based only on weight could put the patients to inappropriately low or high dose resulting in need of additional therapy or serious side effect, respectively. Therefore, the maximum dose-titration based on the lower limit of leukocyte count and tolerability is a novel and a valuable strategy in deciding the dose of thiopurines. 결합조직형성 소원형세포종양은 비교적 최근에 독립된 질환으로 분류되었으며 젊은 나이의 남자에게서 복막에서 기원한 것으로 보이는 종괴가 복강에 다발성으로 있으나 복부 장기에서의 기원이 불명확하며, 여러 장기를 침범하고, 공격적인 성향을 보이는 경우에 복강 내 결합조직형성 소원형세포종양의 가능성을 고려해 봐야 될 것이다. 본 저자들은 다운증후군 환자에서 발생한 결합조직형성 소원형세포종양 1예를 경험하였기에 보고하는 바이다. A desmoplastic small round cell tumor (DSRCT) is a rare, aggressive neoplasm that occurs predominantly in children and young men. It presents as a large mass inside the abdomen, particularly within the pelvis, and may be accompanied by extensive tumor implants throughout the peritoneum. Microscopically, it typically appears as nests of small undifferentiated cells within a desmoplastic stroma. A DSRCT shows a special immunohistochemical staining pattern, expressing epithelial, neural, and muscle markers. A DSRCT is associated with a specific chromosomal translocation, t(11;22) (p13;q12), resulting in a chimeric EWS/WT1 transcript that is helpful for diagnosing this tumor. We experienced a case of DSRCT in a 19-year-old man who had been diagnosed with Down`s syndrome. (Korean J Med 76:471-475, 2009)

Crohn`s disease is characterized by chronic transmural inflammation of the bowel and is associated with serious complications, such as bowel strictures, abscesses, fistula formation, and perforation. As neither medical nor surgical therapy provides a cure for Crohn`s disease, the primary goals of therapy are to induce and maintain remission and prevent complications. As a biologic agent, infliximab, a monoclonal antibody to tumor necrosis factor, is indicated for refractory luminal and fistulizing Crohn`s disease that does not respond to other medical therapies or surgery. Infliximab has proven to be very effective for inducing and maintaining remission in Crohn`s disease; however, infliximab treatment has several potential complications. Here, we report a case of free perforation following a therapeutic response after an initial dose of infliximab for Crohn`s disease.
