보철물 제작 후 부정확한 발음을 가진 환자에서 체계적인 진단 및 치료과정을 통한 보철 수복 증례

Recently, there are cases where anterior esthetic prostheses are fabricated for better esthetics, but biologic, mechanical factors could be overlooked, too focusing on esthetic factor. This leads to changes in neutral zone, dentition, position of tongue and lips, occlusion and anterior guidance causing inaccurate pronunciation. Therefore, consideration of systematic diagnosis and treatment procedure are required. In this case, prosthesis was refabricated through a systematic diagnosis and treatment procedure using four factor (acoustic analysis, esthetic analysis, occlusion, neutral zone) for the patient who complained of inaccurate pronunciation and esthetics of the fixed prosthesis fabricated 10 years ago. Thus, by promoting functional, esthetic recovery, this case report demonstrates satisfying results to both the patient and dentist. 최근 들어 치아의 심미적 개선을 위해 전치부 심미 보철물을 제작하는 치료를 진행하는 경우가 있는데, 이 때 심미적인 요소에 치중하여 생물학적, 기계적 요소를 간과하기 쉽다. 이로 인해 중립대 공간 및 치열에 변화가 발생하고, 혀와 구순의 위치가 변화하고, 교합 및 전방유도 변화로 인해 부정확한 발음문제가 유발될 수 있어 이에 대한 체계적인 진단 및 치료과정에 대한 고려가 필요하다고 사료된다. 본 증례에서는 10년 전 제작한 고정성 보철물로 인해 부정확한 발음 및 비심미성으로 불편감을 호소하는 환자에서 음향학적 분석, 심미적 분석, 교합, 중립대 라는 4가지 요소를 이용하여 체계적인 진단 및 치료과정을 통하여 보철물을 재수복하였고, 이에 기능적, 심미적 회복을 도모하여 환자와 술자 모두에게 만족스러운 결과를 얻었기에 보고하는 바이다.

세계시민성 함양을 위한 중학교 사회과 수업 프로그램 개발: 자율적 협동학습(Co-op Co-op) 모형을 적용하여

본 연구는 세계시민성과 협동학습의 가치를 확인하고, 세계시민교육의 효과적인 교육 방안 중 하나로 협동학습, 특히 자율적 협동학습(Co-op Co-op) 모형을 제시하여 이를 토대로 중학교 사회과 수업에서 활용 가능한 수업 프로그램의 개발 과정을 서술한 것이다. 급속히 진행되는 세계화에 대응하기 위한 방안으로 세계시민교육의 필요성과 중요성이 대두되고 있지만, 아직까지 중·고등학교 교육현장에 적용 가능한 수업 프로그램에 대한 연구는 부족한 상황이다. 따라서 본 연구는 이론적 논의를 통해 세계시민성과 세계시민교육의 주요 가치로 다양성 존중, 세계 문제에 대한 관심, 타인과의 협업, 민주적 절차의 존중, 적극적 참여 등을 제시하였고, 협동학습의 특징 으로 긍정적인 상호의존성, 개별적 책무성, 대면적 상호작용, 이질적인 모둠 구성, 구체적인 수업 목표 등이 있음을 확인하였다. 이후 이러한 세계시민성의 주요 가치와 협동학습의 특징을 모두 함께 잘 함양할 수 있는 협동학습의 모형으로 자율적 협동 학습(Co-op Co-op) 모형을 적용하여, 중학교 사회②의 1단원인 『인권과 헌법』 단원을 중심으로 수업 프로그램을 개발하고 제시하였다. 본 연구에서 개발한 수업 프로그램의 타당성과 신뢰성을 확보하기 위하여 현직 교사들로 이루어진 전문가 집단 에게 수업 프로그램에 대한 검토를 요청하였고, 수정·보완할 내용들을 수렴하였다. The purpose of this study is to identify the value of global citizenship and cooperative learning, and one of the effective ways to educate citizens around the world is to present a Co-op Co-op model for cooperation, which can be used in social studies class in secondary schools. While the need and importance of global citizenship education is emerging as a way to respond to the rapid globalization, there is still a lack of research on the teaching programs available at secondary schools. Therefore, through theoretical discussions, this paper identified diversity, cooperation with others, respect for democratic processes, active participation, and responsibility for global issues as key values of global citizenship. Based on this, class programs were developed and presented based on the first unit of social studies in secondary school 『Human rights and constitutional』 unit using Co-op Co-op among various cooperative learning models. The contents to be modified and supplemented by asking a group of experts from current teachers to review the program to ensure the validity and reliability of the class program developed in this study.

한국에서 연노랑허리솔새(Phylloscopus yunnanensis)의 첫 포획 기록

2021년 10월 22일 인천 옹진군 소청도에서 연노랑허리솔새(Phylloscopus yunnanensis) 1개체가 포획되었으며, 이번 기록은 국내 첫 번째 포획 기록이며 유전적 방법으로 확인된 본 종의 첫 번째 확증 기록이다. 외형적으로 노랑허리솔새(P. proregulus)와 매우 유사하지만, 머리중앙선 앞 부분이 흐리고, 옅은 눈썹선, 둘째날개깃 기부의 반점이 없는 점으로 구분된다. 외부측정치는 노랑허리솔새와 큰 차이가 없지만, 익식은 차이가 있었다. 본 기록은 국내 도래에 대한 명확한 근거가 부족한 솔새과 조류 1종에 대한 포획과 측정 자료를 제공함으로써 국내 생물종 확보 및 생물다양성 증진에 기여하며, 향후 야외에서 본 종에 대한 형태적 동정에 필요한 정보를 제공하는데 의의가 있다. A Chinese Leaf Warbler (Phylloscopus yunnanensis) was caught in mist nets on the Socheong Island (37°45′41.4″N, 124°43′46.8″E), Ongjin County, Korea, on October 22, 2021. This is the first confirmed record of the species in Korea based on morphological and phylogenetic analyses. Despite morphological similarity to Pallas’s Leaf Warbler (P. proregulus), Chinese Leaf Warbler could be distinguished based on differences in the median crown strip, supercilium, and secondary wing feathers, and the wing formula of P. yunnanensis also differed from that of P. proregulus; however, other morphometric measurements were comparable. This record can contribute to the enhancement of biodiversity in Korea based on the morphometry and photography of a Phylloscopidae species, for which there was no substantive evidence of record in Korea. These data are useful to formulate keys for the morphological identification of this species in the field.

교정치료 후 비심미적인 상악 전치부 비율을 가진 환자에서 체계적인 진단 및 치료과정을 통해 심미성이 개선된 보철 수복 증례

상악 전치부와 같은 심미성이 요구되는 부위의 치료 시에는 건강하고, 주위조직과 조화를 이루며 매력적인 미소선의 형성을 염두에 두어야 한다. 이에 관하여 미소선, 연조직 및 경조직의 형태뿐 아니라 치아의 해부학적 형태와 비율 또한 고려하여야 한다. 상악 측절치의 왜소치(peg lateralis)로 인한 치간이개 및 부적절한 치은 형태로 인해 비심미적인 상악 전치부 비율을 가진 환자에서는 교정치료만으로 심미적 수복이 이루어지기 힘들 수 있다. 이런 경우, 교정치료가 끝난 이후 치아-치은-안면 구도에서의 진단 및 분석을 통한 치은절제술 및 보철적 수복을 통해 치아간의 근/원심, 폭/길이 비율을 개선시켜야 심미적으로 만족할 만한 결과를 얻어낼 수 있다. 또한, 상악 측절치의 왜소치(peg lateralis) 치아 비율 개선을 위한 보철물 제작 시 치질삭제가 적으며 인접치와 유사한 색조가 재현 가능하고 투명도가 높은 도재 라미네이트 비니어(Porcelain laminate veneer, PLV)가 선호되고 있다. 본 증례에서는 두 명의 20세 여자 환자로 교정치료 후 상악 측절치의 왜소치로 인한 치간이개 및 부적절한 치은 형태로 인해 비심미적인 상악 전치부 비율을 가진 환자에서의 체계적인 진단 및 치료과정을 통해 심미성이 개선된 보철 수복 증례를 보고하고자 한다. The treatment of areas demanding esthetic requirements, such as maxillary anterior teeth, should take into account the achievement of a healthy, harmonious to the surrounding tissue, and an attractive smile line. In this case report, smile line, soft tissue and hard tissue morphology, and the anatomy and proportion of the tooth, must be considered. In patients with unesthetic maxillary anterior ratio due to inadequate gingival contour and diastema caused by peg lateralis, it would be challenging to achieve an esthetic restoration by orthodontic treatment alone. In such case, after orthodontic treatment, dento-gingivo-facial compositional diagnosis and analysis, followed by gingivectomy and prosthodontic restoration is needed to improve the interdental mesial/distal, width/length ratio to achieve a satisfactory esthetic result. In addition, when improving the tooth proportion of peg lateralis by prosthodontic treatment, Porcelain laminate veneer (PLV), which results in less tooth structure loss, reproduction of similar shade to that of the proximal tooth and high transparency, is recommended. This case report demonstrates esthetic improvements by prosthodontic restoration through systematic diagnosis and treatment procedure in patients with unesthetic maxillary anterior proportion after orthodontic treatment due to peg lateralis by means of two female patients aged twenty years old.

한국 미기록 오색딱다구리 아종(Dendrocopos major brevirostris) 관찰 보고

2019년 12월 6일, 인천광역시 옹진군 대청면 소청도(37°46‘13"N, 124°45"31"E)에서 국내에 서식하는 오색딱다구리 아종인 Dendrocopos major japonicus와 외형적으로 차이가 있는 1개체를 관찰하고 촬영하였다. 형태적 특징과 어깨깃의 흰색 무늬, 머리와 몸 아랫면 흰색의 차이에 의해 본 개체는 국내에서 기록되지 않은 오색딱다구리의 아종 D. m. brevirostris로 확인되었다. 본 관찰은 한국에서 D. m. brevirostris의 첫 기록이며, 아종명은 ‘북방오색딱다구리’로 제안한다. On 6 December 2019, an unusual looking Great Spotted Woodpecker (Dendrocopos major) was observed and photographed at Socheong Island, Daecheong-myeon, Ongjin-gun, Incheon Metropolitan City, Republic of Korea (37°46‘13"N, 124°45"31"E). Depending on its morphological characteristics, extensive white shoulder patch, white face and underparts, the bird was identified as a subspecies brevirostris of Great Spotted Woodpecker which has not been recorded in Republic of Korea yet. This is the first national record of D. m. brevirostris.

Childhood granulomatous periorificial dermatitis (CGPD) is a distinctive granulomatous form of perioral dermatitis. It is characterized by papular eruptions located around the mouth, nose, and eyes, and rarely present eczematous patches or plaques. Histopathologic examination shows upper dermal and perifollicular granulomatous infiltrates. Herein, we report two cases of CGPD that were presented with erythematous plaques. One patient was treated with 0.03% topical tacrolimus and the other patient with oral metronidazole. The patients responded well to their respective therapies, showing resolution of the lesions. 올빼미(Strix aluco) 휴식처로의 동굴 이용

2006년부터 2012년까지 강원도와 충청북도 일대의 동굴 14곳에서 올빼미(Strix aluco)의 서식이 확인되었다. 대부분의 동굴에서 올빼미의 펠릿이 확인되었으며, 올빼미가 직접 관찰된 동굴은 7개소였다. 동굴에서 확인된 올빼미는 대부분 겨울철(11월~3월)에 관찰되었으며, 두 개의 알이 산란된 둥지가 관찰되기도 하였다. 이는 올빼미가 겨울철에 주로 일시적인 휴식처로 동굴을 이용하고 있음을 보여준다. 올빼미가 이용하는 동굴은 입구가 크고, 내부 공간 또한 큰 것으로 나타났다. 따라서 올빼미의 주 서식환경인 산림지역 내 동굴 중에서 올빼미가 이용하기에 적합한 구조를 갖는 동굴의 환경을 보호할 필요가 있다. Cave roosting of the Tawny Owl Strix aluco were observed in Gangwon-do and Chungcheongbuk-do during 2006-2012. We observed roosting owls in corrosion hole and/or pellets on the ground and rock within the cave. Most of cave roosting of the Tawny Owl were observed in the winter season (from November to March), but one breeding nest with two eggs was also observed. This showed that caves might have provided the temporary roosting site for owls, especially during the winter. Caves used by the Tawny Owl as roosts had larger entrance and larger inner structure than non-roosts. It should need to be protected the caves with structural characteristics suitable for owls.

HIV 감염의 발현증상으로 나타난 난치성 소양증 1예

To achieve early detection of HIV infection, physicians must be aware of symptoms related to HIV infection. Dermatologists, in particular, can play an important role, because more than 90% of patients infected with HIV will develop at least one type of dermatologic disorder, which may be the first indicator of the existence of infection. A 26-year-old male presented with a one-year history of intractable pruritus associated with multiple excoriations and small ulcers. Initially, his condition clinically resembled adult atopic dermatitis. However, no improvement was observed despite treatment including narrowband ultraviolet B (UVB), antihistamine, and topical and systemic corticosteroid. After a week, the patient developed seizures and was diagnosed with diffuse large B-cell lymphoma of the brain. HIV testing was performed, and the patient was revealed to be HIV-positive. He began highly active antiretroviral therapy (HAART), and the pruritus and skin condition improved dramatically. Herein, we report a case of HIV infection for which the presenting sign was intractable pruritus. (Korean J Dermatol 2017;55(1):45∼48)
