Objectives: Exposure assessment is a major challenge faced by studies that evaluate the association between pesticide exposure and adverse health outcomes. The objective of this study was to investigate the reliability of information that farmers self-report regarding their pesticide use. Methods: Twenty five items based upon existing questionnaires were designed to focus on pesticide exposure. In 2009, a selfadministrated survey was conducted on two occasions four weeks apart among 205 farmers residing in Gyeonggi and Gangwon provinces. For a reliability measure, we calculated the percentage agreement, the kappa statistics and the intraclass correlation coefficient (ICC) between the two reports according to the characteristics of the subjects. Results: Agreement for ever-never use of any pesticide was 96.4% (kappa 0.61). For both 'years used' and 'age at the first use' of overall pesticides, high agreement was obtained (ICC: 0.88 and, 0.78, respectively), whereas those of 'days used' and 'hours used' were relatively low (ICC: 0.42 and, 0.66, respectively). The kappa value for the use of personal protective equipment ranged from 0.46 to 0.59, and hygiene activities came out at 0.19 to 0.37. The agreement for individual pesticide use ranged widely and there was relatively low agreement due to the low response rates. The reliability scores did not significantly vary according to gender, age, the education level, the types of crop or the years of farming. Conclusions: Our results support that carefully designed, self-reported information on ever-never pesticide use among farmers is reliable. However, the reliability of data on individual pesticide exposure may be unstable due to low response rates and needs to be refined.

This study aimed to determine the feasibility of survey questionnaires for pesticide poisoning. We conducted a pilot study to develop a questionnaire for evaluating pesticide poisoning symptoms and to examine the validity of self-reported information for individual pesticides used. In December 2010 we interviewed 20 farmers with self-reported pesticide poisoning histories residing in Gyeonggi Province. For validity of pesticide information, the purchased pesticide lists from the agricultural cooperative federation were compared with individual self-reported pesticides used. Neurological symptoms were the most common among clinical symptoms related to pesticide poisoning, followed by gastrointestinal, dermal and eye symptoms. Insecticides were the main causative pesticide class for poisoning. Twenty one major symptoms were selected for screening for acute pesticide poisoning in a field study based on the pilot results and literature reviews. Substantial under-reporting was found in data on self-reported individual pesticides used (9.2%), as well as low accuracy (36%) compared with their actual purchase lists. In this pilot study, we suggest the selected symptom lists may apply to a field survey of pesticide poisoning. However, the self-reported information on individual pesticides may not be valid and alternative methods need to be developed.

Objective: This study aimed to examine the level of risk perception related to pesticide contamination of food products and associated sociodemographic factors, as well as investigate the association with environmental pollution preventive behaviors. Methods: Based on data from Social Survey conducted in 2008 among a nationally representative sample of the population 15 years or older, we estimated proportions and 95% confidence intervals of the risk perception of pesticide contamination of food products. Logistic regression analyses were used to analyze the effects of sociodemographic status on risk perception. Results: Among respondents, those who feel anxious about the risks of pesticide contamination of domestic food products accounted for 40.4%, whereas the proportion rose to 87.0% with imported food products. These numbers showed that levels of anxiety about the pesticide contamination of imported food products were ranked at the top, and those for domestic products were at the bottom of the list of all environmental pollutants examined in the survey. Significantly related factors to anxiety about pesticide contamination of food products included women, the 40-50 age group, university graduates, married, non-manual employment, having school-aged children, and owning one's own home. The population attempting to prevent environmental pollution showed higher levels of anxiety about pesticide contamination of food products than did those who were not taking such steps. Conclusion: Compared to other forms of environmental pollution, Koreans show the highest levels of anxiety about pesticide contamination of imported food products. Therefore, it is necessary to establish appropriate regulations and transparent risk communication with the public about the safety of imported food.
