낙엽상 (落葉狀) 천포창 (天疱瘡) 에서 발생한 Kaposi 수두양 (水痘樣) 발진 1예

We present a case of familial benign chronic pemphigus in 50-year-old male patient who had had recurrent oozing, macerated and eroded skin lesions on the groin and scrotum for 20 years. At first, we diagnosed his case as scrotal eczema and/or intertrigo, but after repeated recurrance, it was confirmed as above disease by characteristic histopathologic findings. This report presents a case of a 17-year-old-girl who developed a, Kaposis varicelliforrn eruption(eczema herpeticum) during oral prednisolone therapy in order to control pemphigus foliaceus. Diagnosis was made by the history, clinical feature and histologic examination. Within 24 hours after the initiation of therapy with ribavirin(Viramid), new lesions had ceased to develop.

Author investigated the deposition of immunoglobulins and complements in thc skin of 56 patients with 19 various dermatoses by direct immunofluorescent (DIF) staining. The biopsied specimens were quick-frozen(by dry ice-acetone) and stained with FITC conjugated antihuman immunoglobulin and complement after cutting in a cryostat at -20℃~30℃. The summary of results are as follows: 1) 11 of 14 patients(78.6%) with SLE demonstrated a deposition of IgM(9 cases), IgG(6), IgA(6) and complement(5) at DEJ as granular pattern and 4 of 5 patients(80%) with DLE had IgM(4) and IgA(1) at DEJ as aggregated pattern. 2) 4 of 5 patients(80%) with pemphigus vulgaris demonstrted IgG(3) and complement(2) in ICS, 2 of 5 patients with dermatitis herpetiformis had only IgA at DEJ as granular pattern and 1 of 2 with erythema multiforme IgM at DEJ. 3) In dermal blood vessel, 1 of 3 patitents with morphea demostrated IgM, 2 of 3 with drug eruption had IgM(1) and complement (1) and 2 of 3 with livedo reticularis had IgM (2), IgA (1) and complement(1). 4) In various vasculitis, 1 of 3 patients with Mucha-Habermann(IgM) and anaphylactoid purpura(IgA) and 1 patient each with Wegener granulomatosis and periarteritis nodosa demonstrated IgA in dermal blood vessel. 5) In the skin of 8 patients with herpes simplex, impetigo herpetiformis, Hailey-Hailey disease, Kimura's diseases, amyloidosis, pernio, and erythema nodosum, deposition of immunoglobulin and complement were not detected.
