대동맥 박리증 (DeBakey type IIIa) 의 수술적 치료 -치험 1례-

A successful repair of aortic dissection of descending thoracic aorta was performed in a 48 year old man. The patient was visited ER because of abruptly onset chest pain. On admission, Chest film showed mediastinal widening and undertaken chest CT, echocardiogram and angiogram There was evidence of dilation on descending aorta with internal separation of intimal calcification. Aneurysmal sac with dissection was noted from just below left subclavian artery to 2cm above of diaphragm. He underwent thoracotomy and the impending ruptured aneurysm of the aorta was replaced with a Woven Dacron graft[20Yo Albumin preclotted] using LA-femoral bypass. Postoperative course was uneventful.

직업가치가 이직의도에 미치는 영향에 있어 조직후원인식의 조절효과 - 외식업체 1년 미만 신입종사원을 중심으로 -

본 연구에서는 외식업체에 종사하는 신입사원들을 대상으로 그들이 지각하는 직업가치가 이직의도에 미치는 영향과 이들 관계에서 조직후원인식의 조절효과를 검증하였다. 분석결과 다음과 같은 결과가 도출되었다. 첫째, 직업가치 중 내재적 가치와 이직의도의 관계를 검증한 결과 내재적 가치 중 사회헌신만이 이직의도에 정(+)의 방향으로 영향을 미치는 것으로 나타났으며, 둘째, 직업가치 중 외재적 가치와 이직의도의 관계를 검증한 결과 외재적 가치 중 체면유지와 안정추구만이 이직의도에 정(+)의 방향으로 영향을 미치는 것으로 나타났다. 셋째, 정서적 후원에 따라 내재적 가치가 이직의도에 미치는 영향력의 차이를 검증한 결과 내재적 가치 중 지식추구에 대해 높게 지각한다면 정서적 후원에 대한 지각이 높을수록 이직의도가 더 높아지는 것으로 나타났으며, 반대로, 내재적 가치 중 인간관계중심에 대해 높게 지각한다면 정서적 후원에 대한 지각이 높을수록 이직의도가 더 낮아지는 것으로 나타났다. 넷째, 정서적 후원에 따라 외재적 가치가 이직의도에 미치는 영향력의 차이를 검증한 결과 외재적 가치 중 경제우선에 대해 높게 지각한다면 정서적 후원에 대한 지각이 높을수록 이직의도가 더 낮아지는 것으로 나타났으며, 반대로, 외재적 가치 중 체면유지에 대해 높게 지각한다면 정서적 후원에 대한 지각이 높을수록 이직의도가 더 높아지는 것으로 나타났다. 다섯째, 수단적 후원에 따라 내재적 가치가 이직의도에 미치는 영향력의 차이를 검증한 결과 내재적 가치 중 지식추구에 대해 높게 지각한다면 수단적 후원에 대한 지각이 높을수록 이직의도가 더 높아지는 것으로 나타났다. 여섯째, 수단적 후원에 따라 외재적 가치가 이직의도에 미치는 영향력의 차이를 검증한 결과 외재적 가치 중 경제우선에 대해 높게 지각한다면 수단적 후원에 대한 지각이 높을수록 이직의도가 더 낮아지는 것으로 나타났으며, 반대로, 외재적 가치 중 체면유지에 대해 높게 지각한다면 수단적 후원에 대한 지각이 높을수록 이직의도가 더 높아지는 것으로 나타났다. This study verified the effect of occupational value, as felt by new employees working in the food service industry, on turnover intent and also the moderating effect of perceived organizational support between occupational value and turnover intent. The analysis drew out the following results. First, the study verified that, among inherent values, only social commitment had a positive(+) direction of influence on turnover intent, and second, among extrinsic values, only face-saving and stability seeking had a positive(+) direction of influence on turnover intent. Third, as a result of verifying the effect of inherent values on turnover intent according to emotional support, the study revealed that turnover intent increased, as the perception of emotional support increased, on condition that the item, value of knowledge seeking, among inherent values, was perceived highly, On the other hand, if the item, relationship-centered, scored highly among inherent values, then the higher the perception of emotional support, the lower the turnover intent. Fourth, as a result of verifying the effect of extrinsic values on turnover intent according to emotional support, the study revealed that turnover intent decreased, as the perception of emotional support increased, on condition that the value of economic priority, among extrinsic values, was perceived highly. On the other hand, if the item, face-saving, scored highly among extrinsic values, then the higher the perception of emotional support, the higher the turnover intent. Fifth, as a result of verifying the differing effect of inherent values according to methodical support, the study revealed that turnover intent increased as the perception of methodical support increased, if the item, knowledge-seeking among inherent values was perceived highly. Sixth, as a result of verifying the differing effect of extrinsic values according to methodical support, the study revealed that turnover intent decreased as the perception of methodical support increased, if the item, economic priority among extrinsic values was perceived highly. On the other hand, if the item, face-saving, among extrinsic values, was perceived highly, the higher the perception of methodical support, the higher the turnover intent.

전기 화상 이후 발생한 만성 뇌경질막 노출 상처: 증례 보고

Purpose: Electrical burn of scalp is uncommon. Much more, chronically exposed dura in unstable burn scar is quite exceptional. Hence, we report a case of chronically exposed dura following electrical burn. Methods: A 63 - year - old man presented with a about 40 years history of an ulcerative lesion arising from electrical burn scar with ‘squeeze like sensation' around wound. Wound was about 6 x 8 cm. Area in the center was 3 x 3 cm nonviable dura without sequestrum. Tangential excision with an intraoperative neurosurgical consultation and transposition flap under general anesthesia was done. Intraoperative biopsy was done. Wound was diagnosed as chronic osteomyelitis, not Marjolin ulcer. Flap was taken successfully. But after 5 days, infectious discharge had been appeared during 2 weeks, despite irrigation and drainage. As flap was re - evaluated, we could see remnant necrotic dura. After that, latissimus dorsi muscle free flap with meshed split thickness skin graft was transferred without excision of necrotic dura. Results: Flap was taken successfully. Follow - up at 10 weeks has been uneventful, with good and stable coverage of the wound. Conclusion: It is true that complete excision of devitalized tissue with sagittal sinus obliteration is prerequisite to flap taken. But necrotic dura was tangentially excised instead of total dura excision, because, posterior two - thirds of the sagittal sinus was involved underneath. Muscle is rich in blood vessels and decrease the recipient - site bacterial count effectively. In this case, muscle flap with skin graft without total dura excision is an alternative treatment.

BThere are many materials which can make beautiful, clear eyelid crease with temporary effect. Double fold tape and glue are the most popular materials to make artificial double eye lids. However, the long-term use of these materials seems to make cause many local problems. This study was conducted to identify the long-term side effects of double-fold tape and glue. A total of 191 patients who have experienced double fold tape or glue were enrolled. The information of age, double-fold making product, and side effects were collected, based on medical records and, Patient & Observer scale(1: 'Normal', 5: 'Abnormal/Severe). The mean period of use was 23.7 months. The time required to make double-fold was 5~30 minutes. The most common symptom was itching sense, followed by decrease skin elasticity, skin dryness, and change of skin color. The most common signs was skin laxity, followed by eruption, keratinization, pigmentation, and local inflammation. One patient underwent medical treatment for conjunctivitis after double-fold glue usage. Double fold tape or glue can lead a problem with skin due to lack of oxygen exposure and rubbing eye lids with a stick. Appropriate use of double-fold products and careful monitoring of side effects are needed. (Archives of Aesthetic Plastic Surgery 17: 123, 2011)
