Pigmented villonodular synovitis is a kind of benign inflammatory lesion involving synovial membrane, characterized by yellow-grayish colored villous nodules of synovial membrane, formed by accumulation of cholesterol and hemosiderin with numerous cleftings of synovial membrane. This occurs most commonly in the knee joint as a monoarticular proliferative process. The deveiopment of this disease in the hip joint is rare and moreover, the diagnosis is not easy. Early diagnosis of hip joint involvement is important because destruction of femoral head and acetabulum is the ultimate outcome if treatment is not initiated. A definitive treatment for this disease process is not yet known. In this paper, we report a case of pigmented villonodular synovitis of the hip joint with a review of the literature. It was identified in a 23-year-old young lady who was treated by synovectomy.

Fractures of the distal radius represent the most common fractures of upper extremity and treatment remains challenging. Recently, more extensive therapeutic method is represented for reduction and maintenance of distal radius unstable fracture. Twenty patients, 21 cases of distal radius fracture treated by external fixator between June, 1991 and September, 1995 were followed by more than one year to evaluate the correlation between anatomical and functional results. To assess the functional results, we used Green and O' Brien' s system which scores subjective and objective findings. Grip power ratio was checked with Jamar dynanometer. To assess the anatomical results, volar tilt, radial inclination and radial length were measured. Frykmann type VIII and Universal type IV C fractures are most common form(7 cases, 33%) in our study. The more severe form of fracture of distal radius. the less score in funtional results. There was a little loss of volar tilt, radial inclination and radial length on last follow-up radiographs. In radiographic parameters, radial length showed significant correlation with ulnar deviation(p=0.002). In complication, sudek atrophy(2 cases), ulnar nerve entrapment symptom(1 case), and writing problem(1 case) was seen, but not serious. We concluded that external fixation in distal radius fracture is one of the treatment method expecting good result without serious complication.
천골 제1분절에 발생한 동맥류성 골낭종의 전후방 도달법에 의한 전 절제술 : 1례 보고 Report of a Case

Aneurysmal bone cysts are uncommon lesion, especially in the spine. Among the spinal lesions, it's incidence in sacrum were reported very rare(0.6-3%). The author's case is the lesion of the first sacral segment that involves the body and the lef side ala extensively and showed the collapse due to pathological fracture. Neurological involvement was not observed. After arterial embolization, complete excision of tumor was performed through the combined and posterior procedure under the same anesthesia. In first stage operation, the excision of the posterior elements and whole of left side ala portion was done and transpedicular Cotrel-Dubousset instrumentation was performed between the fifth lumbar vertebra and bilateral iliac tuberosities. And, in secord stage, excision of first sacralbody. and insertion of autologous iliac strut grafts between the bodies fifth lumbar vertebra and 2nd sacral segment were done. Hip spica cast was applied for three months, prstoperatively, solid bony fusion fusion without collapse of grafted bone block was observed at the follow up study of 12 months after operation.
