        골이식재를 사용하지 않은 상악동 거상술

        Various maxillary sinus floor augmentation techniques were common performed and in the most cases, many kinds of bone graft materials were used. The graft materials are autogenous bone or other biomaterials of human, artimal or synthetic origin. but these cases, report describes a new surgical technique by which dental implants are inserted in a void space created by elevating the sinus membrane without additional graft material in atrophic posterior maxilla. We created lateral bony window using piezoelectric device and elevated the schneiderian membrane in ten patients. Dental implants were simultancously placed and lateral window prepared was covered with non-resorbable membrane in five patients and was repositioned with bony window in five patients, without any bone graft. From the clinical and histological results, it is found there is potential capacity for bone formation and placement of implants in the maxillary sinus without the use of bone grafts or bone substitutes.

        정신분열병 환자의 Apolipoprotein E 유전형과 임상변인

        Objectives : In the central nervous system. apolipoprotein E(Apo E) is thought to playa role in neuronal growth and repair. Numerous studies have provided evidence for a genetic association of the Apo E ε4 allele and late onset familial and sporadic Alzheimer's disease. However, previous studies on Apo E genotype distribution in schizophrenia, which would suffer from severe cognitive impairment. have reported conflicting findings. So we have examined the genetic association between schizophrenia and Apo E genotype polymorphism to investigate the possible contribution to schizophrenia. Methods : Genomic DNAs from 100 schizophrenic patients and 60 healthy controls were isolated. The Apo E genotype was determined by the polymerase chain reaction (PCR) with sequence-specific oligonucleotide primers (SSOP) and reverse hybridization using the INNO LiPA(Line Probe Assay) Apo E typing test kit. The frequencies of allele and genotype were compared between patients and controls. And the allele frequencies were compared in terms of positive and negative subtypes according to the PANSS, family history and age of onset. Results : The frequencies of Apo E allele and genotype were not signilicantly different between the patients and controls. But in patients group, the frequencies of allele and genotype between subtypes by PANSS were significantly different. Some other clinical variables such as sex, age of onset and family history were not associated with Apo E genotype. Conclusion : Apo E gene has no causal relation to the development of schizophrenia in Korean population, but it may affect the phenotype of schizophrenia.

        Tetramethylpyrazine이 LPS의 뇌실주입에 따른 생쥐 뇌조직의 Pro-Inflammatory Cytokines 발현에 미치는 영향

        Objectives : Tetramethylpyrazine (TMP) is an active ingredient in Ligusticum wallichii and has a wide range of neuroprotection effects. This study investigated anti-neuroinflammatory effect of TMP on brain regions in intracerebroventricular (i.c.v.) lipopolysaccharide (LPS)-treated C57BL/6 mice, Methods : TMP was administered intraperitoneally at doses of 10, 20, and 30 mg/kg at 1 h prior to LPS (3 mg/kg) i.c.v. injection. mRNA level of pro-inflammatory cytokines, including tumor necrosis factor-α (TNF- α), interleukin (IL)-1β and IL-6, was measured in the cerebral cortex, hippocampus, and hypothalamus tissue using real-time polymerase chain reaction at 24 h after the LPS injection. Cyclooxygenase-2 (COX-2) positive cells in the hypothalamus was also observed using immunohistochemistry at 24 h after the LPS injection, Results : At a dose of 30 mg/kg TMP significantly attenuated up-regulation of TNF-α and IL-1β mRNA in the cerebral cortex and IL-1β mRNA in the hippocampus. In the hypothalamus, doses of 20 mg/kg and 30 mg/kg TMP significantly attenuated up-regulation of TNF-α, IL-1β, and IL-6 mRNA induced by the LPS injection. In addition, TMP (30 mg/kg) significantly reduced the number of COX-2 positive cells in the hypothalamus, Conclusion : These results indicate that TMP has an anti-inflammatory effect on neuroinflammation, especially in the hypothalamus, induced by LPS i.c.v. injection and suggest that TMP-containing Ligusticum wallichii may play a modulatory role on the systemic responses following hypothalamic inflammation.

        심장 점액종의 임상적 고찰

        N/A Background: Cardiac myxoma is uncommon, even though accounts for 50% of primary cardiac tumor, histologically benign disease but may be lethal because of its strategic position. It has diverse clinical manifestations mimicking various diseases, which can be described under the three headings: constitutional, obstructive and embolic and the diagnosis can be easily missed unless clinician has high index of suspicion. A correct diagnosis is made before operation with the development of echocardiography. We reviewed our clinical experience in diagnosis and management of 21 cases of myxomas from 1985 to 2000. Methods: There were 4 males (19%) and 17 females (81%). Their ages ranged from 17 to 66 years (mean 42.2 years). All the patient`s medical records on their clinical features, laboratory findings, chest X-rays, electrocardiograms, echocardiographic findings, operative findings and post operative follow-up were reviewed. Results: Seventeen cases (81%) of the myxomas originated in left atrium and four (19%) in right atrium. Most common symptom was that of intracardiac obstruction such as dyspnea or dyspnea on exertion (62%). Fatigue which accounted for most of the constitutional symptom found in 38% of the 21 patients while embolism accounted for 38%. In all patients, echocardiography was used for diagnosis and the diagnostic accuracy of echocardiography was 100%. All patient s received surgical treatment and immediate postoperative complications occurred in seven patients (33%). Among them, serious complications were found in two cases and one expired due to congestive heart failure. Follow-up duration ranged from 2 months to 15 year s. There was no recurrence or other problem in all patient s during the follow-up period. Conclusion: Although clinical suspicion is difficult due to the nonspecific presentation of myxoma, the diagnosis can be made easily with echocardiographic examination in all cases and surgical excision of myxoma may be curative with good long-term result.(Korean J Med 62:49-57, 2002)

        가족성 고콜레스테롤혈증에서 아포지단백 E 유전자 다형성의 빈도분포 및 관동맥질환과의 연관성

        N/A Objectives: Apolipoprotein E genetic polymorphism was screened in subjects with FH to determine the genetic effects of Apo E polymorphism on basal cholesterol levels, severity of coronary atherosclerosis and response to lipid lowering therapy using HMG CoA reductase inhibitor. Methods: Total C unrelated patients with FH(M:F=24:21, 48.0/11.5yr.) were included in this study. Apolipoprotein E genetic polymorphism was screened. Clinical parameters were checked. Change of lipid profile to lovastatin; 20mg/d(n=19), 4)mg/d(n=12), and of Achilles tendon thickness were analysed. Results: 1) Genotype frequencies of E2/3, 3/3, 4/3 were 8.9, 60.0, 31.1% respectively, and allele frequencies of ε 2, ε 3, and ε 4 were 0.044, 0.800, and 0.155 respectively. 2) Presence and degree of coronary atherosclerosis, the thickness of Achilles tendon and lipid levels were not significantly different by apolipoprotein E genotype, 3) On multivariate study, age, triglyceride and cholesterol /high density lipoprotein were significantly related to presence of coroanry atherosclerosis. 4) Percent reduction of LDL-cholesterol by lovastatin was significantly low in subjects having E4/3 genotype than those having E3/3 genotype (p=0.05; 40mg/d), and the percent reduction of Achilles tendon thickness was significantly low in subjects having E4/3 genotype than those having E3/3 genotype (p=0.037; 20mg/d). Conclusion: The distribution of apolipoprotein E genotype in patients with familial hypercholesterolemia was not significantly different with that in normal subjects. But apolipoprotein E polymorphism may affect the reduction of LDL-cholesterol and Achilles tendon xanthoma with medication of HMG CoA reductase inhibitor in patients with familial hypercholesterolemia.

        가토의 대퇴골에 식립한 3종류의 수산화인회석 코팅 임플란트에 대한 제거회전력 비교 연구

        Introduction: This study compared the strength of osseointegration as determined by the resistance to reverse torque rotation of three different hydroxyapatite coated implants in the rabbit femur model. Materials and Methods: Three hydroxyapatite coated implants (HAPTITE), Tapered Screw-Vent (TSV) and BioTite-H - were used. A total of 40 implants were placed in the femur of 20 adult male rabbits. The animals were divided into two groups. In group A (n=10); one HAPTITE was placed into each right femur and one TSV was placed into each left femur. In group B (n=10); one HAPTITE was placed into each right femur and one BioTite-H was placed into each left femur. Five rabbits of each group were sacrificed at 4 and 8 weeks. The implants were removed by reverse torque rotation using a digital torque-measuring device. A total of 40 implants in 20 rabbits were used for the removal torque measurements. Results: In the Group A, 4 weeks after implant placement, the mean removal torque for the HAPTITE and TSV was 70.7±31.6 N cm and 28.9± 15.1 N cm, respectively. Eight weeks after implant placement, the mean removal torque for the HAPTITE and TSV was 87.9±26.2 N cm and 54.9± 22.4 N cm, respectively. In the Group B, 4 weeks after implant placement, the mean removal torque for the HAPTITE and BioTite-H was 58.0±29.6 N cm and 37.7±14.1 N cm, respectively. Eight weeks after implant placement, the mean removal torque for the HAPTITE and BioTite-H was 91.4± 47.1 N cm and 30.8±9.8 N cm. HAPTITE showed a higher removal torque than the other implants. Conclusion: These results suggest that HAPTITE increases the strength of osseointegration significantly as determined by the resistance to reverse torque rotation.

        Bt 벼의 토양미생물상 영향 비교평가

        경제적 및 농업적 장점은 유전자 변형 작물 재배면적의 증가를 가져왔다. 그러나 유전자 변형 작물의 상업적 재배전에 유전자 변형 작물의 인간건강 및 환경에 미칠 잠재적 위해성에 대한 면밀한 검토가 필수적이다. 본 연구에서는 Bt벼의 토양미생물 군집에 미치는 영향을 조사하였다. 토양화학성분을 분석한 결과, Bt벼와 낙동벼 근권토양 간 화학 성분의 유의성 있는 차이는 없는 것으로 조사되었다. 재배전, 재배초기, 최고 분얼기의 토양미생물 군집밀도를 조사했을 때 Bt벼 근권토양의 세균, 방선균, 진균 군집밀도는 낙동 벼와 유사한 수준으로 나타났다. 시기별 DGGE 분석결과 Bt 벼 근권토양 전체미생물상은 낙동벼와 차이가 없는 것으로 조사되었다. Pyrosequencing을 통한 Bt벼와 낙동벼의 미생 물 군집조성을 조사한 결과 주요 미생물상 분포에 있어서도 매우 유사한 양상을 나타내었다. 위의 결과들을 종합해볼 때 Bt 재배에 따른 토양미생물상에 미치는 영향은 미미한 것으로 사료된다. 수확 후 벼 잔존물이 토양환경에 미치는 영향에 대해서는 좀 더 연구가 진행되어야 할 것이다. The cultivation of genetically modified (GM) crops has increased due to their economic and agronomic advantages. Before commercialization of GM crops, however, we must assess the potential risks of GM crops on human health and environment. The aim of this study was to investigate the possible impact of Bt rice on the soil microbial community. Microbial communities were isolated from the rhizosphere soil cultivated with Bt rice and Nakdong, parental cultivar and were subjected to be analyzed using both culture-dependent and molecular methods. The total counts of bacteria, fungi, and actinomycetes in the rhizosphere of transgenic and conventional rice were not significantly different. Denaturing gradient gel electrophoresis (DGGE) analysis of PCR-amplified 16S rRNA genes revealed that the bacterial community structures during cultural periods were very similar each other. Analysis of dominant isolates in the rhizosphere cultivated with Bt and Nakdong rice showed that the dominant isolates from the soil of Bt rice and Nakdong belonged to the Proteobacteria, Cloroflexi, Actinobacteria, Firmicutes, and Acidobacteria. These results indicate that the Bt rice has no significant impact on the soil microbial communities during cultivation period. Further study remains to be investigated whether the residue of Bt rice effect on the soil environment.

        본 연구의 목적은 Bt 배추의 토양미생물 군집에 미치는 영향을 조사하기 위한 것이다. 토양미생물 조사에 앞서 토양화학성분을 분석한 결과, Bt 배추와 일반품종 배추 근권토양 간 화학성분의 유의성 있는 차이는 없는 것으로 조사되었다. 생육 최대 성숙기의 토양미생물 군집밀도를 조사했을 때 Bt 배추 근권토양의 미생물 군집밀도 범위가 일반 배추 근권토양의 미생물 군집밀도 범위내에 속하는 것으로 나타났다. 월별 DGGE 분석결과 일반품종 배추 근권토양 미생물 군집에 비해 Bt 배추 근권토양 미생물 군집의 변이는 없었다. 형질전환 벡터 유래 유전자부위를 이용하여 유전자의 수평이동성을 조사했을 때 Bt 근권토양 유래 DNA에서는 벡터 유래 유전자가 검출되지 않았다. The aim of this study was to investigate the possible impact of Bt Chinese cabbage on the soil microbial community. Microbial communities were isolated from the rhizosphere of one Bt Chinese cabbage variety and four varieties of conventional ones and were subjected to be analyzed using both culture-dependent and molecular methods. The total counts of bacteria, fungi, and actinomycetes in the rhizosphere of transgenic and conventional Chinese cabbages were observed to have an insignificant difference. Denaturing gradient gel electrophoresis (DGGE) analysis of PCR-amplified 16S rRNA genes revealed that the bacterial community structures were very similar to each other and this genetic stability of microbial communities was maintained throughout the culture periods. Analysis of dominant isolates in the rhizosphere of transgenic and conventional Chinese cabbages showed that the dominant isolates from the soil of transgenic Chinese cabbage belonged to the Bacilli and Alphaproteobacteria, while the dominant isolates from the soil of conventional cabbage belonged to the Holophagae and Planctomycetacia, respectively. These results indicate that the Bt transgenic cabbage has no significant impact on the soil microbial communities.

        의과대학생들의 성격성향과 관련된 환자중심적 태도

        Purpose: The patient-centered attitude is an effort to understand patients themselves as well as their disease. The doctor-oriented approach has been shifting to one where patients and participate in the deciding of medical service. We performed this study to investigate the patient-centeredness and influencing variables associated with personality traits. Methods: We recruited 94 medical students before their psychiatric clinical clerkship, and all subjects were assessed by a structured questionnaire including socio-demographic data, Minnesota Multiphasic Personality Inventory (MMPI), Meyers-Briggs type indicator (MBTI), the patientpractitioner Orientation Scale (PPOS), and the Authoritarian Personality scale (AP), The AP and PPOS questionnaires were repeated after the psychiatric clinical clerkship. Results: After the psychiatric clinical clerkship, the AP scores were significantly lower than before, and the PPOS significantly higher than before. The change in the AP scores were related to MBTI, correlated with MMPI subscales, however, inversely correlated with the changes in PPOS after the clinical clerkship. Conclusion: The authoritarian personality trait was related to, not only the personality trait including MBTI and MMPI subscales, but also to patient-centeredness in medical students.
