        부분공간 간섭 정렬을 이용한 다중 셀 상향링크 시스템에서 합용량 향상을 위한 레퍼런스 벡터 다이버서티

        We propose a reference vector diversity method in multi-cell multi-user uplink system with the subspace interference alignment to obtain higher sum rate capacity. The proposed method transmits several reference vectors before the data transmission, and selects the best reference vector to maximize the cell sum rate. The proposed method provides higher sum-rate capacity compared with the previous interferenc alignment. Simulation result exhibits the proposed method improves the sum-rate capacity by 60%. 본 논문은 다중 셀 상향링크 시스템의 셀 간 간섭을 완화시키기 위한 부분공간 간섭 정렬을 적용했을 때 높은 채널 이득과 높은 합용량을 얻기 위해 간섭 신호공간 형성에 사용되는 레퍼런스 벡터 설정 방법을 제안하였다. 제안된 방법은 레퍼런스 벡터의 설정에 따라 채널과의 매칭이 달라지고 이로부터 셀의 합용량이 변화하는 레퍼런스 벡터의 다이버서티를 얻는 것이다. 각 셀의 처리량을 최대화시키기 위해 전송 전에 여러 개의 레퍼런스 벡터를 전송하고 이중에서 최적의 레퍼런스 벡터를 선정함으로써 기존의 간섭정렬방식보다 높은 채널 이득과 높은 합용량을 얻을 수 있다. 전산 모의실험을 통해 제안된 방법을 적용한 경우 임의로 설정된 레퍼런스 벡터를 설정했을 때의 셀 합용량에 비해 약 60% 향상된 값을 얻었다.

        Bone Regenerative Effects of Biphasic Calcium Phosphate Collagen, Bone Morphogenetic Protein 2, Mesenchymal Stem Cells, and Platelet-Rich Plasma in an Equine Bone Defect Model

        Fractures in the horse industry are challenging and a common cause of death in racehorses. To accelerate fracture healing, tissue engineering (TE) provides promising ways to regenerate bone tissues. This study aimed to evalu- ate the osteogenic effects of biphasic calcium phosphate collagen (BCPC) graft, bone morphogenetic protein 2 (BMP2), mesenchymal stem cell (MSC), and platelet-rich plasma (PRP) treatments in horses. Four thoroughbred horses were included in the study, and, in each horse, three cortical defects with a diameter of 5 mm and depth of 10 mm were formed in the third metacarpal bones (MC) and metatarsal bones (MT). The defects were randomly assigned to one of six treatment groups (saline, BCPC, BMP2, MSC, PRP, and control). Injections of sa- line, BMP2, PRP, or MSCs were made at 1, 3, and 5 weeks after defect surgery. Bone regeneration effects were assessed by radiography, quantitative computed tomography (QCT), micro-computed tomography (μCT), histopathological, and histomorphometric evaluation. The new bone ratio (%) in the histomorphometric evaluation was higher in the BMP2 group than in the control and saline groups. Radiographic and QCT values were significantly higher in the BCPC groups than in the other groups. QCT values of the BMP2 group were significantly higher than in the control and saline groups. The present study demonstrated that BCPC grafts were biologically safe and showed osteoconductivity in horses and the repeated injections of BMP2 without a carrier can be simple and promising TE factors for treating horses with bone fractures.

        Tenoscopy for Acute Septic Digital Flexor Tenosynovitis Treatment in 13 Thoroughbred Horses

        Septic tenosynovitis of the digital flexor tendon sheath (DFTS) is a potentially career-ending and lifethreatening problem in horses. This study aimed to describe the outcomes of tenoscopy for the treatment of acute septic digital flexor tenosynovitis in horses. Tenoscopy was performed on 13 Thoroughbred horses with acute septic tenosynovitis of the DFTS. Surgical time was 56-148 min (mean 85.6 min, median 84.0 min). In the synovial fluid analysis, mean white blood cell count, mean neutrophil proportion, and mean total protein were 42.9 × 103 cells/μl (range, 7.2-109.5 cells/μl), 89.5% (range, 68-97%), and 4.0 g/dl (range, 2.5-5.2 g/dl), respectively. Microbial growth in the synovial fluid culture was detected in 2 of 11 horses. All horses survived and returned to their intended use without complications. The present study demonstrated that the tenoscopy is useful for treating acute septic tenosynovitis of the DFTS in horses.

        Sedative and analgesic effects of intravenous xylazine and tramadol on horses

        This study was performed to evaluate the sedative and analgesic effects of xylazine (X) and tramadol (T) intravenously (IV) administered to horses. Six thoroughbred saddle horses each received X (1.0 mg/kg), T (2.0 mg/kg), and a combination of XT (1.0 and 2.0 mg/kg, respectively) IV. Heart rate (HR), respiratory rate (RR), rectal temperature (RT), indirect arterial pressure (IAP), capillary refill time (CRT), sedation, and analgesia (using electrical stimulation and pinprick) were measured before and after drug administration. HR and RR significantly decreased from basal values with X and XT treatments, and significantly increased with T treatment (p < 0.05). RT and IAP also significantly increased with T treatment (p < 0.05). CRT did not change significantly with any treatments. The onset of sedation and analgesia were approximately 5 min after both X and XT treatments; however, the XT combination produced a longer duration of sedation and analgesia than X alone. Two horses in the XT treatment group displayed excited transient behavior within 5 min of drug administration. The results suggest that the XT combination is useful for sedation and analgesia in horses. However, careful monitoring for excited behavior shortly after administration is recommended.

        Frontonasal Bone Flap for Treating a Paranasal Sinus Cyst Diagnosed by CT in a Thoroughbred Racehorse

        A 3-year-old Thoroughbred gelding was referred to the Jeju National University Equine Hospital with prolonged unilateral nasal discharge for several months. A sinus cyst was suspected based on the history, symptoms, endoscop- ic findings, and radiographic results. Computed tomography (CT) scan revealed that the cyst (7.5 × 8.0 × 10.0 cm) was located between the rostral maxillary sinus and the frontal sinus, causing distortion and remodeling of the overlying bones, swelling of the overlying cutaneous tissues, and a slight deviation of the nasal septum. Standing surgery under sedation was decided to remove the cyst. A frontonasal bone flap was performed using an oscillating bone saw, and the cyst filled with pus was removed. To support the approach to the maxillary sinus, trephination was performed. The horse was treated with sinus lavage via catheterization, nebulization, antibiotics, and nonsteroidal anti-inflammatory drugs. The horse was discharged 18 days after the surgery without signs of any complication. This case showed that a CT scan could be a valuable tool for the diagnosis and subsequent surgical management of paranasal sinus cysts in hors- es. Also, the frontonasal bone flap was useful for exposing and removing the large-sized of cyst in the paranasal sinus.

      • 만성 기관지염 환자에서 격렬한 기침으로 유발된 우측 복벽 내에 발생한 활동성 출혈 및 혈액량감소쇼크 1예

        기침은 갈비뼈 골절, 말로리바이스증후군, 기관기관지 출혈, 기흉, 복부 근육 및 근막의 손상 등 다양한 합병증을 유발할 수 있다. 저자 등은 만성기관지염 환자에서 격렬한 기침으로 우측 복벽 내에 발생한 활동성 출혈과 동반된 혈액량감소쇼크 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 증례: 85세 남자가 2주간 지속된 기침과 우하복부 복통을 주소로 내원하였다. 환자는 40년간 기수련과 복식호흡을 해왔고, 만성기관지염으로 지속적 치료를 받아온 기왕력이 있었다. 내원 시 흉부 엑스선에서 우폐 중엽에 폐렴이 있었고, 우하복부에 압통 및 점상 출혈이 있었으나 당일 시행한 복부단층전산촬영에서 복통과 관련하여 특이 소견은 없었다. 환자는 입원 후 진해제를 투여했으나 산발적으로 격렬한 기침이 있었고 의료진의 만류에도 지속적인 복식호흡을 하였다. 입원 2일째 오전 6시경 어지러움, 발한, 혈압 저하 및 우하복부에 10×10 cm 크기의 압통을 동반한 복부 종괴가 촉지 되었고 재시행한 복부단층전산촬영에서 우측 복벽 내에 활동성 출혈과 10×10 cm 크기의 혈종이 있었다(아래사진). 중심정맥 삽관 및 수혈 등 응급 처치를 하였으나 활동성 출혈에 의한 혈액량감소쇼크가 진행되어 환자는 사망하였다. 결론 및 고찰: 기침에 의해 유발된 복직근초 혈종에 관한 증례는 보고된 바 있으나 우측 복벽 내 활동성 출혈 및 혈종은 매우 드문 증례이다. 본 증례는 만성기관지염 환자에서 기침과 복식호흡으로 인해 우측 복벽 내 동맥의 손상으로 활동성 출혈과 혈종, 저혈량성 쇼크가 나타난 예로, 저자 등은 환자에 대한 자세한 문진과 기침으로 유발될 수 있는 중증 합병증에 대한 신중한 평가의 중요성을 되짚어 볼 수 있었다.

        Computed Tomographic Findings in a Foal with Acute Hindlimb Lameness

        A one-month-old Thoroughbred colt presented with left hindlimb lameness grade 5/5, according to the American Association of Equine Practi- tioners’ lameness scale. The colt started showing signs of lameness two weeks earlier without being involved in an accident. A local veterinarian examined the foal; radiography revealed no significant findings under the hip joint. No im- provement was noted after 15 days of non-steroidal anti-inflammatory drugs (NSAIDs) medication. On presentation at our hospital, ultrasonography was performed, which revealed no significant findings in the iliac wings. The foal underwent a computed tomography (CT) scan under general anesthesia. CT re- vealed bone cysts in the following that could have caused the lameness: the left transverse process of the 5th, 6th lumbar, and the 1st sacrum vertebrae; osteo- phytes in the auricular surface of the ilium, suggestive of sacroiliac arthritis. The foal recovered smoothly from anesthesia with assistance. The foal was treated with NSAIDs and rested for more than six months. The owner reported that the foal showed no lameness one year later. CT revealed bony changes in the lumbo- sacral region that were not detected by radiography and ultrasonography, sug- gesting that CT could be useful for detecting abnormalities in the pelvic region of horses.

        말에서 Granulosa Cell Tumor에 의한 무발정 치료 예

        An 8-year-old Thoroughbred mare showing anestrus for longer than 6 months with left ovarian mass was referred to Veterinary Medical Teaching Hospital of Seoul National University. For 6 months, the mare had received 2 times hormonal therapies with PGF2α and progesterone. The mare had not responded to these therapies and not shown estrous behavior. On rectal palpation, the left ovary was enlarged and contralateral ovary was small and inactive. Transrectal ultrasonographic examination of the left ovary revealed a mass composed of multiple, irregularly shaped follicles. Granulosa cell tumor (GCT) was suspected according to her history. Ovariectomy was performed through the left flank laparotomy with a standing position in a simple chute located outside of horse farm. The mass was diagnosed as GCT on histopathology. The mare was recovered without any problem and became pregnant next spring.

        Isolation of Peripheral Blood-Derived Mesenchymal Stem Cells in Mares and Foals

        Peripheral blood-derived mesenchymal stem cells (PB-MSCs) have shown promise in cell-based therapy, as they can be harvested with ease through minimally invasive procedures. This study aimed to isolate PB-MSCs from foals and mares and to compare the proliferation and cellular characteris- tics of the PB-MSCs between the two groups. Six pairs of mares and their foals were used in this study. MSCs were isolated from PB by direct plating in a tissue culture medium, and cell proliferation (population doubling time [PDT], and col- ony-forming unit-fibroblast assay [CFU-F]), and characterization (morphology, plastic adhesiveness, colony formation, trilineage differentiation) were exam- ined. There was no significant difference in the PB-MSC yield, CFU-F, and PDT between the mares and foals. PB-MSCs from both mares and foals showed typ- ical MSC characteristics in terms of spindle-shaped morphology, plastic adhesive properties, formation of colonies, trilineage differentiation. These results suggest that PB-MSCs isolated from horses, both adult horses, and foals, can be used for equine cell-based therapy.
