        치매노인 가족의 누적스트레스 영향요인

        본 연구는 치매노인 발생이라는 가족 위기에 대하여 이를 돌보는 치매가족의 누적스트레스 정도를 확인하고 누적스트레스에 영향을 미치는 변수를 규명하기 위한 서술적 조사 연구이다. 대상자는 가정 내에서 동거를 하며 치매노인을 돌보는 가족 중 편의표출법에 의해 선택된 102명이었다. 연구도구로는 McCubbin(1987)의 누적스트레스 측정 도구, McCubbin, Patterson과 Thompson(1991)의 가족 강인성 측정 도구, McCubbin, Patterson과 Glynn(1982)의 사회적지지 측정 도구, McCubbin, Larsen과 Olson(1982)의 친척 및 친구지지 측정 도구, 그리고 McCubbin, Larsen 과 Olson(1981)에 의해 개발된 가족의 문제해결 및 대응전략 측정 도구를 이용하였다. 본 연구결과 치매노인을 돌보는 가족의 누적스트레스 정도는 평균 1.74점(범위 1.3-2점)로 나타나 비교적 높은 누적스트레스를 나타내었다. 가족의 강인성, 사회적지지, 친척 및 친구지지, 가족의 문제해결 및 대응전략의 점수가 높을수록 가족의 누적 스트레스 정도가 낮아지는 것으로 나타났다. 가족의 누적스트레스에 영향을 미치는 주요변수들은 8%의 설명력을 보여주었고 여기에 치매노인을 돌보는 가족의 일반적 특성 중 주간호제공자가 인지한 본인의 건강상태, 치매노인의 건강상태, 하루 중 치매노인을 돌보는 시간, 치매노인을 돌본 기간, 치매가족의 총수입, 치매노인을 돌보는데 드는 비용 등 모든 변수를 중심으로 다중 회귀분석을 한 결과 이 모형에서는 21.7%의 설명력을 보여주었다. 이 변수들 중 치매가족의 총수입과 치매노인을 돌보는데 드는 비용이 통계적으로 유의하게 나타났는데, 이는 가족의 누적스트레스를 감소하기 위해서는 그 어떤 변수보다도 가족의 안정을 위한 기본 요소인 경제부분이 많은 영향력을 미치고 있음을 알 수 있었고, 차후 치매노인을 돌보는 비용을 감소할 수 있는 정책적이고 구조적인 장치가 필요함을 시사하고 있다. The purpose of this study was to identify the factors influencing file-up family stress in the caregivers of patients with dementia. Data was collected by questionnaires from 102 families with a member having a dementia, at neurology departments of hospitals, temporary shelter for dementia patient, and nursing homes for the elderly. The data was analyzed using descriptive statistics, pearson correlation coefficients, and multiple regression. In results, the score of file-up stress showed a significantly negative correlation with the score of level of family hardiness(r=-.200, p=.026), social support(r=-.361, p=.004), relative and friend support(r=-.416, p=.001), and F-COPES(r=-.345, p=.048). The multiple regression analysis revealed that the most powerful predictor of file-up family stress was family cost for patients with dementia. The results contribute to the understanding of Korean family caregivers' perceptions of caregiveing. Further researches should be conducted with the consideration of Korean traditional custom that family should take care of the elderly family members.

        The purpose of this study was to develop strategy direction for visiting home care through a Strengths, Weaknesses, Opportunities, and Threats(SWOT) analysis. The methodological research-based study employed an analysis model based on a literature review and expert interviews. The outcomes of the SWOT analysis formed the main components of the strategic planning approach, whose application to the hospital-based visiting home care program of the Seoul Veterans Hospital was expected to be effective in increasing the turnover rate and hospital incomes.

        This paper provides the guidelines from which to develop a visiting health care program at the Public Health Center in Korea and involves an expanded payment compensation system of preventive services based on the new long-term health insurance system in Japan. The function and management methods to achieve the goals practiced in a community contact center for elderly support which have recently been established will guide the specific directions and strategies that the Public Health Center should pursue. That is to say, comprehensive and continuous efforts will be put forth in preventive home visiting care targeting the elderly in certain jurisdictions. At this point in time in which the visiting care nursing program has not yet started, visiting health care provided by the Public Health Center oversees chronic diseases of a vulnerable population. But after it has been developed nationwide, the visiting health care system at the Public Health Center will be distinctive and focused on health promotion and prevention.

        금연의 준비단계이전에서 준비단계로, 준비단계에서 준비단계이후로의 이행에 대한 예측변인 : 청소년과 성인 비교

        Purpose: This study was done to identify predictors differentiating the preparation stage, which is the stage that the smoker is ready to quit smoking, between adolescent smokers and adult smokers. Method: A survey was conducted with 376 adolescent smokers in 4 high schools and 451 adult smokers in community settings in South Korea from August 2003 to April 2005. To identify the predictors before and after preparation in stages of change of smoking, logistic regression was done. Results: The predictors for before preparation in stages of change of smoking were process of change for smoking abstinence for adolescent smokers and depression for adult smokers. The predictors for after preparation in stages of change of smoking were self-efficacy for smoking abstinence for adolescent smoker and self-efficacy for smoking abstinence and smoking temptation for adult smokers. Conclusion: For each group, adolescent smokers and adult smokers, specific smoking intervention methods need to be developed based on the different ways individuals make the decision to quit smoking within their contexts.

        간호대학 졸업반 학생의 도덕성 발달수준 평가

        A study was designed to evaluate the level of moral development of 197 nursing students from five nursing schools in Korea. Data was collected through a self-reported questionnaire completed between March and April 2006. The Korean version of the DIT (Rest`s Defining Issues Test) was used to evaluate the level of moral development as measured by the score of P and stage 4. The data was analyzed by a t-test, ANOVA. The results were as follows: 1) the mean score of P and stage 4 was 41.79 and 16.31 respectively; 2) the score of P did not reveal statistically significant differences with respect to demographic variables; 3) The score of stage 4 revealed significant differences related to the level of education of each participant`s mother. These results indicate that moral development is not influenced by demographic variables. We recommend that a database of DIT scores for students in nursing and other medical fields be created. Such a database would be helpful for the further development of nursing ethics curricula.

        병원간호사의 도덕성 발달수준 평가

        This study was conducted to evaluate the level of moral development of clinical nurses in Korea. Data were collected through self-reported questionnaires received from 187 clinical nurses in Korea from March 1 to 15, April 2006. The Korean version of the DIT (Rest`s Defining Issues Test) was adopted to evaluate levels of moral development, which was measured by the score of P(%) and stage 4. The data were analyzed by a t-test, ANOVA. The results were as follows: 1) The mean score of P(%) and stage 4 was 39.66 (SD=13.34) and 16.20 (SD=8.12) respectively; 2) The score of P(%) revealed significant differences by marital status, nursing ethics education, and employment rank; 3) The score of stage 4 revealed significant differences by age, education level, birthplace, employment rank, and amount of clinical experience. In conclusion, while nursing ethics education should be helpful for enhancing nurses` moral development, this study suggests that it is necessary to develop the content of nursing ethics education and to evaluate its effectiveness.

        간호사의 셀프리더십과 직무만족, 개인성과간의 관계

        The purpose of this study was to identify the relationship among self-leadership, job satisfaction, and perceived outcome in nurses. Method: The subjects of study were nurses as a population who were working for the 2 of university hospitals which have over 500 beds in Kyong Ki Province as well as who have been working for over 6 months. The data was collected by questionnaires from 217 nurses and analyzed using descriptive statistics, pearson correlation coefficient, and multiple regression on SPSS 10. 0 version. Results: The degree of Self-leadership was 3.38 out of 5 as a mean point. There was a significant difference in self-leadership according to age, level of education, working department, and position. There was a significant positive correlation between self-relationship and job satisfaction, self-relationship and perceived outcome, and job satisfaction and perceived outcome. The stepwise multiple regression analysis revealed that the most powerful predictor of job satisfaction and perceived outcome was constructive thinking. Conclusion: In conclusion, the result was obtained that self-leadership is much correlated with job-satisfaction of nurse and outcome of nursing practice. Therefore, as a way to promote efficiency of nursing organization, the constant study about self-leadership with the various aspects is needed focusing on self-management and inner motivation as a new leadership paradigm.

        간호사의 셀프리더십과 간호업무성과간의 관계

        Purpose: This study was undertaken in order to suggest self-leadership as a way to promote organization efficiency of nursing organization. So the relation between self-leadership and outcome of nursing practice inorganization was studied. Method: The subjects of study were nurses as a population who were working for the 2 of university hospitals which have over 500 beds in Kyong Ki Province as well as who have been working for over 6 months. The data was collected by questionnaires from 215 nurses and analyzed using descriptive statistics, perason correlation coefficient, t-test and ANOVA on SAS Program. Results: Self-leadership of nurse got 3.51 grade out of 5 as a mean point. Each of the segments got 3.74 in self-compensation, 3.57 in self-expectation, 3.53 in goal-setting, 3.45 in rehearsal, 3.37 in self-criticism and 3.30 in constructive thinking in order. Independent performance of nursing practice at each items of outcome of nursing practice showed the highest correlations with the self-expectation among self-leadership segments. Constructive thinking, self-compensation, rehearsal, goal-setting were related like this in order, but self-criticism was related just a little. Dependent nursing practice has the highest correlation with the self-expectation, goal-setting, rehearsal, constructive thinking, self-compensation were similar as aboves in order, but self-criticism didn't show any similar correlations. Relationship practice has the highest correlation with the self-expectation, and goal-setting, constructive thinking, self-compensation, rehearsal were similar, while self-criticism did not show any correlations. Conclusion: In conclusion the result was obtained that self-leadership is much correlated with outcome of nursing practice. Therefore, as a way to promote efficiency of nursing organization, the constant study about self-leadership with the various aspects is needed focusing on self-management and inner motivation as a new leadership paradigm.
