Domestic violence is a phenomenon that requires extensive medical attention as it has both short-term and long-term effects on the victims as well as their families and communities. Therefore, clinicians need to have not only a general knowledge of domestic violence, but also need to be skilled in early identification of victimized patients, proper intervention methods, and assessment of sequelae. Furthermore, as health care service providers, clinicians should make efforts to refer patients to appropriate institutions as a means of preventing further violence. This paper presents general ideas about the high-risk domestic violence group, gives guidelines for clinicians who are likely to meet such victims as a patient, and describes sequelae of and treatments for domestic violence victims and their children. In conclusion, through such medical endeavors and the roles of experts, the desire for an increase in social sensitivity to domestic violence and for minimizing short-term and long-term sequelae can be fulfilled. In the future, it is necessary to develop more suitable guidelines for the treatment of domestic violence victims and to undertake studies into the attitudes of medical professionals facing domestic violence victims in Korea.
Risk Factors for Suicidal Ideation of the General Population

The aim of this study was to investigate risk factors of suicide ideation in general population. A total of 1,116 adults were sampled with randomization in an urban area. After excluding 116 participants due to incomplete answer, 1,000 participants (500 males;mean age 39.6 ± 11.6) completed self-report questionnaire including the Center for Epidemiologic Study Depression Scale (CES-D), the Beck Scale for Suicide Ideation (BSI),the Spielberger State-Trait Anger Expression Inventory (STAXI), the Barratt Impulsiveness Scale (BIS), and questions on weekday sleep duration. Results showed higher scores on the BSI were related with higher scores on the STAXI, CES-D, a family history of psychiatric illness, and short or long sleep duration (≤ 5 or ≥ 10 hr) (adjusted R2 = 0.151, P < 0.001,P < 0.001, P = 0.001, and P = 0.008, respectively). The predictive power of the STAXI scores, the presence of psychiatric family history and short or long sleep duration was stronger in the high CES-D group with scores of 16 or higher (adjusted R2 = 0.275,P < 0.001, P < 0.001, and P < 0.001, respectively). These findings suggest that suicide idea in general population may be related with the presence of family history for psychiatric illness, depressive mood, high anger and short or long sleep duration.
아동학대의 조기발견을 위한 제도적 개선 방안: 「아동학대범죄의 처벌 등에 대한 특례법」이 정한 의료인에 의한 신고를 중심으로

아동학대범죄의 처벌 등에 대한 특례법은 아동학대에 '범죄'라는 개념을 도입하고 신고의무자를 정함으로서, 아동학대 신고를 활성화하고자 하는 의도로 제정되었다. 동법 제10조는 의료인을 아동학대 신고의무자로 정하는데, 학대 피해 아동의 발견율이 0.5% 남짓인 현실을 고려한다면, 특정한 의학적 소견으로 비교적 명확하게 아동학대의 사실을 알 수 있는 의료인에게 신고의무를 부과한 입법적 조치는 타당하다. 향후 의료인에 의한 신고가 활성화되도록 하기 위한 제도를 마련하기 위하여 본 논문에서는 크게 세가지 제안을 한다. 첫 번째, 예비의료인과 의료인에게 아동학대 신고의무에 관한 교육을 꾸준하게 실시하는 것이다. 신고의무의 구체적인 내용 및 절차와 더불어, 의료인이 판단하기에 아동학대가 확실한 경우 뿐 아니라 "의심"되는 경우에도 신고의무가 있음을 주지시켜 아동학대를 조기에 발견하고 신고할 수 있도록 하는 것이 바람직하다. 두 번째, 신고 의료인 및 의료기관에 대한 보호 장치를 마련하여 한다. 현행법이 신고자의 보호를 위하여 필요한 조항들을 포함하고 있기는 하지만, 여기서 한발 더 나아가 의료인의 상황에 맞게 신고인을 보호할 수 있는 구체적인 지침을 마련하고, 그러한 지침에 대한 교육을 실시하여야 한다. 신고의료인에 대한 철저한 보호장치를 마련하는 것이 미신고의료인에 대한 면허 박탈 등의 처벌 조치를 강화하는 것보다 더 효과적인 신고의 유인을 제공할 것이다. 세 번째, 의료인이 모든 영유아의 건강상태를 주기적으로 모니터링할 수 있는 영유아건강검진제도를 학대아동 발견의 기회로 활용할 수 있도록 협진 체계를 마련하고, 영유아건강검진을 실시하는 의료인에 대하여 아동학대에 대한 체계적인 교육을 실시하는 것이 바람직하다. The Act on Special Cases Concerning the Punishment, etc. of Child Abuse Crimes intends to encourage reporting and punishment of child abuse by using the concept of 'crime' in child abuse cases. Article 10 of the Act imposes duty to report child abuse on a number of different professions, including medical professionals. Currently, more than 80% of child abuse cases occur among family members and the detection rate of child abuse is as low as 0.5% in Korea. On the other hand, medical professionals can identify child abuse relatively clearly with specific medical opinions. Therefore, it is necessary that medical professions are informed of this duty and does not bear disincentive from reporting. This paper makes policy suggestions in this regard. First, it is necessary that medical students and medical professionals receive regular education about the obligation to report child abuse. Education should include details of the reporting duty, as well as the fact that there is legal obligation to report even if the child abuse is "suspicious", not certain. Second, it is imperative to establish and implement protective programs for medical professionals who report child abuse. The current law provides a rough framework for protection of people who report child abuse, but it is necessary to produce detailed guidelines that are applicable in the context of medical setting. Education for medical students and medical professionals should include the contents of these guidelines, so that they do not hesitate reporting because they fear the aftermath of reporting. Third, it is highly recommended that physicians use the national Baby/Infant Health Checkup Program as an opportunity to detect child abuse. In Korea, the Baby/Infant Health Checkup Program provides physicians to periodically monitor health condition of all babies and children until the age of 71 months. In order to utilize this program for early detection of child abuse, it is imperative that the bBaby/Infant Health Checkup Program is modified to involve child abuse experts and medical professionals who participate in the program are educated about child abuse.
