        전신성 홍반성 루푸스에 병발된 신경인성 방광

        Neurogenic bladder in systemic lupus erythematosus (SLE) has been considered to occur rarely. Myelopathy, cyclophosphamide treatment, vasculitis have been implicated as the causes of the bladder pathology in patients with SLE. In this report, we describe a 46-year-old female with SLE who simultaneously combined with the neurogenic bladder. The diagnosis of neurogenic bladder, attributed to peripheral neuropathy, was made on the basis of cystometrography and clinical symptom. Steroid therapy couldn`t improve the clinical manifestation of neurogenic bladder despite the amelioration of the other lupus symptom.

        한국인 섬유근통증 환자의 결혼생활 만족도

        Objective: Fibromylagia (FM) is a rheumatic disorder characterized by widespread bodily pain, tenderness, and fatigue. It has a major impact on the quality of life including the quality of marital life. The objectives of this study were to assess the quality of marital life in patients with FM in Korea and to identify the correlation of the quality of marital life with disease severity. Methods: Subjects were the patient group, composed of 50 married patients with FM, and a comparison group composed of 50 healthy married adults with similar demographic characteristics. The quality of marital life was measured using the Korean-Marital Satisfaction Inventory (K-MSI). The K-MSI consists of two valid scales, one global affective scale, and eleven additional scales measuring specific dimensions of relationship distress. The clinical parameters were assessed by Korean Fibromyalgia Impact Questionnaire (KFIQ) and tender point, respectively from 50 patients with FM. Results: Of the 50 subjects, 48 (96%) were women with mean (±SD) age of 52.6(±8.8) years. The mean (±SD) duration of disease was 25.7 (±19.8) months. The mean (±SD) scores of KFIQ and tender point count were 52.9 (±14.9) and 13.1 (±1.6), respectively. The T-score of most scales of K-MSI of FM patients were higher than that of healthy controls (p<0.01). The scores of KFIQ were positively correlated with the eight scales of K-MSI and negatively correlated with one scale (Role orientation) of K-MSI. The tender point count was positively correlated with only one of scale (Conflict with in-law) of K-MSI. Conclusion: The results suggest that the quality of marital life in Korean patients of FM is lower than healthy subjects. In a simple correlation analysis, KFIQ was a meaningful variable correlated with the quality of marital life.

        불안정한 지지면에서의 평형 훈련이 편마비 환자의 균형 능력에 미치는 영향

        The purpose of this study was to investigate the balancing training effect on weight bearing ratio and postural sway on the unstable surface for the CVA patients with balance disability. The inclusion criteria were walking independently 20m distances. Eighteen subjects, 4 with right and 14 with left hemiplegic patients were participated in this study. The range of age was from 35 to 75 and the duration from onset time was from 1 to 38 months. The intervention program including balancing training that they were introduced for 10 minutes every other day during 12 weeks to experimental group, and general therapeutic exercise to control group. The result of this study were as following: 1. The balancing training on the unstable surface showed that the postural sway of left/right distribution would appear more effective on sitting with eyes opened. 2. The balancing training on the unstable surface showed that the postural sway of forward/rear distribution, left/right distribution and distance would appear more effective on standing with eyes opened. 3. The balancing training on unstable surface would increase more effective on the weight-bearing ratio of left/right difference and frequency. 4. The balancing training on unstable surface was statistical significant on the postural sway of left/right distribution and area in experimental group, and it was statistical significant on the postural sway of forward/rear distribution, left/right distribution and distance in control group, too.

        요통환자의 기립시 하지체중 지지특성에 관한 연구

        66 patients with low back pain were evaluated to determine if any differences of weight bearing were existed between the same side leg that feel pain(pain side leg) and that without pain (non-pain side leg) of lower extremities while weight bearing. 66 patients were divided into 3 groups according to pain pattern such as low back pain only(26). radiating pain only(21) and low back pain with radiating pain(19). Control group were 28 normal persons. The weight bearing was rated by Limloader(Model LLD-2000, Ver 1.2) and analyzed by paired and independent t-test, one-way ANOVA, Dunan's analaysis and Pearson's correlation analysis. The results of this study were as follows: 1. In patient group, significant difference og weight bearing rating between pain side leg and non-pain side leg was foun. There was borne significantly less weight bearing rating(14.68%) on the pain side leg(p<0001). 2. In normal group, there was no significant difference between left and right leg(P<05). But there was significant difference (3.21%) in absolute difference of both leg(p<.001). 3. The difference of 1 and 2 was 11.47% : more difference of weight bearing rating between both legs in patient group than that of the control group(p<.001). 4. There were significant differences of weight bearing rating in those 3 groups : low back pain group(10.30%). radiating pain group (17.90%) and low back pain with radiating pain group (17.10%) (p<.001). 5. There was significant correlation between pain intensity and difference of weight bearing rating(p<.05). The severer pain intensity, the more difference of weight bearing rating was found. 6. There was no significant correlation between the age, height and duration of symptom, etc(p<.05)

        This article review the animals physical therapy. It is so called veterinary physical therapy. The animals physical therapist is working to physical therapy for animals in veterinary clinical field. The veterinary medicine has not veterinary rehabiliation medicine or physical therapy for animals, also physical therapy field in Korea. So, This research will explain about animals physical therapy of Korea and other countries' by journals and internet information and suggest the future of the animals physical therapist. Finally, The veterinary physical therapy is not used to veterinary clinical field in now. But, The veterinary physical therapy will desire to the view of medical serve and economy by the host of animals and clinical veterinarian the future. Animal physical therapy is a new and rapidly developing field of health care for animals. The benefits of physical therapy have long been recognized in humans. More recently, work in the veterinary field has shown the same benefits of physical therapy to be true for animal patients. Performing orthopaedic or neurological surgery, or fitting a human patient with a cast or splint, and then discharging the patient is an outdated approach. In such cases, physical therapy is clearly warranted. Similarly, recent research has shown that post-surgical rehabiltation and therapy after injuries significantly improves the functional outcomes for animals. Physical Therapy is a healthcare profession directed at evaluating, restoring and maintaining physical function and movement. Working with the owner, veterinarian and often other healthcare professionals, a physiotherapist helps your animal to achieve and maintain optimal health and well-being. Equipped with a specialized university Bacheloriate education and intensively educated in Anatomy, Physiology, Biomechanics, Histology, Neurology, and Pathology, PT's are able to assess, diagnose and treat movement and function. Physical Therapy, Sports Medicine and Rehabilitation are recognized sciences applied to both humans and animals. The goals of physiotherapy are to relieve pain, restore range of motion/movement, improve function, prevent injuries and expand the physical potential of the patient. Once in the field, physical therapists actively continue their education to keep up to date on the latest treatments and technologies. Via continuing education courses, physiotherapists can learn how to apply their unique and specialized knowledge to other animal species.

        파킨슨 질환에 대한 문헌적 고찰

        Parkinson's disease(PD) is a progressive neurodegenerative disease that affects the functioning of the basal ganglia, a brain area that contributes to the control of movement. The disease is caused by the death of nerve cells in the brain that produce dopamine, a chemical messenger. The cells affected usually produce a neurotransmitter(a chemical that transmits nerver impulses) called dopamine, which acts with acetylcholine, another neurotransmitter, to fine-tune muscle control. In Parkinson's disease, the level of dopamine relative to acetylcholine is reduced, adversely affecting muscle control. When the supply of dopamine is depleted, the function of the basal ganglia is disrupted and its ability to control movement deteriorates. The result is that PD patients experience moderate rigidity, difficulty in initiating movements and slowness in executing them, and a rhythmical tremor at rest. Although the cause of Parkinson's disease is not known, genetic factors may be involved. About 3 in 10 people with the disorder have an affected family memeber. About 1 in 100 people over the age of 60 in the US have Parkinson's disease. And Parkinson's disease is slightly more common in men. The course of the disease is variable, but drugs may be the best effective in treating the symptoms and improving quality of life. But, The doctor may arrange physical therapy to help with physical mobility problems. It is important to continue to exercise and take care of your general health. Try to take a walk each day. Stretching exercises can help you maintain your strength and mobility. So, This papers will serve about the information of PD for clinical physical therapist. Finally, The aim of review is increasing approach method and technique for PD patients by the view of physical therapy.

        一部地域 老人들의 主權的 建康水潗에 영향을 미치는 IADL에 관한 硏究

        This research have been made to define how the IADL (Instrumental Activity of Daily Living) performance and the subjective health index of the Aged are affected by their residential circumstance, gender and age, and how deeply these two factors are related and interact. For the period of June 1 to July 31, 2000, we had conducted a questionnaire and direct interview with 693 persons over age sixty-five (65) in Daejon and in other adjacent area, grouping into three different residential types The Aged living at home, The Aged living at welfare facilities and The Aged living alone, and studied on how the IADL performance and the subjective health index of the Aged are influenced and interact as per their characteristics, daily activity and mentality. We had analyzed all the data obtained through this research by the method of : - analysis of frequency as per specific factors by SPSS 10.0/PC+, - x^2 test, - t - test, - ANOVA, - multiple regression analysis by factors. The research concludes followings : a. It appears that the three (3) factors such as gender, age and residential circumstance of the Aged deeply affect the IADL performance and subjective health index of the Aged. (p<0.01) b. With regard to IADL performance of the Aged by the gender, it was analyzed that the female-Aged gains 23.8 point on average, which shows the performance of the female-Aged is less independent. (p<0.01). In addition, it was also found that the IADL performance is becoming less and less independent following their age increasing. In analyzing IADL performance by the residential type, it appears that the Aged living at welfare facilities gains the lowest 21.5 points and is least independent. It was also found that the Aged living at welfare facilities need some assistances from others for their performing IADL. (20-24 point) (p<0.001) c. With regard to the subjective health index of the all-Aged participated in this research, the analysis indicates 8.8 point and this is considered as a point of general standard (7-10 point). In analyzing this index by the gender, the female-Aged gains only 8.6 point which explains a lot of female-Aged consider they are not really healthy. (p<0.001) In analyzing this index by the residential type, the Aged living at welfare facilities and the Aged living alone gain the comparatively lower point, - respectively 8.4 point for the Aged living at welfare facilities and 8.8 point for the Aged living alone. The Aged living at these two residential types express they are obviously in a bad situation of health. (p<0.001) d. With regard to the factors affecting the IADL performance and the subjective health index of the Aged, it was analyzed that the IADL performance can largely be affected by the factors such as depression, frequency of going-out and age rising, and that the subject health index can also deeply be affected by depression, pain and by how much they are satisfied with their current living conditions. e. It was analyzed that the interacting between the IADL performance and the subjective health index is not that strong but even weak. As a result, we were able to conclude that the IADL performances is less independent in case of the female-Aged, the Aged living at welfare facilities, and following the age rising. As for the subjective health state the Aged themselves are aware of, we concluded that the female-Aged, the Aged living at welfare facilities and the Aged living alone, are more critical about their health. From this research, we were able to realize that, when the OMT (Orthopedic Manual Therapy) needed, the physical therapists are really required to have a correct and cautious understanding of the situation in which the aged persons are, and take care of them with more concerns and more improved treatment.
