Mediastinum is the space located between the right & left lung lobes in the center of the thorax, and contains many vital organs such as heart, aorta, trachea, esophagus, vagus nerve, lymphatics. So the infection of mediastinal space causes a serious and potentially fatal process. Like other infections, surgical drainage, aggressive antibiotic therapy and supportive care are recommended for optimal outcome. Airway management in the presence of the neck, glottic edema, elevation of the tongue is a formidible problem especially in the case from odontogenic origin. We have recently encountered such a case resulting in a number of local and systemic complications.

The term osteomyelitis literally means inflammation of bone marrow. It is described as an inflammatory condition of bone primarily engaging the medullary Following circulatory collapse and ischemia, the involved portions of the bone become necrotic. Cellulitis and osteomyelitis are the most frequent complications of fractures of the mandible and maxilla, because there is an abscessed tooth in the line of fracture. Since most fractures are compound fractures, infection may develop because of contact with oral secretions or outside air. The treatment of chronic osteomyelitis is still under debate. Some authors rely on antibiotics alone, while others advocate combination with surgery. But when infection occurs, establish proper drainage and administer antibiotics. Author treated a case of infected mandibular fracture with mandibular osteotomy, sequestrectomy, iliac bone graft and hyperbaric oxygen therapy, who had suffered suppurative osteomyelitis and mandibular nonunion following the open reduction of the mandibular fracture.

Sarcoma is a malignant tumor originated from bone, cartilage, fat tissue, nerve, blood vessel, bone marrow, endothelium, etc. and for this reason it exhibits considerable variation not only clinical but histologic appearance. Osteosarcoma occurs chiefly in young persons and in patients older than 40 years it is usually associated with Paget s disease, irradiated bone, multiple hereditary exostosis or polyostotoc fibrous dysplasia and sometimes with preceding trauma. Radiographically it is divided into three forms: an osteoblastic or sclerosing type, an osteolytic type, and mixed type. Histologically it is divided into osteoblastic type, chondroblastic type, fibroblastic type. The treatment of osteosarcoma is radical excision, combination of radiation and chemotherapy but the prognosis is poor and overall 5-year survival rate is 20-40 %. We present two different type sarcomas of 22-year-old male and 56-year-old male patients which we performed surgical excision, combined chemotherapy and radiation therapy.
