        졸업학년 간호대학생의 임상실습만족도와 간호전문직관의 관계에서 자기효능감의 매개효과

        본 연구는 졸업학년 간호대학생의 임상실습만족도와 간호전문직관 관계에서 자기효능감이 어떤 효과가 있는지 확인하기 위해 시도되었다. 본 연구에는 온라인으로 연구 참여에 동의한 간호학과 4학년 128명이 참여하였으며, 자료수집은 2020년 5월 25일부터 2020년 6월 8일까지 시행하였다. 수집된 자료는 변수의 특성에 따라 빈도와 백분율, 평균과 표준편차, t-test, ANOVA로 분석하였고, 변수 간 상관관계는 Pearson’s correlation coefficient로 분석하였다. 임상실습만족도, 간호전문직관 및 자기효능감의 매개회귀 분석은 Baron과 Kenny(1986)의 분석 방법을 따랐다. 본연구결과, 대상자의 간호전문직관은 주관적 건강상태(F=9.08, p<.001), 대인관계(F=16.81, p<.001), 경제상태(F=5.13, p<.001)에서 유의한 차이를 보였다. 또한 임상실습만족도와 간호전문직관(r=.520, p<.001) 및 자기효능감과 간호전문직관(r=.535, p<.001) 간에 유의한 정적 상관관계가 있었다. 마지막으로 임상실습만족도와 간호전문직관의 관계에서 자기효능감의 부분매개효과가 본연구를 통해 확인되었다(Z=3.58, p=<.001). 이상의 결과를 토대로 간호대학생의 간호전문직관을 향상하는 방안을 마련할 때 임상실습만족도만을 높이는 것보다는 자기효능감도 함께 증진할 수 있는 임상실습 교육전략을 개발하는 것이 필요하며, 임상실습만족도 향상을 위해 임상실습 환경을 지속해서 개선하는 노력 역시 필요하다. This study attempts to confirm the effect of self-efficacy on the association between clinical practice satisfaction and nursing professionalism of graduated nursing students. The online study subjects included 128 fourth-year nursing students who agreed to participate in the survey, and data was collected from May 25 to June 8, 2020. Data were analyzed by frequency and percentage, mean and standard deviation, t-test, and ANOVA according to the characteristics of the variable. Pearsons correlation coefficient was applied to analyze the correlation between variables. Mediated regression analysis followed the analysis method of Baron and Kenny (1986). Results of this study revealed that the subjects nursing professionalism showed significant differences with respect to subjective health status, interpersonal relationship, and economic status. Clinical practice satisfaction and self-efficacy had a significantly positive correlation with nursing professionalism. Lastly, the partial mediating effect of self-efficacy was statistically significant in the relationship between clinical practice satisfaction and nursing professionalism. Our results indicate the necessity to develop clinical practice education strategies that sufficiently reflect opportunities to improve self-efficacy. Furthermore, continuous improvements are required in the clinical practice environment to enhance the nursing professionalism of graduated nursing students.

        종합병원 간호사의 근거기반실무 준비도와 근거기반실무 역량의 영향 요인

        Purpose: The study was done to verify evidence-based practice (EBP) readiness and factors contributing to EBP competency in general hospital nurses. Methods: Participants were 219 nurses working in 7 general hospitals in G-city and J Province. Data were collected from May 12 to May 28, 2015 and analyzed using one-way ANOVA, Pearson’s correlation and stepwise multiple regression. Results: Mean score for EBP competency was 3.89±0.83, for critical thinking disposition, 3.35±0.36, for EBP attitude, 3.53±0.66, for beliefs about value of EBP, 3.42±0.56, for ability in research performance, 2.86±0.57, for time to devote to EBP, 2.57±0.69 and for organization support for EBP, 3.36±0.46. EBP competency was significantly correlated with critical thinking disposition (r=.37, p<.001), beliefs about value of EBP (r=.43, p<.001), ability in research performance (r=.44, p<.001), and time to devote to EBP(r=.33, p<.001). Factors influencing EBP competency in general hospital nurses were research performance (β=.29, p<.001), beliefs about value of EBP (β=.24, p<.001), critical thinking disposition (β=.15, p=.014), recognition of EBP (β=-.18, p=.002), which together explained 35% of total variance. Conclusion: Findings indicate that introduction and activation of EBP in general hospitals is essential and development of systematic training programs to strengthen critical thinking disposition and EBP competency are needed.

        국내 근거기반간호연구의 동향

        Purpose: The purpose of this study was to analyze the current status of research related to evidence-based nursing and to suggest directions for the future of evidence-based nursing practice, education and research. Methods: A search was done of 20 research publications, including domestic nursing journals, nursing master"s theses and doctoral dissertations before November 2016. Finally, 183 studies were selected. The selected papers were analyzed using descriptive statistics and x<SUP>2</SUP> test with the SPSS/WIN 18.0 program. Results: Most of papers examined in this study were journal articles (80.9%). Meta-analysis (35.0%) was the most common study design. Methodological characteristics were as follows: before 2010 about 5% were documents that suggested Priori’ design, generation of PICO, search strategy, quality assessment and description of quality assessment outcome, but after 2011, these designs increased to 30.8%, 73.1%, 41.0%, 91.0% and 65.4%, respectively. The most frequent topics for evidence- based nursing implementation were evidence-based nursing readiness (16 papers). Highest frequency topics in systematic reviews and meta-analysis were studies that confirmed the intervention effect of exercise programs. The highest frequency topics in guideline were temperature control. Conclusion: Researchers" perceptions to improve research methodological quality and education to strengthen the research capability are necessary.

        근거기반실무 과정을 활용한 성인간호학 임상실습 모듈의 시범운영

        본 연구의 목적은 근거기반실무를 활용한 임상실습 모듈이 간호대학생의 근거기반 실무 지식, 태도, 역량, 적용의도 및 비판적 사고성향의 향상에 효과적인지 파악하기 위함이었다. 본 연구는 단일군 전후설계의 실험연구이다. 연구 참여에 동의한 연구대상자는 성인간호학실습에 임할 M대학교 간호학과 3학년 학생 30명이었다. 연구기간은 2016년 5월 2일부터 6월 10일까지였다. 성인간호학실습 모듈은 근거기반실무를 활용하여 간호과정을 수행하는 것을 목표로 2주 동안 진행되었고, 1회기 당 30분에서 3시간씩 총 6회기로 구성되었다. 모듈의 효과는 대응표본 t-test로 분석하였다. 연구결과, 근거기반실무를 활용한 성인간호학 임상실습모듈이 간호대학생들의 근거기반실무에 대한 지식(t=-10.67, p<.001), 태도(t=-2.34, p=.023), 역량(t=-9.03, p<.001), 적용의도(t=-2.40, p=.019)를 유의하게 향상시켰으나 비판적 사고성향의 유의한 차이는 없었다. 근거기반실무를 활용한 임상실습이 학생들의 지식, 태도, 역량은 물론 향후 실무에서의 적용의도를 향상시켰으므로 간호교육과정에 근거기반실무를 다각도로 활용할 필요가 있다. The purpose of this study was to figure out whether Adult Nursing clinical practicum module using evidence-based practice is effective in improving the evidence-based practice knowledge, attitudes, competency, intention to use and critical thinking disposition for Nursing students. This study is a one-group pretest-posttest experimental design. The study participants were third grade nursing students from M University who attended an adult nursing clinical practicum and 30 students agreed to participate in the study. The study period was conducted from May 2, 2016 to June 10, 2016. The Adult Nursing practicum module was conducted for two weeks with the aim of performing the nursing process using the evidence-based practice and consisted of 6 sessions, 30 minutes to 3 hours per session. The effects of the modules were analyzed by paired t-test. Research results showed that Adult Nursing clinical practicum module using evidence-based practice significantly improved student’s knowledge(t=-10.67, p<.001), attitudes(t=-2.34, p=.023), competency(t=-9.03, p<.001), and Intention to use(t=-2.40, p=.019) about evidence- based practice. But, there has not been a significant change in the critical thinking disposition. Since clinical practicum education using evidence-based practice has improved students knowledge, attitude, and competency as well as intention of future use, it is necessary to utilize evidence-based practice in nursing curriculum in various ways

        근거기반실무를 활용한 기본간호학 수업 운영의 효과

        근거기반실무 간호교육은 학습자의 수준에 따라 단계적으로 경험하며 숙련되도록 이끌어야한다. 본 연구는 근거기반실무의 일부를 통합한 간호학과 2학년의 기본간호학 수업을 운영하고 근거기 반실무의 기초역량 향상에 대한 효과를 검증하였다. 연구 설계는 비동등성 대조군 전후 유사실험설계이었다. 대상자는 2개 대학교의 간호학과 2학년 재학생 108명이었다. 실험군은 2016년 2학기 15주 수업기간 중 총 8주, 24시간의 근거기반실무를 활용한 기본간호학 수업에 참여하였고 대조군은 일반적 기본간호학 수업에 참여하였다. 자료는 자가보고형의 문제해결 적극성, 근거기반실무 적용의도, 그리고 비판적 사고성향 측정도구로 수집하였다. 수집된 자료는 SPSS/WIN 23.0 program을 이용하여 동질성은 Chi-square 검정과 독립표본 t-검정으로, 가설 검정은 독립표본 t-검정으로 분석하였다. 분석 결과 실험군은 대조군에 비해 문제해결 적극성과 근거기반실무 적용의도에서 유의한 증가를 보였다. 그러나 비판적 사고는 두 군 간에 차이가 없었다. 근거기반실무를 활용한 기본간호학 수업은 저학년 학생의 근거기반실무 기초역량인 문제해결 적극성과 근거기반실무 적용의도의 증가에 효과적이었다. 근거기반실무에 대한 학년별 목표에 적합한 교과과정에 의한 간호학생들의 근거기반실무 역량과 비판적 사고의 향상에 대한 장기적 노력이 더 필요하다. The nursing education with evidence-based practice (EBP) should set a differentiated education strategy according to the level of the learner, so that the students experience it step by step and it leads them to be skilled. The purpose of this study was to integrate and operate the EBP in the fundamental nursing classes of the second grade students and to verify the effectiveness on improvement of EBP basic competency of them. A non-equivalent control group pretest-posttest design was used. The subjects were 108 students in the second year of nursing at the two universities. The experimental group participated in the class for 8 weeks and 24 hours during the 15-weeks, second semester 2016 and the control group participated in general fundamental nursing class. Data were collected proactivity in problem solving, future use of EBP, and critical thinking disposition instrument and analyzed using SPSS/WIN 23.0 program with Chi-square test, independent t-test. The experimental group had a significant increase in proactivity in problem solving and future use of EBP compared to the control group. However, there was no significant difference in critical thinking disposition between the two groups. Findings indicate that the fundamental nursing class used EBP was an effective lesson to improve proactivity in problem solving and future use of EBP, which is the EBP basic competency for the lower grader. Long-term efforts are needed to improve the EBP competence and critical thinking of nursing students by curriculum appropriate to grade-based goals of EBP.

        초산모의 모유수유 이행 구조모형

        Purpose: This study aimed to develop a structural model to explain breastfeeding behavior of primipara based on information-motivation-behavioral skills. Variables were breastfeeding knowledge, breastfeeding motivation, breastfeeding self-efficacy, and parenting stress. Methods: Participants were first-time mothers who were breastfeeding infants from birth to 6 months. Data were collected from 5 pediatric hospitals and 1 public health center in Jeollanam-do from December 24, 2018 to February 28, 2019. A total of 285 questionnaires were included in the analysis. Data were analyzed using IBM SPSS Statistics ver. 20.0 and AMOS 20.0 programs. Results: Fitness statistics for the hypothetical model were appropriate (χ<sup>2</sup>=48.30 [degrees of freedom= 14, p<0.001], goodness of fit index=0.96, normed fit index=0.89, comparative fit index=0.92, standardized root mean square residual=0.06). Breastfeeding self-efficacy had the most direct and total impact on breastfeeding behaviors (β=0.38, p=0.01). Breastfeeding self-efficacy was directly and totally influenced by breastfeeding knowledge (β=-0.18, p=0.01), breastfeeding motivation (β=0.26, p=0.01). Parenting stress showed a direct effect on breastfeeding self-efficacy (β=-0.14, p=0.02) and breastfeeding behavior (β=0.20, p=0.01), confirming its modulating effect on both variables. Conclusion: Since breastfeeding self-efficacy was the most important variable for the breastfeeding behavior in first-time mothers, nursing interventions should be developed to promote breastfeeding self-efficacy including accurate breastfeeding information and enhanced breastfeeding motivation. Strategies that could relieve or adjust parenting stress should be considered since parenting stress had a causal relationship with breastfeeding self-efficacy and breastfeeding behavior.
