N/A Objective: to determine whether ovarian steroids directly regulate FSH release and B subunit synthesis at the pituitary level Methods: In vitro study. After applying ovarian steroids in cultured rat anterior pituitary cells, we evaluate the amount of relesed FSH and FSH B subunit using dot-blot hybridization. Results: Estradiol alone or GnRH-induced did not modulate FSH release and B ubunit synthesis. Although progesterone alone did not alter FSH release, GnRH-induced FSH release was significantly augmented by the treatment of progesterone. Progesterone alone stimulated FSH B mRNA level. Significant increase in FSH B mRNA level was also observed by the combined treatment of progesterone and GnRH. The effects of progesterone on GnRH-induced FSH release was significantly reduced by the treatment of progesterone antagonist, RU 486. The treatment with progesterone antagonist, RU 486 also abolished progesterone-induced FSH B subunit mRNA biosynthesis. Conclusion: From these results, it is, therefore, concluded that estradiol does not seem to be a major regulator for FSH synthesis but progesterone may exert its action at the pituitary level for the synthesis and release of FSH.
한국 여성에서 생식 노화에 따른 자궁 동맥 혈류 속도 파형의 변화에 관한 연구

N/A Objectives: In order to get the normal value of korean woman and investigate the clinical efficacy of uterine artery velocity waveform measurement in predicting reproductive ageing, Methods: we measured uterine artery doppler waveform using color doppler ultrasound. Study was done on cycle day 5-9 in 231 women. Pulsatility index and resistance index were measured and compared with each other. Results: RI was different significantly among age group 1, 3, 4 compared with group 5. PI was different among age group 2, 3 compared with group 5. When the age group was re-adjusted according to the menopausal state, RI was significant among group 1, 2 compared with group 3. PI was significant only between group I and 3. Conclusion: RI showed better predictive value compared with PI in predicting ovarian ageing.
비정상 자궁 출혈 환자의 진단에 있어서 경질식 초음파 , 식염수 주입 자궁 조영술 및 자궁경술의 진단적 효용성에 관한 연구

N/A Objective: To evaluate prospectively the efficacy and accuracy of transvaginal sonography(TVS), saline infusion sonohysterography(SIS) and hysteroscopy in the exploration of the uterine cavity. Methods: 71 consecutive patients were evaluated with SIS who showed abnormal TVS findings, using saline instilled through endocervically placed balloon catheter with concurrent vaginal sonography. Among them, 41 patients also underwent hysteroscopy and surgery. Transvaginal sonography, sonohysterography, hysteroscopy were compaired with pathologic reports. Results: Fifty-five of 71 sonohysterogram(77.5%) showed abnormal findings, Among them 41 patients have done hysteroscopy and biopsy. According to pathologic reports, sixteen patients were noted to have myoma(39.0%), twelve patients had polyps (21.3%), and both showed most frequent lesions. TVS, SIS, and hysteroscopy had a sensitivity of 94.4%, 91.1%, 94.4%, and a specificity of 40%, 42.8%, 60.0%, respectively and showed not so much different in detection rate. In case of submucosal myoma and polyps, hysteroscopy showed 100% sensitivity, and 92% specificity and showed much higher detection rate compared with SIS (81.2%, 92.0%). Conclusion: Transvaginal sonography and sonohysterography are good office diagnostic test in case of detecting variable gynecologic intrauterine abnormalities.

Two patients, bulging fetal membrane accompanied with cervical dilatation & effacement in the late second-trimester, were treated by modified Shirodkar method. Pregnancies were prolonged effectively and viable fetuses were delivered in both cases. Our cases may help make objective decision prior to attempting cerclage placement when cervix is markedly dilated and something should be done to save pregnancy.
정상월경여성에서 생식노화의 예측시 월경 3일째 FSH와 Estradiol 동시측정의 효용성에 관한 연구

"This study was designed to evaluate the efficacy of measuring day 3 FSH and Estradiol in predicting reproductive ageing in normal menstrating women. The prospective study compared day 3 FSH and Estradiol in 59 normal menstrating women at 40-45 years old with 30 normal menstrating women at 20-25 years old. Day 3 FSH and Estradiol(E2) eleveted significantly in old women compared with young one. However, addition of E2 to day 3 FSH explained an additional 10% of the variance from 0.321(FSH only) to 0.421. Combined measurement of FSH and E2 appears more predictable than any other single ovarian steroid hormone measurement in ptedicting reproductive ageing. "
40~50세 정상여성에서 난소예비의 예측에 있어서 월경주기간 FSH, LH, FSH와 LH 비 및 E2의 효용성에 관한 연구

"The ability of woman to conceive declines with age. But age related decline in reproductive potential ate exclusively within ovary and remaining ovarian potential is referred to as ovarian reserve. In order to investigate the clinical efficacy of ovarian steriod hormone in predicting reproductive ageing, we measured serum levels of follicle stimulating hormone (FSH), luteinizing hormone (LH) and estradiol (E) and its intra- and intercycle variability in healthy female volunteers with regular menstrual cycle between age 40 to 45 in two consecutive cycles. Daily serum samples were obtained on cycle days 1-4, Initially we evaluate above factors between each menstrual cycle and after that compacted day 1-4 as one variable and re-evaluated the difference between cycles as a group. LH showed marked intercycle variability (P<0.0001), but no difference were noted across study days and cycles. FSH showed no difference between study cycles in both compact day (P =0.541) and all individual study days and highest value was noted on day 3 (P=0.98). Also no difference was noted among FSH concentration in day 1-4 in both cycles. Even though FSH and LH ratios were not different among study days, there showed marked difference between cycles. E concentrations were not to have intercycle variability (P=0.228) but their significance were next to FSH. E2 values of day 4 were increased significantly compared to day 1-3 in both study cycles (P =0.0096, 0.004 respectively). Increments of FSH were not collerated with E2 in study cycles. Our data lead us to the conclusion that flexibility measuring in FSH can be introduced among day 1-4 and E can be applied to evaluate reproductive ageing. But to assess the predictive value of both factors and for clinical application, futher investigation should be followed. "
