Zywave; II 수차계로 측정한 굴절교정수술 대상안의 수술 전 안구수차 분석

Purpose: To report the distribution of ocular higher-order aberrations in candidates for refractive surgery measured by Zywave<sup>® II aberrometer. Methods: The present study included 232 eyes of 116 subjects. Ocular aberration data were obtained by measurements per eye using Zywave<sup>® II aberrometer. The mean Zywave spherical equivalent (SE) and higher order aberrations (HOAs) calculated in the central 6-mm zone and expressed as root mean square (RMS) values were analyzed. Correlation analysis was performed to assess the association between ocular HOAs and gender, age, SE refractive error, or central corneal thickness (CCT) and investigate the aberration symmetry between right and left eyes. Results: The average SE was -4.67 ± 1.83 diopters (D). The mean RMS values of total HOA, 3rd, 4th or 5th summated HOAs, coma, trefoil and spherical aberration (SA) were 0.421 ± 0.201 μm, 0.346 ± 0.206 μm, 0.202 ± 0.105 μm, 0.087 ± 0.048 μm, 0.241 ± 0.172 μm, 0.225 ± 0.154 μm and 0.136 ± 0.102 μm, respectively. There was no significant differences of the mean total HOA, summated HOAs, coma, trefoil and SA between genders, age and refractive errors, but the 3rd order trefoil was strongly related with myopia (r = -0.900, p = 0.008). There was symmetry of ocular aberrations between both eyes and the ocular aberrations were not correlated with CCT. Conclusions: Herein, we report the normative distribution of ocular aberrations in the myopic Korean population measured by Zywave<sup>® II aberrometer. 광범위한 장액성 황반부박리를 동반한 푸르처망막병증 1예

Purpose: To report a case of Purtscher’s retinopathy with diffuse serous macular detachment. Case summary: A 63-year-old male patient presented to the hospital with decreased visual acuity. Three days prior to visiting the hospital, he had an blunt injury to the thoracic region caused by a steel beam, and his best corrected visual acuity was 20/50 in the right eye and 20/160 in the left eye. On slit lamp examination nothing unusual was observed in either eye, but fundus examinations showed retinal hemorrhages and a cotton wool spots on the posterior pole of the left eye and nothing unusual in the right eye. On fluorescein angiography, severe nonperfusion was observed at the posterior pole of the left eye. On optical coherence tomography, there was diffuse serous retinal detachment at the posterior pole and inferior retina of the left eye while mild subretinal fluid was observed at the posterior pole of the right eye. Under the suspicion of Purtscher’s retinopathy in both eyes, oral prednisolone (40 mg) was prescribed and the dosage was gradually reduced. Six weeks after the treatment, best corrected visual acuity improved to 20/20 in the right eye, and 20/30 in the left eye. Additionally, subretinal fluid in the right eye completely disappeared. Conclusions: Purtscher’s retinopathy is known as an ocular disease occurring after traumatic events. However, serous detachment of the macula has rarely been observed in Purtscher’s retinopathy, and herein we report a case with diffuse serous macular detachment which responded to oral steroid treatment.

안와농양과 동반된 중심망막동맥 폐쇄 1예

Purpose: To report a case of rapidly progressed orbital abscess after central retinal artery occlusion. Case summary: A 60-year-old man with right periorbital pain and edema starting 3 days earlier visited the hospital. His first visual acuity was 1.0 for the right eye with peripheral edema and tenderness; severe hemorrhagic chemosis, proptosis (approximately 3 mm) and extraocular motility limitation were also observed. According to the CT findings, a 2.5 × 1 cm-sized encapsulated cystic mass was found in the right orbital cavity, along the medial orbital wall; with a diagnosis of orbital abscess, he was hospitalized with systemic antibiotic treatment and abscess drainage. On the second day of hospitalization, the best corrected visual acuity was reduced to light perception, and relative afferent pupillary defect, pale retina and cherry red spot were found. Therefore, emergent incisional drainage, optic massage, and antibiotics were given to the patient. Immediately after the surgery, the peripheral edema and proptosis improved; nevertheless, central retinal artery occlusion did not resolve and vision did not improve. Two months later, best corrected visual acuity for the right eye was counting fingers and central retinal artery occlusion showed no further improvement. Conclusions: No case report on rapidly progressing orbital abscess has been proposed in Korea to date. In the case of rapidly progressed orbital abscess, incisional drainage and antibiotics should be administered promptly. J Korean Ophthalmol Soc 2012;53(10):1523-1527

임베디드 소프트웨어는 국방, 항공 우주, 자동차와 같이 다양한 응용분야에서 그 중요성이 부각되고 있다. 이와 함께 시스템 자원의 효율성을 높이고 응용 소프트웨어 간 안전한 실행환경을 제공하기 위해서 자원 파티셔닝의 필요성이 강조되고 있다. 최근 임베디드 시스템의 응용 분야가 다양해지면서 주기적인 파티션과 함께 비주기적인 파티션들에 대한 요구가 증가하고 있다. 하지만 기존 시스템들은 비주기적인 파티션은 고려하고 있지 않거나, 주기적인 파티션에 비해서 우선순위가 낮게 취급하고 있다. 이러한 문제를 해결하기 위해서 본 논문은 높은 우선순위의 비주기적인 태스크 그룹을 지원하기 위한 구조를 제안하고, 리눅스의 cgroup 프레임워크를 확장하여 구현한다.

최근 모델링 및 시뮬레이션(M&S) 기술이 다양한 분야에서 적용되어 활용되고 있으며, 특히 국방 분야에서의 중요성이 더욱 부각되고 있다. 무기체계 획득 절차 전반에 활용되는 시뮬레이션 시스템을 신속하고 편리하게 개발하기 위해서는 모델링에서 디버깅에 이르는 통합된 환경이 제공되어야 한다. 본 연구에서는 GUI 기반의 DEVS 모델링 도구인 QUEST를 이용해 실제 무기체계와 연동되는 시뮬레이션 시스템의 개발 사례를 소개한다.

가상 문서의 효율적인 관리를 지원하는 대용량 링크정보 관리서버의 개발

지식기반 사회에서 보다 효율적인 정보이용을 지원하기 위해 디지털 도서관 시스템인 MIRAGE가 구현되어졌다. 인터넷이 비약적으로 성장하고, 접속하는 사용자의 수가 증가함에 따라 MIRAGE 시스템에서 관리하는 링크정보는 폭발적으로 증가하게 되었다. 따라서 링크정보를 검색할 경우 웹 정보 객체간의 다양한 링크정보를 이용하여 사용자 중심의 서비스를 제공하도록 요구되어지고 있다. 여기서 제안하는 링크서버는 링크가 가지는 유용한 정보를 체계적으로 분석하고 관리하여 검색에 사용함으로써 효율적인 검색 서비스가 가능하도록 제공하고 있다. 사용자는 자유롭게 웹상의 정보에 대해 링크정보를 만들고, 만들어진 링크정보는 데이터베이스에서 관리가 이루어진다. 하지만 웹상의 링크정보는 삭제가 되거나 변경이 될 경우가 있다. 여기에서는 시스템의 신뢰성을 향상시키기 위해서 링크서버는 따로 링크정보의 유효성을 주기적으로 검사를 하는 에이전트를 두고있다. 에이전트는 데이터베이스에 저장 되어진 링크정보가 항상 유효하도록 주기적인 검사를 통하여 항상 신뢰할 수 있는 링크서버가 동작이 되도록 지원을 하고 있다.

증례보고 : 안와농양과 동반된 중심망막동맥 폐쇄 1예

목적: 급격히 진행된 안와농양과 동반된 중심망막동맥 폐쇄 1예를 보고하고자 한다. 증례요약: 60세 남환이 3일 전부터 발생한 우측 안와 주위 통증 및 부종을 주소로 내원하였다. 우안 나안시력은 1.0이었으며 우안안압은 37 mmHg로 증가되어 있었고 3 mm의 우측 안구 돌출소견과 안구운동 장애를 보였다. 안와 전산화 단층 촬영에서 우측 안와내측의 2.5×1 cm 크기의 격막이 있는 낭성 종괴가 관찰되어 안와 농양 진단하에 전신 항생제 및 배농치료 위해 입원하였다. 입원2일째 우안 안압은 18 mmHg로 감소되었으나 우측 안와 주위 종창 및 발적은 지속되었고 우안 교정시력은 광각으로 감소하였으며 상대적 구심성 동공장애 및 창백해진 망막과 앵두반점 소견 관찰되어 응급 절개 배농술 및 안구마사지를 시행 후 전신 항생제 치료 유지하였다. 술 후 안와 주위 부종 및 안구돌출은 개선되었으나 중심망막동맥은 개통되지 않았으며 시력은 호전되지 않았다. 결론: 급격한 진행을 보이는 안와농양과 동반된 중심망막동맥 폐쇄의 예가 국내에선 알려진 바 없어 보고하고자 하였으며 이러한 안와 농양의 경우 절개 배농 및 전신 항생제 치료 등의 즉각적인 대처가 필요할 것으로 생각한다.
