PURPOSE: The object of the present study is to investigate the effects of the insole supporting medial longitudinal arch while walking in spastic cerebral palsy with pes planus. METHODS: Ten spastic bilateral cerebral palsy children with pes planus participated in this study. The insole were custom-made for the individual child. Muscle activity was measured by surface EMGs attached on tibialis anterior (TA), gastrocnemius (GA), vastus medialis oblique (VMO), biceps femoris long head (BF). temporal-spatial parameters such as velocity, step length, stride length, stance time, toe angle were collected while the subjects walked on the GAITRite system. RESULTS: The results of the present study were summarized as follows: 1. Muscle activities in mean EMGs while walking: Left VMO, Right TA, Left BF and GA revealed significant reductions after applying insole. 2. Muscle activities in peak EMGs while walking: Left TA and BF demonstrated reductions significantly after applying insole. 3. There were improvements in temporal-spatial gait parameters with insole: velocity, both step length, Right stride length and Right toe angle were increased(p<.05). CONCLUSION: Therefore the current study demonstrated that insole supporting the medial longitudinal arch would be effective on gait of the spastic cerebral palsy with pes planus.
트레드밀에서 뒤로걷기 훈련이 경직성 양하지 뇌성마비 아동의 보행에 미치는 영향: 사전 연구

The aim of the current study was to assess the effectiveness of backward gait training on the treadmill in patients with spastic diplegic cerebral palsy (CP). Twelve patients with spastic diplegic CP participated in the study. An 8-week course of backward gait training was administered to the subjects for 3 days per week. Pre-intervention and post-intervention assessments of temporal-spatial gait parameters, the symmetry of the bilateral lower extremity weight bearing, and gross motor function were analyzed using motion analysis system, force plate, and Gross Motor Function Measurement (GMFM). There were significant improvements (p<.05) in the measures of both step length and right stance phase time. Joint kinematics showed increase in right hip abduction in initial contact and terminal swing, right hip external rotation and knee flexion in mid-swing, left ankle dorsiflexion in initial contact and terminal swing (p<.05). The symmetry of the bilateral lower extremity weight bearing and GMFM also significantly increased (p<.05). These findings indicate that backward gait training using a treadmill is beneficial for patients with spastic diplegic CP.
대동작 기능분류 시스템 수준 I과 II 뇌성마비 환자에게 맞춤형 인솔 착용이 보행에 미치는 영향

The purpose of this study was to determine the effect on the temporal-spatial parameters and timed up and go tests with or without customized insoles for spastic cerebral palsy with Gross Motor Function Classification System (GMFCS) leves (I and II). GAITRite system was measured to temporal-spatial gait pareameters and gait speed were measured with time up and go test (TUG). Participants were 19 subjects diagnosed cerebral palsy and the statistical method was paired t-test. The results showed a significant decrease in walking tests after wearing before wearing the insole, and significant changes velocity, TUG, step length, stride length, single support Rt, double support, stance Lt (p< 0.05), but step time, cycle time, single support Lt, stance Rt did no differ significantly (p> 0.05). As a result, if a significant change occurs in the walking speed, temporal-spatial parameters and the difficulty of walking due to various foot deformations, it has a positive effect on walking by wearing a patient-specific insole.

저자들은 불임을 주소로 하는 25세의 여성이 초음파, CT를 거쳐 일측 신형성 부전을 동반한 자궁기형으로 진단되고, 영상-복강경을 통하여 비교통성 흔적자궁각과 동일측 난소와 충수돌기를 포함하는 자궁내막증으로 확진되고, 흔적자궁각과 충수돌기의 제거술을 시행받은 1 예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. Noncommunicating uterine horns are rare, occasionally presenting with functional endometrial cavities, Surgical removal of the noncommunicating horn is commonly performed to prevent endometriosis and rupture of a pregnancy in these patient. We report a case of rudimentary horn with pelvic and appendicular endometriosis treated by laparoscopic resection of those lesions.
