제6 뇌신경 마비시의 Modified Jensen Procedure 1례

Because of the long intracranial course, the 6th nerve is vulnerable to increased intracranial pressure, trauma to the cranial floor, meningeal edema, inflammation in the base of the skull, and any displacement of the brain stem. The authors had experienced a case of abducens nerve palsy resulting non-concomittent esotropia and lateral gaze palsy due to brain trauma by traffic accident. We had performed modified Jensen procedure that had good result. That procedure is presented to aid in preserving the anterior segment blood supply. The anterior ciliary artery in the lateral rectus is not included in the union. The case report is reviewed with the pertinent literature.

Basal cell carcinoma is the most common malignancy of the eyelid. Pigmented basal cell carcinoma is a type of the basal cell carcinoma, and it is needed to differenciate with mealignant melanoma clinically. Authors experienced a case of pigmented basa cell carcinoma which had been thought as a malignant melanoma, but after excision, histopathological study revealed a pigmented basal cell carcinoma. And it had taken about 2 months to invade into the orbital fat after starting to show the lid lesion.

The surgical correction of blepharoptosis is accomplished by operations utilizing the certain materials, the levator, the frontalis or the superior rectus muscle. The perfect postoperative results are symmetrical lift of upper lids, complete cover of the eye on lid closure, adequate mobility when blinking, normal lid fold and no diplopia. The authors obtained good results in two children of congenital ptosis by attaching medial one third of superior rectus to lid margin which was reported by Motais in 1897. The operation which use the superior rectus muscle are not favored recently because complications such as partial corneal exposure, vertical diplopia, difficulty in blinking, and partial obliteration of the upper conjunctival fornix. But, the results of these simple physiologic procedures for binocular paralytic ptosis were remarkable and any complications as aboves did not occur for 3 months of follow up study.

The clinical picture of central retinal artery occlusion is well known. Little is known, however, about the clinical picture of occlusion of the various posterior ciliary arteries individually or together in rhesus monkey. The authors have experienced a case of 21 old women with the visual field defects developed by the humen posterior ciliary artery occlusion via a pathologic embolic process. A brief review of the related literatures is presented.

A total of 75 subjects were examined using Hahn's color vision test & Double 15-Hue test. Among the 75 subjects, 62 subjects detected by screening test among 2700 high school students had defective color vision but good visual acuity and no retinal diseases. The other 13 subjects were detected in out-patient ophthalmic clinic. Results of examination in classifying the types and estimating the extent of color defects were obtained. 1. In the total 75 persons examined, extent of the color defects were as follows; mild--------6cases moderate----31 cases strong------38 cases 2. The classification of types of congenital color defects were as follows; Protan--------2 cases Deutan-------25 cases Unclassified---48 cases Lack of advanced study on color discrimination for congenital color deficiency resulted in existing job discrimination against a color defective person by restricting his/her job opportunity. Therefore this kind of research has very important meanings and should be continued.

There are many causes for thickening and enlargement of optic nerve. Optic neuritis and papilledma are most common causes of enlargement of optic nerve. In many patients with a single episode of optic neuritis, the actual cause is never established. We expereinced a 29-year-old man who complaints of sudden visual loss and mild ocular pain in left eye. It was diagnosed as idiopathic optic neuritis through the clinical sign, fundus appearence and visual field defect. The orbital CT scan showed diffuse enlargement and thickening of introrbital portion of left optic nerve, homogenous enhancement after contrast injection. The thickened left optic nerve was normalized gradually.

약물을 이용한 상안검 Mueller씨근의 기능에 관한 연구

Mueller's superior tarsal muscle and the levator palpebrae superioris act together to elevate the upper lid. Mueller's muscle consists of nonstriated muscle fibers supplied by the sympathetic nerve system, whereas the levator consists of striated muscle. The superior tarsal muscle provides approximately 2mm of lift for the upper eyelid. The function of the sympathetically innervated superior tarsal muscle was defined in 35 young adults by stimulating this muscle pharmacologically in randomized study. The drugs used topically were sympathomimetics (2.5% and 10% phenylephrine HCL) for muscle stimulation. This study provides basic data on the sympathetic tarsal muscle function in young normal subjects by measurement of the MRO (margin-reflex distance). The mean maximum lid elevation was 1.83 mm with 2.5% phenylephrine and was 2.15mm with 10% phenylephrine. The mean of the differences between two concentrations was 0.32 mm. By paired t-test, this difference was not statistically significant (p>0.05).

Mooren's ulcer is a chronic, painful corneal ulceration of unknown etiology. Its spread occurs in three directions: round the periphery, in towards the cornea, and occasionally into the sclera. We experienced a case of Mooren's ulcer in 77-year-old female, in which chronic granulomatous uveitis was complicated along with rubeosis iridis, pars planitis, and peripheral retinal vasculitis. Mooren's ulcer appears to be a distinct entity clinically and a distinct entity by gross pathologic examination but a nonspecific process histologically.
