      • Prognostic Value of Multidetector Coronary Computed Tomography Angiography in Relation to Exercise Electrocardiogram in Patients With Suspected Coronary Artery Disease

        Objectives: This study was designed to determine the prognostic value of multidetector coronary computed tomography angiography (CTA) in relation to exercise electrocardiography (XECG) findings. Background: The prognostic usefulness of coronary CTA findings of coronary artery disease in relation to XECG findings has not been explored systematically. Methods: Patients with suspected coronary artery disease who had undergone both coronary CTA and XECG (<90 days between tests) from 2003 through 2009 were enrolled retrospectively. Coronary CTA results were classified according to the severity of maximal stenosis (normal, mild: <40% of luminal stenosis, moderate: 40% to 69%, severe: ≥70%), XECG results were categorized as positive and negative, and Duke XECG score was calculated. Clinical follow-up data were collected for major adverse cardiac events (MACE): cardiac death, nonfatal myocardial infarction, unstable angina requiring hospitalization, and revascularization after 90 days from index coronary CTA. C-statistics were calculated to compare discriminatory values of each test. Results: Among the 2,977 (58 +/- 10 years) study patients, 12% demonstrated positive XECG results. By coronary CTA, patients were categorized as normal (56%) or having mild (26%), moderate (13%), or severe (5%) disease. During a median follow-up of 3.3 years (interquartile range: 2.3 to 4.6), 97 MACE were observed and the 5-year cumulative event rate was 3.6% (95% confidence interval: 3.0 to 4.3). Although both XECG (C-statistic: 0.790) and coronary CTA (C-statistic: 0.908) improved risk stratification beyond clinical risk factors (C-statistic: 0.746, p < 0.05 for all), XECG in addition to coronary CTA (C-statistic: 0.907) did not provide better discrimination than coronary CTA alone (p = 0.389). In subgroup analyses, coronary CTA stratified risk of MACE in groups with both positive and negative XECG results (all p < 0.001 for trend). However, positive XECG results predicted risk of MACE on coronary CTA only in the moderate stenosis group (hazard ratio: 2.58, 95% confidence interval: 1.29 to 5.19, p = 0.008) and severe stenosis group (hazard ratio: 2.28, 95% confidence interval: 1.19 to 4.38, p = 0.013). Conclusions: In patients with suspected coronary artery disease, coronary CTA discriminates future risk of MACE in patients independent of XECG results. Compared with coronary CTA, XECG has an additive prognostic value only in patients with moderate to severe stenosis on coronary CTA.

Proceedings of International Symposium on East-West Medicine, Seoul. 231-243, 1999.-The oriental medicine Chung-Ri-Ja-Gam-Tang is an herbal medicine which has been used for pulmonary diseases and known as having immune stimulatory effects. It has been known effective in lung cancer. So we studied the effects and the mechanisms of this herbal medicine it the mouse with metastatic lung cancer. The metastatic lung cancer of the mouse was produced with melnoma cell line(B16BL/6).5×10□/mouse tumor cells were injected intravenously to the CDF1 mouse via tail vein. The mice were divided 4 groups. The first group was treated with 50mg/kg extract of Chung-Ri-Ja-Gam-Tang per oral for 10 days after cancer cell injection, second group treated with saline after cancer cell injection, the third group with medicine without cancer cell, and the last group with saline only. After 10 days treatments some of the mice were scarificed and the lung and spleen was removed. The survival duration, weight change, the number of metastatic cancer nodule of the lung, the NK cell activity, the capacity of cytokines(INF-γ,INF-α,IL-2) production and the proliferation activities of mouse lymphocytes were measured. The surival times of the group 1 mice were longer and the weight loss was less than the group 2 significantly. The number of the metastatic nodule of the lung were decresed in the group 1 than the group 2. The production of INF-γ,was increase in group 2 than group 3 and 4, IL-2 production was increased in group 1 than group 2,3,4 significantly. There was no difference in TNF- α production and proliferation activity of lymphocyte in each group. The NK cell activity was significantly increased in group 1 than group 2,3,4. We conclude that the Chung-Ri-Ja-Gam-Tang has the effect of increasing the NK cell activity of the CDF1 mouse with metastatic lung cancer(B16BL/6). And it is suggested that the increased production of the IL-2 is the mechanisms of enhanced NK cell activity.

        Self-Reflective Individual and Disciplinary Power in J. M. Coetzee’s Disgrace

        This paper takes issue with the critical view of David Lurie, the protagonist in J. M. Coetzee’s Disgrace, as a sinner. It argues that David’s resistance is ultimately a cry for individual will and freedom, to keep his private sphere intact from the encroaching influence of the institutional power. The paper investigates how the prototype of institutional domination found in Catholicism is worked into the mechanism of the hearing. In doing so, the parallel between Roman Catholicism’s Sacrament of Penance, the purge of the intellectuals in Mao’s China, and the committee’s demands for David is examined. The ramifications of the hearing’s failure that question the effectiveness of the institutional disciplining of individuals are also explored. Reading David as a resistant hero hints at the possibility that the reconciliation of the history’s past and present, and the vision for a new-fangled future may begin with the distancing of the ruling consciousness from itself sans systematic or external intervention.

        Skin entrance dose for digital and film radiography in Korean dental schools

        Purpose : This study was aimed to compare skin entrance dose of digital radiography with that of film radiography and to show the dose reduction achievement with digital systems at 11 dental schools in Korea. Materials and Methods : Forty six intraoral radiographic systems in 11 dental schools were included in this study. Digital sensors were used in 33 systems and film was used in 13 systems. Researchers and the volunteer visited 11 dental schools in Korea. Researchers asked the radiologic technician (s) at each school to set the exposure parameters and aiming the x-ray tube for the periapical view of the mandibular molar of the volunteer. The skin entrance doses were measured at the same exposure parameters and distance by the technician for each system with a dosimeter (Multi-O-Meter : Unfors instruments, Billdal, Sweden). Results : The median dose was 491.2 μGy for digital radiography and 1,205.0 μGy for film radiography. The skin entrance dose digital radiography was significantly lower than that of film radiography (p<0.05). Conclusion : Fifty-nine percent skin entrance dose reduction with digital periapical radiography was achieved over the film radiography in Korean dental schools.

        Narrative Form, Unheimlich, and the Queer Subject in Jessica Hagedorn’s Dogeaters

        This paper reveals that the disjunctive and cinematic narrative form of Jessica Hagedorn’s Dogeaters organically carves a politicoliteral space for its two principal narrators to show how they cope with the feeling of homelessness. The novel’s peripatetic narrative consists of first-person narrations, third-person narration, newspaper articles, president’s speech, quotes from a book, and an adapted version of prayer script. As Hagedorn reveals in the very beginning of the book, these accounts are neither entirely fictional nor factual. Dogeaters presents a space of quasi-reality that traverses the literal, political, and historical. For Rio Gonzaga, this space becomes a narratival space in which Rio re-arranges, edits, and re-formulates her memories of living in Manila to come to terms with her identity as a transnational migratory subject. During this process, Rio deliberately forgets or belies certain memories. It is Joey Sands who registers the sense of homelessness that Rio strenuously tries to eject. While Joey thinks that he may have found home in the material comforts afforded by performing his queerness, his body tells a different story. His body works as a medium through which Joey’s personal trauma and the Philippines’s national trauma settle in. Rio’s reconstructive and revisional storytelling needs Joey’s visceral body as a site of abjection where Rio’s unheimlich is textually discharged.

      • Identification of Porphyromonas gingivalis lipopolysaccharide-binding proteins in human saliva

        Porphyromonas gingivalis causes periodontal diseases and its lipopolysaccharide (LPS) is considered as a major virulence factor responsible for pathogenesis. Since initial recognition of P. gingivalis LPS (Pg.LPS) in the oral cavity might be crucial for the host response, we identified Pg.LPS-binding proteins (Pg.LPS-BPs) using Pg.LPS-immobilized beads and a high-resolution mass spectrometry. LPS purified from P. gingivalis was conjugated onto N-hydroxysuccinimidyl-Sepharose<SUP>(</SUP>R) 4 Fast Flow beads. Notably, Pg.LPS-conjugated beads could stimulate Toll-like receptor 2 (TLR2) as determined by a TLR2-depdendent reporter expression system using CHO/CD14/TLR2. In addition, the Pg.LPS-conjugated beads induced the production of inflammatory mediators such as nitric oxide and interferon-gamma-inducible protein-10 in the macrophage cell-line, RAW 264.7. These results imply that Pg.LPS retained its immunological properties during the conjugation process. Then, the Pg.LPS-conjugated beads were mixed with a pool of saliva obtained from nine human subjects to capture Pg.LPS-BPs and molecular identities were determined by LTQ-Orbitrap hybrid fourier transform mass spectrometry. Pg.LPS-BPs captured at high frequencies included alpha-amylase, cystatin, prolactin-inducible protein, lysozyme C, immunoglobulin components, serum albumin, lipocalin-1, and submaxillary gland androgen-regulated protein 3B. These proteins are known to be involved in bacterial adhesion and colonization, anti-microbial functions or modulation of immune responses.

      • Cyclin D1 is Associated with Radiosensitivity of Triple-Negative Breast Cancer Cells to Proton Beam Irradiation

        <P>Proton therapy offers a distinct physical advantage over conventional X-ray therapy, but its biological advantages remain understudied. In this study, we aimed to identify genetic factors that contribute to proton sensitivity in breast cancer (BC). Therefore, we screened relative biological effectiveness (RBE) of 230 MeV protons, compared to 6 MV X-rays, in ten human BC cell lines, including five triple-negative breast cancer (TNBC) cell lines. Clonogenic survival assays revealed a wide range of proton RBE across the BC cell lines, with one out of ten BC cell lines having an RBE significantly different from the traditional generic RBE of 1.1. An abundance of cyclin D1 was associated with proton RBE. Downregulation of RB1 by siRNA or a CDK4/6 inhibitor increased proton sensitivity but not proton RBE. Instead, the depletion of cyclin D1 increased proton RBE in two TNBC cell lines, including MDA-MB-231 and Hs578T cells. Conversely, overexpression of cyclin D1 decreased the proton RBE in cyclin D1-deficient BT-549 cells. The depletion of cyclin D1 impaired proton-induced RAD51 foci formation in MDA-MB-231 cells. Taken together, this study provides important clues about the cyclin D1-CDK4-RB1 pathway as a potential target for proton beam therapy in TNBC.</P>

        생쥐초기배아세포의 DNA 합성 및 회복합성에 미치는 MMS 와 CAPTAN 의 영향

        생쥐 초기배아 세포의 DNA합성 및 회복합성에 미치는 MMS와 Captan의 영향을 조사하여 얻은 결과는 다음과 같다. 1. DNA합성 : 0.1mM MMS를 처리한 2 -세기포와 상실기 세포의 DNA합성율은 처리 직후 대조군의 83∼86%정도 억제된다 이같은 억제 작용은 시간이 지남에 따라 더욱 심해져서 처리후 2시간째는 41∼48%, 그리고 4시간째는 44∼56%를 보이는데, 이같은 농도는 HeLa S₃나 CHO세포의 DNA합성에는 별다른 영향을 미치지 않는다. 한펀 0.25mM Captan은 처리후 2시간에 DNA합성의 최소치를 보인후 4시간째는 71∼76%로 증가하여 회복 현상을 나타낸다. 이같은 결과는 생쥐 초기배아 세포가 돌연변이 유발원에 대단히 민감하며, Captan은 MMS에 비해 상해 정도가 경미한 것을 뜻한다. 2. DNA회복 : 0.1∼2.0mM MMS를 처리한 상실기 및 포배기 세포는 모두 비동시 DNA합성을 유발하며 그 정도는 농도의 증가에 따라 직선적으로 증가한다. 0.03∼0.25mM Captan을 처리한 경우에도 비동시 DNA회복합성율은 농도의 증가에 따른 증가를 보이나 그 율은 낮다. 이 두 화학물질에 의한 DNA회복합성율은 상실기 및 포배기에 따른 차이를 보이지 않는다. 이같은 결과는 생쥐 배아세포가 이들 돌연변이 유발원에 의한 DNA상해를 절제회복시키는 능력을 거의 같은 정도로 갖고 있음을 뜻한다. Effects of MMS and captan on DNA synthesis and repair synthesis in early mouse embryo cells were studied and the results obtained were as follows: 1. DNA synthesis: One-tenth mM MMS severely inhibited the rates of DNA synthesis in both 2-cell stage and morula stage cells to 83-86% of control at zero hour. This inhibition was further enhanced with time, representing 41-48% at 2 hours, and 44-56% at 4 hours after treatment, although this concentration did not show any disturbance in DNA synthesis in HeLa S₃ and CHO cells. These results indicate that early embryo cells are quite sensitive to environmental mutagens. 2. DNA repair: One-tenth to 2.0 mM MMS induced unscheduled DNA synthesis in both morula and blastocyst cells. Its rate was dose-dependent and the maximum rates were 23-25% of total cells analyzed. Captan (0.03-0.25 mM) also induced unscheduled DNA synthesis and the maximum rates were 11-13%. These results indicate that both morula and blstocyst cells of early mouse embryo have the capacity to repair damage induced in DNA by MMS and captan, and that MMS may cause more damage than captan does.
