        Age-dependent root canal instrumentation techniques: a comprehensive narrative review

        The aim of this article was to review age-dependent clinical recommendations for appropriate root canal instrumentation techniques. A comprehensive narrative review of canal morphology, the structural characteristics of dentin, and endodontic outcomes at different ages was undertaken instead of a systematic review. An electronic literature search was carried out, including the Medline (Ovid), PubMed, and Web of Science databases. The searches used controlled vocabulary and free-text terms, as follows: ‘age-related root canal treatment,’ ‘age-related instrumentation,’ ‘age-related chemo-mechanical preparation,’ ‘age-related endodontic clinical recommendations,’ ‘root canal instrumentation at different ages,’ ‘geriatric root canal treatment,’ and ‘pediatric root canal treatment.’ Due to the lack of literature with practical age-based clinical recommendations for an appropriate root canal instrumentation technique, a narrative review was conducted to suggest a clinical algorithm for choosing the most appropriate instrumentation technique during root canal treatment. Based on the evidence found through the narrative review, an age-related clinical algorithm for choosing appropriate instrumentation during root canal treatment was proposed. Age affects the morphology of the root canal system and the structural characteristics of dentin. The clinician’s awareness of root canal morphology and dentin characteristics can influence the choice of instruments for root canal treatment.

        Most of the endodontic lesion is associated with the main canal system formed around the main apical foramen, so main canal bacteria reduction is needed for successful root canal treatment. However, the lesion around lateral canal in some cases, the success of the treatment depends on the treatment of bacteria in lateral canal. In a patient that have occurred acute apical periodontitis, lesion formed around the lateral canal on his premolar was observed. Root canal treatment was performed with lateral canal, but the main canal could not be approached because of the calcification. After treatment of accessible lateral canal, the patient’s symptoms was disappeared.

        The fate of overfilling in root canal treatments with long-term follow-up: a case series

        This study describes 6 cases of endodontic overfilling with successful clinical outcomes during long-term (up to 35 years) radiographic follow-up. Successful endodontic treatment depends on proper shaping, disinfection, and obturation of root canals. Filling materials should completely fill the root canal space without exceeding the anatomical apex. Overfilling may occur when the filling material extrudes into the periapical tissues beyond the apex. The present case series describes 6 root canal treatments in which overfilling of root canal sealer and gutta-percha accidentally occurred. Patients’ teeth were periodically checked with periapical radiographs in order to evaluate the outcomes during long-term follow-up. All cases showed healing and progressive resorption of the extruded materials in the periapex. The present cases showed that if a 3-dimensional seal was present at the apical level, overfilling did not negatively affect the long-term outcomes of root canal treatment.

        Prevalence of apical periodontitis and quality of root canal treatment in an adult Kuwaiti sub-population: a cross-sectional study

        Objectives This cross-sectional study evaluated the prevalence of apical periodontitis (AP) and the technical quality of root canal fillings in an adult Kuwaiti subpopulation using cone-beam computed tomography (CBCT) images. Materials and Methods Two experienced examiners analyzed 250 CBCT images obtained from Kuwaiti patients aged 15–65 years who attended government dental specialist clinics between January 2019 and September 2020. The assessment followed the radiographic scoring criteria proposed by De Moor for periapical status and the technical quality of root canal filling. Chi-square and Fisher’s exact tests were used for statistical analysis, with significance level set at p < 0.05. Results Among the 2,762 examined teeth, 191 (6.91%) exhibited radiographic signs of AP, and 176 (6.37%) had undergone root canal filling. AP prevalence in root canal-treated teeth was 32.38%, with a significant difference between males and females. Most of the endodontically treated teeth exhibited adequate root canal filling (71.5%). Conclusions The study demonstrated a comparable prevalence of AP and satisfactory execution of root canal treatment compared to similar studies in different countries.

        단일 내원 근관 치료의 예후

        Traditionally, root canal treatment in multiple visits has been accepted and predictable protocol. In present, the benefits of single visit endodontic treatment have been proposed and many clinicians have considered it. The purpose of this review article is to find out evidence of single visit root canal treatment by discussing previous articles. In this article, it would discuss the outcome between single visit and multiple visits endodontic treatment as post-operative pain, flare-up and healing of periapical tissue. Overall, there are not significant different post-operative complication between single- and multiple- visit treatment. But the case of apical periodontitis and endodontic retreatment is still controversial.

        Surgical endodontic management of infected lateral canals of maxillary incisors

        This case report presents surgical endodontic management outcomes of maxillary incisors that were infected via the lateral canals. Two cases are presented in which endodontically-treated maxillary central incisors had sustained lateral canal infections. A surgical endodontic treatment was performed on both teeth. Flap elevation revealed vertical bone destruction along the root surface and infected lateral canals, and microscopy revealed that the lateral canals were the origin of the lesions. After the infected lateral canals were surgically managed, both teeth were asymptomatic and labial fistulas were resolved. There were no clinical or radiographic signs of surgical endodontic management failure at follow-up visits. This case report highlights the clinical significance and surgical endodontic management of infected lateral canal of maxillary incisor. It is important to be aware of root canal anatomy variability in maxillary incisors. Maxillary central incisors infected via the lateral canal can be successfully managed by surgical endodontic treatment.

        마이크로 기포를 포함한 근관 세척 유동 형태의 수치해석 평가

        This study has the initial purpose of pursuing the efficiency of endodontic treatment by applying microbubbles to a NaOCl cleaning solution. The effects of average apical pressure and wall shear stress were found to determine the cleaning quality of the root canal. Using a G-30 needle and a flow rate of 0.26 ml/s, met acceptance criteria in apical pressure. In the straight root canal shape, the side-vent type enhanced the smear layer removal area more effectively attributed to the wall shear stress. In curved root canals, the flat open type was beneficial in generating wall shear stress relative to the curvature of the curved canal. In the fluidity of the cleaning solution according to the bubble dissolution, a slight change appeared in the average apical pressure. Due to the G-30 needle's small size in bubble flow, it is imperative to adopt a new Reynolds number when applying microbubbles.

      • Root Canal Treatment of Mandibular First Molar with Three Mesial Canals: Case Report

        Thorough knowledge of common root canal anatomy and its variation is very important for endodontic success. There are usually two mesial canals on mandibular first molar but accessory mesial canal-which is middle mesial canalcan be found sometimes. Clinician should consider this anatomic variation when treating mandibular first molar. This case report shows several cases of middle mesial canal on mandibular first molars.
