        한국 성인의 전자담배 사용 관련요인

        이 연구는 한국 성인의 전자담배 사용과 관련된 요소들을 조사하기 위해 수행되었다. 카이제곱 검정, 로지스틱회귀분석을 통해2015년 한국질병통제센터에서 실시한 지역건강조사228,477건을 분석하였다. 전자담배평생 유병율이 남성 11.1%, 여성 0.8%로 나타났다. 현재 전자담배 흡연율은 남성이 24.2%, 여성이 26.1%로 였다. 평생전자담배 흡연경험율은 연령 교육수준, 직업, 소득상태 및 현재흡연상태와 각각 유의미하게 관련되어 있었다. 현재의 보편적인 전자담배흡연이 현 흡연상태와 유의한 관련이 있는 것은 과거에 흡연했던 그룹보다 현재흡연자 그룹이 현저히 높은 경험을 보이고 있는 것이다. 전자담배의 선호는 높았다. 전자담배는 금연을 목적으로 하는 이들이 전자담배가 일반담배에 비해 금연효과가 있는 것처럼 인식하고 있었고, 흡연에서 오는 쾌감이나 중독성을 만족시켜 주었다. 따라서 전자담배는 흡연에 대한 딜레마에서 벗어날 수 있는 해결책으로 반영되어 선호도가 높았다. 2014년 전자담배 수입은 전년도 대비 약 3배 증가했고 세계적으로도 전자담배에 대한 관심이 증가하고 있다. 우리는 전자담배의 예방과 중단에 관련된 특징을 고려해야 한다. 흡연자들이 잘못된 흡연관련 건강인식을 개선하기 위한 금연교육 강화와 금연 위해성에 대한 적극적인 홍보가 필요하다. WHO에서도 전자담배의 안전성을 담보하거나 효과가 있다고 인정한 근거가 부족하다고 권고하고 있다. 전자담배의 사용 동기에 있어서 중요한 환경적인 요인들을 확인하고 전자담배인식과 사용에 대한 연관성을 분석하는 연구들을 모색해야 한다. This study was conducted to investigate the factors associated with electronic cigarette use by Korean adults. To accomplish this, data collected from 228,477 individuals during a community health survey conducted by the Korean Centers for Disease Control and Prevention in 2015 were analyzed by descriptive analysis, the Chi-squared test, and logistic regression analysis. The lifetime prevalence of electronic cigarette smoking was 11.1% for men and 0.8% for women. The current prevalence for electronic cigarettes smoking was 24.2% for men and 26.1% for women among lifetime users of electronic cigarettes. The lifetime experience rate of electronic cigarette smoking was significantly associated with age, educational attainment, occupation, income status and current smoking status, respectively. The current universal smoking e-cigarette is significantly related to the current smoking status, which shows a significantly higher number of current smokers than groups that have smoked in the past. The preference for electronic cigarette smoking was high. E-cigarettes were perceived by those who aimed to quit smoking as having a smoking cessation effect compared to regular cigarettes while also providing the pleasure of smoking and addressing the addiction. Therefore, e-cigarettes were highly favored because they were reflected as a solution to the dilemma of smoking. In 2014, electronic cigarette imports tripled from the previous year, and interest in electronic cigarettes is increasing worldwide. Accordingly, measures related to the prevention and discontinuation of e-cigarettes should be considered. Strengthening anti-smoking education and active promotion of anti-smoking risks is needed to improve smokers" wrong perception of smoking-related health. The WHO also reports that there is a lack of grounds to ensure or acknowledge that e-cigarettes are effective. Accordingly, it is important to identify environmental factors influencing the use of electronic cigarettes and explore the relationship between electronic cigarette recognition and use.

      • Cigarette Smoking among Adolescents aged 13-15 in Viet Nam and Correlates of Current Cigarette Smoking: Results from GYTS 2014 Data

        The aim of this paper is to report the rate of current and ever cigarette smoking and explore correlates of current cigarette smoking among adolescents aged 13-15 in Viet Nam. This analysis was derived from GYTS survey, which comprised of 3,430 adolescents aged 13-15, conducted in 2014 in 13 cities and provinces of Viet Nam. We calculated the weighted rates of current and ever cigarette smoking and reported patterns of smoking behavior. We also performed logistic regression to explore correlates of current cigarette smoking behavior. The weighted rate of ever cigarette smoking was 9.5% (95% confidence interval (CI): 8.5 %-10.5%), in which the weighted rate among males (15.4%; 95% CI: 13.6%-17.0%) was higher than that among females (4.2%; 95% CI: 3.3%-5.1%). The weighted rate of current cigarette smoking was relatively low at 2.5% (95%CI: 2.0%-3.0%) with higher weighted rate among males (4.9%; 95% CI: 3.8%-5.9%) compared to the corresponding figure among females (0.2%; 95% CI: 0.0 %-0.5%). Current cigarette smoking was significantly higher among males than females, in students aged 15 versus 13 years old, and in students who had several or all close friends smoking and students with daily observation of smoking at school. For greater smoking reduction outcomes, we recommend that tobacco interventions for adolescents should consider targeting more male students at older ages, establish stricter adherence to school-based banning of cigarette smoking, engage both smoking and nonsmoking adolescents and empower adolescents to resist peer smoking influence as well as changing their norms or beliefs towards smoking benefits.

      • Age of initiation, Determinants and Prevalence of Cigarette Smoking among Teenagers in Mushin Local Government Area of Lagos State, Nigeria

        Background: Cigarette smoking constitutes a major threat to the health and wellbeing of teenagers. While smoking has been on decline in the developed countries, the reverse is the case in developing countries. The aim of this study was to determine the age of initiation, determinants and prevalence of cigarette smoking among teenagers in Mushin Local Government Area of Lagos state, Nigeria. Materials and Methods: This was a descriptive cross-sectional study among 475 teenagers selected by multistage sampling. A pre-tested, structured, interviewer-administered questionnaire was used for data collection. The study was carried out in November, 2014. Results: Response rate was 84.6%. Mean age of the respondents was $16.4{\pm}1.65years$. Range and mean age of initiation of cigarette smoking were 7 to 17 years and $12.0{\pm}3.32years$ respectively. Teenagers who were above 15 years (OR:5.13, 95%CI: 0.87-30.26), males (OR:5.19, 95%CI: 1.57-17.18), married (OR:8.41, 95%CI: 1.04-63.35), had ${\leq}$primary school education(OR:4.31, 95%CI: 1.07-17.33), influenced by friends(OR:308.84, 95%CI:84.87-1123.81), and influenced by advertisements (OR:27.83, 95%CI: 3.92-197.64) were more likely to have initiated cigarette smoking. Furthermore, teenagers who were males (OR:12.77, 95%CI: 2.90-56.28), married (OR:19.24, 95%CI: 2.05-180.45), had ${\leq}$primary school education(OR:7.85, 95%CI: 2.37-26.01), influenced by friends(OR:28.56, 95%CI: 10.86-75.07), and influenced by advertisements (OR:5.95, 95%CI: 1.72-20.61) were more likely to be current cigarette smokers. In addition, 24.9% had initiated cigarette smoking while 14.7% were current smokers of cigarette. Conclusions: Mean age of initiation of cigarette smoking was $12.0{\pm}3.32years$. Determinants of cigarette smoking were age, gender, marital status, educational background, friends and advertisements. Life time prevalence of cigarette smoking was higher than prevalence of current cigarette smokers. Cigarette smoking reduction programs should take these factors into consideration.

        담배가격 인상에 따른 청소년 흡연율 변화

        본 연구에서는 2015년 담배가격 인상정책 전후 청소년 흡연율 변화, 금연시도율 변화 등에 대해 살펴보고자 하였다. 이에 본 연구는 총 800개교의 중학생 및 고등학생을 대상으로 조사를 실시한 제9차(2013년), 제10차(2014년), 제11차(2015년) 청소년건강행태온라인 조사 원시자료를 이용하여 청소년들의 일반적 특성, 정신건강 및 주관적 학업성적, 음주 및 흡연관련 특성에 따른 담배가격 인상 전후 흡연율 변화 및 금연시도율, 금연시도이유 중 ‘담배값이 너무 비싸서’ 선택률, 흡연량 변화를 분석하였다. 본 연구에서 전체 청소년의 현재 흡연율은 2013년 9.7%에서 2014년 9.2%로 감소하는 모습을 보였고, 담배가격 인상 후인 2015년 흡연율은 7.8%로 전년대비 15.2% 감소율을 보여 담배가격 인상 전보다 큰 폭으로 감소한 것으로 나타났다. 이러한 결과들을 통해 담배가격 인상정책이 청소년 흡연율을 감소시키는데 매우 효과적일 뿐만 아니라 금연동기 및 흡연 예방에도 매우 효과적이라는 것을 확인할 수 있었다. 청소년기 흡연자의 대다수가 성인기 흡연자로 이어진다는 점에서 청소년기의 흡연 예방 및 금연 유도는 매우 중요하다. 향후 흡연실을 허용하지 않는 완전 금연구역의 확대와 담배 광고, 판촉, 후원에 대한 포괄적 금지는 시급히 추진되어야 하며 이미 근거를 바탕으로 작성된 WHO FCTC의 각조약을 충실히 이행할때 금연효과를 극대화 할 수 있을 것이라 사료된다. This study investigated changes in the youth smoking rate and smoking cessation rates before and after a cigarette price increase in 2015. Specifically, the changes in smoking rate, smoking cessation attempt rate, selection rate and amount of smoking were investigated using primary data pertaining to Korean Youth Risk Behavior obtained from a web-based survey of youth general characteristics, mental health, subjective grade, drinking and smoking related characteristics before and after a cigarette price increase. To accomplish this, 800 middle school and high school students in 9th (2013), 10th (2014), 11th (2015) were surveyed. The current youth smoking rate was slightly decreased from 9.7% in 2013 to 9.2% in 2014, while the smoking rate in 2015 decreased significantly by 15.2% after the cigarette price increased to 7.8%. These results confirmed that increasing cigarette price is very effective at decreasing youth smoking rate, providing motivation to stop smoking and preventing smoking. The prevention of smoking and inducement to stop smoking during adolescence is very important because the majority of teenage smokers continue smoking into their adulthood. In addition to expanding a completely non-smoking area and a comprehensive ban on tobacco advertising, promotion and sponsorship should be urgently sought, and the effect can be maximized when the WHO FCTC faithfully implements the already-founded treaty

        우리나라 청소년들의 흡연유형 경험 영향요인

        Objectives: The purpose of this study is to investigate the factors that affect the smoking type among those who have used regular cigarettes, liquid or cigarette-type e-cigarettes. Methods: The subjects of analysis were 6,081 people who had smoked regular cigarettes or e-cigarettes. For data analysis, SPSS ver.25.0 statistical package program was used. Multinomial logistic regression analysis was performed to find out the factors affecting smoking type. Results: Factors affecting the experience of using e-cigarettes compared to regular cigarette smoking are gender and class. Academic performance, living with family members, drinking experience, and secondhand smoke in school. The factors influencing dual use compared to regular cigarette smoking were gender, class, academic performance, economic status, living with family, drinking experience, and experience of secondhand smoke in school. Smoking cessation attempts had an effect on dual use compared to regular cigarette smoking. Conclusion: Smoking cessation experience had a greater effect on e-cigarette use than regular cigarette smoking. .

        Inhalation exposure to cigarette smoke induces endothelial nitric oxide synthase uncoupling and enhances vascular collagen deposition in streptozotocin-induced diabetic rats

        <P><B>Abstract</B></P> <P>Smoking is an acknowledged risk factor for vascular disorders, and vascular complication is a main outcome of diabetes. Hence, we investigated the impact of cigarette smoke on blood vessels in diabetes, postulating that smoking might aggravate diabetic vascular impairment. Sprague–Dawley rats were divided into four groups: control, cigarette smoke-exposed, diabetic, and cigarette smoke-exposed diabetic groups. Streptozotocin-induced diabetic rats were exposed to cigarette smoke by inhalation at total particulate matter concentration of 200 μg/L for 4 h/day, 5 day/week for a total of 4 weeks. Diabetes caused structural change of aorta, but additional cigarette smoke exposure did not induce further alteration. Collagen, a marker for fibrosis, was increased in media of diabetic aorta, and this increase was augmented by cigarette smoke. Cigarette smoke induced endothelial nitric oxide synthase (eNOS) uncoupling in the diabetic group. Malondialdehyde was increased and glutathione was decreased in blood from diabetes, but these effects were not exaggerated by cigarette smoke. Cigarette smoke caused NADPH oxidase (NOX) 2 expression in diabetic aorta and enhanced diabetes-induced NOX4 expression in aorta. Taken together, cigarette smoke exposure can aggravate vascular fibrosis and induce eNOS uncoupling in diabetes under experimental condition, suggesting that smoking might exacerbate diabetic vascular impairments.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Impact of cigarette smoke on the progress of diabetic vascular impairment was investigated. </LI> <LI> Diabetes-induced structural change of aorta was minimally affected by cigarette smoke exposure. </LI> <LI> Cigarette smoke induced endothelial nitric oxide synthase (eNOS) uncoupling in diabetic aortas. </LI> <LI> Cigarette smoke further enhanced diabetes-caused collagen deposition in aorta. </LI> <LI> Cigarette smoke exposure induced NADPH oxidase (NOX) 2 and increased NOX4 expression in diabetic aorta. </LI> </UL> </P>

      • Prevalence of Cigarette Smoking and Associated Factors among Male Citizens in Tehran, Iran

        Background: Cigarette smoking is as the leading cause of cancer mortality and other chronic diseases in males worldwide. The prevalence of cigarette smoking is different across and within countries by age, education level, occupation, and so on. This study aimed to determine the prevalence of cigarette smoking and its relationship with individuals' demographic factors and BMI in adolescent men living in Tehran, Iran. Materials and Methods: This study involved secondary analysis of the 'Urban Health Equity Assessment and Response Tool-2' survey conducted in Tehran, Iran, among men aged 20+, 2011-2012. Using a multistage sampling method, 45,990 men were included in the study. The cigarette smoking status, BMI and demographic factors measured through a self-administered questionnaire. Chi-square, t-test, and logistic regression model were used to examine the relationships between the independents variables and cigarette smoking behavior, using SPSS software version 21. Results: In the total of 45,990 men, the overall prevalence of cigarette smoking was 14.6% (CI 95%: 14.29-14.94). Age (OR=0.96; CI 95%:0.94-0.98), house ownership (OR=0.68; CI 95%: 0.64-0.72), job status (OR=0.60; CI 95%: 0.46-0.86), marital status (OR=0.42; CI 95%: 0.39-0.47) and educational levels (OR=0.50; CI95%: 0.45-0.54) were associated with the prevalence of cigarette smoking. However, associations with BMI, family size, residency years, and district were not statistically significant. Conclusions: Given the relatively high prevalence of cigarette smoking in the study population, policy interventions are required to address this major public health issue, with a focus on the population demographic influences.

        Safety Assessment of Mainstream Smoke of Herbal Cigarette

        Owing to the increase in price of cigarettes in Korea, herbal cigarettes have received increasing attentionas a non-smoking aid; however, its safety has hardly been studied. We analyzed some of the toxic componentsin the mainstream smoke of herbal cigarettes, performed a mutagenicity test on smoke condensatesfor safety assessment, and compared the results with the corresponding values of a general cigarette withthe same tar content. Herbal cigarette “A” was smoked using automatic smoking machine under ISO conditionsin a manner similar to general cigarette “T”. The tar content measured was higher than thatinscribed on the outside of a package. The mainstream smoke of herbal cigarette “A” did not containdetectable levels of tobacco-specific nitrosamines and nicotine. Carbon monoxide and benzo(α)pyrenecontents in herbal cigarette “A” were higher than those in the general cigarette “T”. The phenolic contentssuch as hydroquinone, resorcinol, and catechol in herbal cigarette “A” were higher than those in the generalcigarette “T”, but cresol contents in herbal cigarette “A” were lower than those in the general cigarette“T”. The content of aromatic amines such as 4-aminobiphenyl in herbal cigarette “A” was higher thanthat in the general cigarette “T”; however, this difference was not statistically significant. On the otherhand, 1-aminonaphthalene, 2-aminonaphthalene, and 3-aminobiphenyl contents in herbal cigarette “A”were lower than those in the general cigarette “T”. The smoke condensates of herbal cigarette “A” exhibiteda higher mutagenic potential than the condensates from the general cigarette “T” at the same concentration. We concluded that the mainstream smoke of herbal cigarette contains some toxic components, thesmoke condensates of herbal cigarettes are mutagenic similar to general cigarette because of combustionproducts, and that the evaluation of the chemical and biological safety of all types of herbal cigarettesavailable on the market.

        담배가격 인상 수준에 따른 흡연 청소년의 금연의도

        Purpose: The purpose of this study was to investigate the intention of smoking cessation according to the level of cigarette price increase among adolescent smokers and to suggest a reasonable cigarette price to effectively reduce smoking prevalence. Methods: In 2007, subjects were selected from middle and high school students except twelfth graders using a complex sampling design which employs a two-stage cluster sampling method. In total, 1,001 current smokers were included in the study. To investigate the intention of smoking cessation according to the level of cigarette price increase, cumulative percentages of smoking cessation of every smoking-related subgroup were presented under the assumption that cigarette prices increased to 3,000, 4,000, 5,000, 6,000 or 10,000 won. Non-parametric statistical methods were used to compare the prices at which the subjects intended to quit smoking among the subgroups. Results: More than 50 percent of current smokers intended to quit smoking under the assumption that cigarette prices were doubled to 5,000 won. However, the effect of cigarette prices on smoking cessation was less sensitive when the prices exceeded 5,000 won. In addition, the median of cigarette prices at which the subjects intended to quit smoking was 5,000 won, excluding the subjects who smoked less than a cigarette a day. Conclusion: This study suggests that 5,000 won for one pack of cigarettes is a reasonable price to effectively encourage smoking cessation, considering the price elasticity. Therefore, this finding may be helpful in establishing a new cigarette price policy for anti-smoking.

        성인 흡연자들의 흡연 및 금연 행태와 전자담배사용과의 관련성

        Objectives: The purpose of this study is to provide basic data on the use of electronic cigarettes by identifying the relationship between smoking and smoking behavior and the use of electronic cigarettes. Methods: This study used data from 8,150 participants of the 'Korea National Health and Nutrition Examination Survey (KNHANES)' s 7th (2016). In this study, 1,051 people currently smoking were analyzed. The number of people using electronic cigarettes was 74 among current smokers. Results: The age of onset of smoking was 20.46 years old in the group not using electronic cigarette, and the age of starting smoking was 18.12 years old in the group using electronic cigarette(p<.001). In the education level, the current use of electronic cigarette was 1.82 times higher in high school graduates than college graduates(p<.005). The reason for using electronic cigarette was 4.32 times higher than other reasons for using electronic cigarette for the purpose of smoking cessation(p<.005). Conclusions: For the smoking cessation society, education for the electronic cigarette is needed from the adolescence and selective management for the people who use the electronic cigarette for the purpose of smoking cessation is needed.
