        고용상태에 따른 미충족 치과치료

        Objectives: This study aimed to investigate differences in unmet dental care needs according to employment status and if factors have relevance to unmet dental care needs due to “economic burden” and “no time to spare for visiting”. Methods: Multivariate logistic regression analysis was performed to examine the factors that affected the prevalence of unmet dental care needs due to “economic burden” and “no time to spare for visiting” in an economically active population comprised of subjects aged 20-64 years. Results: Compared to regular workers, part-time workers experienced 1.28-times more unmet dental care needs, while students experienced 0.64-times fewer unmet dental care needs. With regard to reasons related to “economic burden”, the rate of experiencing unmet dental care needs was 1.37-times higher for part-time workers than for regular workers. Regarding reasons related to “no time to spare for visiting”, the rate of experiencing unmet dental care needs was 0.51-times lower in part-time workers compared with regular workers. In economically inactive people, these values were 0.51-times lower in the unemployed, 0.37-times lower in students, 0.23-times lower in house workers, 0.20-times lower in subjects with disease and injury, and 0.20-times lower in early retirees compared to regular workers. Conclusions: Part-time workers experienced more unmet dental care needs compared with regular workers. With regard to “economic burden”, a significant difference in unmet dental care needs was observed between regular workers and part-time workers. Additionally, regarding “no time to spare for visiting”, people who were economically inactive because of unemployment, student or house-worker status, diseases and injury, and retirement experienced fewer unmet dental care needs compared with regular workers.

        한국 노동시장 불안정성과 미충족 치과의료의 관련성: 고용과 소득 불안정성을 중심으로

        Objectives: The aim of this study was to analyze the accessibility of dental care services among individuals with precarious employment in South Korea. Methods: We used the 9th wave of the Korean Health Panel data (2015) and included 7,736 wage and non-wage earners in our study. We determined precariousness in the labor market as a combination of employment relationship and job income, and categorized individuals based on this into the following four groups: Group A comprising those who report job and income security, Group B comprising those who experience job insecurity alone, Group C comprising those who report a stable job but low income, and Group D comprising those who experience both job and income insecurity. Accessibility to dental care services was determined by experience of unmet dental care needs and unmet dental care needs caused primarily by financial burden. Logistic regression analyses were used to assess the effect of precarious work on access to dental care services. Results: Individuals with job insecurity (Group B; OR=1.445; 95% CI=1.22-1.70) and both job and income insecurity (Group D; OR=1.899; 95% CI=1.61-2.24) were more likely to have unmet needs than the comparison group. Both groups B and D were also 2.048 (95% CI=1.57-2.66) times and 4.435 (95% CI =3.46-5.68) times more likely, respectively, to have unmet dental care needs caused by financial burden. Education status, health insurance, and health status were all also effective factors influencing unmet dental care needs. Conclusions: Unstable employment and low income resulted in diminished access to dental care services. Therefore, governments should consider health policy solutions to reduce barriers preventing individuals with employment and income instability from accessing adequate dental care.

        한국인의 미충족 치과치료에 영향을 미치는 요인: 제6기 국민건강영양조사 자료를 이용하여

        Objective: The purpose of the study was to investigate the factors affecting unmet dental care needs of Korean using 6th Korean National Health and Nutritional Examination survey(KNHANES). Methods: The subjects were 13,718 Koreans from the 6th KNHANES data. The questionnaire consisted of general characteristics of the subjects, reason for dental care needs by socio-economic status and unmet dental care needs. Data were analyzed using SPSS 21.0 program. Results: The proportion of unmet dental care needs accounted for 27.4% and the main reasons were financial burden and less priority. Male had 0.21 times less unmet dental needs than female. Low, mid low, and mid high economic income groups had 1.27, 1.24, and 1.08 times respectively more unmet dental care needs than high income group. Those having good and average self-perceived oral health status experienced 0.83 and 0.63 times less dental care needs. Conclusions: Socio-economic factors and health status were the important factors of unmet dental care needs and it is necessary to provide a variety of public service of dental care for the low income group.

        우리나라 성인 치과의료 필요환자의 미충족 치과의료 관련 요인

        Objectives: This study was conducted to analyze the factors related to unmet dental care needs in Korean adults who needed dental treatment. Methods: This study analyzed data from the 7th Korea National Health and Nutrition Examination Survey (2016-2018). Among the adults who participated in the National Health and Nutrition Examination Survey, 2,723 dental treatment who performed oral examinations were the final subjects. General characteristics and oral health characteristics were analyzed with complex sample chi-square test using the IBM SPSS program. Factors related to unmet dental care were analyzed by multiple logistic regression analysis. Results: As for unmet dental care needs related factors, bad oral health was 3.383 times higher, and usually was 1.736 times higher. In the use of dental clinic unmet dental care needs was 0.121 times lower. Unmet dental care needs was high, 1.366 times for those with the experience of tooth pain and 2.736 times for those with chewing difficulty. Conclusions: Based on the above results, it can be seen that oral health status or oral discomfort is related to unmet dental care needs. It is necessary to set goals to improve unmet dental care, recognize that one’s oral health condition is very important, and continuously educate about oral health for each life cycle.

        고령층의 미충족 치과의료 경험에 관한 연구

        The purpose of this study was to identify, by using the Korea Health Panel, the factors for the ‘unmet dental care needs’ —the failure to use dental care service despite the need to get dental care— experienced among older adults. A comparison was made between ‘unmet healthcare needs’ and ‘unmet dental care needs’, taking into account the characteristics of dental care, including the relatively lower health insurance coverage and the larger amount of out-of-pocket costs than the other types of healthcare service. The education level, income, and health insurance type were significantly correlated with both unmet healthcare and unmet dental care needs; age, household type, and alcohol intake were significantly correlated only with unmet dental care needs. On the basis of these results, this study presents policy implications for making oral healthcare more accessible to older adults. 본 연구는 「한국의료패널」을 이용하여 고령층을 대상으로 치과치료 필요가 있음에도 불구하고 치과의료 서비스를 이용하지 못하는 ‘미충족 치과의료’와 관련이 있는 요인을 파악하고자 하였다. 이에 치과의료가 상대적으로 낮은 건강보험 보장률, 높은 본인부담금 등 타 의료서비스와 다른 특성을 고려하여 ‘미충족 의료’와 ‘미충족 치과의료’를 비교・분석하였다. 분석결과 교육수준, 소득, 의료보장 형태, 섭식문제 요인이 공통적으로 미충족 의료 및 미충족 치과의료와 유의한 상관성이 있는 것으로 나타났으며, 연령, 가구유형, 음주 요인은 미충족 치과의료에서만 유의한 상관성이 있는 것으로 확인되었다. 이와 같은 분석결과를 바탕으로 본 연구는 고령층의 구강보건의료의 접근성 향상을 위한 정책적 함의를 제시하였다.

        종사상 지위와 가구 균등화 소득이 치과 의료 미충족에 미치는 영향 : 2018 한국의료패널 자료를 활용한 분석

        Objectives: This study used the 2018 Korea Health Panel Survey data to analyze factors affecting employment status and income relating to unmet dental and medical care needs. Additionally it investigated measures to reduce oral health inequality among various socioeconomic classes. Methods: Descriptive statistics for the subjects’ unmet dental and medical care needs were calculated through chi-square test analysis, and multivariate logistic regression analysis was applied to identify factors affecting the unmet dental and medical care needs. The odds ratio and 95% confidence interval were calculated for each level. These data were analyzed using STATA 17.0 SE (64-bit) version, and the statistical significance level was set to p <0.05. Results: As a result of unmet dental and medical care needs according to general characteristics, the lower the education level (p <0.001), the higher the age (p <0.001) and the lower the household equalization income (p <0.024) and the smoker status (p <0.003) were, respectively. Factors that have a statistically significant impact on unmet dental and medical care needs were divorce, separation and bereavement (p <0.001) in individuals than in married persons, and being smokers than non-smokers (p <0.009). The frequency of unmet dental and medical care needs were found to be lower in the cases of a high school graduate than an elementary school graduate (p <0.018), and of higher household equalization income (p <0.001) than the lowest household equalization income, respectively. Conclusions: It was found that various factors such as age, education level, household equalization income, employment status, type of working hours type, and smoking status affect unmet dental and medical care needs.

        독거노인의 미충족 치과의료 관련요인: 2018년 지역사회건강조사 자료를 이용하여

        Objectives: The purpose of the study was to investigate the factors affecting unmet dental care needs among elderly living alone. Methods: The study subjects were 19,019 elderly living by themselves, who participated in the Korea Community Health Survey 2018 (KCHS, 2018). The data were analyzed using frequency analysis, Rao-Scott chi-square test, t-test, and binary logistic regression analysis, using the SAS 9.4 software. Results: Results from the multiple logistic regression analysis revealed a higher odds ratio (OR) for unmet dental care needs among those with a monthly household income below KRW 1,000,000 (OR=1.64, 95% confidence interval, 95%CI=1.39-1.92), compared to those with an income of above KRW 2,000,000. With regards to stress levels, unmet dental care needs were more prevalent among participants in the “stress moderate” (OR=1.19, 95% CI=1.06-1.33) and “stress” groups (OR=1.68 95% CI=1.48-1.92), compared to the “no stress” group. Furthermore, the OR for unmet dental care needs was significantly higher among participants in the “depression” group (OR=1.84, 95% CI=1.61-2.11) than the “no depression” group. Conclusions: Higher unmet dental care needs were found among participants in the low monthly household income, stress, and depression groups. Further study is needed to analyze the association between factors and needs related to unmet dental care.

        Analysis of the Effects of Unmet Dental Care on Dental Treatment Needs: Based on the 8th Korea National Health and Nutrition Examination Survey (2019)

        Objective: The purpose of this study is to investigate the factors that unmet dental care in terms of oral health problems and dental care requirements affect the need for dental treatment. Methods: We used data from the 8th National Health and Nutrition Survey 1st Year (2019), which surveyed 10,859 participants. We included 5,581 adults aged ≥19 years. To analyze the general characteristics of the participants and determine whether and why their dental care was unmet, survey responses were reclassified and a frequency analysis was conducted. The Rao–Scott chi-square test and composite sample multiple logistic regression analysis was conducted to determine the relationship between general characteristics, oral health behavior, oral conditions, dental needs, and unmet dental care. Results: Multiple logistic regression analysis revealed association between dental needs and access to dental care. Unmet dental care-based analysis revealed that women were 0.77 times more likely to receive dental treatment than men. When prosthetics and implants were in the oral cavity, they were 1.30 and 1.36 times higher. Conclusion: Due to its retrospective cross-sectional design, the study only included participants of the 8th National Health and Nutrition Survey, limiting its ability to establish causal relationships. Future studies are warranted to provide an in-depth analysis by expanding not only the subjective oral health status but also objective oral health indicators according to dental preventive treatment standards and to use these data to determine the factors affecting the systemic health status of citizens.

        한국 노인의 인구·사회경제적 특성과 미 충족 치과진료와의 관련성

        Objectives: This study investigated the relationship between demographic and socioeconomic characteristics of the Korean elderly and their unmet dental care needs, by using the 2015 data from the 6th Korea National Health and Nutrition Examination Survey (NHANES). Methods: In total, 1,372 elderly persons aged 65 and over, who responded to the 6th NHANES, were included in the final analysis. Logistic regression analysis was performed in order to identify any relationship between demographic and socioeconomic characteristics and unmet dental care needs. Results: The rate of unmet dental care needs was shown to be less by 0.799 times in the elderly who reside in dong than those who live in eup and myeon (OR: 0.799, 95% CI: 0.679-0.959). Unmet dental care needs were higher in participants with ‘low’ and ‘below average’ than ‘above average’ income, by 1.645 times (OR: 1.645, 95% CI: 1.087-2.366) and 1.172 times (OR: 1.172, 95% CI: 1.108-1.880), respectively. Elderly individuals living alone had a higher rate of unmet dental care needs than those living with their family by 1.157 times (OR: 1.157, 95% CI: 1.084-1.498). Conclusions: Demographic and socioeconomic factors influenced unmet dental care needs, causing inequality. Proper policy support to the vulnerable should be considered in order to enhance the elderly’s access to dental care.

        국내 미충족 구강진료의 지역간 격차와 관련된 사회결정요인

        Objectives: This study aims to examine the regional differences for unmet dental need, while comparing 16 metropolitan cities in South Korea. Further, this study aims to examine the impacts of social determinants, which relates to the regional difference. Methods: This is a cross-sectional study that analyzes the data, of the 4th round Korea National Health and Nutrition Examination Survey (KNHANES), in depth, as well as an ecologic analysis investigating each area as an aggregate unit. The data obtained from 24,871 subjects was stratified of 16 cities. The dependent variables were the unmet dental needs. There were 12 variables in social determinant, which have been approached by the social status, the social position, the economic status, the urbanization and access to resources. The sex-age adjusted standardization ratio was calculated and a descriptive statistical analysis was performed to compare the cities. The coefficient of variations was calculated. Social determinants affecting regional differences were analyzed through a multiple regression model. PASW statistics 18.0 (SPSS Inc., Chicago, IL, USA) was used. Results: The unmet dental need between the 16 cities showed a 1.7 times the regional difference. On the other hand the unmet dental need, due to financial reason, showed a 3.3 times the regional difference. The correlation analysis showed that the unmet dental need was higher in single-person households, in welfarites and in severely deprived areas. The final regression analysis showed that the local deprivation index (β=-17.19), the ratio of single-person household (β=3.91), and the number of dentists per 10,000 population (β=-2.30), were found to be statistically significant affecting the regional differences of unmet dental need (P<0.1). Conclusions: The unmet dental need in South Korea showed the regional difference, which was affected by the urbanization, the social position and resources of the areas among social determinants.
