Diagnostic accuracy of PET/CT in early stage ovarian cancer: comparison between PET/CT and CT or MRI

The purpose of this study was to compare the diagnostic accuracy of positron emission tomography/computed tomography (PET/CT), abdomino-pelvic computed tomography (CT), and pelvic magnetic resonance imaging (MRI) for detection of early stage ovarian cancer. Medical record of 140 women who underwent PET/CT scanning with suspected early stage ovarian cancer (stage 1-2) from July 2005 to February 2012 was retrospectively reviewed. Preoperative CT, MRI, and PET/CT findings were compared with histopathologic results obtained from the surgery. The accuracy of each modality in detection of malignancy was evaluated by computing the relevant areas under a receiver operating characteristics(ROC) curve and comparisons of the area under the curve (AUC) according to the modality (MedCalc® version 9.6.3.0, Broekstraat 52, 9030 Mariakerke, Belgium). Histopathology showed 7 cases of benign tumors, 23 cases of borderline tumors, and malignant tumors in 110 cases. In detecting malignant lesions in the ovaries and the adnexa, the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of PET/CT were higher than the corresponding values of CT or MRI. In discriminating between cases of benign/borderline and malignant, the accuracy of PET/CT (0.760) trended higher than that of CT or MRI (0.585). PET/CT is more accurate than abdomino-pelvic CT or pelvic MRI in diagnosis of early stage ovarian cancer and in differentiating between cases of benign/borderline and malignant ovarian tumors.
Optimum thickness of GaAs top layer in AlGaAs-based 850 nm VCSELs for 56 Gb/s PAM-4 applications

We studied the influence of GaAs top-layer thickness on the small-signal modulation response and 56 Gb/s four-level pulse-amplitude modulation eye quality of 850 nm vertical-cavity surface-emitting lasers (VCSELs). We considered the proportionality of the gain-saturation coefficient to the photon lifetime. The simulation results that employed the transfer-matrix method and laser rate equations led to the conclusion that the proportionality should be considered for proper explanation of the experimental results. From the obtained optical eyes, we could determine an optimum thickness of the GaAs top layer that rendered the best eye quality of VCSEL. We also compared two results: one result with a fixed gain-saturation coefficient and the other that considered the proportionality. The former result with the constant gain-saturation coefficient demonstrated a better eye quality and a wider optimum range of the GaAs top-layer thickness because the resultant higher damping reduced the relaxation oscillation.

Backgrounds/Aims: Compared with a single urinary biomarker, a composite of multiple urinary biomarkers may be more helpful for differentiating tubulointerstitial inflammation from interstitial fibrosis/tubular atrophy (IFTA) in kidney allografts. Methods: In this cross-sectional cohort study, we collected urine samples from 115 patients with for-cause biopsy, 53 patients with stable allografts, and 50 living kidney donors. We measured the urinary levels of transglutaminase 2 (TG2), syndecan-4 (SDC4), alpha 1 microglobulin (A1M), interferon-inducible protein 10 (IP-10), interleukin 6 (IL-6), and monocyte chemoattractant protein-1 (MCP-1). Results: The for-cause biopsy group showed significantly higher levels of log<SUB>e</SUB>TG2/Cr, log<SUB>e</SUB>A1M/Cr, log<SUB>e</SUB>IL-6/Cr, and log<SUB>e</SUB>MCP-1/Cr compared with other groups. In the for-cause biopsy group, log<SUB>e</SUB>TG2/Cr level was positively correlated with the severity of IFTA. After adjusting for age, sex, body mass index, diabetes, hypertension, cardiovascular disease, and the interval between kidney transplant and biopsy, TG2 and the interval between transplant and biopsy were significantly correlated variables for the severity of IFTA. Regarding tubulointerstitial inflammation, Body mass index, TG2, SDC4, and IP-10 were positively-correlated variables, and MCP-1 and the interval between transplant and biopsy were negatively-correlated variables. Conclusions: Our results show that post-transplant urinary levels of TG2, SDC4, MCP-1 and IP-10 may be a useful biomarker for tubulointerstitial inflammation and IFTA.
