<SUP>18</SUP>F-FDG PET-CT에서 진단된 갑상선 우연종의 악성종양 발견율 및 특징

<B>Purpose: </B>PET-CT is often used to differentiate benign or malignant thyroid incidentalomas. In this retrospective study, we evaluated whether the <SUP>18</SUP>F-FDG uptake pattern and PET-CT findings improved accuracy over the standardized uptake value (SUV). <B>Methods:</B> <SUP>18</SUP>F-FDG PET-CT was performed on 2,178 subjects from August, 2004, to October, 2007, in Sung-ae Hospital. PET-CT was performed on 806 patients (37%) with suspected or known nonthyroidal cancer and 1,372 healthy subjects (63%) without a previous history of cancer. We investigated the clinical characteristics of patients, history, standardized uptake value (SUV), ultrasonography, and hormone levels in blood. Thyroidal cancer was confirmed by ultrasonography-guided fine needle aspiration and pathology after thyroid operation. <B>Results:</B> The prevalence of focal thyroid lesions on PET-CT was 8.8% (191/2178). Thyroid cancer confirmation was 7.9% (15/191). The maximum SUV of malignant thyroid lesions were significantly higher than that of benign lesions (7.00±3.08 vs. 4.49±1.84, P<0.001). <B>Conclusion:</B> PET-CT image interpretation that includes 18F-FDG uptake and SUV is better than PET-CT alone for differentiating benign and malignant lesions. Elevated cAMP level attenuates 2-deoxy-D-ribose-induced oxidative damage in pancreatic β-cells

Glucose toxicity to pancreatic β-cells is defined as irreversible β-cell damage, including apoptosis, caused by chronic exposure to high glucose levels in type 2 diabetes. Oxidative stress is an important mechanism for glucose toxicity to pancreatic β-cells. Reducing sugars produce reactive oxygen species through autoxidation and protein glycosylation. 2-Deoxy-D-ribose (dRib) is a reducing sugar with high reactivity. We investigated whether cAMP-stimulating agents could protect β-cells from dRib-induced oxidative damage. HIT-T15 cells were cultured with various concentrations of dRib for 24h. We measured cell survival, intracellular cAMP and H_(2)O_(2) levels, and apoptosis. dRib decreased cell survival in a dose-and time-dependent manner and markedly increased intracellular H_(2)O_(2) levels and apoptosis. N-Acetyl-L-cysteine decreased dRib-induced rises in intracellular H_(2)O_(2) and apoptosis to control levels. Forskolin, IBMX, and dbcAMP markedly elevated intracellular cAMP levels and significantly attenuated dRib-induced cytotoxicity and apoptosis, but had no influence on the dRib-induced rise in intracellular H_(2)O_(2) levels, These results demonstrate that dRib produced oxidative stress and apoptosis in pancreatic β-cells and that elevated intracellular cAMP levels reduced dRib-induced damage, independent of reactive oxygen species metabolism.
Splenic Littoral Cell Angioma with Hepatitis C Associated Liver Cirrhosis

Littoral cell angioma (LCA) of spleen is an uncommon vascular neoplasm of littoral cell origin. It is well established that LCA may be associated with other malignancy or autoimmune disorders. We describe a 34-year-old woman with viral hepatitis C associated liver cirrhosis who presented with the incidental finding of LCA. She showed ascites, esophageal varix and drowsy mentality. Abdominal computed tomography (CT) showed multiple benign looking nodules in both hepatic lobes, but no abnormality in spleen. Liver transplantation and splenectomy were performed. Microscopic findings revealed narrow anastomosing vascular channels lined with plump cells that exfoliated into the lumen. Immunohistochemically the lining cells were positive for CD31, CD68 and negative for CD34, consistent with LCA. Herein, a second case of an incidentally detected LCA with cirrhosis, viral hepatitis C associated in Korea is reported.

Blood Mononuclear Cell Subsets in Patients with Chronic Obstructive Pulmonary Disease

흡연은 형청 1gE치를 높이며 또한 1gE는 환기장애와 관련이 있다. 한편 흡연은 혈액 T세포 분획에 변화를 초래한다. 이러한 소견들은 흡연자에서 발생하는 기도 폐쇄의 발생기전에 T세포를 매개로 하는 면역학적 이상이 관여할지도 모른다는 것을 시사한다. 이러한 가설을 증명하기 위하여 18명의 정상인과 18명의 만성폐쇄성폐질환 환자에서 혈액 단핵구 분획과 혈청 면역글로블린을 측정하였다. 정상 대조군과 만성폐쇄성폐질환 환자 사이에 백혈구 수 및 감별계산 그리고 T 세포 분획에는 차이가 없었고 호중구만 환자군에서 높았다. 만성폐쇄성폐질환 환자를 폐확산능치가 낮은 10명과 정상인 8명의 두 군으로 나누어 분석하여 보니 폐확산능치가 정상인 환자들은 정상대조군과 확산능이 낮은 환자군보다 유의하게 높은 CD8??T세포와 낮은 CD4??/CD8??비를 보였다. FEV₁/FVC는 폐확산능 정상 환자군에서만 CD4??/CD8??비와 유의한 상관관계를 보였다. 혈청 1gG, 1gM 그리고 1gE치는 세군에서 유사하였고, 1gA치만 확산능 정상 환자군에서 높았다. 이러한 소견들은 확산능 정상 만성폐쇄성폐질환 환자의 기도폐쇄 발생기전에 T세포 매개성 면역조절 이상이 관여할 것을 시사하며 기도폐쇄는 1gE 생산과 무관한 것으로 사료되었다.

Intestinal bypass surgery, particularly jejuno-ileal bypass surgery, performed for the purpose of weight reduction may cause an unexpected exacerbation of nonalcoholic steatohepatitis (NASH). Here, we report a case of NASH caused by small intestinal bacterial overgrowth, which developed after jejuno-colic bypass surgery and resolved dramatically after surgical correction. <P><B>Summary</B></P><P>Two vascular growth factor families, VEGF and the angiopoietins, play critical and coordinate roles in tumor progression and metastasis. A single inhibitor targeting both VEGF and angiopoietins is not available. Here, we developed a chimeric decoy receptor, namely double anti-angiogenic protein (DAAP), which can simultaneously bind VEGF-A and angiopoietins, blocking their actions. Compared to VEGF-Trap or Tie2-Fc, which block either VEGF-A or angiopoietins alone, we believe DAAP is a highly effective molecule for regressing tumor angiogenesis and metastasis in implanted and spontaneous solid tumors; it can also effectively reduce ascites formation and vascular leakage in an ovarian carcinoma model. Thus, simultaneous blockade of VEGF-A and angiopoietins with DAAP is an effective therapeutic strategy for blocking tumor angiogenesis, metastasis, and vascular leakage.</P> <P><B>Highlights</B></P><P>► DAAP simultaneously binds VEGF-A and angiopoietins, and blocks their actions ► DAAP effectively suppresses tumor angiogenesis, metastasis and vascular leakage ► DAAP is superior to VEGF-Trap plus Tie2-Fc in blocking tumor growth and metastasis ► Ang-2 is a therapeutic target to control tumor angiogenesis and vascular leakage</P>

Cooked mature silkworms have received increasing attention as a new source of income for sericultural farms. Quality improvement of these worms as the end-product requires prevention of the following: inclusion of unfertilized silkworm eggs, mixing with impurities, producing undercooked or overcooked mature larvae, and mixing spewed silk substance. Preventing unfertilized eggs and impurities and reducing undercooked or overcooked mature larvae can be achieved by sericultural farms. However, the prevention of spewed silk substance requires the development of new techniques. This study developed new techniques for optimum cold storage temperature, soaking, and hydro-thermal treatment to minimize the spewing of mature larvae prior to steaming: when larvae mature after 7 days on the 5<sup>th</sup> instar, they should be soaked in water at room temperature of 25℃ immediately upon collection and stored at 8℃ for less than 3 days. They should be soaked in boiling water for approximately 10 s immediately before steaming to kill the larvae and cooked with steam. This method allows for the production of high-quality mature larvae without spewed silk substance. These new techniques will considerably improve the production of well-cooked quality mature larvae by minimizing silk substance spewed out from the larvae.
