Lips are a crucial aspect of lower-face aesthetics. The wider and fuller the lips are compared to the width of the face, the more attractive a female appears. However, lip volume decreases with age, increasing the demand for lip augmentation procedures. Hyaluronic acid (HA) fillers have become the most popular choice for lip augmentation due to their effectiveness, biocompatibility, and safety. Vascular complications from HA filler injections are rare. However, vascular occlusion caused by filler injections can lead to serious adverse events such as tissue necrosis. The 830-nm wavelength laser has been used for photobiomodulation (PBM) therapy in dermatology due to the advantage of adequate tissue penetration. It has also been found to be effective in pain control and wound healing. This case study describes the management of a vascular complication due to a lower lip HA filler injection with adjunctive 830-nm wavelength PBM therapy.
Review of high-intensity focused ultrasound for lipolysis: clinical and preclinical studies

High-intensity focused ultrasound (HIFU) is an increasingly popular treatment modality that is used for non-invasive body contouring. Several clinical studies have reported a significant reduction in subcutaneous adipose tissue after HIFU treatment, along with the histologic degradation of adipocytes and fat membranes. Similarly, preclinical studies have confirmed the safety and efficacy of HIFU and identified two mechanisms, namely heat-induced apoptosis and acoustic cavitation. Although the lipolytic effects of HIFU are well documented, insufficient research has been done on the metabolic changes caused by HIFU lipolysis yet. A recent meta-analysis of the outcome of non-invasive fat removal suggests that HIFU may positively lead to weight loss and wasit circumference reduction in the serum lipid profiles, potentially expanding its use from aesthetic to metabolic treatments. In conclusion, HIFU is emerging as an effective and safe tool for fat reduction via heat-induced apoptosis and acoustic cavitation and its use is expected to increase in body contouring. As more data emerges on its metabolic effects, HIFU may also find a place as adjunctive therapy for metabolic disorders.

Keloid is a benign fibroproliferative disorder characterized by excessive collagen production during abnormal wound healing in keloid-prone individuals. Therefore, the treatment of keloid aims to reduce inflammation and reorganize collagen bundles. Intralesional corticosteroid injection, particularly triamcinolone, is a common first-line treatment, but injections can be difficult in very firm lesions. This case report presents a refractory ear keloid treated with a combination therapy of fractional ablative CO2 laser and intralesional triamcinolone injection. The patient had a persistent keloid mass in her left ear despite previous intralesional corticosteroid injections. The ear keloid was treated with the combination therapy of fractional CO2 laser and triamcinolone injection. The keloid size was reduced by more than 50%, and the Vancouver Scar Scale score improved. Combined fractional CO2 laser and triamcinolone injections may have a synergistic effect on drug delivery in addition to each keloid improvement.
