        Comparison of Survival Rate between Posterior-Stabilized and Constrained Condylar Articulations in Revision Total Knee Arthroplasty: A Minimum 5-Year Follow-up Analysis

        Background: Revision total knee arthroplasty (RTKA) is a technically demanding procedure for failed primary TKA. Posterior-stabilized (PS) and constrained condylar knee (CCK) articulations are commonly used for RTKA, but comparison of these articulations in RTKA is scarce. The aim of this study was to compare survival rates and clinical outcomes of RTKAs using PS articulation and CCK articulation. Methods: This study conducted a retrospective analysis of 86 RTKAs (PS, n = 41; CCK, n = 45) with a mean follow-up of 9.15 ± 2.79 years. Clinical outcomes were evaluated using the Hospital for Special Surgery score, Knee Society Score, and The Western Ontario and McMaster Universities Osteoarthritis Index at final follow-up. The survival rate of each group was analyzed by Kaplan- Meier survival analysis and Cox-hazard progression model. Results: Clinical outcomes were improved in both groups without significant difference. Twelve patients had orthopedic complications (4 in PS group and 8 in CCK group). Eight of them underwent re-RTKA (3 in PS group and 5 in CCK group). The articulation design did not influence the failure. The estimated 10-year survival rate was 92.7% in the PS group and 88.2% in the CCK group with no significant difference (p = 0.60). Also in septic failure, there was no significant difference in survival rate (92.7% in PS group and 92.5% in CCK group, p = 0.87). The hazard ratio in the PS group was not significantly different (p = 0.607). Conclusions: In RTKA, both PS and CCK showed similar survival rates and clinical outcomes at a mean follow-up of 9.2 years. Implant articulation did not affect the outcomes when properly indicated.

        Liver Infarction and Venous Thromboembolism after Tamoxifen Use in an ADPKD Patient with Encapsulating Peritoneal Sclerosis: A Case Report

        Encapsulating peritoneal sclerosis (EPS) is a potentially fatal complication after long-term peritoneal dialysis, and tamoxifen can be used for its prevention and treatment. However, tamoxifen is known to increase the risk of venous thromboembolism. A 49-year-old woman was admitted with sudden abdominal pain. The patient had received peritoneal dialysis for 20 years and switched to hemodialysis after the diagnosis of EPS. Tamoxifen (10mg) and prednisolone (20mg) had been administered for 8 months. On computed tomography, the left hepatic lobe was hardly illuminated, leading to a diagnosis of liver infarction. A month later, she was re-admitted due to abdominal pain and extensive deep vein thrombosis of the leg. The administration of tamoxifen was stopped and prednisolone was reduced to 10mg. As her malnutrition progressed, she succumbed to death of gram negative sepsis. The patient was concluded to have liver infarction and extensive venous thrombosis as a side effect of tamoxifen.

        Genetic Analysis of Carotenoids Content in Red Pepper (Capsicum annuum L.) Fruit using Complete Diallel Cross

        Regarding carotenoids content, the genetic basis, heritability and combining ability in six red pepper inbred lines were investigated using full diallel crosses. Both additive and non-additive gene actions govern inheritance of carotenoids content. The mean square of array through variance and covariance analysis (Wr-Vr) was insignificant, which suggest that inbred lines involved in diallel cross may have no epistatic effects. The Vr/Wr graph revealed the influence of partial dominant gene action towards low carotenoids content and the absence of non-allelic interaction. The H2 component was smaller than the H1 and the [H2/4H1] component was 0.187 less than 0.25, indicating unequal proportion of positive and negative alleles in the parents. The estimates of broad and narrow sense heritability for carotenoids content were 0.956 and 0.832, respectively. The variance of general combining ability (GCA) was relatively higher than that of specific combining ability (SCA), which implied that the additive gene effects were predominant as compared to both dominant and epistatic effects for the accumulation of carotenoids in this genetic population. The values of GCA of ‘62024L1’ and ‘62067L2’ were higher than those of the other parents. These 2 inbred lines, therefore, can be considered as useful breeding materials to enhance fruit carotenoids content in other red pepper varieties.

      • Preparation Method of Co<sub>3</sub>O<sub>4</sub> Nanoparticles Using Ordered Mesoporous Carbons as a Template and Their Application for Fischer–Tropsch Synthesis

        <P>Co<SUB>3</SUB>O<SUB>4</SUB> nanoparticles (NPs) with a narrow particle size distribution were fabricated via a facile and novel method using mesoporous carbon materials (CMK-3, MSU-F-C) as sacrificial templates. The particle size distribution of the Co<SUB>3</SUB>O<SUB>4</SUB> NPs varied depending on the pore size of the templates. Synthesis of the NPs with concurrent complete removal of the templates was achieved at 593 K, which is lower than the temperatures utilized in previous reports. It was verified that the carbon template was decomposed by catalytic oxidation with cobalt and NO<SUB><I>x</I></SUB> species generated by thermal decomposition of the cobalt nitrate precursor in air. The prepared NPs, and particularly the Co<SUB>3</SUB>O<SUB>4</SUB> NPs synthesized from CMK-3, acted as excellent catalysts for the Fischer–Tropsch synthesis (FTS).         Genetic Instability in Gastric Epithelial Neoplasias Categorized by the Revised Vienna Classification

        Background/Aims: The aim of this study was to determine the structural chromosomal aberrations, such as loss of heterozygosity (LOH) and microsatellite instability (MSI), at multiple tumor suppressor gene loci in gastric epithelial neoplasia categorized by the revised Vienna classification. Methods: All tissue samples were excised by endoscopic mucosal resection. Sixty category 3 (low-grade adenoma) tissue samples and 51 category 4 samples (high-grade adenoma and intramucosal carcinoma with adenoma) were examined at the 7 sets of microsatellite loci linked to the tumor suppressor gene locus. Results: For category 3 and 4 tissue samples, there were no differences in the frequencies of LOH-positive chromosomes or the extent of chromosomal loss. The Helicobacter-pylori (H. pylori)-positive rate was significantly higher in MSI-positive category 4 samples than in category 3 samples (p=0.04). The frequency of MSI positivity was significantly higher in category 4 samples than in category 3 samples (p=0.003). Conclusions: H. pylori infection is associated with genetic instability of the premalignant lesion. MSI occurs in the early stages of gastric carcinogenesis and its occurrence increases during malignant transformation. Detection of MSI in premalignant gastric lesions may be a surveillant of risk of malignant transformation. (Gut Liver 2010;4:179-185)

        Synergistic cycles of protease activity and inflammation via PPARγ degradation in chronic obstructive pulmonary disease

        Inflammation, oxidative stress, and protease–antiprotease imbalance have been suggested to be a pathogenic triad in chronic obstructive pulmonary disease (COPD). However, it is not clear how proteases interact with components of inflammatory pathways. Therefore, this study aimed to evaluate the effect of neutrophil elastase (NE) on lipopolysaccharide (LPS)-induced interleukin 8 (IL-8) production and determine the molecular mechanism in human bronchial epithelial cells (HBECs). Immortalized bronchial epithelial cells and primary HBECs were used to investigate the impact of NE on LPS-induced IL-8 production. The molecular mechanism by which NE modulated LPS-induced IL-8 production was confirmed in elastase-treated C57BL/6 mice and primary HBECs obtained from COPD patients and healthy controls. The results showed that NE treatment synergistically augmented LPS-induced IL-8 production in both immortalized bronchial epithelial cells and primary HBECs. NE partially degraded peroxisome proliferator-activated receptor gamma (PPARγ), which is known to regulate IL-8 production in the nucleus. Treatment with a PPARγ agonist and overexpression of PPARγ reversed the NE-induced synergistic increase in LPS-induced IL-8 production. Moreover, PPARγ levels were lower in lung homogenates and lung epithelial cells from elastase-treated mice than in those from saline-treated mice. In accordance with the findings in mice, PPARγ levels were lower in primary HBECs from COPD patients than in those from healthy never-smokers or healthy smokers. In conclusion, a vicious cycle of mutual augmentation of protease activity and inflammation resulting from PPARγ degradation plays a role in the pathogenesis of COPD.

        편재형 컴퓨팅 환경에서 XHTML과 VoiceXML간 EXI 문서의 변환시스템 설계와 구현

        편재형 컴퓨팅 환경은 다양한 컴퓨팅 장비의 접속만큼이나 그 응용분야도 다양하다. 이러한 컴퓨팅환경에서 XML은 최적의 데이터 표현이다. W3C 표준인 XML은 XSLT를 이용하여 다른 응용분야에 적절한 데이터 변환이 가능하다. 그러나 XML은 텍스트 기반으로 이진데이터 저장방식에 비해 파일의 크기가 커서 컴퓨팅 파워가 적은 편재형 컴퓨팅 환경에서는 다루기가 어렵다는 단점을 갖고 있다. 본 논문에서는 XML문서의 인코딩 방법에 대하여 살펴보고 자원을 적게 사용하는 편재형 컴퓨팅 환경에서 인코딩 된 EXI 포맷의 XML문서가 다른 응용의 적합한 EXI 포맷 XML 문서으로의 변환방법을 제안한다. 다양한 응용환경 중에서 본 논문에서는 EXI 포맷의 XHTML 문서를 EXI 포맷 VoiceXML로 변환하는 시스템을 제시한다. 이 시스템은 자원이 적은 편재형 컴퓨팅 환경에서 EXI 포맷 XML 문서의 재사용성을 높이고 EXI 포맷의 XML활용에 기여할 수 있다. In a pervasive environment, there are various applications as much as connections of various devices. In this computing environment, XML is the most suitable data representation method. XML is able to transform data for other application areas using XSLT. XML is text-based, the file size of XML document is bigger than binary data file. Therefore, XML has a disadvantage that it is hard to deal with XML in a pervasive environment. In this paper, we survey encoding methods of XML documents, and then we propose a transform method that transforms an encoded EXI format XML document into an EXI format XML document suited for other applications. Among various applications, we present a system that transforms an EXI format XHTML document into an VoiceXML document. This system can improve reusability of EXI format XML documents in a pervasive environment and it is expected to contributes utilization of EXI format XML documents.

        성인에서 Hyrax 장치로 완속상악확장하여 폭경부조화를 개선한 증례

        Approximately 30% of adult patients who want orthodontic treatment have transverse discrepancy with insufficient width of the maxilla. Particularly, in Class III patients requiring orthognathic surgery, the frequency of insufficient width of the maxillary arch related to respiratory problems is high. We report a case of non-surgical maxillary expansion using a Hyrax type expander with an orthognathic surgery, based on the reports that the ratio of non-fused midpalatal suture is not high in adults. A 30 years and 2 months old woman with a long face showed an Angle Class III with a vertical growth pattern. Class III molar and canine relation, anterior edge bite, and mandibular incisor compensatory lingual inclination were observed. The posterior buccal overjet seemed to be appropriate, but I diagnosed that there was a transverse discrepancy, for the following reasons. The inter-canine and inter-molar widths were sufficient but excessive lingual inclination of the mandibular molars was observed when assessing the bucco-lingual inclination based on the center of resistance of the maxillary and mandibular first molar. For this reason, it was expected that intercuspal interference would occur during orthodontic decompensation. Therefore, slow maxillary expansion using Hyrax type expander was performed and 2-jaw rotation surgery was performed to improve aesthetic and occlusion. Adults can also improve width discrepancy by non-surgical methods, which can avoid SARPE requiring additional surgery or segmental surgery lacking stability and predictability.
