      • Breast Reconstruction with Superior Gluteal Artery Perforator Flap in Asian

        Purpose: Breast reconstruction with lower abdominal tissue can produce the best outcome with acceptable rates of long-term complication. However, for cases in which sufficient abdominal tissue is not available, an superior gluteal artery perforator (SGAP) flap can be considered as the next option for autologous breast reconstruction. Materials and Methods: Among a total of 63 women who underwent breast reconstruction with free autologous tissue transfer from July 2010 to April 2011, SGAP flap was performed for four patients. In two cases, patients did not have enough abdominal tissue for sizable breast reconstruction. In another case, the patient had a long abdominal scar due to donor hepatectomy of liver transplantation. In the last case, which was a revisional case after radiation necrosis of a previous pedicled transverse rectus abdominis musculocutaneous (TRAM) flap, a large amount of healthy skin and soft tissue was needed. SGAP flap was elevated in lateral decubitus position. The internal mammary vessels were used for recipient vessels in all cases. Results: Breast reconstruction was performed successfully in all four cases without flap loss. Donor site complication was not observed, except for one case of seroma. The shape of the reconstructed breast was satisfactory in all patients. Conclusion: SGAP flap is an excellent alternative option for the TRAM or deep inferior epigastric artery perforator flap for breast reconstruction. In terms of narrower width, harder consistency of soft tissue, and shorter pedicle, it is clear that the SGAP flap is less competent than the TRAM flap. However, in cases where abdominal tissue is not available, SGAP flap is the only way of providing a large amount of healthy tissue.

        관광휴양형 제2종지구단위계획 수립지침의 문제점과 개선방안 연구

        With the introduction of the new “National Law on Land Planning and Use” in January, 2003, the Tourism & Recreation 2ndDistrict Unit Planning (TR2DP) was launched to as a plan to develop rural areas effectively and to protect them against urban sprawl.The TR2DP is based on the “Plan first and Do it”concept, which tries to rationalize the land use of a developing area and optimize its capability while maintaining or creating environmental quality. However, several problems have been revealed regarding the TR2DP in terms of operation, and these issues have not yet been resolved. Furthermore, these problems can result in potential errors that in turn can lead to unnecessarily excessive monetary expenditures. The possible emergence of environment problems is also of concern. This study attempts to identify the potential operating issues, and subsequently analyzes the P & G & P Resort developing case on Yongpyong Gangwon and Taean Chungnam and Pyeongchang Gangwon. Finally, this research aims to tilize the TR2DP in terms of consideration to investors, rural residents, and operating fficers, and propose effective solutions in the operation of the TR2DP to optimize rural areas’develop benefits.

Patients and Methods:25 patients who was reconstructed with radial forearm free flap for head and neck cancer were reviewed retrospectively. Results:The graft loss rates of full thickness skin graft were less than split thickness skin graft or split thickness skin graft with dermal substitutes. The recovery times of donor site and skin graft donor site of full thickness skin graft were also shorter than split thickness skin graft or split thickness skin graft with dermal substitutes. Skin tex- ture and aesthetic results of donor site were improved and complications as itching and pain sensation were de-creased. Conclusion:Full thickness skin grafts with inguinal skin should be considered for patients requiring a radial forearm free flap.

        Most local government has been interest in tourism development for regional economic growth. It is the key to be successful in increasing money and activating regional economy that the efficient program for regional tourism development must be set to make decision and practice developing project. This study is to suggest a solution through analyzing actual resort developments, such as Phoenix Park, Sungwoo Resort and Hongchun Resort in Kangwon-do. Sure that it helps to go for better tourism development for all, developer, government and residents.

        In order to develope performance assessment of department of physical education, we emphasize the knowledge system of vaulting horse exercise. Because the input variable of propositional knowledge expect positive effect in output variable of an original knowledge through the processing variable of practical knowledge. The teacher analyze the propositional knowledge according to learning target and composed of an practical knowledge for physical education class. Therefore, We provide intentionally learning activity for the variation, development, composition of physical activity. This study was designed to present performance assessment instrument development processing through analysis of propositional knowledge system and practical knowledge system about vaulting horse exercise. First, this study analyzed knowledge system of vaulting horse exercise according to psychomotor skill, cognitive ability, affective behavior. Second, presented new assessment instrument systematized according to step the assessment interval. Through this processing, intend to present the analysis method of learning environment for using in vaulting horse exercise. 1. The teacher should be analyze by the step, domain, time order contents knowledge and assessment knowledge through propositional knowledge system analysis which appointed to input variable in vaulting horse exercise. 2 Through contents knowledge analysis about psychomotor skill, cognitive ability, affective behavior, the teacher should be systematize objectively assessment knowledge which needed in performance assessment instrument development. 3. The psychomotor skill of physical education should be analysis by order of low category, core skill, concrete skill, application skill. 4. Through the composition of practical knowledge system to processing variable in vaulting exercise, the learning knowledge and method knowledge should be compose according to learning environment. 5. The teacher should be compose intentionally in learning environment learning knowledge in time order and learning target through composition of learner traits and learning environment in practical knowledge. 6. The method knowledge of physical education should be compose to the method which could be teach effectively vaulting horse exercise.

      • 하지의 작거나 중등도의 결손 부위 재건을 위한 얕은엉덩휘돌이동맥 천공지 유리 피판술

        Purpose: For reconstruction of lower extremity defects, various flaps can be used and the appropriate flap must be selected and applied according to the size of the defect. In particular, in cases where the defect size is small to moderate, thinner or smaller volume flaps are useful. The authors performed reconstruction of small to moderate defects on the lower extremities using superficial circumflex iliac artery perforator free flaps and are reporting the results. Materials and Methods: Fifteen patients underwent reconstruction of defects on lower extremity areas using superficial circumflex iliac artery perforator free flaps from July 2011 to July 2012 at this hospital. The flaps were elevated from above the deep fat layer, and, in all cases, the vessel diameter of the flaps was less than 1mm, with the exception of superficial vein that accompanied it. Results: The mean follow up period was 4.46 months, and, despite a partial loss in the flap in two cases, there were no total losses. All donor sites were closed with primary closure, and there was no occurrence of complications, such as hematomas, seromas, or lymphorrheas. The patients were highly satisfied with the donor site scar since it could be masked by underwear. Conclusion: Compared to other flaps, superficial circumflex iliac artery perforator free flaps are thinner in thickness and smaller in volume, which results in a more natural contour of the recipient site after the operation. In addition, since the flap can be elevated from supra-deep fat layer, the operation time can be shortened, and lymphorrhea can be prevented, which in turn lessens donor-site morbidity.

      • Virtual surgical planning and patient-specific implant design system for fractured orbital wall reconstruction

        Orbital wall is the thinnest bone that surrounds eyeballs. When there is an external force to mid-upper face, it is blown out to protect the eye from the pressure. Orbital fracture is a common traumatic deformity, but it can cause not only functional problems, but also esthetical problems such as exophthalmos or enophthalmus, and diplopia. The surgeon corrects eyeball position by placing plates to the fractured sites, but limited intra-operative view and complex anatomy make it difficult. 3D surgical software systems have been used for seeming the patient safety and getting reliable outcomes. However, these are not specialized for the surgery and time-consuming manual works are needed frequently. Thus, we propose a virtual surgical planning and implant design system. We focus on increasing usability by automated functions and minimized user interaction. There are four main modules in the proposed system. First, 3D patient’s model is automatically reconstructed from CT data, and the next, the fractured region is detected and visualized by registration of both orbits. Based on the detected area, patient-specific implant is designed using template models. After the surgery, pre-and post-operative 3D models can be compared and analyzed by using synchronized dual view, measurement tools and visualization of the difference of both models. We performed visual inspection for 3D modeling and fractured detection. As a result, the orbital wall of 3D facial model is properly reconstructed without unnecessary holes and the detected regions include real fractured sites properly. The average processing time takes about 4 minutes. In comparison with conventional systems, ours covers most functions and has strengths for automated functions such as orbital wall segmentation and fracture detection. We expect that the automated and optimized functions can reduce pre-operation time and improved surgical outcomes can be derived by using patient-specific 3D printed implant.
