Objectives : To estimate the economic burden of osteoporotic vertebral fracture (VF) from a societal perspective. Methods : From 2002 to 2004, we identified all National Health Insurance claims records for women ${\geq}50$ years old with a diagnosis of VF. The first 6-months was defined as a "clearance period," such that patients were considered as incident cases if their first claim of fracture was recorded after June 30, 2002. We only included patients with ${\geq}$ one claim of a diagnosis of, or prescription for, osteoporosis over 3 years. For each patient, we cumulated the claims amount for the first visit and for the follow-up treatments for 1 year. The hospital charge data from 4 hospitals were investigated to measure the proportion of the non-covered services. Face-to-face interviews were conducted with 106 patients from the 4 study sites to measure the out-of-pocket spending outside of hospitals. Results : During 2.5 years, 131,453 VF patients were identified. The patients had an average of 3.38 visits, 0.40 admissions and 6.36 inpatient days. The per capita cost was 1,909,690 Won: 71.5% for direct medical costs, 20.6% for direct non-medical costs and 7.9% for indirect costs. The per capita cost increased with increasing age: 1,848,078 Won for those aged 50-64, 2,084,846 Won for 65-74, 2,129,530 Won for 75-84and 2,121,492 Won for those above 84. Conclusions : Exploring the economic burden of osteoporotic VF is expected to motivate to adopt effective treatment options for osteoporosis in order to prevent the incidence of fracture and the consequent costs.
Study on a Fixed Bed Gasification of Polyurethane SRF from Electronic Waste

With expanding economic growth, the consumption and changing period of electronics products have increased rapidly last a couple of decades in Korea. A refrigerator generally contains 10% of polyurethane. Since the amount of used refrigerators collected at recycling facilities of E-wastes has increased since 2007, the generation of polyurethane waste has also increased. Polyurethane recycling technologies have been investigated such as polyol production and sound-absorbing materials, which are not commercialized yet due to high cost of the technology. However many kinds of plastic wastes are being used as solid refuse fuel (SRF) like RDF, so polyurethane in E-waste could be utilized as valuable fuels, since it has higher heating values and contains low sulfur, low chlorine, and less hazardous substances. In this study an attempt to fabricate pellet type of SRF from polyurethane waste was conducted to use them as fuels even though it was not easy to make pellet, because characteristics of polyurethane were high volume and low density. Recently, Korean Association of Electronics Environment (KAEE) is producing pelletized SRF. The basic characteristics of polyurethane such as elemental composition, thermal properties and proximate analysis were carried out to assess whether they could be used as fuels or not. Using a fixed bed test reactor, gasification experiments were also conducted at the temperature of 1,000 ℃ by measuring exhaust gas compositions and gas pollutants.

<P>A novel protein-folding function of RNA has been recognized, which can outperform previously known molecular chaperone proteins. The RNA as a molecular chaperone (chaperna) activity is intrinsic to some ribozymes and is operational during viral infections. Our purpose was to test whether influenza hemagglutinin (HA) can be assembled in a soluble, trimeric, and immunologically activating conformation by means of an RNA molecular chaperone (chaperna) activity. An RNA-interacting domain (RID) from the host being immunized was selected as a docking tag for RNA binding, which served as a transducer for the chaperna function for <I>de novo</I> folding and trimeric assembly of RID-HA1. Mutations that affect tRNA binding greatly increased the soluble aggregation defective in trimer assembly, suggesting that RNA interaction critically controls the kinetic network in the folding/assembly pathway. Immunization of mice resulted in strong hemagglutination inhibition and high titers of a neutralizing antibody, providing sterile protection against a lethal challenge and confirming the immunologically relevant HA conformation. The results may be translated into a rapid response to a new influenza pandemic. The harnessing of the novel chaperna described herein with immunologically tailored antigen-folding functions should serve as a robust prophylactic and diagnostic tool for viral infections.—Yang, S. Cutaneous tuberculosis is mostly caused by exogenous reinfection in previously sensitized individuals. The disease spectrum of cutaneous tuberculosis overlaps with that for lupus vulgaris (high immunity), tuberculosis verrucosa cutis (TBVC; moderate/high immunity), scrofuloderma (low immunity), and tuberculosis cutis orificialis (low immunity). TBVC results from direct inoculation of bacilli into the skin of previously infected patients having moderate or high degree of immunity. It usually begins as a solitary keratotic, warty, indurated papule or nodule and slowly progresses in a serpiginous manner, resulting in an irregular and firm reddish-brown warty plaque. The diagnosis is mainly based on the clinical appearance, tuberculin skin tests, culture, and microscopic and histopathological features. We herein report a case of TBVC with negative findings in all usual laboratory tests including a real-time polymerase chain reaction (PCR) for Mycobacterium tuberculosis except the interferon-gamma release assay, but a positive result with a PCR amplification method that we specifically applied for this case.
Calcitriol-induced calcinosis cutis

Calcinosis cutis is defined as an abnormal deposition of calcium phosphate in the skin and subcutaneous tissue. This uncommon disorder is classically divided into four subtypes: dystrophic, metastatic, idiopathic and iatrogenic. Iatrogenic calcinosis has been reported to be associated with intravenous calcium choloride or calcium gluconate therapy. We report a case of calcinosis cutis in a kidney transplant patient taking calcitriol orally. A 66-year-old female was referred to dermatology for the subcutaneous nodules on her extremities for several years. On physical examination, the patient had multiple bean sized flesh colored firm nodules on her extremities. There were no specific symptoms of itching or pain. She had right kidney transplantation in 1993 and then began to take oral calcitriol 5 years ago. Laboratory data showed a total calcium level of 9.2 mg/dL (normal range 8.4-10.2 mg/dL) and a phosphorous level of 3.6 mg/dL (normal range 2.5-4.6 mg/dL). Histophathologic findings showed basophilic-stained calcium deposits in the dermis. Black colored calcium crystals were identified in the Von Kossa stain. Therefore, she was diagnosed with calcitriol induced calcinosis cutis and discontinued calcium supplements for the treatment.

<P><B>Abstract</B></P> <P>In organic tandem solar cells, the morphology of the photo-active layers is not usually stable when their fabrication processes are made with several thermal treatments. Here, we report pentacene-assisted planar tandem organic photovoltaic device based on the active layers of poly(3-hexylthiophene) (P3HT) and (6,6)-phenyl C61-butyric acid methyl ester (PCBM), fabricated in air-ambient. An additive of pentacene was introduced into the active layers of the bottom sub-cells for planarization on their surfaces. The surface morphology of pentacene-based active layer was maintained to be flat from 80 to 160 °C, and allowed for fabricating the planar active layers and interconnecting layer of tandem devices against thermal treatment. State-of-the-art homo-tandem organic solar cells were achieved with an average power conversion efficiency (PCE) of 3.5%, while their best single junction solar cells of P3HT:PCBM achieved a PCE of 3.2%. We showed the first application of high performance tandem organic photovoltaics with three times of high temperature annealing processes at 160 °C. <P>Objective:We investigated whether long-term consumption of Korean mistletoe or Asian Ulmi cortex would prevent or delay menopausal symptoms and progression of osteoarthritis in estrogen-deficient obese rats.Methods:Ovariectomized (OVX) rats were provided a 45% fat diet containing either (1) 0.6% lyophilized water extract of Korean mistletoe (KME)+1.4% dextrose (KME; n=10), (2) 2% lyophilized water extract of Ulmi cortex (UCE; n=10), (3) 30g/kg bw 17-estradiol+2% dextrose (positive control; n=10), (4) 2% dextrose (placebo; OVX-control; n=10), or (5) 2% dextrose (normal-control; n=10) for 4 weeks. At the beginning of the 5th week, OVX rats, except in the normal-control group, were given articular injections of monoiodoacetate into the right knee and the assigned diets were provided for an additional 3 weeks. The rats in the normal-control had injections of saline into the right knee.Results:KME, but not UCE, partially prevented the insulin resistance and the loss of bone mineral density and lean mass. The limping scores were lower in the descending order of the OVX-control > KME and 17-estradiol > UCE > normal-control at day 14 and 21 (P<0.05). The scores for pain behaviors measured by weight distribution on the right leg, maximum running velocity on a treadmill and locomotive activity, were markedly decreased in the same order as limping scores. Monoiodoacetate increased the expression of matrix metalloprotinase-3 and metalloprotinase-13 in the articular cartilage and elevated the production of inflammatory markers such as tumor necrosis factor-, interleukin-1, and interleukin-6, but they were lower in the UCE than in the other groups (P<0.05). Histology of the right knee revealed cartilage damage near the tidemark of the knee and proteoglycan loss was markedly less in UCE.Conclusions:UCE was an effective therapeutic agent for preventing osteoarthritis and KME prevented decreases in lean body mass, bone mineral density, and insulin sensitivity in estrogen-deficient rats.</P>
