Background/Aims: Autoimmune pancreatitis (AIP) is frequently associated with diabetes mellitus (DM). This study evaluated the effect of steroid therapy on the course of DM in AIP. Methods: Glucose tolerance was examined in 69 patients with AIP. DM onset was classified as either a simultaneous onset with AIP or an exacerbation of pre-existing DM. Based on the changes in the HbA1c levels and insulin dose, the responses of DM to steroids were classified as improved, no change, or worsened. Results: Thirty (46%) patients were diagnosed as having DM (simultaneous onset, n=17; pre-existing, n=13). Three months after starting the steroid treatment, the DM improved in 13 (54%) of 24 DM patients. The DM improved in 55%, had no change in 36%, and worsened in 9% of the 11 simultaneous onset DM patients, and it improved in 54%, had no change in 31%, and worsened in 15% of the 13 pre-existing DM patients. At approximately 3 years after starting the steroid treatment, the DM improved in 10 (63%) of 16 patients. The pancreatic exocrine function improved in parallel with the changes in the DM in seven patients. Conclusions: Because approximately 60% of DM associated with AIP is responsive to steroids in the short- and long-terms, marked DM associated with AIP appears to be an indication for steroid therapy. Peroral inoculation of entomopoxvirus (EV) spindles, microstructures composed of the protein fusolin, enhances the infectivity of some insect viruses by disrupting the physical barrier against microbe infection, the peritrophic matrix, in the insect midgut. Here, we examined the temporal persistence of spindles of Anomala cuprea EV (ACEV) that infect Coleopteran species in Bombyx mori larva midgut because spindle solubility over time in the midgut is closely associated with the degree of the enhancement of microbe infectivity by fusolin. A number of ACEV spindles fed to B. mori larvae were retained in the digestive systems even 20 h after the completion of feeding spindles, and a number of spindles were found to be excreted still almost intact in feces under a light microscope. In an in vitro experiment, most ACEV spindles remained intact in B. mori midgut juice 1 h after the start of incubation and some of spindles appeared even overnight in contrast to Bombyx mori nucleoplyhedrovirus polyhedra, which were immediately dissolved in midgut juice. These results suggest spindles are not generally dissolved readily in the midgut of many insects. The difficulty in solubility of ACEV spindles is considered to be mainly due to that fusolin contains many cysteine residues that form a 3D network of disulfide bonds between fusolin dimers. To use spindles at a low cost as additives in microbial insecticides, increasing the solubility of spindles by protein engineering is important to utilize full spindles inoculated.

Background/Aims: The etiology of superficial non-ampullary duodenal epithelial tumors (SNADETs) remains unclear. Recent studieshave reported conflicting associations between duodenal tumor development and Helicobacter pylori infection or endoscopic gastricmucosal atrophy. As such, the present study aimed to clarify the relationship between SNADETs and H. pylori infection and/or endoscopicgastric mucosal atrophy. Methods: This retrospective case-control study reviewed data from 177 consecutive patients with SNADETs who underwent endoscopicor surgical resection at seven institutions in Japan over a three-year period. The prevalence of endoscopic gastric mucosal atrophyand the status of H. pylori infection were compared in 531 sex- and age-matched controls selected from screening endoscopies attwo of the seven participating institutions. Results: For H. pylori infection, 85 of 177 (48.0%) patients exhibited SNADETs and 112 of 531 (21.1%) control patients were non-infected(p<0.001). Non-atrophic mucosa (C0 to C1) was observed in 96 of 177 (54.2%) patients with SNADETs and 112 of 531 (21.1%)control patients (p<0.001). Conditional logistic regression analysis revealed that non-atrophic gastric mucosa was an independent riskfactor for SNADETs (odds ratio, 5.10; 95% confidence interval, 2.44–8.40; p<0.001). Conclusions: Non-atrophic gastric mucosa, regardless of H. pylori infection status, was a factor independently associated with SNADETs.

The temperature dependences of the magnetization and the specific heat of the ferromagneticcompound DyFe2Zn20 have been measured in detail in various magnetic fields. We have observedanomalous magnetic behaviors, i.e., a strong anisotropy at 2 K, the disappearance of this anisotropyat temperatures between approximately 30 K and Tc, and an anomalous behavior of the specificheat in magnetic fields at temperatures near 20 K. These anomalous phenomena have been analyzedbased on the strong exchange interaction between the Fe itinerant electrons and the Dy localizedelectrons, as well as the crystalline electric field, the Zeeman energy, and an usual exchange interactionbetween two Dy atoms. The higher Tc of DyFe2Zn20 compared with that of DyRu2Zn20 iscaused by this exchange interaction between the Fe and the Dy atoms.
Complications of Posterior Fusion for Atlantoaxial Instability in Children With Down Syndrome

Objective: To clarify the complications of posterior fusion for atlantoaxial instability (AAI) in children with Down syndrome and to discuss the significance of surgical intervention. Methods: Twenty pediatric patients with Down syndrome underwent posterior fusion for AAI between February 2000 and September 2018 (age, 6.1±1.9 years). C1–2 or C1–3 fusion and occipitocervical fusion were performed in 14 and 6 patients, respectively. The past medical history, operation time, estimated blood loss (EBL), duration of Halo vest immobilization, postoperative follow-up period, and intra- and perioperative complications were examined. Results: The operation time was 257.9±55.6 minutes, and the EBL was 101.6±77.9 mL. Complications related to the operation occurred in 6 patients (30.0%). They included 1 major complication (5.0%): hydrocephalus at 3 months postoperatively, possibly related to an intraoperative dural tear. Other surgery-related complications included 3 cases of superficial infections, 1 case of bone graft donor site deep infection, 1 case of C2 pedicle fracture, 1 case of Halo ring dislocation, 1 case of pseudoarthrosis that required revision surgery, and 1 case of temporary neurological deficit after Halo removal at 2 months postoperatively. Complications unrelated to the operation included 2 cases of respiratory infections and 1 case of implant loosening due to a fall at 9 months postoperatively. Conclusion: The complication rate of upper cervical fusion in patients with Down syndrome remained high; however, major complications decreased substantially. Improved intra- and perioperative management facilitates successful surgical intervention for upper cervical instability in pediatric patients with Down syndrome.

Objective: Microminipig (MMP) is a miniature pig with an extra small body size for experimental use. In the present study, the occurrence of stillbirths and their genetic association with swine leukocyte antigen (SLA) class II haplotypes were evaluated in a population of MMPs. Methods: The occurrences of stillbirth and genetic association with SLA class II haplotypes using 483 stillborn and 2,246 live piglets, and their parents were compared among the three groups of newborn piglet litters; an all stillborn (AS) group consisting of only stillborn piglet litters, a partial stillborn (PS) group consisting of stillborn and live piglet litters, and an all alive (AA) group consisting of only live piglet litters. Results: The incidence of stillborn piglets was 483/2,729 (17.7%). Distributions of litter sizes, numbers of stillborn piglets in a litter, parities, and gestation periods were distinct among the three groups. The frequencies of low resolution haplotype (Lr)-0.7 or Lr-0.23 were higher in the AS group than in the PS or AA groups. In sires, the frequency of Lr-0.7 associated with the AS group was significantly higher in the AS group than with the AA group. In dams, the frequency of Lr-0.23 was significantly higher in the AS group than in the PS or AA groups, whereas the frequency of Lr-0.7 was not significantly different. Conclusion: The incidence of stillborn piglets in MMPs appears to be higher than those in other pig breeds. Several traits related with stillbirths such as the number of stillborn piglets and parities of the AS group were different from those of the PS and AA groups. Specific SLA class II haplotypes were associated significantly with a high incidence of stillbirths and could be used as genetic markers to adopt breeding strategies to lower the rate of stillbirth in MMPs.

Polyethylene was modified with silanol groups on its surface by photografting of vinyltrimethoxysilane in vapor phase by using benzophenon as a polymerization initiator and by hydrolyzing the methoxysilane groups into the silanol groups with HCI solution. The modified polyethylene formed a dense and homogeneous apatite layer on its surface in a solution with ion concentrations 1.5 times those of human blood plasma within 21 days. This kind of biomimetic process could provide techniques for fabricating apatite-polymer composites with three dimensional structure analogous to the natural bone.
