        Original Article : No association between the IL28B SNP and response to peginterferon plus ribavirin combination treatment in Korean chronic hepatitis C patients

        Background/Aims: There are few available data regarding the association between the single nucleotide polymorphisms (SNPs) of the gene encoding interleukin 28B (IL28B) and a sustained virologic response (SVR) to peginterferon (PEG-IFN) plus ribavirin (RBV) therapy in Korean chronic hepatitis C patients. Methods: This was a retrospective cohort study of 156 patients with chronic hepatitis C virus (HCV) infection who received combination treatment of PEG-IFN plus RBV. Blood samples from these patients were analyzed to identify the IL28B SNPs at rs12979860, rs12980275, rs8099917, and rs8103142. Association analyses were performed to evaluate the relationships between each IL28B SNP and SVRs. Results: Seventy six patients with HCV genotype 1 and 80 with genotype non-1 were enrolled. The frequencies of rs12979860 CC and CT genotypes were 90.4% and 9.6%, respectively; those of rs12980275 AA and AG genotypes were 87.2% and 12.8%, respectively; those of rs8099917 TT and TG genotypes were 92.3% and 7.7%, respectively; and those of rs8103142 TT and CT genotypes were 90.4% and 9.6%, respectively. Among the patients with HCV genotype 1, the SVR rates were 69.7% and 80.0% for rs12979860 CC and CT, respectively (P=0.71). Among the HCV genotype non-1 patients, SVR rates were 88.0% and 100% for rs12979860 CC and CT (P=1.00), respectively. Conclusions: Genotypes of the IL28B SNP that are known to be favorable were present in most of the Korean patients with chronic hepatitis C in this study. Moreover, the IL28B SNP did not influence the SVR rate in either the HCV genotype 1 or non-1 patients. Therefore, IL28B SNP analysis might be not useful for the initial assessment, prediction of treatment outcomes, or treatment decision-making of Korean chronic hepatitis C patients.         Background: The outcome of hepatitis B virus (HBV) infection after liver transplantation (LT) was improved by hepatitis B immunoglobulin (HBIG) and nucles(t)ide analogue (NUA). However, HBV recurrence after LT is critical because the recurrence is occasionally accompanied by a progressive destruction of graft and poor survival. The aims of this study were to investigate the significance HBV recurrence and identity factors associated with HBV recurrence. Methods: From October 1999 to February 2011, a total of 2684 consecutive LT recipients who underwent HBV-associated LT were retrospectively enrolled from 7 transplantation centers in Korea. Results: Prophylaxis regimens were HBIG monotherapy (67.7%) or a combination of HBIG with NUA (22.3%). The recurrence rate of HBV was 6.1% (164 recipients) during mean follow-up duration of 10.9 years. The median time from transplantation to recurrence was 2.1 years (0.1-7.9 years). Of the 1,071 patients with hepatocellular carcinoma (HCC) prior to LT, 155 patients (14.5%) had HCC recurrence after transplantation, and 48 patients (31.0%) had HBV recurrence. Of the 48 patients with recurrence of both HBV and HCC, 25 patients (52.1%) experienced HBV recurrence after HCC recurrence. In the multivariate analysis, pretransplant HCC, pretransplant HBV DNA above 5.5 log copies/mL was independent clinical factors influencing HBV recurrence after LT. The mortality rate among the recipients with HBV recurrence was 34.1% (56 recipients). Mean overall survival was 6.4 years in the HBV-recurrence group and 9.9 years in the HBV-nonrecurrence group (p<0.001). HBV recurrence was not an independent prognostic factor for overall survival. HCC recurrence was the most important factor for overall survival. Conclusions: The overall outcome of LT in HBV-related liver disease was excellent with the current prophylaxis regimen Choon Hyuck David Kwon,8 Suk-Koo Lee8 Pretransplant HBV DNA and HCC were important factors for HBV recurrence. HBV recurrence after LT did not significantly influence on the overall survival without combining of HCC recurrence.

      • Pure 3D Laparoscopic Right Hepatectomy in a Patient with Situs Inversus Totalis

        Aims: Hepatectomy in a patient with situs inversus totalis is technically challenging due to its mirror image anatomy. Especially for laparoscopic approach, the port sites and the position of the surgeons are important matter. The aim of this study is to introduce our pure 3D laparoscopic right hepatectomy in a patient with situs inversus totalis. Methods: A patient with situs inversus totalis who was diagnosed with hepatocellular carcinoma was retrospectively reviewed. Results: 5 port method was used and the port site was bilateral symmetry with laparoscopic right hepatectomy in usual patients. Bilateral symmetry position of the assistant and using left hand when using cavitron ultrasonic surgical aspirator (CUSA) for parenchyme dissection were efficient. The total operation time was 220 minutes and the estimated blood loss was 450 cc. There was no intraoperative complication without transfusion. The patient was discharged on postoperative day 8 with no postoperative complication. Conclusions: Pure 3D laparoscopic right hepatectomy in a patient with situs inversus totalis was safely performed.

        Different Mechanisms for K<SUP>⁢</SUP>-Induced Relaxation in Various Arteries

        <P> [K<SUP>⁢</SUP>]<SUB>o</SUB> can be increased under a variety of conditions including subarachnoid hemorrhage. The increase of [K<SUP>⁢</SUP>]<SUB>o</SUB> in the range of 5∼15 mM may affect tensions of blood vessels and cause relaxation of agonist-induced precontracted vascular smooth muscle (K<SUP>⁢</SUP>-induced relaxation). In this study, effect of the increase in extracellular K<SUP>⁢</SUP> concentration on the agonist-induced contractions of various arteries including resistant arteries of rabbit was examined, using home-made Mulvany-type myograph. Extracellular K<SUP>⁢</SUP> was increased in three different ways; from initial 1 to 3 mM, from initial 3 to 6 mM, or from initial 6 to 12 mM. In superior mesenteric arteries, the relaxation induced by extracellular K<SUP>⁢</SUP> elevation from initial 6 to 12 mM was the most prominent among the relaxations induced by the elevations in three different ways. In cerebral arteries, the most prominent relaxation was produced by the elevation of extracellular K<SUP>⁢</SUP> from initial 1 to 3 mM and a slight relaxation was provoked by the elevation from initial 6 to 12 mM. In superior mesenteric arteries, K<SUP>⁢</SUP>-induced relaxation by the elevation from initial 6 to 12 mM was blocked by Ba<SUP>2⁢</SUP> (30 μM) and the relaxation by the elevation from 1 to 3 mM or from 3 to 6 mM was not blocked by Ba<SUP>2⁢</SUP>. In cerebral arteries, however, K<SUP>⁢</SUP>-induced relaxation by the elevation from initial 3 to 6 mM was blocked by Ba<SUP>2⁢</SUP>, whereas the relaxation by the elevation from 1 to 3 mM was not blocked by Ba<SUP>2⁢</SUP>. Ouabain inhibited all of the relaxations induced by the extracellular K<SUP>⁢</SUP> elevations in three different ways. In cerebral arteries, when extracellular K<SUP>⁢</SUP> was increased to 14 mM with 2 or 3 mM increments, almost complete relaxation was induced at 1 or 3 mM of initial K<SUP>⁢</SUP> concentration and slight relaxation occurred at 6 mM. TEA did not inhibit Ba<SUP>2⁢</SUP>-sensitive relaxation at all and NMMA or endothelial removal did not inhibit K<SUP>⁢</SUP>-induced relaxation. Most conduit arteries such as aorta, carotid artery, and renal artery were not relaxed by the elevation of extracellular K<SUP>⁢</SUP>. Among conduit arteries, trunk of superior mesenteric artery and basilar artery were relaxed by the elevations of [K<SUP>⁢</SUP>]<SUB>o</SUB>. These data suggest that K<SUP>⁢</SUP>-induced relaxation has two independent components, Ba<SUP>2⁢</SUP>-sensitive and Ba<SUP>2⁢</SUP>-insensitive one and there are different mechanisms for K<SUP>⁢</SUP>-induced relaxation in various arteries.

      • Two-dimensional Largest ovarian area to predict ovarian response in in vitro fertilization cycle

        Objective: To evaluate the usefulness of two dimensional largest ovarian area (LOA) as a predictive parameter of oocyte yield and pregnancy in in vitro fertilization (IVF) cycle. Methods: Ninety-two stimulated IVF cycles from sixty-eight infertile women were included. On the day 3 of cycles, antral follicle count (AFC) and LOA were measured by ultrasonography before starting gonadotropins. LOA was calculated by ellipsoid formula (D1×D2×0.8) using two perpendicular diameters in the largest cross-sectional view of each ovary and then summed. Results: Both LOA and AFC had a positive correlation with the number of retrieved oocytes. The best cut-off value of LOA was 6.14 cm2 for predicting poor ovarian response (area under curve [AUC], 0.783; 95% confidence interval [CI], 0.684 to 0.862), 7.70 cm2 for predicting cycle cancellation (AUC, 0.794; 95% CI, 0.697 to 0.871) and 8.36 cm2 for predicting pregnancy (AUC, 0.641; 95% CI, 0.534 to 0.739). For AFC, the best cut-off value was 9 for poor ovarian response (AUC, 0.877; 95% CI, 0.792 to 0.936), 6 for predicting cycle cancellation (AUC, 0.942; 95% CI, 0.872 to 0.980) and 9 for pregnancy (AUC, 0.683; 95% CI, 0.578 to 0.776). The predictability for pregnancy was similar between LOA and AFC. Conclusion: LOA is easy to measure and a good marker for predicting ovarian response and IVF pregnancy.

        Sensitivity of Rabbit Cerebral Artery to Serotonin is Increased with the Moderate Increase of Extracellular K<SUP>⁢</SUP>

Therefore, it was examined in the present study whether the sensitivity of cerebral arteries to vasoactive substances can be changed with the moderate increase of [K<SUP>⁢</SUP>]<SUB>O</SUB>, using Mulvany-type myograph and [Ca<SUP>2⁢</SUP>]<SUB>c</SUB> measurement. The contractions of basilar artery and branch of middle cerebral artery induced by histamine were not increased with the elevation of [K<SUP>⁢</SUP>]<SUB>O </SUB>from 6 mM to 9 mM or 12 mM. On the contrary, the contractions induced by serotonin were significantly increased with the elevation of [K<SUP>⁢</SUP>]<SUB>O</SUB>. The contractions were also significantly increased by the treatment with nitro-L-arginine (10<SUP>⁣4</SUP> M for 20 minutes). In the nitro-L-arginine treated arteries, the contractions induced by serotonin were significantly increased with the elevation of [K<SUP>⁢</SUP>]<SUB>O </SUB>from 6 mM to 12 mM. K<SUP>⁢</SUP>-induced relaxation was evoked with the stepwise increment of extracellular K<SUP>⁢</SUP> from 0 or 2 mM to 12 mM by 2 mM in basilar arterial rings, which were contracted by histamine. But [K<SUP>⁢</SUP>]<SUB>O </SUB>elevation from 4 or 6 mM to 12 mM by the stepwise increment evoked no significant relaxation. Basal tension of basilar artery was increased with [K<SUP>⁢</SUP>]<SUB>O </SUB>elevation from 6 mM to 12 mM by 2 mM steps or by the treatment with ouabain and the increase of basal tension was blocked by verapamil. The cytosolic free Ca<SUP>2⁢</SUP> level was not increased by the single treatment with serotonin or with the elevation of [K<SUP>⁢</SUP>]<SUB>O </SUB>from 4 mM to 8 or 12 mM. In contrast to the single treatment, the Ca<SUP>2⁢</SUP> level was increased by the combined treatment with serotonin and the elevation of [K<SUP>⁢</SUP>]<SUB>O</SUB>. The increase of free Ca<SUP>2⁢</SUP> concentration was blocked by the treatment with verapamil. These data suggest that the sensitivity of cerebral artery to serotonin is increased with the moderate increase of [K<SUP>⁢</SUP>]<SUB>O </SUB>and the increased sensitivity to serotonin is due to the increased [Ca<SUP>2⁢</SUP>]<SUB>i</SUB> induced by extracellular Ca<SUP>2⁢</SUP> influx.

      • Ice Collision Analyses for Membrane Tank Type LNG Carrier

        As arctic energy resource is attracting public attention, arctic shipping market will also be growing in large as expected to increase in LNG trade from Arctic area to the western countries by shipping. During the voyages through such routes, collision with icebergs may be possible. In the present report, ice collision analyses are carried out from a practical point of view to verify the safety of hull structural strength of LNG carriers equipped with GTT $MKIII^{TM}$ membrane type cargo containment system. From the results of collision analyses and the operation-friendly design concept of no-repairing of cargo containment system, a safe operating envelope against ice collision is proposed for LNG carriers of membrane type cargo containment system. Based on the currently proposed safety criteria, it is concluded that LNG carriers with membrane tank type can operate safely with regard to the integrity of CCS in regions where collision between LNG carrier and iceberg is expected.
