An 18-year-old woman was evaluated for a chronic productive cough and dyspnea. She was subsequently diagnosed with mediastinal non-Hodgkin lymphoma (NHL). A covered self-expandable metallic stent (SEMS) was implanted to relieve narrowing in for both main bronchi. The NHL went into complete remission after six chemotherapy cycles, but atelectasis developed in the left lower lobe 18 months after SEMS insertion. The left main bronchus was completely occluded by granulation tissue. However, the right main bronchus and intermedius bronchus were patent. Granulation tissue was observed adjacent to the SEMS. The granulation tissue and the SEMS were excised, and a silicone stent was successfully implanted using a rigid bronchoscope. SEMS is advantageous owing to its easy implantation, but there are considerable potential complications such as severe reactive granulation, stent rupture, and ventilation failure in serious cases. Therefore, SEMS should be avoided whenever possible in patients with benign airway disease. This case highlights that SEMS implantation should be avoided even in malignant airway obstruction cases if the underlying malignancy is curable.

A large portion of the Korean population has been exposed to toxic humidifier disinfectants (HDs), and considering that the majority of the victims are infants, the magnitude of the damage is expected to be considerably larger than what has currently been revealed. The current victims are voicing problems caused by various diseases, including but not limited to lung, upper respiratory tract, cardiovascular, kidney, musculoskeletal, eye, and skin diseases, etc. However, there has been difficulty in gaining validation for these health problems and identifying causal relationships due to lack of evidence proving that toxic HD is the specific causes of extrapulmonary diseases such as allergic rhinitis. Furthermore, the victims and bereaved families of the HD case have not received any support for psychological distress such as post-traumatic stress disorder, depression, feelings of injustice, and anger caused by the trauma. In addition, because the underlying mechanisms of the toxic materials within the HDs such as polyhexamethylene guanidine phosphate, poly(oxyalkylene guanidine) hydrochloride, chloromethylisothiazolinone /methylisothiazolinone have yet to be determined, the demand for information regarding the HD issue is growing. The victims of the HD cases require support that goes beyond financial aid for medical costs and living expenses. There is a desperate need for government-led integrated support centers that provide individualized support through health screenings; in other words, we need an integrated facility that provides the appropriate social support to allow the victims to recover their physical and mental health, so as to well prepare them to return to a normal life. The implementation of such a plan requires not only the close cooperation between those departments already directly involved such as the Ministry of Environment and the Ministry of Health and Welfare, but also active support on a national scale from pan-governmental consultative bodies.
A1 receptors inhibit glutamate release in rat medullary dorsal horn neurons

We have investigated the adenosine-mediated presynaptic inhibition of primary afferent-evoked glutamate release in rat substantia gelatinosa neurons of the trigeminal subnucleus caudalis using a conventional whole-cell patch clamp technique. Adenosine reversibly and concentration dependently decreased the amplitude of glutamatergic excitatory postsynaptic currents and increased the paired-pulse ratio, indicating that adenosine acts presynaptically to reduce glutamate release from primary afferents. The adenosine-induced inhibition of excitatory postsynaptic currents was occluded by a selective A1 receptor antagonist, DPCPX, and was mimicked by a selective A1 receptor agonist CPA. The results suggest that presynaptic A1 receptors decrease action potential-dependent glutamate release from trigeminal primary afferents onto medullary dorsal horn neurons, and thus adenosine A1 receptors could be a potential target for the treatment of pain of orofacial tissues.

목적: Teach a new procedure 방법: The reported incidence of vesicovaginal fistula following hysterectomy is about 0.2%. Most techniques for correcting vesicovaginal fistula consist of transabdominal, transvaginal, or laparoscopic approach. Standard management of vesicovaginal fistula requires a 3-6 month interval from injury to repair to ensure complete resolution of necrosis and inflammation. However, some have advocated early closure of fistulas with good results. In 1942, Latzko developed a technique whereby posthysterectomy vesicovaginal fistulas were treated through a purely vaginal approach of which result was excellent. It has the advantages of a short operating time, minimal blood loss, and low postoperative morbidity while being a simple and minimally invasive procedure. We would like to present the simple and effective Latzko procedure for posthysterectomy vesicovaginal fistula within 1month after hysterectomy.
A case report: malignant placental site trophoblastic tumor with focal choriocarcinoma

Placental site trophoblastic tumor (PSTT) is a very rare and unique form of gestational trophoblastic disease (GTD). This tumor is potentially malignant neoplasm originating from extravillous trophoblast cells. We present a case of a gravida 2 para 2 woman, who presented with 4 years history of amenorrhea after term vaginal delivery. We did pelvic MRI. The result was suspicious invasive mole or choriocarcinoma. There was no evidence of distant metastasis. Initial serum beta hCG level was 816.5 mIU/mL. The patient underwent hysterectmomy and both salpingo-oophorectomy. The final pathologic result was malignant placental site trophoblastic tumor with focal choriocarcinoma. We decided to do single agent chemotherapy; 8 day course methotrexate (MTX). During chemotherapy, the patient’s liver enzyme was elevated. We decided to stop chemotherapy and observe checking serum beta hCG because serum beta hCG level was decreased to 12.26mIU/mL. We checked serum beta hCG weakly. After 2 months, serum beta hCG level was slightly increased. We decided to do chemotherapy using MTX because she was discontinued chemotherapy due to complications but treatment was effective. After resuming chemotherapy, serum beta hCG and liver enzyme were elevated. Currently, we considered changing drug from MTX to dactinomycin (ActD) after liver enzyme was decreased within normal range.
Antiviral Activity of Yogurt against Enterovirus 71 in Vero Cells

Five yogurts were fermented with each bacteria strain. The viability and pH of each yogurt during fermentation or storage were evaluated, and then the cytotoxicity and antiviral activity against enterovirus (EV) 71 of cell-free supernatants (CFS) of the metabolites of each yogurt were compared with those of de Man, Rogosa, and Sharpe (MRS) broth fermented with the same bacteria. As the results, the number of viable bacteria for each yogurt after fermentation or during storage always remained higher than 5 log CFU/mL and the pH of those ranged from 4 to 6. The CFS of all yogurts showed antiviral activity over 45% against EV71, while it didn't exhibit cytotoxicity in Vero cells. Specially, the CFS of yogurt fermented with Lactobacillus plantarum and Bifidobacterium bifidum exhibited high anti-EV71 activity of 92.74 and 90.44%, respectively. In contrast, the CFS of each MRS broth fermented with the same bacteria showed low antiviral activity of less than 30%.
