<P><B>Abstract</B></P> <P>A phytochemical investigation into the bark of <I>Erythrophleum fordii</I> yielded four new compounds, two new cassaine diterpenoids (erythrofordin T and U, <B>1</B> and <B>2</B>) and two new cassaine diterpenoid amines (erythroformine A and B, <B>6</B> and <B>7</B>), as well as nine known compounds. We report for the first time the isolation of erythrofordin V (<B>3</B>) from a natural source and that of the remaining eight known diterpenoids (<B>4</B>–<B>5</B>, <B>8</B>–<B>13</B>) from <I>E. fordii</I>. All structures were elucidated using spectroscopic analysis. Cytotoxic activity of the isolated compounds (<B>1</B>–<B>13</B>) was examined <I>in vitro</I> against three non-small cell lung cancer cell lines (A549, NCI-H1975, and NCI-H1229) using the MTT assay. Cassaine diterpene amines (<B>6</B>–<B>10</B>, <B>12</B>, <B>13</B>) exhibited potent cytotoxic activity against all three cell lines with IC<SUB>50</SUB> values between 0.4μM and 5.9μM. Erythroformine B (<B>7</B>) significantly induced apoptosis in all three cancer cells in a concentration-dependent manner.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>
Promotion of Remyelination by Sulfasalazine in a Transgenic Zebrafish Model of Demyelination

Most of the axons in the vertebrate nervous system are surrounded by a lipid-rich membrane called myelin, which promotes rapid conduction of nerve impulses and protects the axon from being damaged. Multiple sclerosis (MS) is a chronic demyelinating disease of the CNS characterized by infiltration of immune cells and progressive damage to myelin and axons. One potential way to treat MS is to enhance the endogenous remyelination process, but at present there are no available treatments to promote remyelination in patients with demyelinating diseases. Sulfasalazine is an anti-inflammatory and immune-modulating drug that is used in rheumatology and inflammatory bowel disease. Its anti-inflammatory and immunomodulatory properties prompted us to test the ability of sulfasalazine to promote remyelination. In this study, we found that sulfasalazine promotes remyelination in the CNS of a transgenic zebrafish model of NTR/MTZ-induced demyelination. We also found that sulfasalazine treatment reduced the number of macrophages/microglia in the CNS of demyelinated zebrafish larvae, suggesting that the acceleration of remyelination is mediated by the immunomodulatory function of sulfasalazine. Our data suggest that temporal modulation of the immune response by sulfasalazine can be used to overcome MS by enhancing myelin repair and remyelination in the CNS.

A 49-year-old man visited the emergency room of Korea University Ansan Hospital with hematochezia starting the day before the visit. Recently, he was on anti-platelet medication due to hypertension. The patient had no definite symptoms other than hematochezia. Digital rectal exam was positive and laboratory tests showed severe anemia. Sigmoidoscopy was initiated and almost no fecal material was observed in the intestinal tract, allowing insertion into the cecum. Active bleeding from the appendiceal opening was noted. On abdominal CT, contrast enhancement was observed at the tip of the appendix. Under suspicion of acute appendicitis, we consulted with a surgeon. The patient underwent appendectomy with partial cecal resection. Pathologic examination revealed a diagnosis of appendix bleeding due to acute suppurative appendicitis. The patient had no further bleeding after surgery and was discharged in a stable state. Careful observation by the endoscopist is necessary for accurate diagnosis of lower gastrointestinal hemorrhage. Appendiceal hemorrhage is very rarely reported, but it has various pathophysiologies. CT scan is useful when appendiceal hemorrhage is confirmed by endoscopic findings. Surgical treatment was needed in almost all cases reported worldwide. If bleeding from the appendix is confirmed, surgical treatment should be considered for both therapeutic and diagnostic purposes. (Korean J Gastroenterol 2019;73:45-49)

<P>The aims of this paper were to investigate the effectiveness of a newly developed wearable hip assist robot, that uses an active assist algorithm to improve gait function, muscle effort, and cardiopulmonary metabolic efficiency in elderly adults. Thirty elderly adults (15 males/ 15 females) participated in thispaper. The experimental protocol consisted of overground gait at comfortable speed under three different conditions: free gait without robot assistance, robot-assisted gait with zero torque (RAG-Z), and full RAG. Under all conditions, muscle effort was analyzed using a 12-channel surface electromyography system. Spatio-temporal data were collected at 120 Hz using a 3-D motion capture system with six infrared cameras. Metabolic cost parameters were collected as oxygen consumption per unit (ml/min/kg) and aerobic energy expenditure (Kcal/min). In the RAG condition, participants demonstrated improved gait function, decreased muscle effort, and reduced metabolic cost. Although the hip assist robot only provides assistance at the hip joint, our results demonstrated a clear reduction in knee and ankle muscle activity in addition to decreased hip flexor and extensor activity. Our findings suggest that this robot has the potential to improve stabilization of the trunk during walking in elderly adults.</P>

Background: This study aimed to perform meta-analyses to update a previous systematic review (SR) conducted by the US Preventive Services Task Force (USPSTF) to evaluate the benefits and harms of screening for chronic obstructive pulmonary disease (COPD) in asymptomatic adults. Methods: MEDLINE, EMBASE, Cochrane Library, and regional databases were searched from their inception to January 2020. Studies for diagnostic accuracy, preventive services effect, treatment efficacy, and treatment harms were included. Results: Eighteen studies were included, and twelve of these were newly added in this update. In meta-analyses, the pooled sensitivity and specificity for COPD diagnosis using spirometry were 73.4% and 89.0%, respectively. The relative effect of smoking cessation intervention with screening spirometry, presented as abstinence rate, was not statistically significant (risk ratio [RR], 1.21; 95% confidence interval [CI], 0.87–1.67) when all selected studies were pooled, but screening on smoking cessation was effective (RR, 1.58; 95% CI, 1.14–2.19) when limited to studies with smoking cessation programs that provided smoking cessation medicines or intensive counseling at public health centers or medical institutions. Conclusion: In this study, no direct evidence for the impact on health outcomes of screening asymptomatic adults for COPD was identified similar to the previous SR. Further research is necessary to confirm the benefits of COPD screening.
아이스플랜트(Mesembryanthemum crystallinum)의 생화학적 성분 및 생리활성 효과

아이스플랜트(Mesembryanthemum crystallinum)의 일반성분과 cyclitol 화합물을 분석하였으며, 기능성 식품의 소재로 활용 가능성을 알아보기 위하여 추출물의 용매분획별 항산화 효과 및 항당뇨 활성 등을 검정하였다. 무기질 함량은 K와 Na가 각각 1,213.33±2.52와 545.53±12.01 mg/100 g으로 가장 많았고, S, Ca, P, Mg의 순이었다. Cyclitol 화합물의 함량은 D-pinitol이 4.04±0.08 mg/g으로 가장 많았으며, chiro-inositol과 myo-inositol은 각각 2.82±0.01과 0.25±0.01 mg/g으로 비교적 적게 함유되어 있었다. 용매분획별 총페놀 함량은 chloroform과 ethyl acetate 분획에서 각각 35.80±1.33과 23.70±0.62 mg GAE/g으로 많았다. DPPH, ABTS, FRAP 및 lipid/MA assay에 의한 항산화 활성은 공통적으로 chloroform 분획에서 가장 높은 활성을 나타내었으며, ethyl acetate 분획에서 비교적 높은 활성을 나타내었다. 항산화 활성은 용매분획별 총페놀 함량과 비례적이었다. 한편 항당뇨 효과는 α-glucosidase 저해활성에서 모든 용매분획에서 50% 이상의 활성을 나타내었으며, ethyl acetate, butanol 및 chloroform 분획에서 각각 90.33±0.40, 87.98±0.16 및 86.38±0.51%의 높은 활성을 나타내었다. α-Amylase 저해 활성은 25.63±1.45~60.34±2.67%로서 α-glucosidase 저해 활성보다는 낮았으나, 용매분획별 저해 활성은 유사한 경향을 보였다. 이와 같은 연구 결과로 볼 때 아이스플랜트가 항산화 및 항당뇨 기능성 천연소재로 활용이 가능할 것으로 생각되었다. The general components and cyclitol compounds of ice plant (Mesembryanthemum crystallinum) were analyzed to examine the possibility of using extracts as functional food materials, the antioxidant effects and antidiabetic activities of the extracts by solvent fraction were tested. Among the mineral contents, contents of K and Na were the largest (1,213.33±2.52 and 545.53±12.01 mg/100 g, respectively), followed by S, Ca, P, and Mg in order of precedence. Among cyclitol compounds, content of D-pinitol was the largest (4.04±0.08 mg/g) while contents of chiro-inositol and myo-inositol were relatively small (2.82±0.01 and 0.25±0.01 mg/g, respectively). Among total phenol contents by solvent fraction, contents of chloroform and ethyl acetate fractions were large (35.80±1.33 and 23.70±0.62 mg GAE/g, respectively). Among antioxidant activity levels examined by DPPH, ABTS, FRAP, and lipid/MA assays, the chloroform fraction commonly showed the highest level of activity while the ethyl acetate fraction showed relatively high levels of activity. The antioxidant activity levels were proportional to total phenol contents by solvent fraction. As for antidiabetic effects, all solvent fractions showed at least 50% α-glucosidase inhibitory activity levels while the ethyl acetate, butanol, and chloroform fractions showed high levels activity of 90.33±0.40, 87.98±0.16, and 86.38±0.51%, respectively. The α-amylase inhibitory actively levels were in the range of 25.63±1.45∼60.34±2.67%, which was lower than the α-glucosidase inhibitory activity levels, but the inhibitory activity levels by solvent fraction were similar. Given the above study results, ice plant can be utilized as a natural material with antioxidative and antidiabetic functionality.
